Objective: To investigate the prevalence of myco-plasma infections and the sensitivity to antibiotics among patients with nongonococcal urethritis or cer-vicitis (NGU) in Chongqing. Methods: 387 NGU cases with mycopla...Objective: To investigate the prevalence of myco-plasma infections and the sensitivity to antibiotics among patients with nongonococcal urethritis or cer-vicitis (NGU) in Chongqing. Methods: 387 NGU cases with mycoplasma-positive results upon culture were analysed retrospectively. RESULTS: The majority of patients with mycoplasma infections were in the 20-40 year old age group. No significant difference was found between males and females. Ureaplasma urealyticum is the main pathogen of these NGU cases and no clear relationship between its concentration and pathogenic ability was noted. Drug sensitivity was tested against nine antibiotics; the sensitivity rates to josamycim, minocycline and doxycycline were 94.06%, 88.89% and 86.82% respectively, while the resistance rates to lincomycin, ofloxacin, azithromycin and roxthromycin were 74.94%, 42.12%, 41.60% and 40.31% in turn. Conclusions: Josamycin, minocycline and doxycycline could be used as the first choice to treat NGU with mycoplasma infections in Chongqing. It is important to select antibiotics for NGU treatment with mycoplasma infections based on the results of drug sensitivity tests.展开更多
Background: The incidence of sexually transmittedHIV infections is rapidly increasing in China, and theprevalence of AIDS-associated mycoplasmas (includingMycoplasma genitalium, Mycoplasma penetrans, Myco-plasma pirum...Background: The incidence of sexually transmittedHIV infections is rapidly increasing in China, and theprevalence of AIDS-associated mycoplasmas (includingMycoplasma genitalium, Mycoplasma penetrans, Myco-plasma pirum and Mycoplasma fermentans) infections isunknown in patients with sexually transmitted diseases.Objective: To investigate the prevalence of these 4species of AIDS-associated mycoplasmas infections inpatients with nongonococcal urethritis (NGU) andmucopurulent cervicitis (MPC).Methods: In 65 patients with NGU/MPC, detection ofM. genitalium, M. penetrans, M. pirum and M. fermentansin genital and pharyngeal specimens was performed byculture and nested polymerase chain reaction.Results: M. genitalium, M. penetrans, M. pirum and M.fermentans were identified in genital specimens from23.1% (15/65), 12.3% (8/65), 1.5% (1/65) and 0% ofpatients, respectively, and from pharyngeal samples in26.2% (17/65), 15.4% (10/65), 1.5% (1/65) and 0% ofpatients, respectively. M. genitalium was detected in bothgenital and pharyngeal samples in 10.8% (7/65) ofpatients, and M. penetrans in 4.6% (3/65) of patients. M.pirum was found in only 2 cases, and no M. fermentanswas discovered.Conclusions: This study suggests that M. genitaliumand M. penetrans infections are common in patients withNGU/MPC. M. genitalium and M. penetrans may be trans-mitted by genital-genital or oral-genital sex, and maycause urethritis and cervicitis.展开更多

Methods: Retrospective analysis of 878 nongonococcal urethritis / cervicitis cases which received regular treatment and follow-up in our institute from 1st Jan. 2001 to 31st Aug. 2003. Results: The mean cure rate of six methods for Chlamydia trachamatis (Ct) was 57.116%. There were distinct differences among these methods for Ct treatment.The mean cure rate of six methods for Ureaplasma urealyticum (Uu) was 69.556% and there was no difference among these methods for Uu treatment. Coinfection with Ct and Uu dramatically reduced the elimination rate of these two pathogens. Conclusion: The effectiveness of treatment of these antibodies for non-gonococcal urethritis / cervicitis is not currently satisfactory. Importantly, there were many antibiotic-resistant Ct and Uu strains. The factors influencing antibiotic efficacy and mechanisms need further study.展开更多

