Assessment of intraocular pressure measured by Reichert Ocular Response Analyzer,Goldmann Applanation Tonometry,and Dynamic Contour Tonometry in healthy individuals 被引量:4

AIM: To investigate the accuracy of intraocular pressure (IOP) as measured by a Reichert Ocular Response Analyzer (ORA), as well as the relationship between central corneal thickness (CCT) and IOP as measured by ORA, Goldmann applanation tonometry (GAD, and dynamic contour tonometry (DCT). METHODS: A total of 158 healthy individuals (296 eyes) were chosen randomly for measurement of IOP. After CCT was measured using A-ultrasound (A-US), IOP was measured by ORA, GAT, and DCT devices in a randomized order. The IOP values acquired using each of the three tonometries were compared, and the relationship between CCT and IOP values were analyzed separately. Two IOP values, Goldmann-correlated IOP value (IOPg) and corneal-compensated intraocular pressure (IOPcc), were got using ORA. Three groups were defined according to CCT: 1) thin cornea (CCT<520 mu m); 2) normal-thickness cornea (CCT: 520-580 mu m); and 3), thick cornea (CCT>580 mu m) groups. RESULTS: In normal subjects, IOP measurements were 14.95 2.99mmHg with ORA (IOPg), 15.21+/- 2.77mmHg with ORA(IOPcc),1522+/- 2.77mmHg with GAT,and 15.49+/- 2.56mmHg with DCT. Mean differences were 0.01+/- 2.29mmHg between IOPcc and GAT (P >0.05) and 0.28+/- 2.20mmHg between IOPcc and DC (P >0.05). There was a greater correlation between IOPcc and DCT (r =0.946, P =0.000) than that between IOPcc and GAT (r=0.845, P=0.000). DCT had a significant correlation with GAT (r=0.854, P=0.000). GAT was moderately correlated with CCT (r=0.296, P<0.001), while IOPcc showed a weak but significant correlation with CCT (r =0.155, P =0.007). There was a strong negative correlation between CCT and the difference between IOPcc and GAT (r=-0.803, P=0.000), with every 10 m increase in CCT resulting in an increase in this difference of 0.35mmHg. The thick cornea group (CCT>580 mu m) showed the least significant correlation between IOPcc and GAT (r=0.859, 0.000); while the thin cornea group (CCT< 520 mu m) had the most significant correlation between IOPcc and GAT (r= 0.926, P =0.000). The correlated differences between IOPcc and DCT were not significant in any of the three groups (P>0.05). CONCLUSION: Measurement of IOP by ORA has high repeatability and is largely consistent with GAT measurements. Moreover, the ORA measurements are affected only to a small extent by CCT, and are likely to be much closer to the real IOP value than GAT. 展开更多

AIM:To compare intraocular pressure(IOP)measurements before and after laser in situ keratomileusis(LASIK)with a femtosecond laser for flap creation using ocular response analyzer(ORA)and Goldmann applanation tonometry,and to identify factors that may influence the preoperative and postoperative IOP.METHODS:A prospective study conducted on myopic patients who underwent LASIK using a femtosecond laser for flap fashioning.Enrolled patients were evaluated preoperatively,6 wk and 3 mo postoperatively for manifest refraction(MR),keratometric(K)readings and central corneal thickness(CCT)using a scheimpflug-based topography.Corneal resistance factor(CRF),corneal hysteresis(CH),Goldmann correlated IOP(IOPg)and corneal compensated IOP(IOPcc)were measured using ORA besides IOP assessment by Goldman applanation tonometry(GAT).RESULTS:There was a statistically significant decrease in measures of IOPg by 3.35±0.83 mm Hg,followed by GAT which decreased by 2.2±0.44 mm Hg,and the least affected by operation was IOPcc which decreased only by 0.87±0.1 mm Hg after 6 wk.After 3 mo follow up there was a statistically significant decrease in IOPcc which decreased only by 0.76±0.4 mm Hg,followed by IOP GAT by 1.6±0.5 mm Hg,and the most affected by operation was IOPg which decreased by 2.3±0.3 mm Hg.Correspondingly,there was a statistically significant decrease in CH and CRF after 6 wk and 3 mo.At 3 mo,the preoperative MR and preoperative GAT were prominent significant predictors of the postoperative GAT changes.The prediction equation was subsumed.CONCLUSION:IOP measurements and corneal biomechanical factors reduce significantly after LASIK with a femtosecond laser for flap creation.The IOPcc values are less influenced by changes in corneal properties than IOPg and GAT,indicating that IOPcc may provide the most reliable measurement of IOP after this procedure. 展开更多

