A Conversation with Professor Ted Kaptchuk,a Key Architect of Modern TCM in the West

The interview was conducted on November 20th,2022 by Ioannis Solos and Charles Buck.Ted Kaptchuk(泰开阳)is a Professor of Medicine and a Professor of Global Health&Social Medicine at Harvard Medical School.In 1975,he graduated from the Macao Institute of Chinese Medicine,Macao,China.After returning to the West,Ted taught Chinese medicine throughout the States,Europe,Latin America,and Australia.Ted’s book,The Web that Has No Weaver:Understanding Chinese Medicine is a groundbreaking explanation of East Asian medicine that continues to shape the discourse of the Chinese medicine profession in the West and has been translated into 13 languages.In 1990,Ted was recruited in Harvard Medical School to help research Chinese medicine.After encountering high placebo responses in multiple acupuncture clinical trials,Ted decided to switch his career to primarily studying placebo.He has published over 300 peer-reviewed articles.He has been awarded the Lifetime Achievement Award from the Society of Acupuncture Research(2015),the Lifetime Achievement Award from the Society for Interdisciplinary Placebo Studies(2021),and the William Silen Lifetime Achievement Award in Mentoring from Harvard Medical School(2022).

基于TCM模式的护理干预对肺癌手术患者情绪、肺功能及生活质量的影响

目的 探讨基于TCM模式的护理干预对肺癌手术患者情绪、肺功能及生活质量的影响。方法 选择2020年2月—2022年2月江苏省人民医院收治的126例行胸腔镜肺癌根治术患者为研究对象,以随机数字表法分为2组,每组63例,对照组给予胸外科临床基础性护理干预,试验组在对照组护理基础上应用基于TCM模式的护理干预。比较2组患者干预前及干预30 d后的情绪(SAS评分、SDS评分)、肺功能指标[1 min内最大通气量(MVV)、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))]、生活质量(KPS评分)及护理满意度。结果 干预30 d后,2组患者SAS评分、SDS评分均有不同程度的下降,且组间比较试验组明显低于对照组,差异均有统计学意义(P均<0.05);2组患者的KPS评分以及MVV、FVC、FEV_(1)均升高,且组间比较试验组的KPS评分及肺功能指标均高于对照组,差异均有统计学意义(P均<0.05);试验组患者的总体满意度为96.83%(61/63),显著高于对照组的80.95%(51/63),差异有统计学意义(P<0.05)。结论 基于TCM模式的护理干预效果明显优于常规护理,能够显著改善肺癌手术患者的肺功能,缓解负性情绪,提高患者生活质量,继而获得患者认同。

从美国针灸流派的产生发展看中医针灸理论的国际传播——以TCM针灸流派和医学针灸流派为例

传统中医针灸流派和医学针灸流派分属美国针灸流派的两大阵营,且最具代表性。从二者的产生以及在美国的传播发展历程看,针灸作为一门传统医疗技术,在美国实现“本土化”,以及进一步走向国际化和现代化的进程中,其面临的理论国际传播问题最为突出。站在医学史的角度,并结合针灸医学的自身特点,从理论整合重构、科学研究深化以及针灸海外教育三个方面入手,冲破针灸医学在国际传播中的局限,向世界展现中医针灸理论的全貌,是目前针灸医学理论实现国际化和现代化的前进方向。

Discussion on the Treatment of Palmoplantar Keratosis by TCM Soaking and Washing

By combing the mechanism and advantages of traditional Chinese medicine(TCM)soaking and washing and combining TCM syndrome differentiation with western medicine differentiation of diseases,guided by the view that"the principle of external treatment is that of internal treatment",this article discusses the differentiation and treatment ideas of palmoplantar keratosis by TCM soaking and washing,which will provide a reference for clinical treatment.

The Significance of Constructing TCM Psychosomatic Medicine from the Holistic View of the Unity of Nature and Man

With the development of medical technology, the problem of fragmentation of medical research has become increasingly prominent, and a series of iatrogenic diseases, drug-induced diseases and drug toxicity and side effects have resulted in an increase in the number of diseases. In the face of these problems, the medical concept has been re-examined and re-thought. The concept of the holistic view of the unity of nature and man in traditional Chinese medicine has certain enlightenment for dealing with the problems in the current medical development. Based on the holistic view of the unity of nature and man, the overall connection between human’s heart and body and nature is explained, and the further construction of TCM psychosomatic medicine is conducive to guiding the diagnosis and treatment of clinical diseases, and providing new enlightenment for the current medical concept thinking and clinical disease research.

