Efficacy of combining traditional Chinese medicine fumigation with Western medicine for diabetic peripheral neuropathy:A systematic review and meta-analysis 被引量:3

Objective:A systematic review and meta-analysis was conducted to evaluate the effectiveness of supplementing Western medicine with Traditional Chinese medicine(TCM)fumigation in patients with diabetic peripheral neuropathy.Methods:The China Biomedical Literature,Chinese full-text periodical,China National Knowledge Infrastructure,WanFang,PubMed,EMbase,and Cochrane Library databases were searched for relevant randomized controlled trials published from inception through May 2015.The methodological quality of eligible studies was evaluated using the Cochrane Risk Bias Assessment tool,and summarized effects were calculated using Reviewer Manager 5.1 software.Subgroup analysis was conducted based on the duration of intervention.Results:The initial search identified 312 relevant studies,of which 40 randomized controlled trials involving 3497 patients were eligible for analysis.The results indicated that TCM fumigation significantly improved the curative effects[risk ratio(RR)=1.34,95%CI(confidence interval):1.29e1.39],common peroneal motor nerve conduction velocity[standard mean difference(SMD)=2.93,95%CI:2.26e3.61],common peroneal sensory nerve conduction velocity(SMD=2.23,95%CI:1.46e3.01),and plasma viscosity(SMD=1.02,95%CI:1.35e0.69)compared to Western medicine alone(all p<0.01).A subgroup analysis indicated that the curative effects were significant after 15 days(RR=1.31,95%CI:1.21e1.42),30 days(RR=1.33,95%CI:1.26e1.40),and 60 days(RR=1.50,95%CI:1.32e1.71)of combined treatment(all p<0.01). 展开更多

The hemodynamic mechanism of rolling manipulation (RM) of traditional Chinese medical massage (TCMM) is investigated. An axisymmetrical nonlinear model and an arbitrary Lagrangian-Eulerian finite element method (ALE-FEM) with rezoning algorithm were introduced to study the viscous flow through an axisymmetrical rigid tube with axially moving stenosis to simulate the rolling manipulation. Flow rate and wall shear stress were obtained by solving complete Navier-Stokes equations numerically. The numerical results show that the stenosis moving frequency, namely the frequency of rolling manipulation, has great effect on the disturbance of flow and the wall shear stress. The stenosis coefficient, which characterizes the severity of the stenosis, another adjustable parameter in rolling manipulation, also shows the significant effect on flow rate and wall shear stress. These numerical results may provide some data that can be taken into consideration when massage is used in clinic. 展开更多

Objective:To explore the diagnostic value of traditional Chinese medical(TCM)dialectical classification in Hashimoto's thyroiditis complicated with suspicious nodules.Methods:The clinical data of patients with Hashimoto's thyroiditis complicated with thyroid nodules in the Department of Breast and thyroid surgery of Weifang Hospital of traditional Chinese Medicine from January 2018 to December 2019 were collected.The patients were examined by 2 or more experienced TCM doctors,and the four diagnostic data were obtained,and then the relevant syndrome types of the patients were judged according to the data.According to the color Doppler ultrasonographic features of thyroid nodules,the patients who met the indication of fine needle aspiration biopsy of thyroid nodules were selected and underwent fine needle aspiration biopsy of thyroid nodules before operation.To analyze the clinical diagnostic value of that the ultrasonic mode used in this study and thyroid cytopathology Bethesda report system combine dialectical classification of traditional Chinese medicine in Hashimoto's thyroiditis complicated with suspected thyroid nodules.Result:A total of 89 patients with Hashimoto's thyroiditis complicated with thyroid nodules were collected.according to the ultrasonic mode,the difference between different modes was statistically significant(P<0.05).The mode of color ultrasound is also related to the dialectical classification of traditional Chinese medicine.The patients with high malignant risk score are mainly qi depression and phlegm stagnation,phlegm and blood stasis,while those with low score are exuberant liver fire and heart liver yin deficiency.According to the study of different The Bethesda System for Reporting Thyroid Cytopathology(TBSRTC)classification,the dialectical classification of patients with higher TBSRTC classification was more inclined to qi depression and phlegm stagnation,phlegm and blood stasis,and there was significant difference between different classification(P<0.05).Conclusion:Qi depression and phlegm obstruction,phlegm and blood stasis have high ultrasound malignant risk score and high TBSRTC classification grade in patients with Hashimoto's thyroiditis complicated with suspected thyroid nodules,which has important clinical diagnostic value. 展开更多

