Systematic analysis of the safety and benefits of transvaginal hybrid-NOTES cholecystectomy

AIM: To evaluate transvaginal hybrid-NOTES cholecystectomy(TVC) during its clinical establishment and compare it with the traditional laparoscopic technique(LC).METHODS: The specific problems and benefits of TVC were reviewed using a registry analysis,a comparative cohort study and a randomized clinical trial. At first,feasibility,safety and specific complications of the TVC were analyzed based on the first 488 data sets of the German NOTES Registry(GNR). Hereafter,we compared the early postoperative results of our first 50 TVC-patients with those of 50 female LCpatients matched by age,BMI and ASA classification. The same cohort was contacted an average of two years later to evaluate long-term results concerning pain and satisfaction with the aesthetic results and the overall postoperative results as well as sexual intercourse by means of two domains of the German version of the Female Sexual Function Index(FSFI-d). Consequently,we performed a randomized clinical trial comparing 20 TVC-patients with 20 needlescopic/3-trocar cholecystectomies(NC) also concerning the early postoperative results as well as pain,satisfaction and quality of life by means of the Eypasch Gastrointestinal Quality of Life Index(GIQLI) in the later course. Finally,we discussed the results in accordance with other published studies.RESULTS: The complication(3.5%) and conversion rates(4.1%) for TVC were low in the GNR and comparable to those of the LC. Access related intraoperative complications included injuries to the bladder(n = 4; 0.8%) and bowel(n = 3; 0.6%). The study cohort revealed less postoperative pain after TVC comparing to the LC-patients on the day of surgery(NRS,1.5/10 vs 3.1/10,P = 0.003),in the morning(NRS,1.9/10 vs 2.8/10,P = 0.047) and in the evening(NRS,1.1/10 vs 1.8/10,P = 0.025) of postoperative day(POD) one. The randomized clinical trial consistently found less cumulative pain until POD 2(NRS,8/40 vs 14/40,P = 0.043),as well as until POD 10(NRS,22/190 vs 41/190,P = 0.010). Furthermore,the TVC-patients had a better quality of life on POD 10 than did the LC-patients(GIQLI,124/144 vs 107/144,P = 0.028). The complication rates were comparable and no specific problems were detected in the long-term follow-up for sexual intercourse for either group. The TVC-patients were more satisfied with the aesthetic result in the long-term course in the matched cohort analysis(1.00 vs 1.88,P < 0.001) as well as in the randomized clinical trial(1.00 vs 1.70,P < 0.001) when compared with the LC-patients.CONCLUSION: TVC is a feasible procedure with a high safety profile and has advantages in regard to postoperative pain and aesthetic results when compared with LC or NC.

Function of note strings in Japanese Tit alarm calls to the Common Cuckoo:a playback experiment

Background: Birds produce alarm calls to convey information about threats. Some Passerine alarm calls consist of several note strings, but few studies have examined their function. Previous studies have shown that Japanese Tits(Parus minor) can alter the calling rate and number and combination of notes in response to predators. We previously found the combinations of note types in Japanese Tit alarm calls to be significantly different in response to the Sparrowhawk(Accipiter nisus) and Common Cuckoo(Cuculus canorus).Methods: Through playback experiments, we tested whether the note strings in Japanese Tit alarm calls to the Common Cuckoo have different functions in conveying information. The note strings of selected alarm calls were divided into the categories of C and D, and different calls were then constructed separately based on the two note string categories. Original alarm calls(C–D), C calls and D calls were played back to male Japanese Tits during the incubation period.Results: Male Japanese Tits had a significantly stronger response to C calls than to C–D calls, and they showed a significantly stronger response to both C and C–D calls than to D calls, suggesting that Japanese Tits discriminated between the C and D calls.Conclusions: Our study demonstrated that the C-and D-category note strings of Japanese Tit alarm calls to the Common Cuckoo have different functions, which supports the previous finding that different note strings in an alarm call can provide different information to receivers. However, the exact meanings of these note strings are not yet known, and further investigation is therefore required.

