This study determined if it is possible to use the X-ray system installed in each medical facility to calculate the time constant of a semiconductor survey meter and confirm the meter’s function. An additional filter was attached to the medical X-ray system to satisfy the standards of N-60 to N-120, more copper plates were added as needed, and the first and second half-value layers were calculated to enable comparisons of the facility’s X-ray system quality with the N-60 to N-120 quality values. Next, we used a medical X-ray system to measure the leakage dose and calculate the time constant of the survey meter. The functionality of the meter was then checked and compared with the energy characteristics of the meter. The experimental results showed that it was possible to use a medical X-ray system to reproduce the N-60 to N-120 radiation quality values and to calculate the time constant from the measured results, assuming actual leakage dosimetry for that radiation quality. We also found that the calibration factor was equivalent to that of the energy characteristics of the survey meter.展开更多

It integrates stimulation and recording on 16 pairs of electrodes. Two switches(SAS3 T16/SAS1 T16 X2) fabricated in high-voltage 0.8 μm process with online re-configurable function are proposed. These two switches are combined with commercial off-the-shelf(COTS) electronics to implement the closed-loop implantable system in compact module. The system includes amplifier for recording neural signals, high-voltage stimulator, power transmission device, central processing module and flexible implantable electrodes. Two customized switches route any electrode to amplifier or stimulator, and nerve stimulation and signal recording are performed through lead wire-driven channels. The entire system is able to operate at up to 28 V, and is a biocompatible package with a volume of 42 mm×35 mm×8 mm. This system solves several problems encountered in implantable devices: low flexibility, negative influence of stimulus artifacts on neural detection and low integration of electrodes.展开更多

DATA SOURCE: A computer-based online search of publications was conducted using the Vi...OBJECTIVE: To evaluate feasibility of biofeedback therapy in China Medical Institutions to improve dysfunction following stroke. DATA SOURCE: A computer-based online search of publications was conducted using the Vip and PubMed Databases to identify publications that addressed biofeedback. The search key words included "electromyogram", "biofeedback", and "stroke". In total, 81 articles were retrieved. DATA SELECTION: Studies closely related to biofeedback, or studies with contents recently published in the same study field or in authorized journals, were included. Duplicated articles were excluded. Following full-text retrieval of selected articles, a total of 14 articles were collected, which addressed randomized, controlled trials of biofeedback therapy for dysfunction after stroke. Methodological quality was assessed for randomized, controlled trials using criteria from Cochrane reviewers' handbook. Results were analyzed using Revman 4.2 software. MAIN OUTCOME MEASURES: Outcomes and evaluation indices were expressed by odds ratio (OR), weighted mean difference (WMD), and 95% confidence interval (95% CO. Potential publication bias was presented using the funnel plot. RESULTS: The study included 14 randomized, controlled trials of 1 147 patients. Following biofeedback therapy, meta-analysis results demonstrated that: (1) The total effective rate was significantly greater in the biofeedback therapy group compared with the control group [OR = 3.46, 95% Cl (2.09, 5.73), P = 0.62]. (2) Electromyogram changes were better in biofeedback therapy patients compared to the control group [WMD= 22.31, 95% C/(17.19, 27.43), P〈 0.001]. (3) Motor function was better in biofeedback therapy patients compared with the control group [WMD = 12.43, 95% CI (6.71, 18.16), P 〈 0.001]. (4) Daily living activities were better in biofeedback therapy patients compared with the control group [WMD= 18.11, 95% C/(15.77, 20.44), P= 0.36]. (5) Joint range of motion was better in biofeedback therapy patients compared with the control group [WMD = 6.43, 95% Cl (4.44, 8.41), P = 0.77]. Sensitivity analysis also demonstrated similar results after eliminating articles that described unknown diagnostic criteria and statistical methods. CONCLUSION: Following stroke, biofeedback therapy for dysfunction was shown to result in significant and valid outcomes, increased motor function and electromyogram values, improved joint range of motion, and improved daily living activities.展开更多

