Objective: To explore the practice and application of infection prevention and control strategies in risk departments during the COVID-19 epidemic, and to formulate the infection prevention and control measures to provide advice and guidance in risk departments. Methods: According to the latest plan of diagnosis and treatment, prevention and control issued by the National Health Commission, expert advice and consensus, combined with the actual situation in our hospital, a series of infection prevention and control measures of COVID-19 in risk department was formulated. Results: During the epidemic period, the prevention and control measures of nine risk departments including emergency operation, anesthesiology, endoscopy center, blood purification center, otolaryngology, stomatology, medical imaging department, medical cosmetology department and pulmonary function room were established from six aspects, including pre-examination and screening, medical technology control, personnel management, personal protection, environmental disinfection, medical waste disposal, etc. Conclusion: During the epidemic period, the infection prevention and control strategy of risk departments is one of the key links to control the spread of the epidemic, and risk departments must pay attention to and strictly implement various infection prevention and control measures. 展开更多

Objective: To evaluate the role of prevention and control strategies for nosocomial infection in a tertiary teaching hospital during the sudden outbreak of Corona Virus Disease 2019 (COVID-19). Methods: The hospital initiated an emergency plan involving multi-departmental defense and control. It adopted a series of nosocomial infection prevention and control measures, including strengthening pre-examination and triage, optimizing the consultation process, improving the hospital’s architectural composition, implementing graded risk management, enhancing personal protection, and implementing staff training and supervision. Descriptive research was used to evaluate the short-term effects of these in-hospital prevention and control strategies. The analysis compared changes in related evaluation indicators between January 24, 2020 and February 12, 2020 (Chinese Lunar New Year’s Eve 2020 to lunar January 19) and the corresponding lunar period of the previous year. Results: Compared to the same period last year, the outpatient fever rate increased by 1.85-fold (P P Conclusion: The nosocomial infection prevention and control strategies implemented during this specific period improved the detection and control abilities for the COVID-19 source of infection and enhanced the compliance with measures. With the petroleum industry endeavoring to develop promising oil and gas in deeper water, gas hydrates prevention is a serious concern for oil and gas producing companies producing at conditions in the hydrate region. This paper details lessons learned from the successful field deployment of AA LDHI and proper implementation strategies used for 3 different practical fields as case studies in the Gulf of Mexico. From the 3 field experiences, the AA LDHI has been used to replace the conventional thermodynamic hydrate inhibitor due to its numerous benefits during steady state operations and transition operations where AA LDHI is injected prior to extended shut in and restart for fields producing at low water cut. However, the strategy to develop a cost effective chemical management of hydrates for fields producing at high water cut is by pumping methanol or diesel to push down the wellbore fluid below the mud line during planned and unplanned shut-ins to delay water production, it also secures the riser with non hydrate fluids. This illustrates how the AA LDHIs are used in conjunction with more conventional hydrate management approaches to reach an optimal cost effective field hydrate management solution. However, this shows that the key to overall success of hydrate prevention is a full integration of a good front end design, a comprehensive deployment and an effective down hole monitoring system. 展开更多

The variant has many mutations and spreads quickly. It also has immune escape to existing COVID-19 vaccines, and the infection rate and repeated infection rate have increased worldwide. For such variant, at this stage, foreign countries mainly adopt three strategies to prevent and control the epidemic. First, big data is used to track the potential infection of the epidemic, trace the spread of the epidemic, screen close contacts, and control the scope of infection. The second is to maximum the coverage rate of vaccination, especially the coverage rate of the third injection, so as to reduce the infection level of Omicron variant. At last, they are developing a general coronavirus vaccine to prevent infection caused by SARS-CoV-2 variants, including the Omicron variant, so as to fundamentally eliminate the potential threat of variants to human beings. Appraisal of Existing HIV/AIDs Prevention and Control Measures and Presentation of Innovative Strategies to End HIV/AIDS Epidemic by 2030

