The aims of this study were: 1) to investigate whether a visual analogue scale (VAS) could be linear with the LSAS and substitute for the LSAS, 2) to relate such a VAS instrument to patient demographics. Methods: Fifty SAD patients were assessed using the LSAS and VAS instruments completed by both patients and doctors at the same session. We then drew distributions and calculated the Spearman’s ρ and κ coefficient values (divided at the median for each scale) between patient and doctor assessments. Next, each pair among the scores for the LSAS, the patient VAS and the doctor VAS was compared using Wilcoxon rank sum tests according to patient life profile data. Results: Scatter plots of pairs of scores were obtained. Spearman’s ρ was 0.661 between the LSAS and the patient VAS, 0.461 between the LSAS and the doctor VAS, and 0.494 between VAS scores of patients and doctors. The κ coefficients were 0.501 between the LSAS and patient VAS, 0.251 between the LSAS and doctor VAS, and 0.425 between patient VAS and doctor VAS (for all six, p < 0.001). The Wilcoxon rank sum tests indicated a significant difference between the groups with/ without “employment” (LSAS, patient/doctor VAS), with/without “graduation from junior college/university” (doctor VAS) (p < 0.05) and with/without marital history (the age of first consultation) (p < 0.01). Conclusions: A patient VAS may substitute for the LSAS and offer the versatility necessary to capture patient states and life profiles.展开更多

A clinical study was conducted to determine the effect of orally ingested caffeine on visual analogue scale (VAS) responses in healthy, moderate caffeine-consuming volunteers through the use of population pharmacokinetic-pharmacodynamic (PK-PD) modeling. Twelve subjects were recruited for a three-period cross-over study which utilized caffeine containing beverages. Each visit included 8-hour blood plasma and VAS response collection for PK and PD assessment respectively. The VAS used in the study, also called the caffeine analog scale, has been previously validated for caffeine. Population PK-PD modeling was conducted with NONMEM 7.2. Simultaneous and sequential modeling of PK-PD was attempted. Final model selection was based on parameter estimate precision, diagnostic plots, and visual predictive check (VPC) plots. Results showed that a one-compartment open model with first-order absorption and elimination best described the pharmacokinetics of caffeine. Sequential PK-PD modeling was successful and an effect compartment model with linear slope and baseline parameter best described caffeine pharmacodynamics. Diagnostic plots showed no major bias and VPC plots showed agreement between observations and predictions. The model was able to link VAS responses to caffeine concentration in healthy volunteers and may be useful in clinical trial simulations and design.展开更多

In the literature, there are more than 34 shoulder function assessment scoring instruments;the Modified Constant Murley Score (M-CMS) is one of the most popular scores. Although, the M-CMS had been translated and culturally adapted to Danish, Brazilian and Turkish versions, there is no Arabic version found in the literature. We aim to translate and culturally adapt M-CMS into the Arabic language. Method: The M-CMS was translated using previously published guidelines. The translation and cultural adaptation were done in five stages, initial translation by two bilingual translators then a synthesis of the translations after that, back translation by two native English speakers. Then an expert committee meeting approved the pre-final Arabic version. Finally, a pilot test was conducted on 41 patients to ensure its validity. Results: The M-CMS was successfully translated from the original English version to the Arabic version;no difficulties in the translation process were faced. Conclusion: A validated Arabic version of the M-CMS was produced and ready to be used for functional assessment of different shoulder pathologies in Arabic-speaking countries. Future study is needed for translation and cultural adaptation of the English standardized test protocol to assure the reproducibility of the Arabic version of the M-CMS.展开更多

This study aims to evaluate the effect of low-intensity continuous ultrasound (LICUS) treatment on the knee of osteoarthritis patients through home-based intervention using the LICUS medical device. 2) Methods: The clinical trials were designed in a single-arm, open-label, and intervention study. Thirty-five participants, including those who dropped out (12%), were screened and enrolled. The patients received LICUS (1.1 MHz, 1.5 W/cm2, collimated beams) on the knee by the instructions of the investigator at home (5 min/session, 3 times/day, for four-weeks). Outcome measures were assessed using the Visual Analog Scale (VAS) as a primary endpoint and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) as a secondary endpoint to evaluate pain relief and functional recovery of the knee between pre-treatment (baseline) and post-treatment (four-weeks). 3) Results: Knee pain scores measured using the VAS and WOMAC indices were significantly reduced after a four-week treatment with LICUS compared to baseline. Knee stiffness and functional capacity were significantly reduced after the LICUS application. In addition, there were no reports of adverse effects during the study period. 4) Conclusion: Long-term and home-based application of LICUS can be recommended as an alternative option for the treatment of OA patients, as evidenced by the effect of pain relief and knee function recovery.展开更多