Corneal neuromas,also termed microneuromas,refer to microscopic,irregula rly-shaped enlargements of terminal subbasal nerve endings at sites of nerve damage or injury.The formation of corneal neuromas results from damage to corneal nerves,such as following corneal pathology or corneal or intraocular surge ries.Initially,denervated areas of sensory nerve fibers become invaded by sprouts of intact sensory nerve fibers,and later injured axons regenerate and new sprouts called neuromas develop.In recent years,analysis of corneal nerve abnormalities including corneal neuromas which can be identified using in vivo confocal microscopy,a non-invasive imaging technique with microscopic resolution,has been used to evaluate corneal neuropathy and ocular surface dysfunction.Corneal neuromas have been shown to be associated with clinical symptoms of discomfort and dryness of eyes,and are a promising surrogate biomarker for ocular surface diseases,such as neuropathic corneal pain,dry eye disease,diabetic corneal neuropathy,neurotrophic keratopathy,Sjogren's syndrome,bullous keratopathy,post-refra ctive surgery,and others.In this review,we have summarized the current literature on the association between these ocular surface diseases and the presentation of corneal microneuromas,as well as elaborated on their pathogenesis,visualization via in vivo confocal microscopy,and utility in monitoring treatment efficacy.As current quantitative analysis on neuromas mainly relies on manual annotation and quantification,which is user-dependent and labor-intensive,future direction includes the development of artificial intelligence software to identify and quantify these potential imaging biomarkers in a more automated and sensitive manner,allowing it to be applied in clinical settings more efficiently.Combining imaging and molecular biomarkers may also help elucidate the associations between corneal neuromas and ocular surface diseases. 展开更多

The eye,a complex organ isolated from the systemic circulation,presents significant drug delivery challenges owing to its protective mechanisms,such as the blood-retinal barrier and corneal impermeability.Conventional drug administration methods often fail to sustain therapeutic levels and may compromise patient safety and compliance.Polysaccharidebased microneedles(PSMNs)have emerged as a transformative solution for ophthalmic drug delivery.However,a comprehensive review of PSMNs in ophthalmology has not been published to date.In this review,we critically examine the synergy between polysaccharide chemistry and microneedle technology for enhancing ocular drug delivery.We provide a thorough analysis of PSMNs,summarizing the design principles,fabrication processes,and challenges addressed during fabrication,including improving patient comfort and compliance.We also describe recent advances and the performance of various PSMNs in both research and clinical scenarios.Finally,we review the current regulatory frameworks and market barriers that are relevant to the clinical and commercial advancement of PSMNs and provide a final perspective on this research area. 展开更多

AIM:To explore the role of positron emission tomographycomputed tomography(PET-CT)examination in the diagnosis and treatment of ocular adnexal mucosa associated lymphoid tissue lymphoma(OAML).METHODS:The general clinical data,postoperative PET-CT results,treatment regimens,and the prognosis of 21 histopathologically confirmed OAML patients between October 2017 and September 2021 were collected.Among the 21 patients,five patients underwent surgical treatment alone,13 patients underwent surgical treatment combined with radiotherapy,and three patients underwent surgical treatment combined with chemotherapy.RESULTS:The follow-up period ranged from 8 to 79mo,with four cases of recurrence and no deaths.Through PETCT examination,two patients exhibited both local ocular metabolic elevation and systemic metastasis,and one of these patients had cervical lymph node metastasis,while the other had submandibular and parotid gland metastasis.Nine patients showed only local ocular metabolic elevation,while 10 patients had no abnormal metabolic activity locally.CONCLUSION:PET-CT examination plays a crucial role in detecting residual lesions and recurrence following tumor resection,aiding in precise disease staging,and facilitating the development of personalized treatment plans,ultimately improving patient prognosis. 展开更多

