Effectiveness of topical oxygen therapy in wound healing for patients with diabetic foot ulcer

Objectives:Non-healing wounds have been one of the major challenges in health care because of increased morbidity,especially for those who have diabetes mellitus.Numerous regimens are being innovated to produce an evidence-based practice that would minimize complications and promote healing.Topical oxygen therapy is an innovation in wound care that has been considered influential in the wound healing process.This intervention aims to increase the oxygen concentration in the affected limb to promote wound healing.Methods:This research applied an experimental design that targeted a total of 60 adult patients aged 45–64 years with diabetic foot ulcers.A randomized systematic sampling technique was used to allow equal chances and prevent bias.In total,30 patients in the control group received usual care for diabetic foot ulcers,and the remaining 30 patients in the experimental group received topical oxygen therapy together with standard care for diabetic foot ulcers.Subjects were assessed using the Wagner-Meggitt Wound Classification System.Results:The result proved that there was a significant difference in the wound grade of patients in the experimental group after the application of the usual wound care plus the topical oxygen therapy using Friedman's test.The control and experimental groups were compared using Mann–Whitney statistical analyses,and the results showed that there was a significant difference between the control and experimental groups after the application of topical oxygen therapy.Conclusions:Topical oxygen therapy was demonstrated to be effective to aid in the wound healing process of patients with diabetic foot ulcers.Fur ther research was recommended to improve the application of topical oxygen therapy to patients with chronic wounds and promote the wound healing process.

Review of the Therapeutic Advance of Acne Treatment

Acne vulgaris is among the most frequent chronic skin disorders in adolescents, characterized by inflammation of the pilosebaceous unit. The psychological distress due to the disfiguring is the main problem that affects the quality of life for patients suffering from acne. Drugs for the treatment of acne are one of the main contents of drug research on common skin diseases. Acne treatment has remained relatively unaltered over the last years. Still, in conjunction with the increase of bacterial resistance to antibiotics throughout the world, there is a global trend away from antibiotic monotherapy treatment. This review discussed the currently used treatment options and evaluated the issues and evidence supporting their usage for patients with acne, including drug therapy with topical, oral, hormonal therapy, and so on. Drug therapy is the most common method in the clinic. Due to the multifactorial etiology of acne vulgaris, most drugs cannot meet the treatment requirements. Therefore, the screening and development of excellent drugs are one of the key targets for treating acne in the future.

Experimental Study on Topical Treatment of Diabetic Skin Ulcers with Yi Medicine "Yi Bu A Jie(以布阿节)”Extract

Objective： To study the role and mechanism of the Yi medicine, Yi Bu A Jie （以布阿节） extract, in topical treatment of diabetic skin ulcers, with a view to finding a breakthrough natural drug for the prevention and treatment of diabetic skin ulcers. Methods： A model of diabetic skin ulcers in Kunming mice was developed. Yi Bu A Jie was extracted in a Soxhlet extractor. Two different concentrations of the extract （0.005 mg/mL and 0.01 mg/mL） were applied to the wound of diabetic skin ulcers once every 3 days, and local skin appearance and histopathological changes were observed. Resalts： The shortest healing time was 25.25 ±2.06 day with a low concentration （P=0.0037 compared with the high concentration group, 33.14＋2.21 day; P=0.0082 compared with control group, 28.21 ± 2.14 days）. The longest healing time was in the high concentration group （P=0.0025 compared with the control group）. In both groups, a large number of inflammatory neutrophil cells were exuded dudng the experimental period. In the low concentration group, capillary-rich granulation tissue and actively growing fibroblasts appeared in the wound, while there was much necrotic tissue in the high concentration group. Conclusion： Yi Bu A Jie extract has an inhibitory effect on diabetic skin ulcers in mice, and the low concentration is more suitable.

Consensus on the application of negative pressure wound therapy of diabetic foot wounds

Because China is becoming an aging society,the incidence of diabetes and diabetic foot have been increasing.Diabetic foot has become one of the main health-related killers due to its high disability and mortality rates.Negative pressure wound therapy(NPWT)is one of the most effective techniques for the treatment of diabetic foot wounds and great progress,both in terms of research and its clinical application,has been made in the last 20 years of its development.However,due to the complex pathogenesis and management of diabetic foot,irregular application of NPWT often leads to complications,such as infection,bleeding and necrosis,that seriously affect its treatment outcomes.In 2020,under the leadership of Burns,Trauma and Tissue Repair Committee of the Cross-Straits Medicine Exchange Association,the writing group for‘Consensus on the application of negative pressure wound therapy of diabetic foot wounds’was established with the participation of scholars from the specialized areas of burns,endocrinology,vascular surgery,orthopedics and wound repair.Drawing on evidence-based practice suggested by the latest clinical research,this consensus proposes the best clinical practice guidelines for the application and prognostic evaluation of NPWT for diabetic foot.The consensus aims to support the formation of standardized treatment schemes that clinicians can refer to when treating cases of diabetic foot.
