BACKGROUND There are very few cases of cardiac occluder detachment,and it is rare to completely remove the occluder using interventional methods without undergoing thoracotomy surgery after detachment.This case innova...BACKGROUND There are very few cases of cardiac occluder detachment,and it is rare to completely remove the occluder using interventional methods without undergoing thoracotomy surgery after detachment.This case innovatively used ultrasound guidance combined with digital subtraction angiography(DSA)to completely remove the occluder,accumulating some experience.CASE SUMMARY The patient underwent left atrial appendage occlusion surgery in our hospital due to atrial fibrillation.After the surgery,the occluder fell off and became free in the left ventricle,which is very dangerous.We innovatively used ultrasound guidance,combined with DSA,and interventional surgery to successfully capture the free occluder using a catcher,completely remove it,and then re implant a new left atrial appendage occluder.After the surgery,the patient recovered very well.CONCLUSION The size selection of the occluder is slightly conservative,and the shape of the left atrial appendage opening is irregular.展开更多
BACKGROUND Combined hepatocellular-cholangiocarcinoma(CHC)is a rare type of primary liver cancer.Due to its complex histopathological characteristics,the imaging features of CHC can overlap with those of hepatocellula...BACKGROUND Combined hepatocellular-cholangiocarcinoma(CHC)is a rare type of primary liver cancer.Due to its complex histopathological characteristics,the imaging features of CHC can overlap with those of hepatocellular carcinoma(HCC)and intrahepatic cholangiocarcinoma(ICC).AIM To investigate the possibility and efficacy of differentiating CHC from HCC and ICC by using contrast-enhanced ultrasound(CEUS)Liver Imaging Reporting and Data System(LI-RADS)and tumor biomarkers.METHODS Between January 2016 and December 2019,patients with histologically confirmed CHC,ICC and HCC with chronic liver disease were enrolled.The diagnostic formula for CHC was as follows:(1)LR-5 or LR-M with elevated alphafetoprotein(AFP)and carbohydrate antigen 19-9(CA19-9);(2)LR-M with elevated AFP and normal CA19-9;or(3)LR-5 with elevated CA19-9 and normal AFP.The sensitivity,specificity,accuracy and area under the receiver operating characteristic curve were calculated to determine the diagnostic value of the criteria.RESULTS After propensity score matching,134 patients(mean age of 51.4±9.4 years,108 men)were enrolled,including 35 CHC,29 ICC and 70 HCC patients.Based on CEUS LI-RADS classification,74.3%(26/35)and 25.7%(9/35)of CHC lesions were assessed as LR-M and LR-5,respectively.The rates of elevated AFP and CA19-9 in CHC patients were 51.4%and 11.4%,respectively,and simultaneous elevations of AFP and CA19-9 were found in 8.6%(3/35)of CHC patients.The sensitivity,specificity,positive predictive value,negative predictive value,accuracy and area under the receiver operating characteristic curve of the aforementioned diagnostic criteria for discriminating CHC from HCC and ICC were 40.0%,89.9%,58.3%,80.9%,76.9%and 0.649,respectively.When considering the reported prevalence of CHC(0.4%-14.2%),the positive predictive value and NPV were revised to 1.6%-39.6%and 90.1%-99.7%,respectively.CONCLUSION CHCs are more likely to be classified as LR-M than LR-5 by CEUS LI-RADS.The combination of the CEUS LI-RADS classification with serum tumor markers shows high specificity but low sensitivity for the diagnosis of CHC.Moreover,CHC could be confidently excluded with high NPV.展开更多

Methods: A total of 52 patients with anal fistula admitted to the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from December 2017 to July 2018 were selected. They were all undergoing transperineal 3D volume ultrasound combined with 2D high-frequency ultrasound examination, and the diagnosis results were analyzed. The results of ultrasonography and surgical pathology were compared. Results: Among 52 patients, 3D volume ultrasound combined with 2D high-frequency ultrasound were used to diagnose 32 cases of anal fistula intersphincteric type, 14 cases of transsphincter type, 5 cases of supra-sphincter type, and 1 case of extra-sphincter type. T supervisor classification accuracy rate is 90%. The detection rate of branch pipes was 