Validity and reliability of the evaluation tool of TCM confidence in students with TCM learning experience

Objective:This study aimed to design an evaluation scale of traditional Chinese medicine(TCM)confidence and then evaluate its validity and reliability.Methods:Three parallel samples with TCM learning experience were included in this current study.Randomly selected 500 nursing students were included factor analysis and reliability and validity analyses.After 4 weeks,200 nursing students were recruited to retest the reliability.The evaluation tool of TCM confidence included 6 dimensions and 25 entries.Results:Cronbach's a coefficient of the evaluation scale was 0.95 and that of the 6 observed aspects were higher than 0.90,and splithalf reliability was 0.81.For the retest reliability,Cronbach's a coefficient of the evaluation scale was 0.93 and that of the 6 examined aspects was all over 0.80.The index of content validity Scale Content Validity Index(S-CVI)was 0.92 and Item Content Validity Index(I-CVI)was 0.83–1.00.Conclusions:Observed results may indicate that the designed tool can be used to assess the TCM confidence level of students with TCM studied experience and also can evaluate results of TCM education during studying TCM lectures.

In this article,we summarize the clinical experience ofProfessor Ruixia Pei,a famous traditional Chinese medicine practitioner in Shaanxi Province,China,in treating hyperthyroidism.The etiology and pathogenesis,syndrome differentiation,and medication experience are introduced in detail.This paper summarizes the advantages of Professor Pei's methods of syndrome differentiation and treatment of hyperthyroidism.This may help enrich the clinical treatment of hyperthyroidism,and provide some diagnosis and treatment ideas.

王静波临床经验总结

王静波教授从事中医眼科工作40余年,积累了丰富的临床经验。特别是在中晚期青光眼的治疗方面,王静波教授研习古籍,结合自己的临床经验,认为中晚期青光眼及术后视神经萎缩、小视野者,主要是因为阴不涵阳,肝阳失于制约,肝阳上亢,阳热耗气伤阴,终至气阴两亏;气阴亏损目之窍道无力以通,无物以养而视物不清。制定了益气养阴开窍为治则治疗中晚期青光眼,并创制了益气养阴、开窍明目的益阴明目合剂。为中晚期青光眼的治疗提供了新的思路。同时在春季卡他性结膜炎、麦粒肿等疾病方面也有独特的治疗方法。治疗春季卡他性结膜炎时,王静波老师治疗本病从整体出发,通过辨证论治调理体质,联合中药外熏洗及内服、药渣外敷等多种中医疗法,既能固其本,又能治其标,内外结合、标本兼治,缩短病程,减少复发。王静波教授还根据多年临床经验,总结出经验方内服结合药渣外敷并配合耳尖放血治疗麦粒肿,内外治结合收到显著疗效。该文对以上进行总结,旨在继承名老中医经验,促进中医眼科的学术发展。

Summary on Professor QIAO Bao-zhang's Experience in Syndrome Differentiation and Treatment for Pancreatic Cancer

Objective:To summarize clinical experience of Professor QIAO Bao-zhang in treatment for pancreatic cancer,and to provide reference for clinicians.Methods:Through learning from Professor QIAO during his clinical practice and listening to his lectures,Professor QIAO Bao-zhang's clinically proved cases of treating pancreatic cancer based on syndrome differentiation were organized and summarized.From aspects like etiology,pathogenesis,treatment principles and treatment methods of pancreatic cancer,Professor QIAO's experience in diagnosis and treatment for this disease was analyzed and explored,and characteristics of medication were summarized.Two typical cases were selected as proof,and his treatment ideas and methods were explored.Results:Professor QIAO believes that the pathogenesis is mostly deficiency of vital Qi(正气),and stagnation of phlegm,heat and dampness in Zang-Fu(脏腑)and meridians.Around intermingled deficiency and excess,syndrome differentiation and treatment were performed.Due to treatment methods such as strengthening vital Qi to eliminate pathogenic factor,clearing heat,eliminating dampness and resolving phlegm,as well as removing toxicity and resolving hard mass,progress of malignant tumors was inhibited and recurrence and metastasis of malignant tumors were delayed.Conclusion:Professor QIAO's experience in treating pancreatic cancer is worth learning.