From the small beginnings of the Miller and Black period, in which microbiologists followed Koch's postulates, took the reductionist approach to try to study the complex oral microbial community by analyzing individual species; to the modem era when oral researchers embrace "holism" or "system thinking", adopt new concepts such as interspecies interaction, microbial community, biofilms, poly-microbial diseases, oral microbiological knowledge has burgeoned and our ability to identify the resident organisms in dental plaque and decipher the interactions between key components has rapidly increased, such knowledge has greatly changed our view of the oral microbial flora, provided invaluable insight into the etiology of dental and periodontal diseases, opened the door to new approaches and techniques for developing new therapeutic and preventive tools for combating oral polymicrobial diseases.展开更多

Varicella-zoster virus, which is responsible for varicella(chickenpox) and herpes zoster(shingles), is ubiquitous and causes an acute infection among children, especially those aged less than six years. As 90% of adul...Varicella-zoster virus, which is responsible for varicella(chickenpox) and herpes zoster(shingles), is ubiquitous and causes an acute infection among children, especially those aged less than six years. As 90% of adults have had varicella in childhood, it is unusual to encounter an infected pregnant woman but, if the disease does appear, it can lead to complications for both the mother and fetus or newborn. The major maternal complications include pneumonia, which can lead to death if not treated. If the virus passes to the fetus, congenital varicella syndrome, neonatal varicella(particularly serious if maternal rash appears in the days immediately before or after childbirth) or herpes zoster in the early years of life may occur depending on the time of infection. A Microbiology laboratory can help in the diagnosis and management of mother-child infection at four main times:(1) when a pregnant woman has been exposed to varicella or herpes zoster, a prompt search for specific antibodies can determine whether she is susceptible to, or protected against infection;(2) when a pregnant woman develops clinical symptoms consistent with varicella, the diagnosis is usually clinical, but a laboratory can be crucial if the symptoms are doubtful or otherwise unclear(atypical patterns in immunocompromised subjects, patients with post-vaccination varicella, or subjects who have received immunoglobulins), or if there is a need for a differential diagnosis between varicella and other types of dermatoses with vesicle formation;(3) when a prenatal diagnosis of uterine infection is required in order to detect cases of congenital varicella syndrome after the onset of varicella in the mother; and(4) when the baby is born and it is necessary to confirm a diagnosis of varicella(and its complications), make a differential diagnosis between varicella and other diseases with similar symptoms, or confirm a causal relationship between maternal varicella and malformations in a newborn.展开更多

Methods: This prospective study included 60 patients with otitis media with effusion diagnosed at the otorhinolaryngology (ENT) outpatient clinic with age ranged from 2 to 16 years, 36 males and 24 females. Results: Predisposing factors of OME were rhinosinusitis in 58.3% of cases, adenoid in 20% of cases, adenotonsillitis in 16.7% of cases and tonsillitis in 5% of cases. Microorganisms in middle ear fluids were negative in 70% of cases, isolation of streptococcus pneumonia in 16.7% of cases, Haemophilus influenzae 6.7% and Moraxella catarrhalis 6.7%. Conclusion: Rhinosinusitis was the most frequent predisposing factor of cases of OME. Positive bacterial culture was found in 30% of cases.展开更多

Rhinovirus contamination was found in the gloves, cuffs of protective wear, inner surface of biological safety cabinet (BSC) windows, and trash handles. Remarkably, high contamination was found on the inner walls of the centrifuge and the inner surface of centrifuge tube casing in the rotor. Spilling infectious medium on the surface of centrifuge tubes was found to contribute to contamination of centrifuge surfaces. Exposure to sodium hypochlorite containing no less than 0.2 g/L available chlorine decontaminated the surface of the centrifuRe tubes from Rhinovirus after 2 min.展开更多