92%, and the compliance rate of internal fistula was 95%. Two-dimensional high-frequency ultrasound was used to diagnose 34 cases of anal fistula intersphincteric type, 14 cases of transsphincter type, 4 cases of supra-sphincter type, and 0 cases of extra-sphincter type. The detection rate of branch canals was 42%, and the accuracy of type classification was 90%. The coincidence rate was 95%. BACKGROUND Synchronous combined hepatocellular-cholangiocarcinoma(CHC)and hepatocellular carcinoma(HCC)is very rare,with few literature reports and poor clinical outcomes associated with the disorder.Surgical resectio...BACKGROUND Synchronous combined hepatocellular-cholangiocarcinoma(CHC)and hepatocellular carcinoma(HCC)is very rare,with few literature reports and poor clinical outcomes associated with the disorder.Surgical resection is the main treatment,which makes the preoperative diagnosis very important.However,due to imaging manifestations overlapping with HCC,diagnosis of this type of synchronous cancer is challenging and it tends to be misdiagnosed as multiple HCC.Herein,we report the contrast-enhanced ultrasound(CEUS)manifestations of a case of synchronous CHC and HCC,aiming at adding to the understanding of this disease.CEUS displayed exquisite vascularity and tissue perfusion in real time with good spatial and temporal resolution and more accurately reflect tumor washin and washout times than contrast-enhanced computed tomography(CT)in this case.CASE SUMMARY The patient was a 69-year-old female with a 20-year history of chronic hepatitis B.Due to months of epigastric pain and anorexia,she reffered to our hospital for treatment.Five days before hospitalization,abdominal magnetic resonance imaging performed at another hospital detected a space-occupying lesion in the liver.After her hospitalization,laboratory tests showed elevated alpha-fetoprotein and carbohydrate antigen 19-9 level.Two suspicious liver lesions located in S4 and S6,respectively,were identified in a cirrhotic background by abdominal contrast-enhanced CT(CECT).Furthermore,the lesion in S4 and S6 were detected by CEUS and assigned to CEUS LI-RADS 5 and M categories,respectively.The patient underwent tumor radical resections.Post-operative pathology confirmed the S4 and S6 lesions to be HCC and CHC,respectively.A newly-found suspicious liver nodule with potential malignancy was detected in liver S1 by both CEUS and CECT 7 mo after operation.CONCLUSION The CEUS characteristics of CHC and HCC are different.CEUS features in combination with clinical information could help in effective diagnosis,clinical decision-making and better prognosis.展开更多

Current clinicallyavailable diagnostic methods are mainly based on the use of either X-rays or ultrasound(US). All X-ray based methods provide a measure of bone mineral density(BMD), but it has been demonstrated that other structural aspects of the bone are important in determining fracture risk, such as mechanical features and elastic properties, which cannot be assessed using densitometric techniques. Among the most commonly used techniques, dual X-ray absorptiometry(DXA) is considered the current 'gold standard' for osteoporosis diagnosis and fracture risk prediction. Unfortunately, as other X-ray based techniques, DXA has specific limitations(e.g., use of ionizing radiation, large size of the equipment, high costs, limited availability) that hinder its application for population screenings and primary care diagnosis. This has resulted in an increasing interest in developing reliable pre-screening tools for osteoporosis such as quantitative ultrasound(QUS) scanners, which do not involve ionizing radiation exposure and represent a cheaper solution exploiting portable and widely available devices. Furthermore, the usefulness of QUS techniques in fracture risk prediction has been proven and, with the last developments, they are also becoming a more and more reliable approach for assessing bone quality. However, the US assessment of osteoporosis is currently used only as a pre-screening tool, requiring a subsequent diagnosis confirmation by means of a DXA evaluation. Here we illustrate the state of art in the early diagnosis of this 'silent disease' and show up recent advances for its prevention and improved management through early diagnosis.展开更多