AIM:To investigate the effect of lacidipine,thiamine pyrophosphate(TPP)and the combination of lacidipine and TPP against oxidative and inflammatory eye damage induced by bilateral common carotid artery ligation in rats.METHODS:Male albino Wistar rats were categorized as those who underwent sham surgery(SG),right and left common carotid cross-clamping and unclamping procedure(CCU),lacidipine+CCU(LCCU),TPP+CCU(TCCU),and combination of lacidipine and TPP(LTC)+CCU(LTCCU).One hour before anesthesia,the LCCU(n=6)received lacidipine(4 mg/kg,orally)and the TCCU(n=6)received TPP(20 mg/kg,intraperitoneally).The SG(n=6)and CCU(n=6)received the same volume of distilled water from the same route.After anesthesia(60 mg/kg ketamine,intraperitoneally),the necks of the rats were opened in the midline.Ischemia was created for 10min by placing clips on the right and left common carotid arteries.Rats in the SG only underwent subcutaneous incision.After 10min,the clips were removed and reperfusion was achieved for six days.Then,the animals were euthanized(120 mg/kg ketamine,intraperitoneally)and the levels of oxidant,antioxidant and proinflammatory cytokines in the eye tissues were determined.The retinal tissue of the eye was also examined histopathologically.RESULTS:Lacidipine,TPP,and LTC significantly prevent the increase in malondialdehyde,tumor necrosis factoralpha,interleukin-1β(IL-1β),and IL-6 levels,decrease in total glutathione levels,superoxide dismutase and catalase activities and histopathological retinal damage in eye tissue induced by bilateral common carotid artery ligation in rats.The impact of these drugs on protection is determined to be LTC>lacidipine>TPP.CONCLUSION:As a result of the study,it is concluded that LTC may be more effective than lacidipine and TPP alone in treating ocular ischemic syndrome. 展开更多

AIM:To quantitatively measure ocular morphological parameters of guinea pig with Python technology.METHODS:Thirty-six eyeballs of eighteen 3-weekold guinea pigs were measured with keratometer and photographed to obtain the horizontal,coronal,and sagittal planes respectively.The corresponding photo pixels-actual length ratio was acquired by a proportional scale.The edge coordinates were identified artificially by ginput function.Circle and conic curve fitting were applied to fit the contour of the eyeball in the sagittal,coronal and horizontal view.The curvature,curvature radius,eccentricity,tilt angle,corneal diameter,and binocular separation angle were calculated according to the geometric principles.Next,the eyeballs were removed,canny edge detection was applied to identify the contour of eyeball in vitro.The results were compared between in vivo and in vitro.RESULTS:Regarding the corneal curvature and curvature radius on the horizontal and sagittal planes,no significant differences were observed among results in vivo,in vitro,and the keratometer.The horizontal and vertical binocular separation angles were 130.6°±6.39°and 129.8°±9.58°respectively.For the corneal curvature radius and eccentricity in vivo,significant differences were observed between horizontal and vertical planes.CONCLUSION:The Graphical interface window of Python makes up the deficiency of edge detection,which requires too much definition in Matlab.There are significant differences between guinea pig and human beings,such as exotropic eye position,oblique oval eyeball,and obvious discrepancy of binoculus.This study helps evaluate objectively the ocular morphological parameters of small experimental animals in emmetropization research. 展开更多