Despite continuous progress,the prevention and treatment of human immunodeficiency virus(HIV)/acquired immune deficiency syndrome(AIDS)remain the world’s most serious public health challenges.A key problem is the degree of immune function reconstruction after antiretroviral therapy.Antiretroviral therapy has enriched the treatment of HIV/AIDS and improved the present conditions and the life quality of HIV/AIDS patients.However,some patients still fail to achieve normalization of CD4+T lymphocyte counts although persistent virological suppression.These patients are referred to as immunological non-responders,and usually present with severe immunological dysfunction.To date,since the underlying mechanism of incomplete immune reconstitution in HIV/AIDS has not been fully elucidated,remaining to be the focus and difficulties of current research.It is still a challenge to explore a safe,effective,and reliable therapeutic method for immunological non-responders.Due to fewer side effects and lower drug resistance,traditional Chinese medicine is often sought to provide alternative pharmacotherapy for regulating the immunity of immunological non-responders in China.In this review,we aimed at summarizing the latest and most comprehensive information on traditional Chinese medicine therapeutic methods for promoting immune reconstruction.In addition,outlooks and perspectives for possible future research that related are also discussed. 展开更多

Cerebral ischemia-reperfusion is a process in which the blood supply to the brain is temporarily interrupted and subsequently restored.However,it is highly likely to lead to further aggravation of pathological damage to ischemic tissues or the nervous system.,and has accordingly been a focus of extensive clinical research.As a traditional Chinese medicinal formulation,Sanhua Decoction has gradually gained importance in the treatment of cerebrovascular diseases.Its main constituents include Citrus aurantium,Magnolia officinalis,rhubarb,and Qiangwu,which are primarily used to regulate qi.In the treatment of neurological diseases,the therapeutic effects of the Sanhua Decoction are mediated via different pathways,including antioxidant,anti-inflammatory,and neurotransmitter regu-latory pathways,as well as through the protection of nerve cells and a reduction in cerebral edema.Among the studies conducted to date,many have found that the application of Sanhua Decoction in the treatment of neurological diseases has clear therapeutic effects.In addition,as a natural treatment,the Sanhua Decoction has received widespread attention,given that it is safer and more effective than traditional Western medicines.Consequently,research on the mechanisms of action and efficacy of the Sanhua Decoctions in the treatment of cerebral ischemia-reperfusion injury is of considerable significance.In this paper,we describe the pathogenesis of cerebral ischemia-reperfusion injury and review the current status of its treatment to examine the therapeutic mechanisms of action of the Sanhua Decoction.We hope that the findings of the research presented herein will contribute to a better understanding of the efficacy of this formulation in the treatment of cerebral ischemia-reperfusion,and provide a scientific basis for its application in clinical practice. 展开更多

Heat-sensitive suspended moxibustion has a neuroprotective effect against focal cerebral ischemia/reperfusion injury, but the underly- ing mechanisms remain unclear. The duration of heat-sensitive suspended moxibustion (usually from 30 minutes to 1 hour) is longer than traditional suspended moxibustion (usually 15 minutes). However, the effects of 15- and 35-minute suspended moxibustion in rats with cerebra/ischemia/reperfusion injury are poorly understood. In this study, we performed 15- or 35-minute suspended moxibustion at acupoint Dazhui (GV14) in an adult rat model of focal cerebral ischemia/reperfusion injury. Infarct volume was evaluated with the 2,3,5-triphenyltetrazolium chloride assay. Histopathological changes and neuronal apoptosis at the injury site were assessed by hematoxy- lin-eosin staining and terminal deoxynucleotidyl transferase dUTP nick end labeling assay. Caspase-9 and caspase-3 expression at the in- jury site was detected using immunofluorescent staining. Bax and Bcl-2 expression at the injury site was assessed using western blot assay. In the 35-minute moxibustion group, infarct volume was decreased, neuronal apoptosis was reduced, caspase-9, caspase-3 and Bax expres- sion was lower, and Bcl-2 expression was increased, compared with the 15-minute moxibustion group. Our findings show that 35-minute moxibustion has a greater anti-apoptotic effect than 15-minute moxibustion after focal cerebral ischemia/reperfusion injury. 展开更多

