Advanced glycation end products:Key mediator and therapeutic target of cardiovascular complications in diabetes 被引量:4

The incidence of type 2 diabetes mellitus is growing in epidemic proportions and has become one of the most critical public health concerns.Cardiovascular complications associated with diabetes are the leading cause of morbidity and mortality.The cardiovascular diseases that accompany diabetes include angina,myocardial infarction,stroke,peripheral artery disease,and congestive heart failure.Among the various risk factors generated secondary to hyperglycemic situations,advanced glycation end products(AGEs)are one of the important targets for future diagnosis and prevention of diabetes.In the last decade,AGEs have drawn a lot of attention due to their involvement in diabetic pathophysiology.AGEs can be derived exogenously and endogenously through various pathways.These are a nonhomogeneous,chemically diverse group of compounds formed nonenzymatically by condensation between carbonyl groups of reducing sugars and free amino groups of protein,lipids,and nucleic acid.AGEs mediate their pathological effects at the cellular and extracellular levels by multiple pathways.At the cellular level,they activate signaling cascades via the receptor for AGEs and initiate a complex series of intracellular signaling resulting in reactive oxygen species generation,inflammation,cellular proliferation,and fibrosis that may possibly exacerbate the damaging effects on cardiac functions in diabetics.AGEs also cause covalent modifications and cross-linking of serum and extracellular matrix proteins;altering their structure,stability,and functions.Early diagnosis of diabetes may prevent its progression to complications and decrease its associated comorbidities.In the present review,we recapitulate the role of AGEs as a crucial mediator of hyperglycemia-mediated detrimental effects in diabetes-associated complications.Furthermore,this review presents an overview of future perspectives for new therapeutic interventions to ameliorate cardiovascular complications in diabetes. 展开更多

Molybdenum-based materials have been intensively investigated for high-performance gas sensor applications.Particularly,molybdenum oxides and dichalcogenides nanostructures have been widely examined due to their tunable structural and physicochemical properties that meet sensor requirements.These materials have good durability,are naturally abundant,low cost,and have facile preparation,allowing scalable fabrication to fulfill the growing demand of susceptible sensor devices.Significant advances have been made in recent decades to design and fabricate various molybdenum oxides-and dichalcogenides-based sensing materials,though it is still challenging to achieve high performances.Therefore,many experimental and theoretical investigations have been devoted to exploring suitable approaches which can significantly enhance their gas sensing properties.This review comprehensively examines recent advanced strategies to improve the nanostructured molybdenum-based material performance for detecting harmful pollutants,dangerous gases,or even exhaled breath monitoring.The summary and future challenges to advance their gas sensing performances will also be presented. 展开更多

BACKGROUND Robotic surgery has been considered to be significantly better than laparoscopic surgery for complicated procedures.AIM To explore the short-term effect of robotic and laparoscopic spleen-preserving splenic hilar lymphadenectomy(SPSHL)for advanced gastric cancer(GC)by Huang’s three-step maneuver.METHODS A total of 643 patients who underwent SPSHL were recruited from April 2012 to July 2017,including 35 patients who underwent robotic SPSHL(RSPSHL)and 608 who underwent laparoscopic SPSHL(LSPSHL).One-to-four propensity score matching was used to analyze the differences in clinical data between patients who underwent robotic SPSHL and those who underwent laparoscopic SPSHL.RESULTS In all,175 patients were matched,including 35 patients who underwent RSPSHL and 140 who underwent LSPSHL.After matching,there were no significant differences detected in the baseline characteristics between the two groups.Significant differences in total operative time,estimated blood loss(EBL),splenic hilar blood loss(SHBL),splenic hilar dissection time(SHDT),and splenic trunk dissection time were evident between these groups(P<0.05).Furthermore,no significant differences were observed between the two groups in the overall noncompliance rate of lymph node(LN)dissection(62.9%vs 60%,P=0.757),number of retrieved No.10 LNs(3.1±1.4 vs 3.3±2.5,P=0.650),total number of examined LNs(37.8±13.1 vs 40.6±13.6,P=0.274),and postoperative complications(14.3%vs 17.9%,P=0.616).A stratified analysis that divided the patients receiving RSPSHL into an early group(EG)and a late group(LG)revealed that the LG experienced obvious improvements in SHDT and length of stay compared with the EG(P<0.05).Logistic regression showed that robotic surgery was a significantly protective factor against both SHBL and SHDT(P<0.05).CONCLUSION RSPSHL is safe and feasible,especially after overcoming the early learning curve,as this procedure results in a radical curative effect equivalent to that of LSPSHL. 展开更多

