In fact, a non-negligible percentage of patients under- going pancreaticoduodenectomy (PD) for suspected cancer has a benign lesion at final histology. We describe an approach for patients with diagnostic uncertainty between cancer and chronic pancreatitis, with the aim of minimizing the incidence of PD for suspicious malignancy finally diagnosed as benign disease. METHODS: Eighty-eight patients (85.4%) with a clinicoradio- logical picture highly suggestive for malignancy received for- mal PD (group 1). Fifteen patients (14.6%) in whom preopera- tive diagnosis was uncertain between pancreatic cancer and chronic pancreatitis underwent pancreatic head excavation (PHEX) for intraoperative tissue diagnosis (group 2): those diagnosed as having cancer received PD, whereas those with chronic pancreatitis received pancreaticojejunostomy (P]). RESULTS: No patient received PD for benign disease. All pa- tients in group I had adenocarcinoma on final histology. Eight patients of group 2 (53.3%) received PD after intraoperative diagnosis of cancer, whereas 7 (46.7%) received PJ because no malignancy was found at introperative frozen sections. No signs of cancer were encountered in patients receiving PHEX and PJ after a median follow-up of 42 months. Overall survival did not differ between patients receiving PD for cancer in the group 1 and those receiving PD for cancer after PHEX in the group 2 (P=0.509). CONCLUSION: Although the described technique has been used in a very selected group of patients, our results suggest that PHEX for tissue diagnosis may reduce rates of unnecessary PD, when the preoperative diagnosis is uncertain between cancer and chronic pancreatitis.展开更多

A new procedure called duodenum-preserving pancreatic head resection(DPPHR) has been introduced, and has little effect on the structure and function of the digestive system. With the development of minimally invasive surgical techniques, treatment of CP can be performed with laparoscopic DPPHR(LDPPHR). We present a case of CP that was successfully treated with LDPPHR. The postoperative pathological diagnosis was pancreatitis, demonstrating the feasibility of LDPPHR. We recommend this minimally invasive surgical method as preferred treatment for CP.展开更多

METHODS: Eighty three patients, who presente...AIM: To prospectively analyse the clinical, biochemical and radiological characteristics of the mass lesions arising in a background of chronic calcific pancreatitis (CCP). METHODS: Eighty three patients, who presented with chronic pancreatitis (CP) and a mass lesion in the head of pancreas between February 2005 and December 2011, were included in the study. Patients who were identified to have malignancy underwent Whipple' s procedure and patients whose investigations were suggestive of a benign lesion underwent Frey's procedure. Student t-test was used to compare the mean values of imaging findings [common bile duct (CBD), main pancreatic duct (MPD) size] and laboratory data [Serum bilirubin, carbohydrate antigen 19-9 (CA 19-9)] between the groups. Receiver operating characteristic curve (ROC curve) analysis was done to calculate the cutoff valves of serum bilirubin, CA 19-9, MPD and CBD size. The sensitivity, specificity, positive predictive valve (PPV) and negative predictive value (NPV) were calculated using these cut off points. Multivariate analysis was performed using logistic regression model. RESULTS: The study included 56 men (67.5%) and 27 women (32.5%). Sixty (72.3%) patients had tropical calcific pancreatitis and 23 (27.7%) had alcohol related CCP. Histologically, it was confirmed that 55 (66.3%) of the 83 patients had an inflammatory head mass and 28 (33.7%) had a malignant head mass. The mean age of individuals with benign inflammatory mass and those with malignant mass was 38.4 years and 45 years respectively. Significant clinical features that predicted a malignant head mass in CP were presence of a head mass in CCP of tropics, old age, jaundice, sudden worsening abdominal pain, gastric outlet obstruction and significant weight loss (P ≤ 0.05). The ROC curve analysis showed a cut off value of 5.8 mg/dL for serum bilirubin, 127 U/mL for CA 19-9, 11.5 mm for MPD size and 14.5 mm for CBD size. CONCLUSION: Elevated Serum bilirubin and CA 19-9, and dilated MPD and CBD were useful in predicting malignancy in patients with CCP and head mass.展开更多

Therapeutic ERCP interventi...The main treatment strategies for chronic pancreatitis in young patients include therapeutic endoscopic retrograde cholangio-pancreatography (ERCP) intervention and surgical intervention. Therapeutic ERCP intervention is performed much more extensively for its minimally invasive nature, but a part of patients are referred to surgery at last. Historical and follow-up data of 21 young patients with chronic pancreatitis undergoing duodenum-preserving total pancreatic head resection were ana- lyzed to evaluate the outcomes of therapeutic ERCP intervention and surgical intervention in this study. The surgical complications of repeated therapeutic ERCP intervention and surgical intervention were 38% and 19% respectively. During the first therapeutic ERCP intervention to surgical intervention, 2 patients developed diabetes, 5 patients developed steatorrhea, and 5 patients developed pancreatic type B pain. During the follow-up of surgical intervention, 1 new case of diabetes occurred, 1 case of stea- torrhea recovered, and 4 cases of pancreatic type B pain were completely relieved. In a part of young patients with chronic pancreatitis, surgical intervention was more effective than therapeutic ERCP in- tervention on delaying the progression of the disease and relieving the symptoms.展开更多
目的探讨保留十二指肠的胰头切除术(duodenum-preserving pancreatic head resection,DPPHR)与胰管切开取石胰腺管空肠吻合术(Partington手术)对于胰腺头部结石手术治疗的疗效差别,评估二者手术的优缺点,并对其进行一定的优化和改进。...目的探讨保留十二指肠的胰头切除术(duodenum-preserving pancreatic head resection,DPPHR)与胰管切开取石胰腺管空肠吻合术(Partington手术)对于胰腺头部结石手术治疗的疗效差别,评估二者手术的优缺点,并对其进行一定的优化和改进。方法通过回顾性分析2017年1月1日至2022年10月1日以来,昆明医科大学第二附属医院肝胆胰外科一至四病区收治的139例需行手术治疗的胰腺头部结石患者的临床、影像和病理资料;根据对患者所施行的手术方式不同,将其分为DPPHR组和Partington组,分析2组患者临床特征以及外科治疗的近期和远期疗效。结果术前一般资料分析差异均无统计学意义(P>0.05);DPPHR组以Ⅰ型结石为主(80.2%),而Partington组以Ⅰ、Ⅲ型结石为主(45.3%、47.2%),差异有统计学意义(P<0.001);近期疗效中,DPPHR组在手术时间、术中出血量、术后住院天数和术后并发症发生率方面较Partington组具优势(P<0.05);远期疗效中,DPPHR组在结石复发再手术、术后胰腺功能等方面均优于Partington组(P<0.05);Logistic多因素分析显示CP≥5 a(95%CI 1.057~3.884,P=0.012)、术后长期有饮酒史(95%CI 0.987~3.128,P=0.025),CP≥5a和术后长期有饮酒史均是影响胰腺头部结石术后无疼痛复发生存期的独立危险因素。结论胰腺头部结石患者近期疗效(手术时间、术中出血量、术后住院天数和术后并发症发生率)和远期疗效(结石复发再手术、术后胰腺功能)分析,DPPHR手术均优于Partington术,可认为DPPHR手术目前成为一种有效的治疗PDS的方法;但还需为患者制定个体化手术策略,并重视术后护理和随访,以便为患者提供最佳预后。展开更多