Introduction: Gonorrhoeae and antimicrobial resistance AMR of gonococci is a major health problem today, because emerged resistance to last line empirical treatment for gonorrhoeae cephalosporins in many countries is ...Introduction: Gonorrhoeae and antimicrobial resistance AMR of gonococci is a major health problem today, because emerged resistance to last line empirical treatment for gonorrhoeae cephalosporins in many countries is predictable to be untreatable disease in near future. WHO GASP, WHO GLASS and WHO’s global action plan on AMR recommends to expand nationally and internationally to collect data to monitor AMR of gonococci for public health policies. Objective: Our aim is to detect resistance of gonococci to Cepha- losporins and determine the most effective empirical treatment for un-com- plicated gonococcal urethritis in males in Egypt. Methods: We depended in our methodology on selected gonococci from male urethral discharge specimens on Thyer Martien medium;collected 33 isolates during three years from 2017 to 2020;used antibiotics with MIC according to international standards and measuring IZD according to antimicrobial susceptibility testing reference ranges in international standards. Results: By statistical studies, resistance to cephalosporins was as follows: Cephradine 97%, Cefaclor 87.9%, Cefoxitin 97%, Ceftriaxone 90.9% and 42.4% to Cefepime, that shows hetero-genecity in resistance inside cephalosporin group;while resistance to Macrolides group represented by Azithromycin and Tetracyclins group represented by Doxycycline was as follows: Azithromycin 39.4%, Doxycycilne 27.3%;finally fluoroquinolones, the most effective group, resistance, was as: Levofloxacin 15.2%, Ciprofloxacin 15.2% and Ofloxacin 24.2%. Conclusion: The most effective empirical treatment for uncomplicated gonococcal urethritis in males in EGYPT is Fluoroquinolone;especially Levofloxacin ranks first susceptibility as 78.8% and 15.2% resistance followed by Ciprofloxacin susceptibility as 69.7% and 15.2% resistance, finally Ofloxacin susceptibility as 66.7% and 24.2% resistance;for Ceftriaxone not more recommended in EGYPT as empirical treatment for uncomplicated gonococcal urethritis, it is susceptibility as 6.1% and 90.9% resistance;in addition, we can use combination therapy of Fluoroquinolones with Azithromycin or Doxycycline, whose susceptibility is 30.3% for Azithromycin and 42.4% for Doxycycycline, while resistance is 39.4% for Azithromycin and 27.3% for Doxycycline. It is worth noting that only Cefepime in Cephalosporins group represents 42.4% susceptibility and 42.4% resistance;in addition to the Carbapenems group, it represents as 42.4% susceptibility for Imipenem and 45.5% resistance, then 42.2% susceptibility for Meropenem and 48.5% resistance, which can play role in combination therapy.展开更多
Objective:To conduct a systematic literature review on urethral calculi in a contemporary cohort describing etiology,investigation,and management patterns.Methods:A systematic search of MEDLINE and Cochrane Central Re...Objective:To conduct a systematic literature review on urethral calculi in a contemporary cohort describing etiology,investigation,and management patterns.Methods:A systematic search of MEDLINE and Cochrane Central Register of Controlled Trials(CENTRAL)databases was performed.Articles,including case reports and case series on urethral calculi published between January 2000 and December 2019,were included.Full-text manuscripts were reviewed for clinical parameters including symptomatology,etiology,medical history,investigations,treatment,and outcomes.Data were collated and analyzed with univariate methods.Results:Seventy-four publications met inclusion criteria,reporting on 95 cases.Voiding symptoms(41.1%),pain(40.0%),and acute urinary retention(32.6%)were common presenting features.Urethral calculi were most often initially investigated using plain X-ray(63.2%),with almost all radio-opaque(98.3%).Urethral calculi were frequently associated with coexistent bladder or upper urinary tract calculi(16.8%)and underlying urethral pathology(53.7%)including diverticulum(33.7%)or stricture(13.7%).Urethral calculi were most commonly managed with external urethrolithotomy(31.6%),retrograde manipulation(22.1%),and endoscopic in situ lithotripsy(17.9%).Conclusion:This unique systematic review of urethral calculi provided a summary of clinical features and treatment trends with a suggested treatment algorithm.Management in contemporary urological practice should be according to calculus size,shape,anatomical location,and presence of urethral pathology.展开更多