AIM:To assess and compare the variations and agreements across different ocular biometric parameters using swept-source optical coherence tomography(SS-OCT)and Scheimpflug tomography in patients diagnosed with cataract.METHODS:This prospective case series was conducted at Tianjin Medical University Eye Hospital.In total,212 eyes from 212 patients scheduled for phacoemulsification were included.Eyes were evaluated preoperatively using two SSOCT devices(IOLMaster700 and CASIA2)and Scheimpflug tomography(Pentacam).Central corneal thickness(CCT),anterior chamber depth(ACD),aqueous depth(AQD),white-to-white distance(WTW),flat simulated keratometry(Kf),steep simulated keratometry(Ks),mean keratometry(Km),and total corneal keratometry(TKm)were measured.Intraclass correlation coefficient(ICC),95%confidence intervals(CI)and limits of agreement(LoA)widths were conducted to assess differences and correlations between devices.RESULTS:All parameters,except for Ks,were significantly different.Pairwise comparison revealed no significant differences between keratometry obtained by IOLMaster 700 and Pentacam.LoA widths of all paired comparisons for Ks were>0.80 D.Except for WTW between IOLMaster 700 and CASIA2 and between CASIA2 and Pentacam,other Pearson’s coefficients between devices showed a strong correlation(all r>0.95).The ICC of WTW(ICC=0.438,95%CI 0.167-0.625)showed poor reliability.The reliability of CCT,ACD,and AQD was excellent(all ICC>0.95),whereas that of TKm was good(ICC=0.827,95%CI 0.221-0.939).A significant linear correlation was also observed among devices.CONCLUSION:The ocular parameters derived from the use of IOLMaster700,CASIA2,and Pentacam exhibit significant discrepancies;as such,measurements from these devices should not be deemed as interchangeable. 展开更多

AIM:To invastigate intra-and interobserver reliability of interferometry,tear meniscus height(TMH)measurement and meibography(MBG)of an ocular surface analyzer,LacryDiag(Quantel Medical,France).METHODS:Five consecutive measurements and subsequent analysis of interferometry,TMH,and MBG were recorded by two examiners using the LacryDiag.To assess intra-and interobserver reliability,we used Cohen’s kappa for categorical variables(interferometry),or intraclass correlation coefficient for continuous variables(TMH,MBG).RESULTS:Thirty eyes of 30 examinees were included.For both observers,there was excellent intraobserver reliability for MBG(0.955 and 0.970 for observer 1 and 2,respectively).Intraobserver reliability for observer 1 was substantial for interferometry(0.799),and excellent for TMH(0.863).Reliability for observer 2 was moderate for interferometry(0.535)and fair to good for TMH(0.431).Interobserver reliability was poor for interferometry(0.074)and fair to good for TMH(0.680)and MBG(0.414).CONCLUSION:LacryDiag ocular surface analyzer in our study proves to be a reliable noninvasive tool for the evaluation of TMH and MBG.As for interferometry,poor interobserver reliability,fair to good intraobserver reliability for observer 1,and moderate for observer 2,leave room for improvement. 展开更多

Mobile technology is developing significantly.Mobile phone technologies have been integrated into the healthcare industry to help medical practitioners.Typically,computer vision models focus on image detection and classification issues.MobileNetV2 is a computer vision model that performs well on mobile devices,but it requires cloud services to process biometric image information and provide predictions to users.This leads to increased latency.Processing biometrics image datasets on mobile devices will make the prediction faster,but mobiles are resource-restricted devices in terms of storage,power,and computational speed.Hence,a model that is small in size,efficient,and has good prediction quality for biometrics image classification problems is required.Quantizing pre-trained CNN(PCNN)MobileNetV2 architecture combined with a Support Vector Machine(SVM)compacts the model representation and reduces the computational cost and memory requirement.This proposed novel approach combines quantized pre-trained CNN(PCNN)MobileNetV2 architecture with a Support Vector Machine(SVM)to represent models efficiently with low computational cost and memory.Our contributions include evaluating three CNN models for ocular disease identification in transfer learning and deep feature plus SVM approaches,showing the superiority of deep features from MobileNetV2 and SVM classification models,comparing traditional methods,exploring six ocular diseases and normal classification with 20,111 images postdata augmentation,and reducing the number of trainable models.The model is trained on ocular disorder retinal fundus image datasets according to the severity of six age-related macular degeneration(AMD),one of the most common eye illnesses,Cataract,Diabetes,Glaucoma,Hypertension,andMyopia with one class Normal.From the experiment outcomes,it is observed that the suggested MobileNetV2-SVM model size is compressed.The testing accuracy for MobileNetV2-SVM,InceptionV3,and MobileNetV2 is 90.11%,86.88%,and 89.76%respectively while MobileNetV2-SVM,InceptionV3,and MobileNetV2 accuracy are observed to be 92.59%,83.38%,and 90.16%,respectively.The proposed novel technique can be used to classify all biometric medical image datasets on mobile devices. 展开更多