Xiaohong Cheng’s Clinical Experience in Treating Membranous Nephropathy

Membranous nephropathy is the most common pathological type of nephrotic syndrome in adult patients.In recent years,the incidence of membranous nephropathy has been increasing year by year.The pathogenesis of membranous nephropathy has not been completely clear yet.Professor Xiaohong Cheng has accumulated a lot of clinical experience in the treatment of membranous nephropathy,and believes that the main pathogenesis of membranous nephropathy is“spleen and kidney deficiency,kidney blockage.”The main treatment method is“invigorating spleen and kidney,promoting blood circulation and clearing collaterals,”which has achieved remarkable clinical efficacy.

Summary of Chief Physician Hao Mingxia's Experience in "Treating Liver Disease in Four Chinese Medicinal Formulae"

This paper summarizes Hao Mingxia's experience in treating chronic hepatitis B with traditional Chinese medicine.Director Hao discussed the treatment from the dynamic changes of damp-heat,toxin,deficiency and blood stasis,which mainly involved liver,spleen and kidney.Chronic hepatitis B is a chronic wasting disease,which always belongs to the combination of deficiency and excess,asthenic healthy qi with pathogen lingering,and pathogenic qi does not go for the progress of the disease.The four directions of this paper are based on supporting healthy qi,combining attack with tonic,so as to attack evil-qi without hurting positive.It not only follows the ancient method of TCM for syndrome differentiation and treatment,but also combines modern pharmacological effects to clarify the direction of disease.There are more accurate and comprehensive coverage of disease symptoms,so as to achieve the effect of systemic conditioning.Director Hao is good at comprehensive treatment,according to the physiological characteristics of viscera and body pathology,unified qi,blood and yin-yang of whole body,medication to often reach,knowing constantly and achieving change,the effect is immediate.

长期模拟失重实验中医证候诊断量表的研究

目的制订适用于长期模拟失重实验受试者整体健康状态评价的中医证候诊断量表,并对其信度与效度进行初步评价。方法本研究基于对长期航天飞行不同阶段中的人体中医病机的认识,通过文献调研结合专家咨询的方法制订量表。采用重测信度和内部一致性系数来检验量表信度;进行为期90 d的头低位卧床实验,采用项目分析和因子分析法评价量表的效度,采用旋转成分矩阵分析各条目与维度间的相关性。结果长期模拟失重实验中医证候诊断量表包含8个维度、57个条目。信度分析结果:30名受试者前后两次量表评分总分相关系数为0.889,表明量表的稳定性很高;量表的克龙巴赫α系数为0.934,表明量表内部一致性非常好;分半信度校正后为0.858,表明量表的信度很高。效度分析结果:57个条目在高分组和低分组的差异均有统计学意义(P<0.01),各条目与量表总分的皮尔逊相关系数均>0.4(P<0.001);采用主成分因子分析法提取8个公因子,累积贡献率为55.293%;旋转成分矩阵分析结果经医理分析和专家反馈显示,与初始8个维度的重合度高达87.5%,表明量表的结构效度良好。结论量表具有良好的信度和效度,能够进行中医证候诊断,适用于长期模拟失重实验,可为受试者主观感受的评价提供客观量化新方法。