Objective: To observe the clinical efficacy of Qingqiao Capsule (清窍胶囊, QQC) in treating patients with secretory otitis media (SOM). Methods: A total of 90 patients were randomly assigned into the treated group (n:45) and the control group (n=45). Patients in the treated group were administrated with QQC, 5 capsules each time, 3 times a day for totally 10-14 days, and those in the control group were given per os cefaclor capsules 0.5g each time for adult, 3 times a day, or 20mg/(kg·d) for children, for 10-14 days. The therapeutic efficacy of treatment on the patients was observed and compared after treatment and followed up for 3-6 months. Results: (1) The clinical efficacy in the treated group was superior to that in the control group with significant statistical difference (P〈0.01); (2) Comparison of the efficacies in patients of three different TCM syndrome types (the external pathogenic wind invasion caused auditory orifice stuffiness type, the Gan-Dan damp-heat steaming up auditory orifice type and the Pi-deficiency dysfunction induced dirty dampness blocking ear type) showed no statistically significant difference(P〉0.05); (3) The vanishing rate and time needed of the main symptoms and signs in the treated group were superior to those in the control group on ear muffle, tinnitus, hearing impairment, hydrotypanum, pure tone threshold and abnormal tongue figure, and the difference was statistically significant (P〈0.05 or P〈0.01), only those of earache, otopiesis and abnornal pulse figure were insignificantly different between the two groups (P〉0.05). Conclusion: QQC is an effective Chinese composite medicine on patients with SOM, and shows no obvious adverse reaction. 展开更多

Objective To study the medication regularity of Chinese herbal medicines based on hyperuricemia patents. Methods Access database was established to summarzie the patents regarding hyperuricemia disclosured by State Intellectual Property Office(SIPO) of the People's Republic of China in 1986-2015. Statistically analysis was performed by Chinese Medicine Inheritance Support System(TCMISS). Results The literatures of Chinese herbal medicines treating hyperuricemia were 1223, and 138 papers were included by excluding and screening, and contain 282 kinds of traditional Chinese medicine. Conclusion By analyzing the frequency, efficacy, flavor, channel tropism,and composition of the herbs used, we recognize that the efficacy of herbs in curing hyperuricemia is mostly clearing heat and promoting diuresis, invigorating the circulation of blood, dispersing stasis, alleviating water retention, and reinforcing deficiency concurrently. Flavors are mostly sweet, bitter, and cold. Channel tropism is mainly to liver, spleen, stomach, lung, and kidney. It provides the medication consideration for clinical treatment of hyperuricemia. 展开更多

To observe the effects of Yifei Kangliu Yin(YFKLY) in treating non-small cell lung cancer (NSCLC). Methods:Two hundred and seventy-one patients with NSCLC were randomly divided into three groups, Group A treated only by YFKLY, Group B treated by the combination of YFKLY and chemotherapy, and Group C treated only by chemotherapy as the control group for control. Results: (1) Of the 127 cases in Group A, 1 case got complete remission (CR), 13 got partial remission (PR), 89 had no change (NC), and 24 had progression of disease (PD), thus CR+PR+NC accounting for 81.10%; of the 80 patients in Group B, 17 got PR , 53 got NC, 10 got PD, PR+NC accounting for 87.50%; of the 64 cases of chemotherapy group, 7 cases got PR, 39 cases got NC, 18 cases got PD, PR+NC accounting for 71.88% (P<0.01). (2) The metastasis rate was 23.52% in Group A, 20.00% in Group B and 35.71% in Group C respectively after treatment. (3) The 1-, 2-, 3- and 4-year survival rate were 73.09%, 32.01%, 13.18% and 13.18% in Group A, 71.85%, 46.35%, 29.19% and 23.35% in Group B and 37.61%, 13.67%, 9.7% and 0% in Group C. The symptoms were improved, and Karnofsky score was elevated in Group A and B. Conclusion: YFKLY could increase survival rate and quality of life, decrease metastasis rate, and enhance the immune function in NSCLC patients. 展开更多

This study conducted clinical observation of the treatment of noninsulin dependent dia-betes mellitus (type ) with Semen Persical decoction for purgation with addition (SPDPA) in compari-son with a control group treated with glyburide. The results of the clinical observations show the total effective rate of SPDPA on 106 cases was 79%. which was equivalent to levels in the control group, butSPDPA was more effective in relieving symptoms. The animal experimental study showed SPDPA couldreduce tasting blood sugar in diabetic patients and rats. Its mechanism may be due to improved secretionof insulin, inhibited excretion by the endocrine cell, increased endocrine pellet inhibition ot insular cellsand improved composition of hepatic glycogen. In traditional Chinese medical theory, the mechanism oftherapeutic action of SPDPA in diabetes mellftus is based on synergistic regulation benefiting the vital en-ergy and nourishing Yin, the activation of blood circulation to dissipate blood stasis and the loosening ofthe bowel to relieve constipation. 展开更多
