A Comparative Analysis of the Control Effect of Medical Expenses between General Hospitals and Traditional Chinese Medicine Hospitals from 2012 to 2021

Objective To analyze the changing trend of average medical expenses and structure in general hospitals and traditional Chinese medicine(TCM)hospitals and the effects and differences achieved by the two kinds of hospitals through controlling unreasonable growth of medical expenses,so as to provide reference for controlling the rapid rise of medical cost in public hospitals and optimizing the cost structure.Methods Based on the changes of related indicators of medical expense control from 2012 to 2021,the overall characteristics,changes of cost structure and trends of medical expenses in general hospitals and TCM hospitals were investigated.Results and Conclusion From 2012 to 2021,the increase of medical expenses in general hospitals and traditional Chinese medicine hospitals had slowed down,the proportion of drug revenue to medical income began to decline,and the medical service income increased.However,the proportion of inspection,test and sanitary materials income has increased instead of decreasing,but the management cost has decreased.The two kinds of hospitals have achieved certain cost control results,the structure of medical cost has changed greatly,and the technical service and labor value of medical personnel have been reflected to a certain extent.However,it is still necessary to explore a more scientific and reasonable cost control mechanism to promote the further optimization of medical cost structure. 展开更多

Study on the Changes of Medical Income Structure in Governmentrun Hospitals of Traditional Chinese Medicine from 2012 to 2021

Objective To study the changing characteristics and trend of medical income structure in the government-run hospitals of traditional Chinese medicine(TCM),evaluate the effects of relevant reform measures,and to put forward corresponding suggestions for further optimizing their income structure.Methods The data related to the average medical income of government-run hospitals of TCM from 2012 to 2021 were sorted out.Then,descriptive analysis method was used to analyze the changes of related indicators.Besides,structural change method was applied to investigate the changes of outpatient income and inpatient income.Results and Conclusion From 2012 to 2021,the growth of medical income in government-run hospitals of TCM tended to be stable,and the proportion of medical service income increased from 22.62%(2012)to 29.38%(2021),but the average annual growth rate was only 0.68%.The main items that caused the change of outpatient income structure were medicine revenue,laboratory tests,diagnosis and treatment,and the cumulative contribution rate was 89.15%.The main items that caused the change of inpatient income structure were medicine revenue,sanitary materials,and auxiliary examinations income,with a cumulative contribution rate of 80.04%.However,the contribution rate of registration,diagnosis,treatment,surgery and nursing income reflecting the value of medical personnel’s technical labor was relatively small.The medical income structure of government-run hospitals of TCM underwent great changes and gradually became reasonable,but the medical service income increased slowly,and not all indicators achieved the expectations.To promote the sustainable development of public hospitals of TCM and enable them to provide high-quality and efficient TCM medical and health services,it is necessary to further improve the relevant policy mechanism. 展开更多

Background:This study uses a literature review and the Delphi expert consultation method to construct a competency evaluation model for infection control personnel in traditional Chinese medicine(TCM)hospitals.The aim is to strengthen infection control management in TCM hospitals,assess the competency of infection control personnel in their positions,and assist them in identifying the competencies that need improvement.Methods:Based on the literature research method and the Delphi expert consultation method,a competency model for the position was constructed through two rounds of expert consultations,analyzing the relationships between various factors and establishing a hierarchical structure model.Pairwise comparisons were made among the elements at the same level to construct a judgment matrix.Through the analytic hierarchy process,the weight coefficients of the indicators at each level in the competency model were obtained.Results:This study conducted a comprehensive assessment of various capabilities and practices related to hospital infection control.The survey results indicate that participants excelled in multiple areas,with a high overall satisfaction rate.95.28%of participants were able to develop hospital infection monitoring plans based on national infection control policies,demonstrating a good understanding and execution of these policies.94.09%of participants were familiar with high-risk populations and key departments in the hospital.91.73%of participants were able to establish monitoring scopes based on the trends of multidrug-resistant bacteria and conduct information monitoring,reflecting strong response capabilities.92.91%of participants were able to collaborate with relevant departments to conduct bacterial resistance monitoring,showing a good team spirit.94.49%of participants were able to perform targeted monitoring,including surgical site infections,indicating that they have effective monitoring strategies.91.34%of participants were able to collect and organize monitoring data and establish a systematic database,demonstrating good data management skills.90.16%of participants were able to interpret laws and regulations related to hospital infection management,indicating a high level of legal knowledge.89.37%of participants generally possessed good communication skills.92.52%of participants were able to guide medical staff on occupational safety and protective knowledge,showing an emphasis on occupational health.Participants demonstrated a strong desire to learn and innovate,with 87.01%actively participating in continuing education and research activities,reflecting a pursuit of professional development.Conclusion:Based on the results of the two rounds of expert consultations,a competency evaluation model for infection control personnel in TCM hospitals was formed.Through the analytic hierarchy process,the weight coefficients of various indicators at different levels in the model were obtained,and the research results have good scientific validity and reliability. 展开更多