This study describes a dual method of CTS interrogation by EXD and US with the results applied to a single numerical score;the Combined Median Score (CMS). This is the product of CSA of median nerve at Carpal Tunnel inlet and CSI. In a series of 100 suspected cases, standard EDX identified 89 positive cases, CSI identified 92 positives, CMS identified 95 positives. Objective: To analyze the non-periodic, unstable and even chaotic echoes scattered from microbubbles which are extremely sensitive and may easily collapse, fragment or shrink when ultrasound contrast agents are expose...Objective: To analyze the non-periodic, unstable and even chaotic echoes scattered from microbubbles which are extremely sensitive and may easily collapse, fragment or shrink when ultrasound contrast agents are exposed to ultrasound (US) irradiation. Methods: The combined time-frequency analysis was applied to the original signals instead of the traditional Fourier spectral analysis technique. Results: The results obtained from simulation as well as experiment showed that the subharmonic, 2nd harmonic and ultra harmonic of the microbubbles occurred during the oscillation and varied with time. The dependence on the incident ultrasonic amplitude and microbubble parameters were established. Conclusion: The transient echoes backscattered from the ultrasound agent in the evaluation of the blood perfusion can be analyzed thoroughly by the technique of combined-frequency analysis and the time detail of the frequency contents can be revealed.展开更多

Lung ultrasound is a relatively simple bedside method helping early diagnosis of these conditions. </span><b><span style="font-family:Verdana;">Purpose:</span></b><span style="font-family:Verdana;"> To compare the findings of chest X-ray (CXR) and of lung ultrasound (LUS) among children with lower respiratory tract infections. </span><b><span style="font-family:Verdana;">Subjects and Methods:</span></b><span style="font-family:Verdana;"> A comparative cross-sectional study that included 100 children aged 6 months to 6 years with lower respiratory tract infections and 100 healthy age- and sex-matched children (controls). Included children were recruited from Mansoura University children hospital during the period from September 2020 to June 2021. Detailed findings of chest X-ray and lung ultrasound were compared. </span><b><span style="font-family:Verdana;">Results:</span></b><span style="font-family:Verdana;"> Among 100 patients with LRTIs, the diagnosis of pneumonia, bronchiolitis and bronchitis were reported in 41%, 36% and 23% of cases respectively. Lung ultrasound was more sensitive than chest X-ray for diagnosing pneumonia (97.6% for LUS vs. 92.7% for CXR) and bronchiolitis (91.7% for LUS vs. 77.8% for CXR). </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> Lung ultrasound is a sensitive tool for the assessment of lower respiratory tract infections. 目的探讨以问题为导向的教学法(problerm-based learning,PBL)、情景教学法联合放射学信息系统(radiography information system,RIS)在超声医学本科实习教学中的应用价值。方法选取2020年6月—2022年6月在重庆医科大学附属大学城医院...目的探讨以问题为导向的教学法(problerm-based learning,PBL)、情景教学法联合放射学信息系统(radiography information system,RIS)在超声医学本科实习教学中的应用价值。方法选取2020年6月—2022年6月在重庆医科大学附属大学城医院超声科实习的医学影像专业本科实习生48名为研究对象,随机分为观察组24名学生,对照组24名学生,察组采用PBL、情景教学法联合RIS进行教学,对照组采用传统教学法进行教学,对2组出科考核成绩(理论、操作技能)、问卷调查进行比较。结果观察组的出科成绩理论为(88.24±2.18)分,操作为(84.47±3.49)分,均高于对照组的出科成绩理论(74.48±2.32)分,操作(71.33±3.24)分,差异有统计学意义(P<0.001);观察组满意度为95.83%,对照组满意度为79.17%,差异有统计学意义(P<0.05);观察组83.33%的学生认为PBL教学法+情景教学+RIS激发了学习兴趣,87.50%的学生认为训练了临床思维能力,91.67%的学生认为培养了团队协作能力,83.33%的学生认为提高了沟通能力,而对照组54.17%学生认为传统教学激发了学习兴趣,54.17%的学生认为训练了临床思维能力,45.83%的学生认为培养了团队协作能力,50.00%的学生认为提高了沟通能力,观察组的学习兴趣、学习主动性、团队协作意识、人文关怀意识、沟通能力、理论知识掌握、技能操作熟练程度、临床思维能力方面均优于对照组,差异有统计学意义(P<0.05)。结论PBL、情景学法联合RIS在超声实习教学中不仅有助于提高学生的专业素养,还有助于提高学生的综合素质,能提高教学效果,提升学生对教学的满意度。展开更多

Results: The results obtained from simulation as well as experiment showed that the subharmonic, 2nd harmonic and ultra harmonic of the microbubbles occurred during the oscillation and varied with time. The dependence on the incident ultrasonic amplitude and microbubble parameters were established. Conclusion: The transient echoes backscattered from the ultrasound agent in the evaluation of the blood perfusion can be analyzed thoroughly by the technique of combined-frequency analysis and the time detail of the frequency contents can be revealed.
