目的探讨多参数心脏磁共振(cardiac magnetic resonance,CMR)在低龄儿童急性心肌炎诊断和短期随访中的应用价值。材料与方法前瞻性分析2022年1月至2024年2月就诊于我院的60例8岁以下的急性心肌炎患儿和30例对照组儿童。收集所有受试者...目的探讨多参数心脏磁共振(cardiac magnetic resonance,CMR)在低龄儿童急性心肌炎诊断和短期随访中的应用价值。材料与方法前瞻性分析2022年1月至2024年2月就诊于我院的60例8岁以下的急性心肌炎患儿和30例对照组儿童。收集所有受试者的临床资料和CMR数据[包括左心室心肌的初始T1值、心肌细胞外容积百分数(extracellular volume fraction,ECV)、T2值、T2信号比值、晚期钆增强(late gadolinium enhancement,LGE)情况]。使用受试者工作特征(receiver operating characteristic,ROC)曲线评价单一和联合参数的诊断效能,并计算曲线下面积(area under the curve,AUC)、敏感度、特异度和准确度。采用配对检验比较低龄急性心肌炎患儿初诊和随访时的CMR数据。结果急性心肌炎组的初始T1值、ECV值、T2值、T2信号比值和LGE阳性病例数均显著高于对照组(P均<0.001)。初始T1值、ECV值、T2值、T2信号比值的最佳截断值分别为1232.50 ms、30.50%、53.50 ms、1.75。初始T1值、T2值、T2信号比值的AUC(分别为0.917、0.889、0.839)均高于其他单一参数,但三者之间无显著差异(P均>0.05),其中初始T1值的敏感度和准确度最高,分别为81.7%和83.3%。2018版路易斯湖标准(Lake Louise Criteria,LLC)、排除ECV的2018LLC、初始T1值联合T2值的AUC(分别为0.933、0.917、0.892)均高于其他联合参数,但三者之间无显著差异(P均>0.05),其中2018LLC的敏感度和准确度最高,分别为90.0%和92.2%。与初诊时相比,短期随访时初始T1值、ECV值、T2值、T2信号比值、LGE累及心肌节段数和符合2018LLC的病例数总体上均降低(P均<0.05)。结论多参数CMR可作为诊断低龄儿童急性心肌炎的有效方法,在短期随访时能够监测患儿治疗后的病情变化。展开更多
Turbulent fluxes at the air-sea interface were estimated with data collected in 2011-2020 with a low-profile platform named OCARINA during eight experiments in five regions:2011,2015,and 2016 in the Iroise Sea;2012 in...Turbulent fluxes at the air-sea interface were estimated with data collected in 2011-2020 with a low-profile platform named OCARINA during eight experiments in five regions:2011,2015,and 2016 in the Iroise Sea;2012 in the tropical Atlantic;2014 in the Chilie-Peru upwelling;2017 and 2018 in the Mediterranean Sea,and 2018 and 2020 in Barbados.The observations were carried out with moderate winds(2-10 m s^(-1))and average wave heights of 1.5 m.In this study,the authors used the fluxes calculated by the bulk method using OCARINA-sampled data as the input.These data can validate the fluxes estimated from ERAS reanalysis data.The OCARINA and ERA5 data were taken concomitantly.To do this,the authors established an algorithm to extract the OCARINA data as closely as possible to the reanalysis data in time and position.The measurements of the OCARINA platform can conclude on the relevance of the widely used reanalysis data.展开更多

Atmospheric forcing dat...The Louis scheme and the COARE algorithm （version 3.0） are tested against eddy covariance and inertial dissipation methods for friction velocity estimates in different wind-sea/swell regimes. Atmospheric forcing data, tabulated by Donelan et al. （1997.J Phys Oceanog, 27：2087-2099）, were collected from a mast on the foredeck of a SWATH （small water-plane area, twin hull） ship in deep sea off the State of Virginia during the surface wave dynamics experiment. These data are representative of low to moderate wind regimes. The aerodynamic roughness length is determined by using the Charnock relationship. The intercomparison shows that the Louis scheme and the COARE algorithm underestimate the friction velocity by 6% and 3% respectively under pure wind sea conditions, 15% and 13% respectively under cross swell conditions, and 21% and 17% respectively under counter swell conditions. The analysis shows that these underestimations were caused by the method chosen to determine the aerodynamic roughness length because it significantly underestimates the aerodynamic roughness length. It is especially true under the cross swell and counter swell conditions.展开更多
Rocio (ROCV), Saint Louis encephalitis (SLEV) and West Nile (WNV) are Flavivirus (Flaviviridae) probably carried by birds and transmitted by Culex mosquitoes. We show here a review on infections of the central nervous...Rocio (ROCV), Saint Louis encephalitis (SLEV) and West Nile (WNV) are Flavivirus (Flaviviridae) probably carried by birds and transmitted by Culex mosquitoes. We show here a review on infections of the central nervous system by St. Louis Encephalitis, Rocio and West Nile Flaviviruses in Brazil, 2004-2014. In the last 10 years, serologic surveys in horses showed high proportions of seropositive animals which point out that SLEV and ROCV have circulated infecting horses in west-central, southeast and other regions of Brazil and that WNV has been introduced into Brazil and circulates mostly in Pantanal region. However humans infected by WNV have not been reported. In the State of Sao Paulo: SLEV was isolated from a case clinically diagnosed as dengue in 2004;in 2006, 6 SLEV patients including 3 cases of menigoencephalitis were reported in the middle of a large epidemic of dengue type 3;and in 2008, 1 patient with acute febrile illness that was IgM-positive for dengue was found infected by SLEV by detection of the virus genome. In 2010, ROCV genome was detected in the cerebrospinal fluids of 2 patients from the northern region with meningoenchephalitis and also AIDS. This was the first report of infections by ROCV in the last 34 years and curiously, it occured more than 2000 km from where the virus was firstly found. It is necessary to improve the surveillance of SLEV, ROCV and WNV in Brazil.展开更多