Several epidemiological studies have clearly identified diabetes mellitus(DM)as a major risk factor for cognitive dysfunction,and it is going to be a major public health issue in the coming years because of the alarmi...Several epidemiological studies have clearly identified diabetes mellitus(DM)as a major risk factor for cognitive dysfunction,and it is going to be a major public health issue in the coming years because of the alarming rise in diabetes prevalence across the world.Brain and neural tissues predominantly depend on glucose as energy substrate and hence,any alterations in carbohydrate metabolism can directly impact on cerebral functional output including cognition,executive capacity,and memory.DM affects neuronal function and mental capacity in several ways,some of which include hypoperfusion of the brain tissues from cerebrovascular disease,diabetes-related alterations of glucose transporters causing abnormalities in neuronal glucose uptake and metabolism,local hyper-and hypometabolism of brain areas from insulin resistance,and recurrent hypoglycemic episodes inherent to pharmacotherapy of diabetes resulting in neuronal damage.Cognitive decline can further worsen diabetes care as DM is a disease largely self-managed by patients.Therefore,it is crucial to understand the pathobiology of cognitive dysfunction in relation to DM and its management for optimal long-term care plan for patients.A thorough appraisal of normal metabolic characteristics of the brain,how alterations in neural metabolism affects cognition,the diagnostic algorithm for patients with diabetes and dementia,and the management and prognosis of patients when they have this dangerous combination of illnesses is imperative in this context.This evidence-based narrative with the backup of latest clinical trial reviews elaborates the current understanding on diabetes and cognitive function to empower physicians to manage their patients in day-to-day clinical practice.展开更多
BACKGROUND Traditional treatments for major depressive disorder(MDD),including medication and therapy,often fail and have undesirable side effects.Electroconvulsive therapy(ECT)uses electrical currents to induce brief...BACKGROUND Traditional treatments for major depressive disorder(MDD),including medication and therapy,often fail and have undesirable side effects.Electroconvulsive therapy(ECT)uses electrical currents to induce brief seizures in the brain,resulting in rapid and potent antidepressant effects.However,owing to misconceptions and controversies,ECT is not as widely used as it could and often faces stigmatization.AIM To evaluate the efficacy and safety of ECT compared to those of medication and/or therapy in patients with severe MDD.METHODS This prospective cohort study included 220 individuals with severe MDD who were divided into the ECT and non-ECT groups.The patients in the ECT group underwent bilateral ECT three times a wk until they either achieved remission or reached a maximum of 12 sessions.The non-ECT group received medication and/or therapy according to clinical guidelines for MDD.The primary outcome was the variation in the hamilton depression rating scale(HDRS)score from treatment/ECT initiation to week 12.In addition,patients’quality of life,cognitive abilities,and biomarkers were measured throughout the study.RESULTS Although both groups showed significant improvements in their HDRS scores over time,the improvement was more pronounced in the ECT group than in the non-ECT group.Additionally,the ECT group exhibited a more substantial improvement in the quality of life and cognitive function than those of the non-ECT group.Compared with the non-ECT group,the ECT group exhibited evidently lower variations in the brain-derived neurotrophic factor(BDNF)and cytokine interleukin-6(IL-6)levels.The side effects were generally mild and comparable between the two groups.ECT is safer and more potent than medication and/or therapy in mitigating depressive symptoms,enhancing wellbeing,and bolstering cognitive capabilities in individuals with severe MDD.ECT may also affect the levels of BDNF and IL-6,which are indicators of neuroplasticity and inflammation,respectively.CONCLUSION ECT has emerged as a potentially advantageous therapeutic approach for patients with MDD who are unresponsive to alternative treatments.展开更多

However, little attention is paid to evaluate the functional state of a patient in sleep. The efficiency of prescribed medicines is also determined in wakefulness. The effects of these drugs during sleep are usually out-of-sight of a physician. Human psychoemotional state during wakefulness is closely related to sleep. Various serious disturbances in physiological functions appear during the night sleep. They include cardiovascular disorders (hypertensive crisis, myocardial infarction, arrhythmias, stroke, etc.), which may cause sudden death. A large body of evidence shows that stress is followed by sleep disorders. They serve as a factor, which contributes to the development of stress. It should be noted that sleep is the major anti-stress factor. The vicious circle is formed, which can be revealed by a complex control of human physiological functions during wakefulness and sleep. We developed a new methodology and constructed an information-and-equipment device for monitoring of vital physiological functions of humans during sleep in a real everyday life. This approach holds much promise for the diagnosis of disease and evaluation of therapeutic efficacy.展开更多