文摘<strong>Background:</strong><span style="font-family:;" "=""><span style="font-family:Verdana;"> Lower respiratory tract infections (LRTIs) are among the leading causes of morbidity and mortality among children during early childhood. Lung ultrasound is a relatively simple bedside method helping early diagnosis of these conditions. </span><b><span style="font-family:Verdana;">Purpose:</span></b><span style="font-family:Verdana;"> To compare the findings of chest X-ray (CXR) and of lung ultrasound (LUS) among children with lower respiratory tract infections. </span><b><span style="font-family:Verdana;">Subjects and Methods:</span></b><span style="font-family:Verdana;"> A comparative cross-sectional study that included 100 children aged 6 months to 6 years with lower respiratory tract infections and 100 healthy age- and sex-matched children (controls). Included children were recruited from Mansoura University children hospital during the period from September 2020 to June 2021. Detailed findings of chest X-ray and lung ultrasound were compared. </span><b><span style="font-family:Verdana;">Results:</span></b><span style="font-family:Verdana;"> Among 100 patients with LRTIs, the diagnosis of pneumonia, bronchiolitis and bronchitis were reported in 41%, 36% and 23% of cases respectively. Lung ultrasound was more sensitive than chest X-ray for diagnosing pneumonia (97.6% for LUS vs. 92.7% for CXR) and bronchiolitis (91.7% for LUS vs. 77.8% for CXR). </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> Lung ultrasound is a sensitive tool for the assessment of lower respiratory tract infections. Further steps for application and validation are recommended.</span></span>
文摘目的探讨以问题为导向的教学法(problerm-based learning,PBL)、情景教学法联合放射学信息系统(radiography information system,RIS)在超声医学本科实习教学中的应用价值。方法选取2020年6月—2022年6月在重庆医科大学附属大学城医院超声科实习的医学影像专业本科实习生48名为研究对象,随机分为观察组24名学生,对照组24名学生,察组采用PBL、情景教学法联合RIS进行教学,对照组采用传统教学法进行教学,对2组出科考核成绩(理论、操作技能)、问卷调查进行比较。结果观察组的出科成绩理论为(88.24±2.18)分,操作为(84.47±3.49)分,均高于对照组的出科成绩理论(74.48±2.32)分,操作(71.33±3.24)分,差异有统计学意义(P<0.001);观察组满意度为95.83%,对照组满意度为79.17%,差异有统计学意义(P<0.05);观察组83.33%的学生认为PBL教学法+情景教学+RIS激发了学习兴趣,87.50%的学生认为训练了临床思维能力,91.67%的学生认为培养了团队协作能力,83.33%的学生认为提高了沟通能力,而对照组54.17%学生认为传统教学激发了学习兴趣,54.17%的学生认为训练了临床思维能力,45.83%的学生认为培养了团队协作能力,50.00%的学生认为提高了沟通能力,观察组的学习兴趣、学习主动性、团队协作意识、人文关怀意识、沟通能力、理论知识掌握、技能操作熟练程度、临床思维能力方面均优于对照组,差异有统计学意义(P<0.05)。结论PBL、情景学法联合RIS在超声实习教学中不仅有助于提高学生的专业素养,还有助于提高学生的综合素质,能提高教学效果,提升学生对教学的满意度。