The Correlation Between Note Features and Consecutive Interpreting Quality for English Majors

Note-taking skill is a necessary component in interpreter training programs,and previous research has yielded findings such as note-taking training methods or features of interpreter trainees’notes.However,little research has been done to investigate the changes in note features and correlations between note features and interpreting quality concerning Chinese students’C-E(Chinese-English)and E-C(EnglishChinese)interpreting.Using the framework of Daniel Gile’s Effort Model and Interpretive Theory of Translation,this paper examined how 45 English Majors’notes develop within one semester(seventeen weeks)and the relationship between note features(quantity,form,and language choice of notes)and consecutive interpreting quality.The participants of this study were all beginner interpreting trainees,and the note-taking training was introduced in Week 6.The study employed note manuscripts,interpreting tests,and semi-structured interviews to track the features and changes in students’notes.Correlation analyses and T-tests showed that(a)after the note-taking training,the number of notes increased from Week 8 to Week 17,and it was positively correlated with interpreting quality(fidelity and delivery)for both C-E and E-C interpreting;(b)as for forms of notes,participants primarily employ single Chinese words and the percentages of abbreviations and symbols rose prominently from Week 8 to Week 17 for C-E interpreting.Besides,correlation analyses show that interpreting quality improves with fewer single Chinese words and more abbreviations and symbols.For E-C interpreting,notes were mainly in English,especially single English words and abbreviations.The percentages of single Chinese words and abbreviations ascended whereas those of single English words and symbols decreased.Furthermore,results show that the more abbreviations and symbols,the better target-text fidelity,and fewer abbreviations,the better the targettext delivery;(c)concerning language choice,notes were mainly in source language for both C-E and E-C interpreting and the percentage of target language notes went up significantly for C-E interpreting.Consequently,the percentage of target language notes was positively correlated with interpreting quality.Interviews indicate that most participants do not pay much attention to language selection in the first stage,and if the source text a familiar topic with little difficult vocabulary,he or she records the target language.Otherwise,it was safer to use the source language.

Historical Research on Rights in the Eyes of Young Marx

Karl Marx’s insightful critique of bourgeois rights,represented in the Declaration of the Rights of Man and of the Citizen adopted by France,can be attributed to the short-term and longterm perspectives he adopted successively in his research.By reading works such as History of France in the Revolutionary Era and History of the French Revolutionary Parliament,Marx was able to examine the original intentions of the legislators in the period of the French Revolution.This short-term examination was the most intuitive and logical approach to studying the Declaration of the Rights of Man and of the Citizen,in which Marx focused on the research of the period of the French National Convention and its idea on property rights.The enigmatic political-legal facts that emerged in 1793 during the French Revolution,along with other incidents,presented an opportunity for Marx to turn to long-term rights surveys.In this context,Marx gradually shifted his attention from the legislators who were the shining protagonists of the“grand political historical drama”to the underlying forces that worked behind them,and in doing so understood how the two major achievements of the French Revolution provided an explanation for the historicity of the phenomenon of rights.Marx’s longterm rights surveys from the perspective of historical materialism were directly inspired by the Kreuznach Notes as well as famous scholars including Francois Guizot,Augustin Thierry,and Henri Saint-Simon.

Rupture of a Posterior Fossa Dermoid Cyst Overlying the Torcular with Extracranial Extension: Technical Note

Intracranial dermoid cysts are congenital benign neoplasms mostly diagnosed in the pediatric?hood and usually involve the midline structures. They count approximately less than 1% of all intracranial neoplasms and are believed to arise from ectopic cell rests incorporated in the closing neural tube. These dermoid cysts, especially those involving the posterior fossa and overlying the torcular, are uncommon. We report perhaps the first case of this entity in a third-year-old boy and discuss the physiopathogenesis, the imaging features and the best technical note to manage this cyst in this location.

Notes on the variability of reflected inner core phases

Recent events beneath Central America have produced excellent sets of inner core reflection （PKiKP phase） at high frequency recorded by USArray ranging from 18° to 30°. However, the amplitude of this phase displays considerable scatter with a factor of six or more. Such scatter has been attributed to upper-mantle scattering and the Inner Core Boundary （ICB） in combination. Here, we show that neighboring events share upper-mantle scatterers beneath the receivers, and their ratio allows a clearer image of deep earth structure. Alter confirming some of the measured variation is indeed due to deep structure, we stacked nearby traces to reduce fine scale variations which are mostly due to shallow structure. Then, the remaining relatively large scale variation pattern of PKiKP phase is caused by the inner core boundary, as demonstrated by numerical experiments. After migration of data to the 1CB, we observe a consistent image. We find such a pattern can be explained by a patch of mushy material of a few kilometers high where the material changes gradually from that of the outer core to that of the inner core.