The simultaneous presence of positive and negative charges renders the zwitterion-based materials with electrostatically induced hydration properties, which enables a high resistance towards oily pollutants, nonspecific protein adsorption, bacterial adhesion and biofilm formation. This review starts from the working mechanism of zwitterions and covers the fabrication strategies of zwitterion-based functional coatings, namely the zwitterion-bearing binder route, the zwitterion-bearing additive route and the post-generation of coatings containing zwitterionic precursors. The applications of zwitterion-based functional coatings are discussed, including medical implants, marine antifouling and oil-resistant coatings, with focus on the relevant mechanisms of the zwitterion-containing coatings for a specific performance. Finally, some comments and perspectives on the current situation and future development of zwitterion-based functional coatings are given.展开更多
BACKGROUND Children with attention-deficit/hyperactivity disorder(ADHD)often exhibit behaviour challenges and deficits in executive functions(EF).Psychostimulant medications[e.g.,methylphenidate(MPH)]are commonly pres...BACKGROUND Children with attention-deficit/hyperactivity disorder(ADHD)often exhibit behaviour challenges and deficits in executive functions(EF).Psychostimulant medications[e.g.,methylphenidate(MPH)]are commonly prescribed for children with ADHD and are considered effective in 70%of the cases.Furthermore,only a handful of studies have investigated the long-term impact of MPH medication on EF and behaviour.AIM To evaluate behaviour and EF challenges in children with ADHD who were involved in an MPH treatment trial across three-time points.METHODS Thirty-seven children with ADHD completed a stimulant medication trial to study the short-and long-term impact of medication.Children with ADHD completed three neuropsychological assessments[Continuous Performance Test(CPT)-II,Digit Span Backwards and Spatial Span Backwards].Parents of children with ADHD completed behaviour rating scales[Behaviour Rating Inventory of Executive Functioning(BRIEF)and Behaviour Assessment System for Children-Second Edition(BASC-2)].Participants were evaluated at:(1)Baseline(no medication);and(2)Best-dose(BD;following four-week MPH treatment).Additionally,18 participants returned for a long-term naturalistic follow up(FU;up to two years following BD).RESULTS Repeated measure analyses of variance found significant effects of time on two subscales of BRIEF and four subscales of BASC-2.Neuropsychological assessments showed some improvement,but not on all tasks following the medication trial.These improvements did not sustain at FU,with increases in EF and behaviour challenges,and a decline in performance on the CPT-II task being observed.CONCLUSION Parents of children with ADHD reported improvements in EF and behaviours during the MPH trial but were not sustained at FU.Combining screening tools and neuropsychological assessments may be useful for monitoring medication responses.展开更多

A base box, placement drawer, platform board and display are used as the base module and the base module is filled with specific functional components. Lifting and lowering using motors and spiral base, moving using universal wheels. These devices together constitute the training platform. A survey of students and teachers was conducted through a questionnaire, and they all gave very good feedback that the multi-functional platform was very practical and useful. This platform effectively solves the drawbacks of the original training platform which is time-consuming, laborious and inconvenient, and is worthy of further promotion and research.展开更多

文摘Medical devices are a major component of precision medicine and play a key role in medical treatment,particularly with the rapid development of minimally invasive surgery and wearable devices.Their tissue contact properties strongly affect device performance and patient health(e.g.,heat coagulation and slipperiness on surgical graspers).However,the design and optimization of these device surfaces are still indistinct and have no supporting principles.Under such conditions,natural surfaces with various unique functions can provide solutions.This review summarizes the current progress in natural functional surfaces for medical devices,including ultra-slipperiness and strong wet attachment.The underlying mechanisms of these surfaces are attributed to their coupling effects and featured micronano structures.Depending on various medical requirements,adaptable designs and fabrication methods have been developed.Additionally,various medical device surfaces have been validated to achieve enhanced contact properties.Based on these studies,a more promising future for medical devices can be achieved for enhanced precision medicine and human health.

文摘The quantum vibrational energy-spectra including high excited states of the protein molecules have been calculated by new theory of bio-energy transport along the protein molecules and its dynamic equation, discrete nonlinear Schrodinger equation, appropriate to the protein molecules on the basis of the level of molecular structure. This energy-spectra obtained are basically consistent with the experimental values by infrared absorption and radiated measurement of person’s hands and laser-Raman spectrum from metabolically active E.Coli.. From this energy-spectra we know that the infrared lights with (1-3)x1000nm and (5-7)x1000nm wavelength can be absorbed by the protein molecules in the living systems. In accordance with the non-linear theory of the bio-energy transport we know that the energy of the infrared light absorbed by the proteins can result in vibrations of amide-I in amino acids and can facilitate the bio-energy transport along the protein molecular chains from one place to other for the growth of living bodies. This processe is non-thermal. This is just non-thermal effect of the infrared lights. According to the mechanism we explained further the medical functions of the infrared lights absorbed.

Pat.,Phellinus igniarius(L.ex Fr.) in mulberry tree,Phellinus pini,Fomitopsis rosea Karst,Pyropolyporus adamantinus,Fomes fomentarius,Ganoderma mastoporum Pat,Inonotus obliquus,Fuscoporia punctate,Ganoderma tsugae,Trametes versicolor,Trametes orientalis Imaz,Laetiporus sulphureus,Ganoderma capense(Lloyd) Teng,Hericium erinaceus,Lycoperdon bovista,Polyporus umbellatus Fries,Wolfiporia extensa and Cordyceps barnesii Thwaites.In this paper,the main functions of the major fungi species in eastern forest region of Zhongtiao Mountain in Shanxi Province were introduced.