Resu...Purpose: A 40-year-old male with a severe corneal melting, peripheral infiltration and nasal perforation in the right eye after a not affiliated case of urethritis is reported. Methods: Retrospective case report. Results: A sample of conjunctival discharge was sent to culture, in where Gram-diplococci grew and therefore a systemic antibiotic treatment was initiated. Given the severity of the clinic, a penetrating keratoplasty is conducted two days after admission and the corneal button was sent to microbiology, which polymerase chain reaction was positive for Neisseria gonorrhoeae. Conclusion: Despite it being a rare process in an adult, keratitis caused by Neisseria gonorrhoeae should be suspected in all sexually active patients with conjunctivitis and with a clinic genitourinary medical history. The systemic therapy showed a superior effectiveness in combination with topical therapy than the last one on its own.展开更多

In particular, the Eswab system (by Copan) simplifies and streamlines specimen collection and represents the only liquid system supporting the recovery of all types of bacteria (aerobic, anaerobic, and fastidious bacteria). In addition, LBM offers advantages in the efficiency of microorganism recovery and ease of sampling, transport, and storage. LBM also allows the introduction of true automation in the laboratory: either by using Copan ? (Walk-Away Specimen Processor) or any other commercially available specimen processor that utilizes LBM. In this paper, we illustrate how LBM can positively change laboratory workflow by illustrating several years of our experience with LBM. LBM allows clinical specimen optimization and has several important advantages: cost reduction (due to the smaller number of different devices used), time savings for medical or nursing staff (less confusion in collection device selection and fewer samples being collected), time savings for laboratory staff (fewer samples to access and handle for individual investigations), and patient comfort improvement (multiple sample collection can be avoided). A unique collection device for several investigations also guarantees quality due to the uniformity of the sample and standardization of procedures.展开更多

The objective of this research, as part of the multidisciplinary project “Sustainabil...Insects or algae are considered future solutions to substitute imported feed proteins like soybean meal (SBM) in animal nutrition. The objective of this research, as part of the multidisciplinary project “Sustainability transitions”, focused on replacing 50% SBM by partly defatted larvae meal from black soldier flies (Hermetia illucens) or the blue-green algae Spirulina platensis in meat type chicken diets. The current study aimed to evaluate the effects on body composition, protein quality, apparent precaecal digestibility (apcD) and parameters of intestinal microbiota. In total, 288 one-day-old male growing chickens (Ross 308) from a commercial hatchery were randomly allotted to 48 pens (6 birds per pen) across five diets, which were fed at a free choice level. The control diet utilized SBM, wheat and corn as main ingredients, and experimental diets replaced 50% of SBM by the alternative proteins under study. Amino acid (AA) supplementation of the final diets was conducted both at a basic level (diets HM, SM;Lys and Met added equal to the control diet) and an extended level of AA fortification (diets HM+, SM+;extended supplementation of Lys, Met, Thr, Arg). After finishing the growth study, 4 individual birds per diet were slaughtered for whole body analyses to derive nutrient utilization and dietary protein quality parameters. Additionally, pooled chyme samples from 16 birds per diet (control, HM and SM) were analyzed to assess apcD and microbial parameters. Diets HM and SM with a basic level of AA fortification led to significant depressions in nutrient deposition and dietary protein quality. However, HM+ and SM+ diets with an extended level of AA supplementation led to significantly improved responses;however these were still generally not on par with the control diet. The HM+ diet provided superior CP deposition and dietary protein quality, as compared to the SM+ diet, but equal to the control diet. Results of gut microbiology yielded no significant effects due to feeding the alternative protein sources under study.展开更多

