BACKGROUND With the increasing trend of vaginal birth after caesarean delivery(VBAC),evaluation of the feasibility and safety of a second VBAC with grand multiparity is worth considering.Intrapartum uterine rupture is diagnosed in approximately one-fifth of all VBAC cases following successful vaginal delivery.To our knowledge,no report is available on the application of laparoscopy to repair postpartum uterine rupture after a successful second VBAC in China.CASE SUMMARY A 31-year-old woman(gravida 5,para 2)at 39 wk and 5 d of gestation was admitted to the hospital in labour.After a successful VBAC and observation for approximately 13 h,the patient complained of progressive abdominal pain.Given the symptoms,signs,and auxiliary examination results,intraperitoneal bleeding was considered.Because the patient was stable and ultrasound imaging was the only method available to assess the possibility of rupture,we recommended laparoscopy to clarify the diagnosis and for prompt laparoscopic uterine repair or exploratory laparotomy if necessary.Operative findings included transverse uterine scar rupture at the lower uterine segment of approximately 5.0 cm in length and 800 mL of intraoperative pelvic haemoperitoneum.Finally,she successfully underwent laparoscopic repair of uterine rupture and recovered very well according to three-dimensional magnetic resonance imaging at 42 d postpartum.CONCLUSION Routine postpartum intrauterine exploration is not beneficial to the mother and may even increase the risk of rupture.This case highlights a laparoscopic approach for repairing uterine rupture in the immediate postpartum period. 展开更多

BACKGROUND Uterine rupture is a serious obstetric emergency,a severe event,and a serious threat to maternal and fetal life.It is a rare and not well characterized by the fact that multiple operations of uterine cavities contribute to uterine rupture during pregnancy.Atypical uterine rupture is easily misdiagnosed as other obstetric or surgical diseases.In current guidelines,abdominocentesis is a contraindication for late pregnancy.Therefore,the cases presented in this report provide new ideas for clinical diagnosis and treatment of uterine rupture.CASE SUMMARY Case 1,a 34-year-old woman(gravida 5,para 2),32 wk and 4 d of gestation,presented with acute upper abdominal pain for 8 h with nausea and vomiting.Computed tomography(CT)revealed pelvic and abdominal effusion.We extracted 3 mL unclotted blood from her abdominal cavity.An emergency caesarean section was performed.A uterine rupture was found,and the fimbrial portion of the left fallopian tube was completely adhered to the rupture.The prognosis of both the mother and the infant was good.Case 2,a 39-year-old woman(gravida 10,para 1)at 34 wk and 3 d of gestation complained of persistent lower abdominal pain for half a day.Her vital signs were normal.CT revealed a high probability of pelvic and abdominal hemoperitoneum.We extracted 4 mL dark red blood without coagulation.An emergency laparotomy was performed.Uterine rupture was identified during the operation.Postoperative course in both the mother and infant was uneventful.CONCLUSION For pregnant women in the second or the third trimester with persistent abdominal pain,abdominal effusion,fetal distress and even fetal death,the possibility of uterine rupture should be highly suspected.CT can identify acute abdominal surgical or gynecological and obstetric diseases.Abdominocentesis is helpful for diagnosing and clarifying the nature of effusion,but its clinical value need to be confirmed by further clinical studies. 展开更多

Indeed, it is a Affection obstetrical condition whose maternal-fetal prognosis is poor in terms of morbidity and mortality. </span><span style="font-family:Verdana;"><strong>Objectives:</strong></span><span style="font-family:Verdana;"> To assess the risk factors for uterine rupture in the Koutiala Health District. </span><span style="font-family:Verdana;"><strong>Methods:</strong></span><span style="font-family:Verdana;"> This was a descriptive and analytical cross-sectional prospective collection study from January 1, 2019 to December 31, 2019, a 12-month period. In our study, all patients admitted to the maternity ward were included during the study period for which the diagnosis of uterine rupture was made. </span><span style="font-family:Verdana;"><strong>Results:</strong></span><span style="font-family:Verdana;"> Out of a total of deliveries, we recorded 27 cases of uterine rupture, a frequency of 1.04%. The average age of our patients was 32 years with extremes of 19 and 45 years. We notice 92.6% of uterine ruptures during the transfer. Almost 3/4 of our parturients were out of school 70.4% versus 11.1% in primary school and 18.5% in secondary school. The majority of patients affected by uterine rupture came from rural areas 85.2%. Only 14.8% were from Koutiala city. The admission time to the Koutiala Reference Health Centre was 2 hours 30 minutes in 50.85% of our patients with extremes of 15 minutes and 4 hours 30</span><span style="font-family:" color:windowtext;font-weight:normal;"=""> </span><span style="font-family:Verdana;">min. The average parity was 6.30 - 3036;large </span><span style="font-family:Verdana;">multiparous</span><span style="font-family:Verdana;"> accounted for 63% in our sample followed by </span><span style="font-family:Verdana;">multiparous</span><span style="font-family:Verdana;"> 18.5%. As for pregnancy follow-up, 100% of the patients had not performed any prenatal consultations. In 85.2% hysterrhaphy was performed intervention and hysterectomy in 14.8%. Maternal prognosis was satisfactory in 96.30% of cases. From the point of view of morbidity: 1 case of bladder-vaginal fistula was recorded and corrected by the bladder survey at home for 15 days. Late complications were the occurrence of anemia in 16 patients who were transfused and 1 case of phlebitis. Measurements of the Lower Uterine Segment at Term in Women with Previous Cesarean Delivery