文摘Several epidemiological studies have clearly identified diabetes mellitus(DM)as a major risk factor for cognitive dysfunction,and it is going to be a major public health issue in the coming years because of the alarming rise in diabetes prevalence across the world.Brain and neural tissues predominantly depend on glucose as energy substrate and hence,any alterations in carbohydrate metabolism can directly impact on cerebral functional output including cognition,executive capacity,and memory.DM affects neuronal function and mental capacity in several ways,some of which include hypoperfusion of the brain tissues from cerebrovascular disease,diabetes-related alterations of glucose transporters causing abnormalities in neuronal glucose uptake and metabolism,local hyper-and hypometabolism of brain areas from insulin resistance,and recurrent hypoglycemic episodes inherent to pharmacotherapy of diabetes resulting in neuronal damage.Cognitive decline can further worsen diabetes care as DM is a disease largely self-managed by patients.Therefore,it is crucial to understand the pathobiology of cognitive dysfunction in relation to DM and its management for optimal long-term care plan for patients.A thorough appraisal of normal metabolic characteristics of the brain,how alterations in neural metabolism affects cognition,the diagnostic algorithm for patients with diabetes and dementia,and the management and prognosis of patients when they have this dangerous combination of illnesses is imperative in this context.This evidence-based narrative with the backup of latest clinical trial reviews elaborates the current understanding on diabetes and cognitive function to empower physicians to manage their patients in day-to-day clinical practice.

文摘BACKGROUND Traditional treatments for major depressive disorder(MDD),including medication and therapy,often fail and have undesirable side effects.Electroconvulsive therapy(ECT)uses electrical currents to induce brief seizures in the brain,resulting in rapid and potent antidepressant effects.However,owing to misconceptions and controversies,ECT is not as widely used as it could and often faces stigmatization.AIM To evaluate the efficacy and safety of ECT compared to those of medication and/or therapy in patients with severe MDD.METHODS This prospective cohort study included 220 individuals with severe MDD who were divided into the ECT and non-ECT groups.The patients in the ECT group underwent bilateral ECT three times a wk until they either achieved remission or reached a maximum of 12 sessions.The non-ECT group received medication and/or therapy according to clinical guidelines for MDD.The primary outcome was the variation in the hamilton depression rating scale(HDRS)score from treatment/ECT initiation to week 12.In addition,patients’quality of life,cognitive abilities,and biomarkers were measured throughout the study.RESULTS Although both groups showed significant improvements in their HDRS scores over time,the improvement was more pronounced in the ECT group than in the non-ECT group.Additionally,the ECT group exhibited a more substantial improvement in the quality of life and cognitive function than those of the non-ECT group.Compared with the non-ECT group,the ECT group exhibited evidently lower variations in the brain-derived neurotrophic factor(BDNF)and cytokine interleukin-6(IL-6)levels.The side effects were generally mild and comparable between the two groups.ECT is safer and more potent than medication and/or therapy in mitigating depressive symptoms,enhancing wellbeing,and bolstering cognitive capabilities in individuals with severe MDD.ECT may also affect the levels of BDNF and IL-6,which are indicators of neuroplasticity and inflammation,respectively.CONCLUSION ECT has emerged as a potentially advantageous therapeutic approach for patients with MDD who are unresponsive to alternative treatments.
文摘The diagnosis of disease and prescription of therapy are mainly realized through the control of physiological functions in humans during wakefulness. However, little attention is paid to evaluate the functional state of a patient in sleep. The efficiency of prescribed medicines is also determined in wakefulness. The effects of these drugs during sleep are usually out-of-sight of a physician. Human psychoemotional state during wakefulness is closely related to sleep. Various serious disturbances in physiological functions appear during the night sleep. They include cardiovascular disorders (hypertensive crisis, myocardial infarction, arrhythmias, stroke, etc.), which may cause sudden death. A large body of evidence shows that stress is followed by sleep disorders. They serve as a factor, which contributes to the development of stress. It should be noted that sleep is the major anti-stress factor. The vicious circle is formed, which can be revealed by a complex control of human physiological functions during wakefulness and sleep. We developed a new methodology and constructed an information-and-equipment device for monitoring of vital physiological functions of humans during sleep in a real everyday life. This approach holds much promise for the diagnosis of disease and evaluation of therapeutic efficacy.
文摘Objective To present a novel modified level set algorithm for medical image segmentation. Methods The algorithm is developed by substituting the speed function of level set algorithm with the region and gradient information of the image instead of the conventional gradient information. This new algorithm has been tested by a series of different modality medical images. Results We present various examples and also evaluate and compare the performance of our method with the classical level set method on weak boundaries and noisy images. Conclusion Experimental results show the proposed algorithm is effective and robust.

