Fu's subcutaneous needling for orthostatic hypotension due to Guillain-Barré syndrome: A case report

A 28-year-old female developed symptoms of frequent dizziness and occasional syncope while standing up,accompanied by paresthesia in her extremities and occasional constipation.She was diagnosed with Guillain-Barré syndrome(GBS).Her upright blood pressure was undetectable,and cerebrospinal fluid analysis showed protein-leukocyte separation.A nerve conduction study showed that the sensory nerves of her lower limbs had been injured,but the motor nerves were almost normal.She was treated with immunotherapy and hormonotherapy,which partially relieved her paresthesia and constipation.However,her orthostatic hypotension(OH) persisted after two months of treatment.After three courses of treatment consisting of Fu’s subcutaneous needling,OH significantly improved,with a much less pronounced decrease(<20 mmHg) in systolic blood pressure in the upright position than observed be fore treatment.She was sympto m-free and able to stand and walk for short distances after 3 months of follow-up.This case indicates that Fu’s subcutaneous needling treatment is effective for OH caused by GBS,and it may be a suitable alternative when conventional treatment does not produce ideal results. 展开更多

Subcutaneous needling is a new therapeutic method which is based on the features of the meridian-collateral theory of cutaneous regions in traditional Chinese medicine and the neurohumeral theory and skin anatomy of modern medicine and is of the features of “fixed meridian but not fixed acupoints”, “micro stimulation”, and “subcutaneous needling with filiform needles” to achieve the goal of treating diseases. This method is indicated clinically to the analgesia and relaxation of spasm especially the pain caused by the disorders of the internal organs and marked therapeutic effect has been obtained. With method, 150 cases of epigastric pain were treated by our department and the total effective rate was over 90%. It is presented herein below. 展开更多

Objective:Fu's subcutaneous needling(FSN)has been shown potential positive effectiveness in clinical practice.However,its specific effects are still not clear.This study aims to evaluate the blinding effect of a non-penetrating sham FSN needle,developed for the first time by the authors'research team in healthy participants.Furthermore,to provide a reliable control tool to assess the specific effects of FSN.Methods:We conducted a randomized,sham-controlled trial in 60 healthy participants divided into FSN group and sham FSN group in a 1:1 ratio.The participants were assessed according to blinding index(BI),pain intensity,sensation type and acceptability of needling.Results:The James'BI was(0.56,95%CI[0.45,0.67])for two groups.The Bang's BI was(0.50,95%CI[0.24,0.76])in the FSN group and(-0.36,95%CI[-0.67,-0.07])in the sham FSN group.There was no significant difference between needling pain intensity(P>0.05)and type of needling sensation.A total of 96.6%participants who received sham FSN and 86.6%participants who received FSN considered the needling acceptable or better.Conclusion:This study confirmed the effective blinding capability of the sham FSN needles in healthy participants,indicating that the novel non-penetrating sham FSN needle can be served as eligible control to evaluatethe specific effects of FSN. 展开更多

Objective To observe the efficacy of combining therapy in treatment of 90 patients with scapulohumeral periarthritis in different phases. Method Ninety patients with scapulohumeral periarthritis were treated with Fu's subcutaneous needling(FSN) combined with physiotherapy in painful phase, acupotomology and massage in adhesion phase, and appropriate functional exercise in rehabilitation phase. Seven days were considered as one course of treatment, and efficacy evaluation was performed after three courses. Function evaluation of shoulder joint was made. Result The total effective rate was 100%(38/38) in acute phase, 93.5%(29/31) in adhesion phase, and 100%(21/21) in rehabilitation phase. After treatment, the range of motion(ROM) of shoulder joint, pain, muscle force, activities of daily living(ADL) and joint local morphology of the patient have been improved obviously. Conclusion Better clinical efficacy is achieved in treatment of scapulohumeral periarthritis in different phases by combining therapy centering on FSN. 展开更多

Objective: To observe the therapeutic efficacy of Fu's subcutaneous needling(FSN) in treating scapulohumeral periarthritis(SP).Methods: Eighty patients with SP were randomly divided into an observation group and a control group, 40 cases in each group. Patients in the observation group received FSN treatment while those in the control group received conventional acupuncture treatment. Visual analogue scale(VAS) and range of motion(ROM) were observed before and after the treatment, and the therapeutic efficacy was also evaluated.Results: After the treatment, the VAS scores decreased significantly in the two groups, and the VAS score in the observation group was significantly lower than that in the control group(P〈0.01). After the treatment, the ROM increased significantly in both groups(both P〈0.01), and the ROM in the observation group was significantly higher than that in the control group(P〈0.01). The recovery rate and the total effective rate in the observation group were respectively 40.0% and 95.0%,significantly higher than 12.5% and 77.5% in the control group(P〈0.01, P〈0.05).Conclusion: FSN can effectively relieve pain and improve the joint motion in treating SP, thus offering a novel option to the treatment of this kind of musculoskeletal disorder. 展开更多