文摘The Veterinary Microbiology course is centered around the diagnosis and testing of pathogenic microorganisms,with the core value of“moral education and character development.”It reconstructs multidimensional teaching resources by integrating disciplinary achievements with clinical cases and implements a hybrid teaching approach combining virtual simulation and problem-based learning(PBL)through the“three stages+four models+three reflections”framework.Dual-qualification teachers employ various teaching methods,create a“six-in-one”model for ideological and political education,and conduct formative assessments based on the principles of diversified objectives and process emphasis.The hybrid teaching reform addresses issues such as fragmented knowledge,insufficient class hours,weak animal disease diagnostic abilities among students,limited application and expansion of knowledge points,and students’lack of proactive critical thinking skills.The application of hybrid teaching has shown significant advantages and effectiveness,providing a reference for teaching reform in similar microbiology courses.

文摘Objective: To investigate the prevalence of myco-plasma infections and the sensitivity to antibiotics among patients with nongonococcal urethritis or cer-vicitis (NGU) in Chongqing. Methods: 387 NGU cases with mycoplasma-positive results upon culture were analysed retrospectively. RESULTS: The majority of patients with mycoplasma infections were in the 20-40 year old age group. No significant difference was found between males and females. Ureaplasma urealyticum is the main pathogen of these NGU cases and no clear relationship between its concentration and pathogenic ability was noted. Drug sensitivity was tested against nine antibiotics; the sensitivity rates to josamycim, minocycline and doxycycline were 94.06%, 88.89% and 86.82% respectively, while the resistance rates to lincomycin, ofloxacin, azithromycin and roxthromycin were 74.94%, 42.12%, 41.60% and 40.31% in turn. Conclusions: Josamycin, minocycline and doxycycline could be used as the first choice to treat NGU with mycoplasma infections in Chongqing. It is important to select antibiotics for NGU treatment with mycoplasma infections based on the results of drug sensitivity tests.
文摘Background: The incidence of sexually transmittedHIV infections is rapidly increasing in China, and theprevalence of AIDS-associated mycoplasmas (includingMycoplasma genitalium, Mycoplasma penetrans, Myco-plasma pirum and Mycoplasma fermentans) infections isunknown in patients with sexually transmitted diseases.Objective: To investigate the prevalence of these 4species of AIDS-associated mycoplasmas infections inpatients with nongonococcal urethritis (NGU) andmucopurulent cervicitis (MPC).Methods: In 65 patients with NGU/MPC, detection ofM. genitalium, M. penetrans, M. pirum and M. fermentansin genital and pharyngeal specimens was performed byculture and nested polymerase chain reaction.Results: M. genitalium, M. penetrans, M. pirum and M.fermentans were identified in genital specimens from23.1% (15/65), 12.3% (8/65), 1.5% (1/65) and 0% ofpatients, respectively, and from pharyngeal samples in26.2% (17/65), 15.4% (10/65), 1.5% (1/65) and 0% ofpatients, respectively. M. genitalium was detected in bothgenital and pharyngeal samples in 10.8% (7/65) ofpatients, and M. penetrans in 4.6% (3/65) of patients. M.pirum was found in only 2 cases, and no M. fermentanswas discovered.Conclusions: This study suggests that M. genitaliumand M. penetrans infections are common in patients withNGU/MPC. M. genitalium and M. penetrans may be trans-mitted by genital-genital or oral-genital sex, and maycause urethritis and cervicitis.
文摘Objective: To analyze the efficacy of six antibiotic methods for treating non- gonococcal urethritis /cervicitis and the factors influencing efficacy. Methods: Retrospective analysis of 878 nongonococcal urethritis / cervicitis cases which received regular treatment and follow-up in our institute from 1st Jan. 2001 to 31st Aug. 2003. Results: The mean cure rate of six methods for Chlamydia trachamatis (Ct) was 57.116%. There were distinct differences among these methods for Ct treatment.The mean cure rate of six methods for Ureaplasma urealyticum (Uu) was 69.556% and there was no difference among these methods for Uu treatment. Coinfection with Ct and Uu dramatically reduced the elimination rate of these two pathogens. Conclusion: The effectiveness of treatment of these antibodies for non-gonococcal urethritis / cervicitis is not currently satisfactory. Importantly, there were many antibiotic-resistant Ct and Uu strains. The factors influencing antibiotic efficacy and mechanisms need further study.
文摘Since the initial observations of oral bacteria within dental plaque by van Leeuwenhoek using his primitive microscopes in 1680, an event that is generally recognized as the advent of oral microbiological investigation, oral microbiology has gone through phases of "reductionism" and "holism". From the small beginnings of the Miller and Black period, in which microbiologists followed Koch's postulates, took the reductionist approach to try to study the complex oral microbial community by analyzing individual species; to the modem era when oral researchers embrace "holism" or "system thinking", adopt new concepts such as interspecies interaction, microbial community, biofilms, poly-microbial diseases, oral microbiological knowledge has burgeoned and our ability to identify the resident organisms in dental plaque and decipher the interactions between key components has rapidly increased, such knowledge has greatly changed our view of the oral microbial flora, provided invaluable insight into the etiology of dental and periodontal diseases, opened the door to new approaches and techniques for developing new therapeutic and preventive tools for combating oral polymicrobial diseases.