Objective: To explore the application effect of flipped classroom combined with problem-based learning teaching method in clinical skills teaching of standardized training for resident doctors of traditional Chinese Medicine. Methods: The study used the experimental control method. The study lasted from September to November 2022. The subjects of this study were 49 students of standardized training for resident doctors of traditional Chinese Medicine from grades 2020, 2021 and 2022 of Dazhou integrated TCM & Western Medicine Hospital. They were randomly divided into experiment group (25) and control group (24). The experiment group adopted flipped classroom combined with problem-based learning teaching method, and the control group adopted traditional teaching method. The teaching content was 4 basic clinical skill projects, including four diagnoses of traditional Chinese Medicine, cardiopulmonary resuscitation, dressing change procedure, acupuncture and massage. The evaluation method was carried out by comparing the students’ performance and a self-designed questionnaire was used to investigate the students’ evaluation of the teaching method. Results: The test scores of total scores in the experimental group (90.12 ± 5.89) were all higher than those in the control group (81.47 ± 7.96) (t = 4.53, P P Conclusions: The teaching process of the flipped classroom combined with problem-based learning teaching method is conducive to improving the efficiency of classroom teaching, cultivating students’ self-learning ability, and enhancing students’ willingness to learn. 展开更多

Experimental Study of the Anti-human Papilloma Virus Mechanisms by a Traditional Chinese Medicine “Keyouling”

To investigate the anti-human papilloma virus (HPV) effect of the external liniment of traditional Chinese medicine “Keyouling”, the efficacy of this drug acting upon the proliferation and growth of the normal epidermis cells of rabbits′ penis prepuce in different concentrations was observed. An experimental model of pathology of histocytes infected with HPV in vitro was established by using the human HPV infected prepuce epidermis cells as virus carrier to infect the sub-cultured epidermis cells of rabbits′ penis prepuce. The direct killing effect and the blocking effect of infections of epidermis cells in rabbits′ penis prepuce induced by HPV with different concentrations of “Keyouling” were determined by 3 kinds of culture methods: (1) the infected epidermis cells of rabbits′ penis prepuce were challenged by co-culture with HPV suspension and with culture medium containing drug; (2) the co-culture with epidermis cells of rabbits′ penis prepuce and HPV suspension was challenged with drug; (3) co-culture with culture medium containing drug and epidermis cells of rabbits′ penis prepuce was challenged with HPV suspension. It was found that “Keyouling” had little effect on the proliferation and growth of the normal epidermis cells of rabbits′ penis prepuce, but it had direct killing effect on HPV, and blocked effectively the infection of the epidermis cells of rabbits′ penis prepuce by HPV. It concludes that “Keyouling” has significant killing effect to HPV, but not normal histocytes in vitro , indicating that it is safe for clinical use. Its blocking effect on the HPV infection of rabbits′ penis prepuce epidermis cells suggested “Keyouling” may be clinically used to prevent the development of tumors or to inhibit the further progression or metastasis of tumors. 展开更多

The Five Plague Gods(五瘟神) in the Shuilu(水陆) murals from the Ming and Qing dynasties exemplify a distinctive manifestation of plague gods image developed under the pedigree of Taoist gods.As a significant system for expelling disasters in Shuilu,the depiction of the Five Plague Gods prominently merges human beings and livestock.This shape form to a large extent,draws on the image of Nuo(傩),a figure reflecting an imagination of the host of plague pathogens.In terms of the numbers of gods,it aligns with the conceptual framework of Wu Xing(五行 the five elements),a foundational principle in traditional Chinese medicine.The internal cultural connection highlights the means of plague prevention and control in China,as well as the external reflection of plague culture.Through multi-dimensional interpretations with the double proof method,this article dissects the formation,core elements,and cultural characteristics of the beliefs in the Five Plague Gods reflected in the Shuilu murals,and transcends mere visual semantics,offering a deeper understanding of these complex cultural symbols. 展开更多