Background: Globally, UNAIDS report 2022 shows, there are 84.2 million people affected by HIV/AIDS and 40.1 million deaths from AIDS since the start of epidemic. In sub-Saharan Africa, women and girls accounted for 63% of all new HIV infections in 2021 with, six in seven new HIV infections among adolescents aged 15 - 19 years being girls. Key populations accounted for 70% of HIV infections globally in 2021, with 51% of these new HIV infections in sub-Saharan Africa. Reflecting on the 4 decades’ journey of HIV epidemic amidst local, national and international efforts, the UN target of ending AIDS as a public health threat by 2030 remains questionable unless new innovative ways are used. This study aimed at analyzing existing HIV/AIDS interventions, discuss UN interventions in line with ending HIV/AIDS by 2030 then, suggest and discuss new innovative ways of ending HIV scourge by 2030. Methods: Systematic literature review methodology was used to extract existing published information on HIV prevention strategies from 1981 to 2023. The articles were previewed by 2 experts for quality and grouped by intervention. Of the 637 articles accessed, on HIV prevention/control only 45 met the inclusion criteria. Data were synthesized using a narrative synthesis approach following standard guidelines on synthesis without meta-analysis. Descriptive analysis was done, strength and limitations were identified. UNAIDS recommendations for ending HIV/AIDS by 2030 identified and analyzed. New Innovations in HIV/AIDS were presented and discussed. The scope of the reviewed literature was limited to HIV preventive strategies practiced between 1981 and 2023. Results: Findings show that, Uganda’s HIV prevalence was at a peak in 1991 of 15% (30% among pregnant women in urban areas). ABC strategy is claimed to have turned sharply downward the prevalence through the mid-1990s and reached 5% (14% for pregnant urban women) by 2001. Analysis of the strategy showed that the achievements of the strategy could not be sustained, subsequently HIV prevalence rose again. This is because none of the ABC components can independently reduce HIV problem. In the real world, 100% abstinence has failed, condom use only reduces infection by 90% (WHO), and lifelong monogamy is impractical. Such limitations weaken ABC strategy. The study established that Post-exposure prophylaxis (PEP) i.e. taking HIV medicines within 72 hours (3 days) after a possible exposure to HIV infection is a safe, effective and a globally practiced HIV preventive intervention in emergency situations of HIV exposure. However, PEP is limited to care sought within 72 hours after exposure and yet timely access especially in rural areas and for key populations remains a big challenge. Oral PrEP was also identified as effective HIV preventive measure that can reduce HIV risk from sex by about 99% and from injection drug users by 74%. However, like PEP, timely access especially in rural areas and for key populations remains a big challenge. The UNAIDS 95-95-95 strategy (i.e. 95% of people know their HIV status, 95% with +HIV status be on sustained ART and 95% on ART get viral load suppression) formed the basis for setting the target of ending HIV/AIDS epidemic by 2030. However, our analysis shows that this target is unrealistic given the above highlighted limitations/ barriers in preventive measures and the unlikely perfect adherence (100%) to ART by all enrolled HIV positive persons. Conclusion: Ending HIV/AIDS by 2030 cannot be achieved by implementing the current preventing strategies and control measures. This study established that most of the existing HIV preventive strategies and control measures have a number of limitations. However, with sustained UN 95-95-95 strategy supplemented with additional innovative ways, there is hope that the UN dream of ending HIV/AIDS though not necessarily by 2030, can in the long run be achieved. 展开更多

Gastric cancer(GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half... Gastric cancer(GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half of all GC cases worldwide occur in East Asia, notably China. Globally, overall incidence rates of GC are declining, which is potentially attributed to a decrease in Helicobacter pylori(H. pylori) infection and the use of refrigeration to preserve foods rather than salt. GC is a multifactorial disease, and its occurrence and development were impacted by environmental and genetic factors. H. pylori infection is the primary risk factor for GC, especially for non-cardia. The prognosis of GC is poor due to stages at the first diagnosis. The 5-year survival rate is less than 10% when patients are diagnosed at an advanced stage, but the rate is as high as 85% if patients are detected at an earlier stage. Endoscopic screening can potentially prevent GC by early diagnosis and early treatment and has been widely adopted in screening programs in East Asian countries, such as Japan and Korea. This review summarizes updated epidemiological aspects, risk factors, and prevention strategies of GC in recent years to help researchers determine the most effective intervention strategies for reducing risk of GC. 展开更多