文摘Varicella-zoster virus, which is responsible for varicella(chickenpox) and herpes zoster(shingles), is ubiquitous and causes an acute infection among children, especially those aged less than six years. As 90% of adults have had varicella in childhood, it is unusual to encounter an infected pregnant woman but, if the disease does appear, it can lead to complications for both the mother and fetus or newborn. The major maternal complications include pneumonia, which can lead to death if not treated. If the virus passes to the fetus, congenital varicella syndrome, neonatal varicella(particularly serious if maternal rash appears in the days immediately before or after childbirth) or herpes zoster in the early years of life may occur depending on the time of infection. A Microbiology laboratory can help in the diagnosis and management of mother-child infection at four main times:(1) when a pregnant woman has been exposed to varicella or herpes zoster, a prompt search for specific antibodies can determine whether she is susceptible to, or protected against infection;(2) when a pregnant woman develops clinical symptoms consistent with varicella, the diagnosis is usually clinical, but a laboratory can be crucial if the symptoms are doubtful or otherwise unclear(atypical patterns in immunocompromised subjects, patients with post-vaccination varicella, or subjects who have received immunoglobulins), or if there is a need for a differential diagnosis between varicella and other types of dermatoses with vesicle formation;(3) when a prenatal diagnosis of uterine infection is required in order to detect cases of congenital varicella syndrome after the onset of varicella in the mother; and(4) when the baby is born and it is necessary to confirm a diagnosis of varicella(and its complications), make a differential diagnosis between varicella and other diseases with similar symptoms, or confirm a causal relationship between maternal varicella and malformations in a newborn.
文摘Objective: To detect different etiological factors of otitis media with effusion (OME) and different types of microorganisms in middle ear fluids. Methods: This prospective study included 60 patients with otitis media with effusion diagnosed at the otorhinolaryngology (ENT) outpatient clinic with age ranged from 2 to 16 years, 36 males and 24 females. Results: Predisposing factors of OME were rhinosinusitis in 58.3% of cases, adenoid in 20% of cases, adenotonsillitis in 16.7% of cases and tonsillitis in 5% of cases. Microorganisms in middle ear fluids were negative in 70% of cases, isolation of streptococcus pneumonia in 16.7% of cases, Haemophilus influenzae 6.7% and Moraxella catarrhalis 6.7%. Conclusion: Rhinosinusitis was the most frequent predisposing factor of cases of OME. Positive bacterial culture was found in 30% of cases.

文摘To understand the potential causes of laboratory-acquired infections and to provide possible solutions that would protect laboratory personnel, samples from a viral laboratory were screened to determine the main sources of contamination with six subtypes of Rhinovirus. Rhinovirus contamination was found in the gloves, cuffs of protective wear, inner surface of biological safety cabinet (BSC) windows, and trash handles. Remarkably, high contamination was found on the inner walls of the centrifuge and the inner surface of centrifuge tube casing in the rotor. Spilling infectious medium on the surface of centrifuge tubes was found to contribute to contamination of centrifuge surfaces. Exposure to sodium hypochlorite containing no less than 0.2 g/L available chlorine decontaminated the surface of the centrifuRe tubes from Rhinovirus after 2 min.