DRG改革前后南通市某三级综合医院中医药发展实践对比分析

[目的]总结DRG改革前后南通市某三级综合医院中医药发展实践经验,并提出相应对策,为解决中医药发展面临的困境提供参考。[方法]以南通市某三级综合医院为例,分析该院DRG改革前后中医科病房中医药费用、病房中医外治法费用、门诊人次及收入、与其它科室中医费用对比、中医优势病种情况。[结果]DRG改革后,中医科住院的中草药费用、中医外治法费用、医疗总费用和中医费用占比均先下降再升高,第二年升高率为14.45%;人均中草药费用、人均外治法费用、人均总费用及中医费用占比基本保持稳定,差异无统计学意义;针刺、穴位贴敷、刮痧、中药熏药费用降低,差异均有统计学意义(P<0.05);火罐费用明显升高,差异有统计学意义(P<0.001);外治法总费用保持稳定,差异无统计学意义。中医科门诊的月门诊人次、月门诊中草药收入、月门诊外治法收入、月门诊总费用均升高,差异均有统计学意义(P<0.01)。中西医结合肝病科人均住院中医费用及人均中医费用占比均较改革前升高,差异均有统计学意义(P<0.05);中医优势病种为短暂性大脑缺血性发作、肺部感染和恶性肿瘤中医治疗等。[结论]DRG支付背景下综合医院中医药发展仍存在种种困境,未来需探索一系列相关对策以促进中医药发展。

以提升患者就医体验为导向的中医药特色夜门诊实践探索

满足不同就医人群的需求,提供夜间诊疗服务,是改善患者就医体验的重要途径之一。夜门诊的开设有利于医院科学合理调整门诊空间,整合有限的医疗资源。为了提升患者就医体验,保障患者医疗安全,夜门诊开设需要优化医护技等人员的合理配置,加强医院多部门间的配合联动。浙江省中医院通过前期调研,结合中医医院特色、特长,开设具有中医药特色的夜门诊后,门诊运行效率进一步提高,医疗服务质量得到持续改进,得到患者的一致好评。

国医大师张磊从脾胃论治耳鸣经验探析

国医大师张磊教授善从脾胃论治耳鸣,认为脾胃功能失调,中焦升降失司,气血生化乏源,升清降浊无力,上窍无以濡养是其主要病机。此病源头为脾胃虚弱失养,痰湿化热困脾,临证时将其证候分为脾气虚弱、痰湿中阻、脾虚肝郁等证型。治疗从脾胃着手,调畅中土,以后天之精濡养全身脏腑,使正气充盈清窍,邪不侵犯人体,而耳鸣自愈。

医疗机构基于“三结合”中药新药研发及管理体系的构建

目的:在中药审评审批制度改革的政策背景下,围绕医疗机构,探索中医药理论、人用经验和临床试验相结合(简称“三结合”)的审评证据体系落地举措,为医疗机构中药制剂及中药新药研发提供参考。方法:根据“三结合”审评证据体系下中药新药研发的相关要求,从机构层面加强人用经验研究相关配套建设,从管理层面搭建中药研发、成果转化服务平台,从临床层面规范开展人用经验研究,构建医疗机构中药新药研发及管理体系。结果:医疗机构中药新药研发及管理体系的构建,有助于将临床诊疗数据转变为支持中药新药注册的人用经验证据,通过提高中药研发水平促进中药新药转化。结论:在医疗机构构建基于“三结合”的中药新药研发及管理体系,是“三结合”审评证据体系落地、政策引导研发实践的路径之一。

基于数据挖掘王晓燕教授治疗儿童抽动障碍脾虚肝亢证用药规律

目的运用数据挖掘技术分析并总结王晓燕教授治疗儿童抽动障碍脾虚肝亢证的用药经验。方法收集王教授门诊治疗的脾虚肝亢证抽动障碍患儿,运用中医传承辅助平台(V 2.5)建立数据库,对数据库中处方予频次分析、关联规则分析和复杂系统熵聚分析。结果共收集122首方,涉及中药64味,用药频次前11位分别为麸炒白术、麸炒苍术、炙甘草、炒白芍、蝉蜕、柴胡、陈皮、炒莱菔子、炒酸枣仁、炒僵蚕、珍珠母。药物四气以温寒药居多,五味以苦辛甘为主,归经以肝脾肺居多。药物组合频次超过115次的共30组。得到潜在核心组合6组,潜在新方3首。结论王教授治疗儿童抽动障碍脾虚肝亢证用药以寒温并举,辛苦兼施为主,主张肝脾同调,土木并举,选用自拟方柴术调肝理脾汤为基础加减治疗。