However, it is impossi... Taiwan has disadvantageous conditions for sediment-related disasters such as debris flows. The construction of engineering structures is an effective strategy for reducing debris flow disasters. However, it is impossible to construct engineering structures in all debris flow areas in a short period. Therefore, the government aims to gradually develop non-structural preventive strategies, including evacuation planning, debris flow disaster emergency action system, disaster resistant community program, recruitment of debris flow professional volunteers, debris flow warning systems, and land management strategies, to mitigate disasters and secure the safety of residents. This review describes the processes and effects of recent debris flow non-structural preventive strategies in Taiwan. The average number of casualties prior to the year 2000 was far higher than the corresponding number after 2000 because debris flow evacuation drills have been promoted since 2000 and the debris flow disaster emergency action system has been progressively improved since 2002. Furthermore, the changes in risks caused by debris flow disasters before and after the implementation of non-structural preventive strategies were used to explain the effectiveness of these strategies at the community level. The results showed that softwarebased non-structural preventive strategies can effectively reduce the casualties caused by debris flows at both the national and community levels. 展开更多

Historically,frequent and heavy snow disaster(SD)has caused serious livestock death and casualties,resulting in a devastating impact on animal husbandry development in the Three Rivers Source Region(TRSR).From winter in 2018 to spring in 2019,the largest SD occurred in this area over the past 10 years,especially in core zones of the Lancang River Source Region.Field research results show that the main causes of the major SD include weak infrastructure(i.e.,roads,communications,warm sheds,and insufficient forage reserve),low rate of domestic animals for sale before the SD,and low loss settlement rate.SD occurrence could furtherly reduce the ability of disaster prevention,mitigation and relief of disaster loss.In the future,heavily affected SD areas should improve the forecasting ability of snowfall incidents,strengthen infrastructure construction,implement grass and livestock balance strategies,optimize livestock structure,improve loss settlement rate,and develop a modern compound model of animal husbandry development model that combines breeding,slaughtering and deep processing of animal product. 展开更多

Many countries use similar strategies in prevention campaigns, but despite that the spread of HIV is unabated. The basic messages of the current liberal prevention strategies aim to promote changes in individual behaviour so as to prevent HIV transmission. The strategies avoid any kind of regulation, prescription and controls, and trust solely on the readiness of the sexually active part of the general population, and in particular the “at-risk populations” to cooperate with those messages. However, only a small number of people have cooperated. The increase in the incidence of HIV over the past decade in the countries listed discloses the failures of the current prevention strategies. With respect to the goal of trying to influence those people prone to high-risk sexual lifestyles, and the injecting drug users, the prevention strategies have not had a meaningful effect in the long term. This is also reflected in the increase of other STIs such as hepatitis C, syphilis, and the human papilloma virus, which are worse when there is co-infection with HIV. The liberal prevention strategies turned out to be inadequate for the goal, and must be adjusted to real life situations to counteract their misuse. Meanwhile there are ample evidence-based measures which must be implemented into concerted efforts by complementing the current strategies with respect to a person-to-person bound infection. Medical care policies have to consider the non-adherence behaviour of those in need. 展开更多

<strong>Objectives:</strong> To analyze the clinical features of falls in hospitalized patients with chronic kidney disease (CKD), and to provide basis for preventing the occurrence of falls in hospitalize... <strong>Objectives:</strong> To analyze the clinical features of falls in hospitalized patients with chronic kidney disease (CKD), and to provide basis for preventing the occurrence of falls in hospitalized patients with CKD. <strong>Method: </strong>A retrospective study was conducted to analyze the 27 adverse events of falls in hospitalized patients from the department of nephrology in Peking University Shenzhen Hospital during May 2009 and May 2019. <strong>Results: </strong>Among the 27 patients with falls, people over 65 years old accounted for 55.56%;74.07% of the patients had caregivers;the peak period of falls was from June to August, accounting for 40.74%, mainly occurring from 7:00 to 8:00, 12:00 to 13:30 and 19:00 to 21:00;the locations of falls were mostly near the bed and in the bathroom, accounting for 33.33% and 22.22%, respectively;suffering from anemia, hypoproteinemia, abnormal serum potassium, calcium-phosphorus metabolism disorders accompanied by abnormal parathyroid hormone and hypertension accounted for 88.89%, 85.19%, 29.63%, 96.30% and 81.48% respectively. The inpatients on dialysis therapy accounted for 66.67%. <strong>Conclusion:</strong> The occurrence of falls in hospitalized patients with CKD was not only related to age, caregivers, time, months, locations or other conventional factors, but also the complications and dialysis therapies. To effectively reduce the occurrence of falls, it’s necessary to enhance the sense of safety and responsibility of caregivers. In recent years,the conditions of continuous warm winter,more rain and high humidity are very beneficial to the breeding and survival of sugarcane diseases and pests.Coupled with the degradation of single varieties,more attention on new plants and less on ratoon cane and the unscientific and unreasonable use of chemical pesticides,the types of sugarcane diseases and pests in the Menglian sugarcane area are increasing day by day,and the damage is increasing year by year,resulting in a substantial reduction in production and sugar,and a serious disaster threat to sugarcane production.Based on the field survey and the current sugarcane production practice in Menglian,this paper systematically reviewed the occurrence and damage dynamics and outbreak reasons of catastrophic diseases and pests that seriously influence sugarcane production,and proposed corresponding prevention and control strategies and integrated prevention and control technology according to the occurrence and damage characteristics of catastrophic diseases and pests. 展开更多