The purpose of this study was to demonstrate quantitatively and qualitatively the physical and psychological burden due to positioning and breast compression during mammography. Muscle activity of each part of the body during positioning for the standard imaging method, to obtain craniocaudal (CC) view images, was measured in 15 adult females using surface EMG. The associated pain was analyzed using visual analogue scale (VAS) scores. During positioning for the CC view, muscle activity was highest in the biceps (24.44 iEMG/s) followed by the trapezius (17.78 iEMG/s) on the imaging side. Muscle activity of the biceps and the sternocleidomastoid on the imaging side showed significant differences compared with pre-imaging activity (biceps P < 0.031, sternocleidomastoid P < 0.005). The pain during mammography was rated as moderate to severe pain (VAS = 3.3 - 6.7) for CC views. As a result, the activities of not only the muscles directly involved in mammography positioning but also those indirectly involved were high as compared with the normal state. Measurement of muscle activity during mammography is expected to be used effectively, such as in the care to reduce pain for the subjects.展开更多

The response to the pruritus was assessed both weekly and at the end of the study period using a visual analogue score(VAS).RESULTS The average VAS score at the end of the study was 3.14 ± 1.59, which was significant compared to the baseline value of 7.75 ± 1.02(P < 0.05). Improvements in symptoms were noted in 19 out of 21(90.4%) patients. However, relapse occurred in six out of the 19 patients who responded. The dropout rate was high during the study period(33.3%).CONCLUSION Narrow band UVB is effective as an add-on therapy in peritoneal dialysis patients with refractory uremic pruritus. However, the present regime is cumbersome and patient compliance is poor.展开更多

Currently, there are limited options available for alleviating pain related to mammography. A non-medicinal approach to the alleviation of pain involves the effects of laughter on physical and psychological wellbeing. We therefore examined the possibility that humorous stimuli would alleviate the physical burden on women undergoing mammography. We assessed 29 women, 15 women received only conventional mammography (neutral group), while 14 women (humor group) watched a funny video during the same examination. The intensity of pain experienced during mammography was assessed by visual analogue scale (VAS) and the VAS results showed a statistically significant difference (P = 0.007) between the two groups, with the humor group experiencing less pain. In an additional experiment, 14 women in the humor group also underwent conventional mammography without exposure to the funny video and pain was assessed by VAS. We found that the pain experienced during conventional mammography without the funny video was significantly greater than the pain experienced during the same mammography but with the funny video (P = 0.047). These findings suggested the possibility of alleviating pain during mammography by humorous stimuli.展开更多

The patients with checkrein deformity mainly report distending pain in toe tips, pain when walking or wearing shoes, and gait instability. Previous studies have mainly reported surgical treatments for checkrein deformity, while few studies have reported using comprehensive rehabilitation alone to improve the checkrein deformity.CASE SUMMARY A 28-year-old woman was admitted to the hospital due to unstable gait caused by pain in the right hallux, for which she was unable to stretch for over three months. The patient had undergone "resection of ameloblastoma at the right mandible, mandibulectomy, and autogenous right fibula grafting". The patient’s hallux toe, as well as the second and third toes of the right foot could not be stretched, with pain in all the toes during walking. Based on the medical records of the patient, as well as the results of physical and auxiliary examinations, the main diagnosis was checkrein deformity in the right foot. Since the patient refused surgical treatment, rehabilitation was the only treatment option. At discharge, the patient reported evident improvement in the pain in the toes, gait stability, as well as increased ability to climb up and downstairs.CONCLUSION Comprehensive rehabilitation therapy could effectively alleviate the manifestations of checkrein deformity and improve the walking ability of the patients.展开更多

This condition was highly prevalent that approximately 85% patients having back pain were brought on by prolonged sitting. With the rapid development of modern technology, sitting has now become the most common posture in today’s work- place. Idea of using motor control learning approach provides the optimal control and coordination of the spine. The McKenzie evaluation was received using repeated movements and sustained positions. Therefore high quality randomized clinical trial was required to compare the effectiveness of these treatments for work related low back pain. Objectives: To compare the effectiveness of motor control exercises and McKenzie exercises in reducing pain and disability in work related low back pain. Method: The study included 40 subjects with work related low back pain due to prolonged sitting. They were randomly allocated into two groups (Group A and Group B). Group A was treated with motor control exercises and group B was treated with McKenzie exercises for 4 weeks. Results: Both the groups have shown statically significant improvement in vas with p < 0.0001 and ODI with p < 0.0001. When the comparison was done after the 4 weeks, the percentage of improvement in group A was much higher than Group B. Conclusion: The study concluded that motor control exercises have shown statically and clinically significant improvement in reducing pain and disability when compared to McKenzie exercises among work related low back pain subjects.展开更多

These findings suggested the possibility of alleviating pain during mammography by humorous stimuli.

文摘BACKGROUND In clinical practice, checkrein deformity is usually found in patients with calf injuries after ankle fracture or distal tibial fracture. At discharge, the patient reported evident improvement in the pain in the toes, gait stability, as well as increased ability to climb up and downstairs.CONCLUSION Comprehensive rehabilitation therapy could effectively alleviate the manifestations of checkrein deformity and improve the walking ability of the patients.
文摘Background: Work related low back pain has been identified as a one of the most costly disorders among the worldwide working population. Conclusion: The study concluded that motor control exercises have shown statically and clinically significant improvement in reducing pain and disability when compared to McKenzie exercises among work related low back pain subjects.
