Western medicine mainly uses drugs such as proton pump inhibitors to inhibit gastric acid secretion, but some patients are accompanied by symptoms such as non-acid reflux and gas reflux, which cannot effectively treat the disease. It is necessary to actively explore other treatment schemes. Traditional Chinese medicine (TCM) has a long history of research on gastroesophageal reflux disease, which emphasizes the treatment based on syndrome differentiation as a whole. Through the treatment of various and multi-component TCM prescriptions, the patient’s body condition can be adjusted, and the treatment effect on gastroesophageal reflux disease is reliable, which has obvious therapeutic advantages. To further clarify the treatment of gastroesophageal reflux disease, this study reviewed and analyzed the research progress of the treatment of liver disease with modified prescriptions, and the report is as follows.展开更多

TP represents the curative treatment of pancreatic cancer which simultaneously involves the head and the body/tail of the pancreas. Other possible indications for TP comprise multifocal parenchymal diseases, such as intraductal papillary mucinous neoplasms, pancreatic neuroendocrine tumors, multiple endocrine neoplasms, and chronic pancreatitis. Sometimes, TP is carried out in patients scheduled for pancreaticoduodenectomy(where only the head of the pancreas is removed) when a cancer located in the pancreatic head is intraoperatively found to also extend to the pancreatic body [1].展开更多

文摘Aseptic abscesses are an emergent entity and have been described in inflammatory bowel disease, especially in Crohn's disease, and in other diseases. However, aseptic abscesses associated with Beh et's disease are extremely rare.We report a Japanese male diagnosed with an incomplete type of Beh et's disease who developed multiple aseptic abscesses of the spleen and liver. In 2002,the spleen abscesses were accompanied by paroxysmal oral aphthous ulcers and erythema nodosum. As the patient's response to antibiotic treatment was inadequate,a splenectomy was performed.Severe inflammatory cell infiltration, largely of polymorphonuclear neutrophils, was observed without evidence of bacterial or fungal growth.Although the patient had no history of ocular symptoms or genital ulcers,a diagnosis of incomplete Beh et's disease was made according to the Japanese diagnostic criteria because of the presence of paroxysmal arthritis and epididymitis since 2002.In 2005,multiple liver abscesses developed with right hypochondrial pain and seemed to be attributed to Beh et's disease because the abscesses yielded negative results during a microbiologic investigation and failed to go into remission under antibiotic therapy.Oral prednisone(15 mg/d) was started in May 2006,and the abscesses dramatically disappeared 4 wk after treatment.Although the patient had a relapse of the liver abscesses in association with the tapering of prednisone, the augmentation of prednisone dosage yielded a response. The abscesses of the liver and spleen were strongly suggested to be attributed to Beh et's disease.Clinician should be aware of the existence of aseptic abscesses as uncommon manifestations of Beh et's disease.
文摘According to the theory of traditional Chinese medicine, the tongue is thought to be an outer manifestation of the status of the viscera. It can be divided to spleen-stomach area,liver-gall area, kidney area, and heart-lung area. The tongue coating is formed by stomach-Qi.the tongue, especially spleen-stomach area, and the tongue coating may reflect the status of the spleen and stomach. This article summarizes the research overview of TCM tongue diagnosis in spleen and stomach disease.

文摘The pediatric body of pure yang, the liver is often more than enough, the spleen is often insufficient,Yang moving surplus, and Yin essence is insufficient,at the same time, children are the body of immature yin and yang, prone to imbalance of yin and Yang,so deficiency of phlegm accumulation and endogenous liver wind, feel ill at ease, dysfunction of lung qi,Insufficiency of kidney yin are the fundamental pathogenesis of children with tic syndrome. This article discusses the relationship between children with tic syndrome and the five Zang-organs,and how to regulate the viscera through regulating the spleen and stomach system, treating both manifestation and root cause of disease, and put forward a new treatment that the spleen and stomach system should be taken as the center to regulate the five Zang-organs in children with tic syndrome, which should be resolved in order of importance and urgency and treated in stages.