文摘Introduction: Gonorrhoeae and antimicrobial resistance AMR of gonococci is a major health problem today, because emerged resistance to last line empirical treatment for gonorrhoeae cephalosporins in many countries is predictable to be untreatable disease in near future. WHO GASP, WHO GLASS and WHO’s global action plan on AMR recommends to expand nationally and internationally to collect data to monitor AMR of gonococci for public health policies. Objective: Our aim is to detect resistance of gonococci to Cepha- losporins and determine the most effective empirical treatment for un-com- plicated gonococcal urethritis in males in Egypt. Methods: We depended in our methodology on selected gonococci from male urethral discharge specimens on Thyer Martien medium;collected 33 isolates during three years from 2017 to 2020;used antibiotics with MIC according to international standards and measuring IZD according to antimicrobial susceptibility testing reference ranges in international standards. Results: By statistical studies, resistance to cephalosporins was as follows: Cephradine 97%, Cefaclor 87.9%, Cefoxitin 97%, Ceftriaxone 90.9% and 42.4% to Cefepime, that shows hetero-genecity in resistance inside cephalosporin group;while resistance to Macrolides group represented by Azithromycin and Tetracyclins group represented by Doxycycline was as follows: Azithromycin 39.4%, Doxycycilne 27.3%;finally fluoroquinolones, the most effective group, resistance, was as: Levofloxacin 15.2%, Ciprofloxacin 15.2% and Ofloxacin 24.2%. Conclusion: The most effective empirical treatment for uncomplicated gonococcal urethritis in males in EGYPT is Fluoroquinolone;especially Levofloxacin ranks first susceptibility as 78.8% and 15.2% resistance followed by Ciprofloxacin susceptibility as 69.7% and 15.2% resistance, finally Ofloxacin susceptibility as 66.7% and 24.2% resistance;for Ceftriaxone not more recommended in EGYPT as empirical treatment for uncomplicated gonococcal urethritis, it is susceptibility as 6.1% and 90.9% resistance;in addition, we can use combination therapy of Fluoroquinolones with Azithromycin or Doxycycline, whose susceptibility is 30.3% for Azithromycin and 42.4% for Doxycycycline, while resistance is 39.4% for Azithromycin and 27.3% for Doxycycline. It is worth noting that only Cefepime in Cephalosporins group represents 42.4% susceptibility and 42.4% resistance;in addition to the Carbapenems group, it represents as 42.4% susceptibility for Imipenem and 45.5% resistance, then 42.2% susceptibility for Meropenem and 48.5% resistance, which can play role in combination therapy.
文摘Objective:To conduct a systematic literature review on urethral calculi in a contemporary cohort describing etiology,investigation,and management patterns.Methods:A systematic search of MEDLINE and Cochrane Central Register of Controlled Trials(CENTRAL)databases was performed.Articles,including case reports and case series on urethral calculi published between January 2000 and December 2019,were included.Full-text manuscripts were reviewed for clinical parameters including symptomatology,etiology,medical history,investigations,treatment,and outcomes.Data were collated and analyzed with univariate methods.Results:Seventy-four publications met inclusion criteria,reporting on 95 cases.Voiding symptoms(41.1%),pain(40.0%),and acute urinary retention(32.6%)were common presenting features.Urethral calculi were most often initially investigated using plain X-ray(63.2%),with almost all radio-opaque(98.3%).Urethral calculi were frequently associated with coexistent bladder or upper urinary tract calculi(16.8%)and underlying urethral pathology(53.7%)including diverticulum(33.7%)or stricture(13.7%).Urethral calculi were most commonly managed with external urethrolithotomy(31.6%),retrograde manipulation(22.1%),and endoscopic in situ lithotripsy(17.9%).Conclusion:This unique systematic review of urethral calculi provided a summary of clinical features and treatment trends with a suggested treatment algorithm.Management in contemporary urological practice should be according to calculus size,shape,anatomical location,and presence of urethral pathology.
文摘Purpose: A 40-year-old male with a severe corneal melting, peripheral infiltration and nasal perforation in the right eye after a not affiliated case of urethritis is reported. Methods: Retrospective case report. Results: A sample of conjunctival discharge was sent to culture, in where Gram-diplococci grew and therefore a systemic antibiotic treatment was initiated. Given the severity of the clinic, a penetrating keratoplasty is conducted two days after admission and the corneal button was sent to microbiology, which polymerase chain reaction was positive for Neisseria gonorrhoeae. Conclusion: Despite it being a rare process in an adult, keratitis caused by Neisseria gonorrhoeae should be suspected in all sexually active patients with conjunctivitis and with a clinic genitourinary medical history. The systemic therapy showed a superior effectiveness in combination with topical therapy than the last one on its own.
文摘Liquid-based microbiology (LBM) is the future for the technological development of microbiology laboratories. In particular, the Eswab system (by Copan) simplifies and streamlines specimen collection and represents the only liquid system supporting the recovery of all types of bacteria (aerobic, anaerobic, and fastidious bacteria). In addition, LBM offers advantages in the efficiency of microorganism recovery and ease of sampling, transport, and storage. LBM also allows the introduction of true automation in the laboratory: either by using Copan ? (Walk-Away Specimen Processor) or any other commercially available specimen processor that utilizes LBM. In this paper, we illustrate how LBM can positively change laboratory workflow by illustrating several years of our experience with LBM. LBM allows clinical specimen optimization and has several important advantages: cost reduction (due to the smaller number of different devices used), time savings for medical or nursing staff (less confusion in collection device selection and fewer samples being collected), time savings for laboratory staff (fewer samples to access and handle for individual investigations), and patient comfort improvement (multiple sample collection can be avoided). A unique collection device for several investigations also guarantees quality due to the uniformity of the sample and standardization of procedures.