背景:梯度人工骨修复支架模拟了骨骼系统中的独特特征,在骨骼系统再生中具有巨大的应用潜力。目的:综述梯度人工骨修复支架在骨骼系统组织工程中的最新研究进展,并阐述了其优势与制造策略。方法:由第一作者检索Web of Science和PubMed... 背景:梯度人工骨修复支架模拟了骨骼系统中的独特特征,在骨骼系统再生中具有巨大的应用潜力。目的:综述梯度人工骨修复支架在骨骼系统组织工程中的最新研究进展,并阐述了其优势与制造策略。方法:由第一作者检索Web of Science和PubMed数据库2000-2023年发表的文献,英文检索词为“gradient,bone regeneration,scaffold”,最终筛选后对76篇文献进行分析总结。结果与结论:①作为骨骼系统组织高效、高质量修复的重要手段,梯度人工骨修复支架目前针对骨组织、骨-软骨、肌腱-骨组织的天然梯度特征进行了仿生设计,这些支架能够一定程度地从结构、成分上模拟原生组织的细胞外基质,从而促进细胞黏附、迁移、增殖和分化,促进受损组织向原生状态再生恢复。②先进制造技术为梯度人工骨修复支架制备提供了更多可能;目前已经开发了通过空间差异化纤维排布和生物活性物质加载构建的梯度电纺纤维支架;分层叠加、分级孔隙率与生物3D打印技术制造的梯度3D打印支架;原位分层注射、简单逐层叠加、冷冻干燥法制造的梯度水凝胶支架;另外还包括其他方式或多方法联用的支架;这些支架在体外实验中展示了良好的生物相容性,在小型动物实验中能够加速组织再生并且观察到组织学结构明显改善。③目前开发的梯度人工骨修复支架仍需进一步优化,提高在梯度尺度上的匹配性,进一步明确材料与组织相互作用,避免降解产物导致的副反应等问题,未来需要结合相关学科优势与临床需求进一步优化。 展开更多

硬纤维瘤(desmoidtumor,DT)是一种罕见的局部侵袭性肿瘤,无转移能力,但具有浸润性生长及局部复发的倾向,可严重影响患者功能,造成沉重的临床负担甚至死亡。近20年来,该病的治疗理念发生了巨变,从传统的积极手术转变为以主动监测为一线... 硬纤维瘤(desmoidtumor,DT)是一种罕见的局部侵袭性肿瘤,无转移能力,但具有浸润性生长及局部复发的倾向,可严重影响患者功能,造成沉重的临床负担甚至死亡。近20年来,该病的治疗理念发生了巨变,从传统的积极手术转变为以主动监测为一线治疗的保守策略,治疗手段也更加丰富,呈现多学科管理的发展趋势。相关研究指出DT的发病与Wnt及Notch信号通路失调有关,以nirogacestat为代表的多款针对其发病机制的新型药物正在研发中,有望成为该病未来治疗的新方向。本文就DT的分子发病机制和治疗进展进行综述,为该病的临床治疗及未来研究提供依据和方向。 展开更多

目的:探究高龄辅助生殖(ART)女性困顿感影响因素并提出应对策略.方法:采用方便抽样法,选取2022年9月-2022年12月本院门诊就诊的高龄ART女性316例,应用一般资料调查问卷、困顿感量表(ES)、不孕症患者羞辱感量表(ISS)、简易应对方式量表(S... 目的:探究高龄辅助生殖(ART)女性困顿感影响因素并提出应对策略.方法:采用方便抽样法,选取2022年9月-2022年12月本院门诊就诊的高龄ART女性316例,应用一般资料调查问卷、困顿感量表(ES)、不孕症患者羞辱感量表(ISS)、简易应对方式量表(SCSQ)、家庭韧性评估量表(FRAS)进行调查.采用多元线性逐步回归分析探讨高龄ART女性困顿感独立影响因素.结果:纳入研究的287例高龄ART女性ES评分32.15±4.79分,其中内部困顿感维度评分11.96±2.41分,外部困顿感维度评分20.19±3.58分.不同家庭人均月收入、配偶是否独生子女、对母亲身份重要性的认知、不孕治疗时间的高龄ART女性ES评分有差异;Pearson相关分析显示,高龄ART女性ISS评分、SCSQ评分中消极应对评分与ES评分呈正相关,SCSQ评分中积极应对评分、FRAS评分与ES评分呈负相关(均P<0.05).多元线性逐步回归分析显示,对母亲身份重要性的认知低、不孕治疗时间长、ISS评分高、SCSQ评分中消极应对评分高、积极应对评分低、FRAS评分低是高龄ART女性困顿感主要影响因素(均P<0.05).结论:本次调查高龄ART女性困顿感处于中等水平,影响因素多样应引起关注;建议采取积极认知、心理及家庭系统干预,改善患者生育观念、病耻感及应对方式,提高家庭韧性,从而期减轻患者困顿感. 展开更多

Locally advanced rectal cancer requires a multidisciplinary approach based on total neoadjuvant treatment with radiotherapy(RT)and chemotherapy(ChT),followed by deferred surgery.Currently,alternatives to the standard total neoadjuvant therapy(TNT)are being explored,such as new ChT regimens or the introduction of immunotherapy.With standard TNT,up to a third of patients may achieve a complete pathological response(CPR),potentially avoiding surgery.However,as of now,we lack predictive markers of response that would allow us to define criteria for a conservative organ strategy.The presence of muta-tions,genes,or new imaging tests is helping to define these criteria.An example of this is the diffusion coefficient in the diffusion-weighted sequence of magnetic resonance imaging and the integration of this imaging technique into RT treatment.This allows for the monitoring of the evolution of this coefficient over successive RT sessions,helping to determine which patients will achieve CPR or those who may require intensification of neoadjuvant therapy. 展开更多