The therapeutic potential of Annexin A1,an important member of the Annexin superfamily,has become evident in results of experiments with multiple human systems and animal models.The anti-inflammatory and pro-resolving effects of Annexin A1 are characteristic of pathologies involving the nervous system.In this review,we initially describe the expression sites of Annexin A1,then outline the mechanisms by which Annexin A1 maintains the neurological homeostasis through either formyl peptide receptor 2 or other molecular approaches;and,finally,we discuss the neuroregenerative potential qualities of Annexin A1.The eye and the nervous system are anatomically and functionally connected,but the association between visual system pathogenesis,especially in the retina,and Annexin A1 alterations has not been well summarized.Therefore,we explain the beneficial effects of Annexin A1 for ocular diseases,especially for retinal diseases and glaucoma on the basis of published findings,and we explore present and future delivery strategies for Annexin A1 to the retina. 展开更多

In his beautiful book,Consilience:The Unity of Knowledge,the eminent biologist Edward O Wilson,advocates the need for integration and reconciliation across the sciences.He defines consilience as“literally a‘jumping together’of knowledge with a linking of facts…to create a common groundwork of explanation”.It is the premise of this paper that as much as basic biomedical research is in need of data generation using the latest available techniques–unifying available knowledge is just as critical.This involves the necessity to resolve contradictory findings,reduce silos,and acknowledge complexity.We take the cornea and the lens as case studies of our premise.Specifically,in this perspective,we discuss the conflicting and fragmented information on protein aggregation,oxidative damage,and fibrosis.These are fields of study that are integrally tied to anterior segment research.Our goal is to highlight the vital need for Wilson’s consilience and unity of knowledge which in turn should lead to enhanced rigor and reproducibility,and most importantly,to greater understanding and not simply knowing. 展开更多

AIM:To examine the disparities in macular retinal vascular density between individuals with connective tissue disease-related interstitial lung disease(CTD-ILD)and healthy controls(HCs)by optical coherence tomography angiography(OCTA)and to investigate the changes in microvascular density in abnormal eyes.METHODS:For a retrospective case-control study,a total of 16 patients(32 eyes)diagnosed with CTD-ILD were selected as the ILD group.The 16 healthy volunteers with 32 eyes,matched in terms of age and sex with the patients,were recruited as control group.The macular retina’s superficial retinal layer(SRL)and deep retinal layer(DRL)were examined and scanned using OCTA in each individual eye.The densities of retinal microvascular(MIR),macrovascular(MAR),and total microvascular(TMI)were calculated and compared.Changes in retinal vascular density in the macular region were analyzed using three different segmentation methods:central annuli segmentation method(C1-C6),hemispheric segmentation method[uperior right(SR),superior left(SL),inferior left(IL),and inferior right(IR)],and Early Treatment Diabetic Retinopathy Study(ETDRS)methods[superior(S),inferior(I),left(L),and right(R)].The data were analyzed using Version 9.0 of GraphPad prism and Pearson analysis.RESULTS:The OCTA data demonstrated a statistically significant difference(P<0.05)in macular retinal microvessel density between the two groups.Specifically,in the SRL and DRL analyses,the ILD group exhibited significantly lower surface density of MIR and TMI compared to the HCs group(P<0.05).Furthermore,using the hemispheric segmentation method,the ILD group showed notable reductions in SL,SR,and IL in the superficial retina(P<0.05),as well as marked decreases in SL and IR in the deep retina(P<0.05).Similarly,when employing the ETDRS method,the ILD group displayed substantial drops in superficial retinal S and I(P<0.05),along with notable reductions in deep retinal L,I,and R(P<0.05).In the central annuli segmentation method,the ILD group exhibited a significant decrease in the superficial retinal C2-4 region(P<0.05),whereas the deep retina showed a notable reduction in the C3-5 region(P<0.05).Additionally,there was an observed higher positive likelihood ratio in the superficial SR region and deep MIR.Furthermore,there was a negative correlation between conjunctival vascular density and both deep and superficial retinal TMI(P<0.001).CONCLUSION:Patients with CTD-ILD exhibits a significantly higher conjunctival vascular density compared to the HCs group.Conversely,their fundus retinal microvascular density is significantly lower.Furthermore,CTD-ILD patients display notably lower superficial and deep retinal vascular density in comparison to the HCs group.The inverse correlation between conjunctival vascular density and both superficial and deep retinal TMI suggests that detecting subtle changes in ocular microcirculation could potentially serve as an early diagnostic indicator for connective tissue diseases,thereby enhancing disease management. 展开更多