文摘Gastroesophageal reflux disease(GERD)is one of refractory digestive disease in clinic characterized by high prevalence, complex and recurrent symptoms.Traditional Chinese medicine(TCM)has a significant effect on the treatment of GERD.Spleen deficiency and damp-heat syndrome becomes one of the main syndrome patterns of GERD adapting with the changes of modern lifestyle.Although the therapy of strengthening the spleen, clearing heat and dampness is widely used in treatment of GERD,further studies still need to be conducted. The article comprehensively analyses the theoretical basis, mechanism and clinical efficacy of the therapy in order to provide more ideas for clinical treatment.
文摘According to the original works of an ancient Chinese medicine book"Treatise on Spleen and Stomach"(Detailed Analysis of Spleen and Stomach Diseases;1249 C.E.)written by Li Dongyuan(1180-1251 C.E),and related explanations of later physicians, the efficacy and application of Ginseng(Ginseng Radix et Rhizoma)in related prescriptions were investigated from the aspects of dosage, usage and treatment methods.Ginseng appeared in 27 named prescriptions, including 20 decoctions,4 pill and 3 powder. In terms of dosage and usage, the common dosage of Ginseng is five points, mainly decoction. Most of the processing methods are"Fuju"(Bite up the medicine with your mouth so it can be taken.)and decoction with water. In terms of treatment methods,Ginseng is used in the sweet temperature and heat removing method and Nourishes Qi and Eliminates Evils Method(build your body to cure disease)in Li Dongyuan's"Treatise on Spleen and Stomach".Explaining Sweet Temperature Heat Removal Method(Use sweet and warm medicines to treat Qi-deficiency fever or Blood-deficiency fever;there are many reasons for fever, and cold and cooling medicines with heat-clearing effect are generally used for treatment.Use bitter cold medicine,so as not to consume the body's Yang.)takes Buzhong Yiqi Decoction(BYD)as an example, and explaining the Nourishes Qi and Eliminates Evils Method takes Mahuang-renshen-shaoyao decoction as an example. At the same time,"Treatise on Spleen and Stomach"matured in the Jin and Yuan Dynasties(1115-1368 C.E.).Its system of weights and measures, terminology and medical theory are quite different from those of modern Chinese medicine. Therefore, when researching the dosage, usage and treatment methods of Ginseng,we will explore the controversial points, and explain them by using modern Chinese medicine theory,so as to guide the clinical medication more accurately.
文摘Total pancreatectomy(TP) is a surgical procedure which includes the complete removal of the pancreatic gland, the spleen, the duodenum, the gallbladder, and the distal part of the stomach. Sometimes, TP is carried out in patients scheduled for pancreaticoduodenectomy(where only the head of the pancreas is removed) when a cancer located in the pancreatic head is intraoperatively found to also extend to the pancreatic body [1].
文摘BACKGROUND The overlap of imaging manifestations among distinct splenic lesions gives rise to a diagnostic dilemma.Consequently,a definitive diagnosis primarily relies on his-tological results. The ultrasound(US)-guided coaxial core needle biopsy(CNB)not only procures sufficient tissue to help clarify the diagnosis, but reduces the incidence of puncture-related complications.CASE SUMMARY A 41-year-old female, with a history of pulmonary tuberculosis, was admitted to our hospital with multiple indeterminate splenic lesions.Gray-scale ultrasono-graphy demonstrated splenomegaly with numerous well-defined hypoechoic ma-sses.Abdominal contrast-enhanced computed tomography(CT)showed an en-larged spleen with multiple irregular-shaped,peripherally enhancing, hypodense lesions.Positron emission CT revealed numerous abnormal hyperglycemia foci. These imaging findings strongly indicated the possibility of infectious disease as the primary concern, with neoplastic lesions requiring exclusion. To obtain the precise pathological diagnosis, the US-guided coaxial CNB of the spleen was ca-rried out.The patient did not express any discomfort during the procedure.CONCLUSION Percutaneous US-guided coaxial CNB is an excellent and safe option for obtaining precise splenic tissue samples,as it significantly enhances sample yield for exact pathological analysis with minimum trauma to the spleen parenchyma and sur-rounding tissue.