文摘Insects or algae are considered future solutions to substitute imported feed proteins like soybean meal (SBM) in animal nutrition. The objective of this research, as part of the multidisciplinary project “Sustainability transitions”, focused on replacing 50% SBM by partly defatted larvae meal from black soldier flies (Hermetia illucens) or the blue-green algae Spirulina platensis in meat type chicken diets. The current study aimed to evaluate the effects on body composition, protein quality, apparent precaecal digestibility (apcD) and parameters of intestinal microbiota. In total, 288 one-day-old male growing chickens (Ross 308) from a commercial hatchery were randomly allotted to 48 pens (6 birds per pen) across five diets, which were fed at a free choice level. The control diet utilized SBM, wheat and corn as main ingredients, and experimental diets replaced 50% of SBM by the alternative proteins under study. Amino acid (AA) supplementation of the final diets was conducted both at a basic level (diets HM, SM;Lys and Met added equal to the control diet) and an extended level of AA fortification (diets HM+, SM+;extended supplementation of Lys, Met, Thr, Arg). After finishing the growth study, 4 individual birds per diet were slaughtered for whole body analyses to derive nutrient utilization and dietary protein quality parameters. Additionally, pooled chyme samples from 16 birds per diet (control, HM and SM) were analyzed to assess apcD and microbial parameters. Diets HM and SM with a basic level of AA fortification led to significant depressions in nutrient deposition and dietary protein quality. However, HM+ and SM+ diets with an extended level of AA supplementation led to significantly improved responses;however these were still generally not on par with the control diet. The HM+ diet provided superior CP deposition and dietary protein quality, as compared to the SM+ diet, but equal to the control diet. Results of gut microbiology yielded no significant effects due to feeding the alternative protein sources under study.