Ocular surface disease(OSD)can have a severe impact on patients as it can lead to visual impairment and persistent discomfort.Ocular surface reconstruction(OSR)is an approach to the management of ocular diseases that cause structural damage to the ocular surface.OSR encompasses both medical and surgical treatment options.In this review,we discuss the medical and surgical strategies used in OSR.Medical management often aims to treat tear insufficiency,inflammation,and keratinization.Surgical treatments may be employed for a variety of reasons,including failure of medical management.This may include improving the oculo-palpebral structures in order to improve lid positioning and tear film.Additional therapies focus on improving tear production,such as through salivary gland transplantation.In situations where the ocular surface is so severely damaged that there is loss of limbal stem cells,limbal stem cell transplant(LSCT)may be indicated.Other surgeries such as amniotic membrane transplant(AMT)and conjunctival flaps(CFs)can help promote corneal healing.Finally,in severe situations where the cornea is beyond salvage,corneal transplantation,such as a penetrating keratoplasty(PKP),can be considered.OSR often requires a combination of medical and surgical approaches targeted to each specific patient’s presentation in order to achieve optimal outcomes. 展开更多

Changes of corneal properties induced by laser in situ keratomileusis (LASIK) results in low inaccurate intraocular pressure (IOP) readings by Goldmann applanation tonometry (GAT). Before and after LASIK, the applied value of IOP, measured by dynamic contour tonometry (DCT) in comparison to GAT, was evaluated. Before and 1, 4 weeks after LASIK, the IOP in 30 cases (60 eyes) was measured by GAT and DCT respectively. The obtained results were statistically processed by SPSSll. 5 statistical software. The results showed that central corneal thickness (CCT) could affect GAT measurements but not DCT measurements. The comparison of IOP one and 4 weeks after LASIK revealed that the readings from GAT was separately decreased by 5.00±1.12 and 5.45±1.13 mmHg as compared with those before LASIK, while those from DCT had no significant difference. It was concluded that LASIK-induced changes of CCT could influence the accuracy of GAT measurements, but had no influence on those from DCT. DCT was more beneficial to the measurements of IOP in normal eyes and those subject to LASIK surgery. 展开更多