文摘Computer-aided diagnosis based on image color rendering promotes medical image analysis and doctor-patient communication by highlighting important information of medical diagnosis.To overcome the limitations of the color rendering method based on deep learning,such as poor model stability,poor rendering quality,fuzzy boundaries and crossed color boundaries,we propose a novel hinge-cross-entropy generative adversarial network(HCEGAN).The self-attention mechanism was added and improved to focus on the important information of the image.And the hinge-cross-entropy loss function was used to stabilize the training process of GAN models.In this study,we implement the HCEGAN model for image color rendering based on DIV2K and COCO datasets,and evaluate the results using SSIM and PSNR.The experimental results show that the proposed HCEGAN automatically re-renders images,significantly improves the quality of color rendering and greatly improves the stability of prior GAN models.

文摘Zwitterion-based materials by virtue of their special physical and chemical characteristics have attracted researchers to utilize them for fabricating functional coatings. The simultaneous presence of positive and negative charges renders the zwitterion-based materials with electrostatically induced hydration properties, which enables a high resistance towards oily pollutants, nonspecific protein adsorption, bacterial adhesion and biofilm formation. This review starts from the working mechanism of zwitterions and covers the fabrication strategies of zwitterion-based functional coatings, namely the zwitterion-bearing binder route, the zwitterion-bearing additive route and the post-generation of coatings containing zwitterionic precursors. The applications of zwitterion-based functional coatings are discussed, including medical implants, marine antifouling and oil-resistant coatings, with focus on the relevant mechanisms of the zwitterion-containing coatings for a specific performance. Finally, some comments and perspectives on the current situation and future development of zwitterion-based functional coatings are given.

文摘BACKGROUND Children with attention-deficit/hyperactivity disorder(ADHD)often exhibit behaviour challenges and deficits in executive functions(EF).Psychostimulant medications[e.g.,methylphenidate(MPH)]are commonly prescribed for children with ADHD and are considered effective in 70%of the cases.Furthermore,only a handful of studies have investigated the long-term impact of MPH medication on EF and behaviour.AIM To evaluate behaviour and EF challenges in children with ADHD who were involved in an MPH treatment trial across three-time points.METHODS Thirty-seven children with ADHD completed a stimulant medication trial to study the short-and long-term impact of medication.Children with ADHD completed three neuropsychological assessments[Continuous Performance Test(CPT)-II,Digit Span Backwards and Spatial Span Backwards].Parents of children with ADHD completed behaviour rating scales[Behaviour Rating Inventory of Executive Functioning(BRIEF)and Behaviour Assessment System for Children-Second Edition(BASC-2)].Participants were evaluated at:(1)Baseline(no medication);and(2)Best-dose(BD;following four-week MPH treatment).Additionally,18 participants returned for a long-term naturalistic follow up(FU;up to two years following BD).RESULTS Repeated measure analyses of variance found significant effects of time on two subscales of BRIEF and four subscales of BASC-2.Neuropsychological assessments showed some improvement,but not on all tasks following the medication trial.These improvements did not sustain at FU,with increases in EF and behaviour challenges,and a decline in performance on the CPT-II task being observed.CONCLUSION Parents of children with ADHD reported improvements in EF and behaviours during the MPH trial but were not sustained at FU.Combining screening tools and neuropsychological assessments may be useful for monitoring medication responses.

With the improvement of people’s medical awareness, people cannot meet the simple surgical treatment, and there is a great demand for nursing treatment. At the same time, the demand for efficiency and convenience of medical nursing practice teaching continues to improve, and the multi-functional medical nursing training innovation platform has been paid more and more attention. The rapid development of material technology as well as digitalization has brought about a huge change, and we have created a multifunctional, spatially efficient and easy-to-transfer information platform for medical care training. A base box, placement drawer, platform board and display are used as the base module and the base module is filled with specific functional components. Lifting and lowering using motors and spiral base, moving using universal wheels. These devices together constitute the training platform. A survey of students and teachers was conducted through a questionnaire, and they all gave very good feedback that the multi-functional platform was very practical and useful. This platform effectively solves the drawbacks of the original training platform which is time-consuming, laborious and inconvenient, and is worthy of further promotion and research.