The choosing of its insertion points doesn't obey the principles of traditional Chinese medicine. Objective:To observe and compare the therapeutic effect on shoulder periarthritis between Fu’s subcutaneous needling(FSN)and electroacupuncture(EA).Methods:A total of 68 patients with periarthritis of shoulder were divided into a FSN group and an EA group according to the random number table,34 cases in each one.In the FSN group,FSN was adopted,once every two days,totally for 10 times.In the EA group,EA was used once daily,consecutively for 20 times.Shoulder pain score and shoulder function score(Constant-Murley score)before and after treatment were compared between the two groups.The clinical therapeutic effect was analyzed and evaluated statistically.Results:After 20-day treatment,shoulder pain score was reduced obviously as compared with that before treatment in the patients of either group(both P<0.01).After treatment,shoulder pain score in the FSN group was lower significantly than that in the EA group(P<0.01).In 20 days of treatment,CMS was improved obviously as compared with that before treatment in the patients of both groups(both P<0.01)and CMS in the FSN group was higher significantly than that in the EA group after treatment(P<0.01).The effective rate was 94.11%in the FSN group and was 91.17%in the EA group.The remarkably effective rate was 73.52%in the FSN group,significantly higher than 20.58%in the EA group(P<0.01).Conclusion:Both FSN and EA are effective in treatment of periarthritis of shoulder,but FSN obtains a much higher remarkably effective rate in treatment of periarthritis of shoulder and its effects for relieving shoulder pain and improving shoulder function are superior to EA.Because of its effective treatment and simple operation,FSN deserves to be promoted in clinical practice for periarthritis of shoulder. 展开更多

Objective: To observe the therapeutic efficacy of Fu's subcutaneous needling(FSN) with laser for postherpetic neuralgia and analyze its action mechanism. Methods: A total of 102 cases with postherpetic neuralgia were randomly allocated into a treatment group and a control group, 51 in each group. FSN with laser was used for patients in the treatment group, whereas electroacupuncture was used for patients in the control group. After 6 months of treatment, the total effective rates, action time and cure courses in recovery cases were compared between the two groups. Three months after the treatment was completed, a follow-up was done to compare relapse rates between the two groups. Results: The recovery and total effective rates in the treatment group were 56.9% and 94.1% respectively, versus 35.3% and 68.6% in the control group, showing statistical differences(P<0.05); the action time and cure courses in the observation group were significantly shorter than those in the control group(P<0.05); the relapse rate in the treatment group was 6.9%, versus 44.4% in the control group, showing a statistical difference(P<0.05). Conclusion: FSN with laser can obtain better effect for postherpetic neuralgia than electroacupuncture and pose lower relapse risks. 展开更多

One case of acute Bell’s palsy was reported and treated with Fu’s subcutaneous needling(FSN).Present symptoms:facial muscle stiffness on the right side,ptosis of labial angle,failure to frown the right eyebrow,the eyeball exposed about 2 mm when gently closing the eyes,air leaking when bulging the cheeks,drooling when brushing the teeth and gargling,and food retained in the cheek when eating.Western medicine diagnosis:peripheral facial palsy.Traditional Chinese medicine diagnosis:facial paralysis.The subcutaneous swaying movement and reperfusion action of FSN were adopted on the affected muscles and achieved the remarkably effects on incomplete closure of eyes and drooping of the mouth.The patient was recovered quickly.Before and after treatment,the laser speckle contrast imaging was used to observe the improvement of blood flow on the face,which provides the idea for the research and clinical practice of Fu’s subcutaneous needling in treatment of Bell’s palsy. 展开更多

In this paper, the authors studied the manipulation of "dry needling", reviewed Chinese ancient and modern literature of acupuncture-moxibustion, and found that "dry needling" is very similar to triple needling, superficial needling, joint valley needling, surround needling, and other traditional Chinese acupuncture- moxibustion manipulations; even if in modern China, before and after the invention of "dry needling", some unique therapies, such as oblique insertion therapy at Ashi point, sliver needling, Fu's subcutaneous needling, and long-round needling, were also invented. It can be seen that "dry needling" therapy is actually a kind of differentiation of traditional Chinese acupuncture- moxibustion in the international communication. The spread of traditional Chinese acupuncture-moxibustion in the West is the basis for the invention of "dry needling" therapy, and "dry needling" therapy develops and enriches traditional Chinese acupuncture-moxibustion. 展开更多

As one of the most important concepts in traditional Chinese medicine, cutaneous regions have a close relationship with skin in modern medicine. As one of the most important concepts in traditional Chinese medicine, cutaneous regions have a close relationship with skin in modern medicine. According to the doctrine of traditional Chinese medicine, the cutaneous regions are the corresponding projections of twelve meridians in the superficial layer of the body and play an important role in body surface-viscera correlation. The symptoms of visceral disorders will reflect on cutaneous regions and one of the major patterns of manifestation is referred visceral pain accompanied by tenderness and hyperalgesia on the body surface. On the contrary, cutaneous regions participate in the treatment of visceral disorders. As major clinical applications of the cutaneous regions hypothesis, superficial acupuncture, subcutaneous needling, massage(tuina), moxibustion, and wrist-ankle acupuncture have achieved favorable therapeutic outcomes in clinical practice. Modern researches have gained insights into the biological background of cutaneous regions, however, further studies are needed to clarify the mechanisms. 展开更多