Purpose: The accuracy of Goldmann applanation tonometry (GAT) has been shown to depend on several biomechanical properties of the cornea. Newer tonometry devices (e.g., the Dynamic Contour Tonometer PASCAL? [DCT] and the Tono-Pen? XL [TP]) have been designed to accurately measure intraocular pressure (IOP) independent of corneal thickness (CCT) and pathology. This study investigates the influence of corneal pathologies on the accuracy of these IOP measuring devices, and compares this accuracy to that of direct intracameral IOP measurement. Methods: 8 eyes of 8 patients suffering from corneal pathologies scheduled for penetrating keratoplasty, and 10 eyes of 10 patients scheduled for cataract surgery (control group) were examined. Before surgery, the anterior chamber was cannulated at the temporal corneal limbus. In a closed system, the intraocular pressure (IOP) was directly set to 10, 20, and 30 mmHg with a manometric water column. Intraocular pressure measurements taken by GAT, DCT, and TP were compared to intracameral measurements obtained by a precision reference pressure sensor. Results: Control group: All three methods showed good agreement with the intracameral readings (mean deviation of all three devices, -0.9 mmHg). Group with corneal pathologies: The TP yielded the most exact IOP values in the group with corneal pathologies when taking all diagnoses into account. The mean deviations from the intracameral IOP measurements were -0.9 mmHg ± 3.2 mmHg (mean ± SD) for TP, -2.9 mmHg ± 3.3 mmHg for GAT, and -5.0 mmHg ± 7.9 mmHg for DCT. For bullous keratopathy, the most exact IOP readings were obtained by the TP (mean deviation -0.2 mmHg ± 3.5 mmHg). The TP and GAT devices underestimated IOP in the patients with Fuchs’ endothelial dystrophy;all 3 devices underestimated adjusted IOP after keratoplasty. DCT showed the greatest deviations from adjusted IOP in the case of non-herpetic scars. In the control group, none of the devices showed a statistically relevant dependency on CCT. Nevertheless, in the group with corneal pathologies, only TP showed no dependency on CCT. Conclusion: Our results suggest that the Tono-Pen XL? is the most accurate measurement device to determine IOP in patients with corneal pathologies, especially in patients suffering from corneal edema (bullous keratopathy). GAT yielded surprisingly exact IOP values in patients suffering from irregular corneal surface. DCT showed a high degree of deviation from the adjusted IOP, and should not be used to determine IOP in corneas with the disorders listed here. 展开更多

AIM:To assess intraocular pressure(IOP)during the daily curve of intraocular pressure(DCPo)in keratoconic eyes and compare Goldmann applanation tonometer(GAT),without and with astigmatism correction(n GAT and c GAT)and Tono-Pen AVIA(TPA)assessment methods.METHODS:Thirty-nine keratoconic eyes of 24 patients were assessed.DCPo was evaluated with five IOP measurements;four were performed with a GAT(n GAT and c GAT),and a Tono-Pen AVIA(TPA)at various times throughout the day.RESULTS:Mean IOP DCPo values(mm Hg)were:n GAT,9.9±2.6;c GAT,11.3±2.6;TPA 12.3±3.1.Mean IOP DCPo differences(mm Hg)and Spearman’s correlation coefficients were as follows:c GATc-n GAT,1.32±1.31,r_(s)=0.879(P<0.01);c GAT-TPA,-1.02±2.08,r_(s)=0.723(P<0.01);and n GAT-TPA,-2.35±2.23,r_(s)=0.730(P<0.01).Bland-Altman analysis for agreement between c GAT-TPA and n GAT-TPA mean IOP DCPo measurements revealed a mean difference of 1.02(95%CI,0.35-1.70)and 2.35(95%CI,1.62-3.07)mm Hg,respectively.Regression analysis yielded the following equation:TPA IOP=5.49+0.775×c GAT-0.015×ACD-0.299×corneal astig matism,which allowed us to infer TPA IOP values from other parameters.CONCLUSION:In keratoconic eyes,IOP peaks of DCPo measurements are identified at 6 a.m.,independent of the tonometer.The mean DCPo values are:TPA>c GAT>n GAT.IOP TPA measures are predictive of c GAT values,adjusted according to anterior chamber depth and corneal astigmatism. 展开更多

Mesenchymal stem cells have neuroprotective effects that limit damage to the retina and photoreceptors,and which may be mediated by extracellular vesicles(or exosomes)released by mesenchymal stem cells.To investigate the neuroprotective effect of extracellular vesicles derived from umbilical cord mesenchymal stem cells on glaucoma,we established rat models of chronic ocular hypertension by injecting conjunctival fibroblasts into the anterior chamber to mimic optic nerve injury caused by glaucoma.One week after injury,extracellular vesicles derived from umbilical cord-derived mesenchymal stem cells were injected into the vitreous cavity.We found that extracellular vesicles derived from mesenchymal stem cells substantially reduced retinal damage,increased the number of retinal ganglion cells,and inhibited the activation of caspase-3.These findings suggest that mesenchymal stem cell-derived extracellular vesicles can help alleviate optic nerve injury caused by chronic ocular hypertension,and this effect is achieved by inhibiting cell apoptosis. 展开更多