Effects of TCM Nursing Based on Syndrome Differentiation on Pulmonary Function and Quality of Life in Patients with Acute Exacerbation of COPD

[Objectives] To investigate the effects of TCM nursing based on syndrome differentiation on pulmonary function and quality of life in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). [Methods] A total of 92 patients with AECOPD who came to Nanchong Chinese Medicine Hospital from March 2022 to February 2023 were selected for the study, and the intervention group (TCM nursing based on syndrome differentiation, 46 cases) and the conventional group (basic nursing, 46 cases) were selected for the study, and the pulmonary function and quality of life of the two groups were compared. [Results] Before nursing, there was no significant difference in levels of forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and percentage of forced expiratory volume in one second to forced vital capacity (FEV1/FVC) between the intervention group and conventional group ( P >0.05). After 3 months of nursing, the levels of FVC, FEV1 and FEV1/FVC in the intervention group were higher than those in the conventional group ( P <0.05). Before nursing, there was no significant difference in the scores of health, emotion and social functions between the two groups ( P >0.05). At three months of nursing, the scores of health, emotion, and social functions in the intervention group were higher than those in the conventional group ( P <0.05). [Conclusions] The implementation of TCM nursing based on syndrome differentiation in patients with AECOPD can effectively improve the pulmonary function and quality of life of patients, and has significant clinical implementation value.

Validity and reliability of the evaluation tool of TCM confidence in students with TCM learning experience

Objective:This study aimed to design an evaluation scale of traditional Chinese medicine(TCM)confidence and then evaluate its validity and reliability.Methods:Three parallel samples with TCM learning experience were included in this current study.Randomly selected 500 nursing students were included factor analysis and reliability and validity analyses.After 4 weeks,200 nursing students were recruited to retest the reliability.The evaluation tool of TCM confidence included 6 dimensions and 25 entries.Results:Cronbach's a coefficient of the evaluation scale was 0.95 and that of the 6 observed aspects were higher than 0.90,and splithalf reliability was 0.81.For the retest reliability,Cronbach's a coefficient of the evaluation scale was 0.93 and that of the 6 examined aspects was all over 0.80.The index of content validity Scale Content Validity Index(S-CVI)was 0.92 and Item Content Validity Index(I-CVI)was 0.83–1.00.Conclusions:Observed results may indicate that the designed tool can be used to assess the TCM confidence level of students with TCM studied experience and also can evaluate results of TCM education during studying TCM lectures.

HXD3CA型机车TCMS系统地面试验平台构建研究

列车网络控制系统(Train Control and Management System,TCMS)是利用网络技术将机车各控制设备连接在一起,设立主控设备,进行统一控制和信息共享,是机车的信息通信核心。HXD3CA型机车TCMS系统以东芝成熟的列车通信网络技术为基础,结合国际标准的列车通信网络(TCN)开发研制而成。HXD3CA型机车TCMS系统地面试验平台(以下简称TCMS试验平台)用于备品备件测试,岗位员工培训,故障现象再现。本文主要介绍了TCMS试验平台的构建过程和各种模拟器硬件实现方法。

基于TCMS落地数据的存储、检索、显示软件平台

基于TCMS落地数据的存储、检索、显示软件平台是一套兼顾数据准确性、完整性和实时性的信息传输解决方案,流式处理时序性数据以应用于城市轨道交通行业低延时海量实时数据的特性。该平台由车载数据分类发送、地面服务器解析存储和对外可视化Web服务三部分组成。其中车载数据分类发送作为客户端软件可编译为通用程序包;地面服务端程序处理实时数据流;可视化Web系统主要结合MVC思想(Model_模型,View_视图,Controller_控制器),以Spring Boot开源框架作为基础架构搭建[1]。

轨道车辆技术TCMS终端系统仿真装置设计

在城市轨道交通车辆运营中,车辆的安全可靠运行关系到乘客的人身安全问题。针对城轨车辆整车故障排查与处理,文章给出了基于TCMS终端系统仿真装置的设计、开发、验证和维护,实现对列车网络系统的接口、车载电气控制系统的逻辑控制、故障诊断及排除等方面的分析验证,弥补车辆故障排查与处理研究的不足,为轨道车辆技术赛项提供了平台支撑。

TcmYiAnBERT:基于无监督学习的中医医案预训练模型

目的/意义充分挖掘中医医案中的文本信息,提高中医药信息化程度和中医医案症状术语抽取、关系抽取等下游任务的准确率。方法/过程通过光学字符识别和爬虫技术获取大量中医医案数据并进行预处理,构建面向中医医案领域预训练数据集,使用BERT模型预训练方法,经过多轮训练得到首个面向中医领域专有预训练模型TcmYiAnBERT,并将该模型开源。结果/结论中医领域专有预训练模型TcmYiAnBERT在中医命名实体识别任务中比未使用该模型的预训练模型F 1值提高2.8个百分点。

熵权多目的地TCM模型及其在游憩资源旅游价值评估中的应用——以武夷山景区为例

多目的地环程旅行问题是TCM模型研究应用的难点和热点,减少费用分成权重确定中的主观因素成为旅游价值评估的关键性问题。论文提出基于熵权的费用分成法,综合考虑了游客在旅游区的消费情况、旅游资源要素价值、资源影响力等多方面影响因素,权重确定完全基于组合景点间的差异,避免了主观因素的影响,使费用分成合理、科学。通过对武夷山景区国内旅游价值的分析计算结果表明2,002年武夷山景区游憩资源的国内旅游价值为22.301×108元,其中消费者支出15.407×108元,人均实际支出1 452元;消费者剩余6.894×108元,人均消费者剩余为650元。研究结果表明,熵权多目的地TCM模型是正确可行的。