The gut microbiome is an extensive variety of bacteria with a range of metabolic capabilities that can be pathogenic,beneficial,or opportunistic.Changes in the gut microbiota's composition can affect the link between gut integrity and host health as well as cause disruptions to numerous neurological systems.The second most prevalent mental health problem,insomnia has a negative social and economic impact.Currently,it is becoming increasingly obvious how crucial it is to preserve the delicate balance of gut microbiota to treat illness-related symptoms like insomnia.Although traditional Chinese medicine has proposed an effective strategy against insomnia through gut microbiota alteration in animal models,studies in human models are limited.This decreases the predictive value of the studies in terms of human outcomes.This editorial places an emphasis on cultural sensitivity rather than scientific reasoning that promotes the use of traditional Chinese medicine(TCM).We aim to emphasize the concern that promoting TCM could divert resources from conventional medical research,leading to suboptimal care. 展开更多

Objective:The objective of this study was to evaluate the effectiveness of flotation therapy combined with psychological resilience training in improving emotional regulation and sleep quality in patients recovering from viral infections and to assess the application of the traditional Chinese medicine(TCM)theory of“form and spirit co-nourishing”in modern healthcare interventions.Methods:A total of 20 patients recovering from viral infections with anxiety and sleep disturbances were randomly assigned to the experimental group(n=10)and the control group(n=10).The experimental group received a combination of resilience training,targeting five key areas:physiological regulation,cognitive restructuring,emotional management,self-efficacy,and social support,alongside flotation therapy,with 45-min weekly sessions.The control group received conventional medical treatment.Anxiety levels were measured using the Hamilton Anxiety Scale,while sleep quality was assessed using the Pittsburgh Sleep Quality Index,both before and after the intervention.Results:Compared with intervention before,the resilience level of the experimental group significantly increased after the intervention(P<0.001),while no notable changes were detected in the control group(P>0.05);the experimental group demonstrated significantly lower anxiety level and sleep disorder level and higher resilience level compared to the control group after the intervention(all P<0.001).Conclusion:The combined use of flotation therapy and resilience training,grounded in the TCM principle of“form and spirit co-nourishing,”significantly improves emotional regulation and sleep quality in patients recovering from viral infections.This integrative approach supports both physical and psychological healing,aligning with the TCM holistic treatment model,and offers a promising avenue for modern mental health interventions. 展开更多

Objective:This study is to address the health beliefs and health behavior of Chinese immigrants residing in the Greater Vancouver area of British Columbia(BC)Province in Canada.This article discussed Chinese immigrants’traditional Chinese medicine(TCM)use,health beliefs,and health behaviors.Methods:Information used in this study is based on data collected in the Chinese-speaking community in the Greater Vancouver area of BC in 2020–2022.Quantitative and qualitative methods were applied to this study.The first stage recruited 314 participants for the quantitative study to cross-validate an instrument tool,followed by the 2nd stage of 20 stratified random sampling out of the 314 participants for TCM-related in-depth qualitative interviews.This study focuses on the second stage of TCM qualitative interviews.Results:Results indicated that TCM health beliefs have cultural and spiritual meanings tied to the Chinese-speaking participants.There are barriers for the Chinese-speaking population to access the existing healthcare services due to their TCM health beliefs and other health needs,for example,family doctors,integrated medicine for better health outcomes,and mental health services especially during the coronavirus disease 2019(COVID-19)pandemic.Conclusion:Integrating TCM health beliefs and behaviors of Chinese-speaking immigrants into existing Canadian mainstream health services are strongly recommended. 展开更多

Rooted in Chinese traditional culture, Traditional Chinese Medicine contains abundant humanistic thoughts, for instance, pursuing the unity of human and nature, advocating the vital importance of life, sticking to the virtue of "medicine being humane art", abiding by the principle of "Benevolence prior to interest", complying with the medical rule of "Respecting peers". These are the very concrete reflections of medical humanistic spirit.
