The Third International Meeting of Tropical Medicine and Parasitology organized by the Society of Infectious and Parasitic Diseases, Chinese Medical Association was held on April 8-10, 1994 in Kunming, Yunnan Province...The Third International Meeting of Tropical Medicine and Parasitology organized by the Society of Infectious and Parasitic Diseases, Chinese Medical Association was held on April 8-10, 1994 in Kunming, Yunnan Province, China. 178 participants from China’s mainland,5 from Taiwan, China and 24 from 16 countries attended the meeting.The president of the XVI International Congress of Tropical Medicine and Malaria,Prof.and Dr.Keizo Matsumoto was invited to attend the meeting.He offered a warm congratulatory speech at the openning ceremony. During this assembly, the Society of Tropical Medicine and Parasitology, Chinese Medical Association was established and its first session of executive council was formed,which will certainly promote the international academic communication and collaboration with those relevant foreign organizations and institutions.展开更多
Background Rabies continues to pose significant public health challenges in many developing countries including Bhutan.A probable case of rabies was admitted to our hospital and its reporting led to the uncovering of ...Background Rabies continues to pose significant public health challenges in many developing countries including Bhutan.A probable case of rabies was admitted to our hospital and its reporting led to the uncovering of an outbreak in domestic and wild animals.We discuss the challenges in the diagnosis and management of rabies in a resource-limited setting.Case presentation A 35-year-old male presented with intermittent fever,bilateral lower limb weakness that was rapidly progressive,urinary incontinence with episodes of palpitations and sweating.He had sustained a Category III bite on the right lower thigh with four bite marks,inflicted by a stray dog.He had received post-exposure prophylaxis with intra-dermal anti-rabies vaccine.On initial examination,the patient was in distress but cooperative for the interview.He had pulse rate ranging from 60 to 100/min with episodes of diaphoresis and palpitations,but with normal capillary blood glucose.In the lower limb,the muscle power was zero with absent tendon reflexes in the lower limb and impaired abdominal reflex below T10 level.He had hyperaesthesia below T8,hydrophobia,aerophobia and photophobia.He had multiple spontaneous fasciculations in both the thighs and right deltoid and these later involved the intercostal muscles,neck and face muscles.He had altered sensorium and desaturation for which he required mechanical ventilation.Polymerase chain reaction for rabies virus was negative in cerebrospinal fluid and saliva.Rabies virus neutralizing antibody was negative in cerebrospinal fluid but had high titres in the serum.He received Human Rabies Immunoglobulin after admission.He was managed in the intensive care unit and died 23 days later.After this case was notified,a rapid response team was deployed in the field,and uncovered rabies outbreak in animals in the locality.Conclusions This case called for a serious evaluation of the country’s efforts in achieving zero rabies deaths by 2030.The management of this case identified several critical areas of context-specific interventions in Bhutan.There is also an urgent need to improve diagnostic capabilities at the national reference laboratory and enhance the technical competencies of healthcare workers in the management of dog bite cases.展开更多
Background:Loiasis is an uncommon and poorly understood parasitic disease outside endemic areas of Africa.The aim of this study was to describe the clinical and biological patterns and treatment of imported loiasis by...Background:Loiasis is an uncommon and poorly understood parasitic disease outside endemic areas of Africa.The aim of this study was to describe the clinical and biological patterns and treatment of imported loiasis by sub-Saharan migrants diagnosed in Madrid,Spain.Methods:A retrospective study was conducted with sub-Saharan immigrants seen at the Tropical Medicine Unit of the Carlos III Hospital in Madrid,Spain,a reference center,over 19 years.Categorical variables were expressed as frequency counts and percentages.Continuous variables were expressed as the mean and standard deviation(SD)or median and interquartile range(IQR:Q3–Q1).Chi-square tests were used to assess the association between categorical variables.The measured outcomes were expressed as the odds ratio(OR)with a 95%confidential interval.Continuous variables were compared by Student’s t-tests or Mann-Whitney U tests.Binary logistic regression models were used.P<0.05 was considered a statistically significant difference.Results:One hundred thirty-one migrants from tropical and subtropical areas with loiasis were identified.Forty-nine patients were male(37.4%).The migrants’mean age(±SD)was 42.3±17.3 years,and 124(94.7%)were from Equatorial Guinea.The median time(IQR)between arrival in Spain and the first consultation was 2(1–7)months.One hundred fifteen migrants had eosinophilia,and one hundred thirteen had hyper-IgE syndrome.Fifty-seven patients had pruritus(43.5%),and thirty patients had Calabar swelling(22.9%).Seventy-three patients had coinfections with other filarial nematodes(54.2%),and 58 migrants had only Loa loa infections(45.8%).One hundred two patients(77.9%)were treated;45.1%(46/102)patients were treated with one drug,and 54.9%(56/102)patients were treated with combined therapy.Adverse reactions were described in 14(10.7%)migrants.Conclusions:Our patients presented early clinical manifestations and few atypical features.Thus,physicians should systematically consider loiasis in migrants with a typical presentation.However,considering that 72.5%of the patients had only positive microfilaremia without any symptoms,we suggest searching for microfilaremia in every migrant from endemic countries for loiasis presenting with eosinophilia.展开更多