It has been well identified that the extreme events significantly impact the system operation. In this work, a comprehensive operating strategy, which coordinates the preventive strategies and system restoration, was proposed. A re-dispatch model that limits the potential loss under extreme events, as well as provides adequate resources for system restoration after the extreme events, was proposed. An optimal power flow based method was established to solve the proposed model. The availabilities of components and the remaining system due to the extreme events were restored by extending the EPRI’s System Restoration Navigator (SRN) with a little modification. Case studies demonstrate the proposed model and methods. 展开更多

Objective:This study aimed to explore more on contributing factors and prevention of overweight and obesity among Pacific islanders.Methods:Using Cochrane library guideline,four electronic databases,PubMed,Embase,Scopus and CINAHL,were systematically searched from 2010 to 2020.Search strategy included key concepts,free text terms,and database-controlled vocabulary terms to identify articles on determinants and preventative strategies of overweight and obesity in the Pacific countries.A data extraction sheet was developed to extract relevant information required for analysis and theme development for this study.Results:A total of 22 articles were summarized and categorized.Determinants of overweight and obesity identified two themes:environmental related factors 13.6%(3/22)and sociocultural related factors 27.3%(6/22).The preventative strategies of overweight and obesity was categorized into behavioural change 22.7%(5/22),school-related 18.2%(4/22),and policies 50%(11/22).The quality assessment of articles showed“Good”68.2%(15/22),and“Fair”31.8%(7/22).Conclusion:This study highlighted two contributing,non-medication factors affecting overweight and obesity are both environmental and socio-cultural issues.Overweight and obesity preventative strategies identified in the Pacific context considered behavioural change,knowledge enhancement,and policy development.Further research should aim at exploring the current preventative strategies in-depth,to appreciate the most feasible and recommended preventative approach within the Pacific society. 展开更多

Introduction: While the prevalence of HIV/AIDS is shown to be decreasing in the last few decades, it still remains a serious public health issue in Pacific countries. This study is aimed to review the common barriers and also preventive strategies for HIV/AIDS among Pacific Islanders. Methodology: This systematic review study on HIV/AIDS was developed based on the Cochrane Library Guideline. Seven online databases which were more frequently used in finding relevant studies were used to search literatures. Using relevant key words, all studies found in the search were reviewed by two independent reviewers and their quality and relevance were assessed. All previous qualitative and quantitative published articles included in the study were published from 2000 to 2016 and in the English language. A data extraction sheet was developed and a descriptive statistic was used to analyze the data. Results: Twenty-two studies met the study inclusion and exclusion criteria. Many of the studies were conducted in American Pacific countries (50%). Community-based studies were the most common type of studies (72.7%). In quantitative studies, the most common barriers were lack of accessibility to health services and lack of knowledge, while in the qualitative studies they were cultural barriers and access to health services. Educational trainings on sexual health topics were identified as the main preventive strategy. Conclusion: This study highlights the main barriers of HIV/AIDS among Pacific Islanders. It also provides a guide for decision makers to develop and implement new strategies which are sensitive and based on Pacific cultures and beliefs. 展开更多