Purpose: To evaluate the accuracy of sonographic measurements of the lower uterine segment (LUS) thickness at term in predicting uterine scar defects in women with previous Cesarean delivery (CD). Methods: Eighty-nine pregnant women who underwent CD between 37 and 41 weeks of gestation from 2013 to 2015 were enrolled in this study and divided into two groups. Group A consisted of women with previous CD, and Group B consisted of women with previous vaginal deliveries. We performed an ultrasound evaluation of the myometrial and full thickness of LUS (mLUS and fLUS) transvaginally before a CD and evaluated the appearance of LUS during surgery, which was defined as follows: grade I, well-developed;grade II, thin without visible content;grade III, translucent with visible content;and grade IV, either dehiscence or rupture. Results: The median mLUS and fLUS were 1.50 and 4.07 mm in the group A, and 2.75 and 5.37 mm in the group B. We observed significant differences in the median mLUS and fLUS between grades I/II (2.07 and 4.37 mm) and grades III/IV (0.67 and 2.52 mm). Both mLUS and fLUS were predictive factors for grades III/IV and cutoff values were 0.97 mm of mLUS and 3.13 mm of fLUS, having a sensitivity of 87.5% and 75.0%, and a specificity of 87.7% and 91.4% in mLUS and fLUS measurement, respectively. Conclusion: Sonographic measurements of LUS at term may be a feasible and reliable method to predict uterine rupture or uterine dehiscence in women with prior CD. 展开更多

BACKGROUND Mifepristone-induced abortion(MIA)has been used worldwide to terminate pregnancies.However,the association between placenta accrete(PA)and MIA has seldom been reported.CASE SUMMARY A 26-year-old pregnant woman presented with painless vaginal bleeding at 35 wk of gestation.She had a medical abortion(mifepristone followed by misoprostol)1 year ago at the sixth week of gestation.Her personal history for previous surgery was negative.Abdominal ultrasonography showed a normal foetus with complete placenta previa.The foetal membrane ruptured with massive vaginal bleeding and severe abdominal pain.An emergency Caesarean section was performed,and the newborn was delivered.The placenta failed to expel and manual extraction was carried out.A large defect was noted in the uterine fundus and repair of the uterine rupture was conducted immediately.The postoperative pathology report showed placenta accreta.CONCLUSION The evidence suggests a possible etiologic role of MIA in PA,as the incidence of PA after MIA is much higher than general population.Millions of pregnancies are complicated by PA each year,some of which result in fatality.To prevent subsequent placental complications after MIA,hormonal supplementation might be a promising therapeutic options.However,further studies are needed to identify the high-risk factors and to confirm the effectiveness of estrogen supplement therapy. 展开更多

Background: The perilous path of the African woman’s pregnancy often ends in difficult situations putting her life in danger. One of the solutions to save her is often to “remove” her uterus, in a hemorrhage and emergency context. It is the hemostasis hysterectomy. It’s generally practiced in difficult conditions with complicated operative follow-ups. The tragedy is that finally the patient will die (almost 1 time out of 2 in our study). This is the sad reality of resource-limited countries. How many women will continue to die under these conditions? This study attempts to explore the different aspects of this reality. Objective: To study the epidemiological, clinical, and therapeutic aspects of hemostasis hysterectomies at the Teaching Hospital of Souro Sanou University Hospital in Burkina Faso. Methodology: This was a prospective cross-sectional descriptive study conducted over 12 months from January 1 to December 31, 2015. Included in our study were all patients received in our department and who required a total or subtotal hysterectomy in the management of a severe postpartum hemorrhage, during the study period. Results: The frequency of hemostasis hysterectomies was 0.4% compared to all deliveries. The average age was 29.65 years old. The average parity was 4.3. Multiparas and large multiparas accounted for 65%. The main indications were uterine rupture (40%) and uterine inertia (60%). Hysterectomy was subtotal in 95% of cases. Maternal morbidity was dominated in 90% of cases by hemorrhagic shock, hemostatic disorders, and obstetrical fistulas. The prognosis was poor, with 8 deaths or 40% of cases. Conclusion: Better management of pregnancy and childbirth would reduce cases of hemostasis hysterectomies. The availability of blood products would improve the maternal prognosis. 展开更多

Background:The rate of cesarean delivery has significantly increased in China in the last decade.Women with prior cesarean history tend to have a higher risk of uterine rupture during termination of the pregnancy in mid-trimester than those without such a history.The aim of our study was to evaluate the influences of the potential risk factors on uterine rupture in women with prior cesarean.Methods:We conducted this retrospective study of women with prior cesarean section,who underwent mid-trimester pregnancy termination between January 2006 and December 2013 in Peking Union Medical College Hospital.The protocol was oral administration ofmifepristone and misoprostol for the patients with the gestational ages below 16 weeks or intra-amniotic injection of ethacridine lactate (EL) for those with at least 16 weeks of gestational ages.The thickness of the lower uterine segment (LUS) was measured before the termination of pregnancy.Logistic regression was used to study the risk factors of uterine rupture.Results:The total rate of successful abortion was 93.9% (62/66).Four patients failed in induction,and one of them received curettage,whereas the other three experienced uterine rupture (4.5%).The successful rates of abortion were 85.7% (30/35) for women treated with mifepristone-misoprostol and 86.1% (31/36) for those treated with EL.There was a significant difference in the mean LUS thickness between the uterine rupture group (3.0 ± 2.0 mm) and the nonrupture group (7.0 ± 3.0 mm) (P < 0.05).The LUS thickness of<3 mm was associated with uterine rupture during mid-trimester pregnancy termination in women with prior cesarean (odds ratio,94.0; 95% confidence interval 4.2-2106.1) after adjusted maternal age,gestational age,interdelivery interval and prior cesarean section.Severe bleeding that required transfusion occurred in one case (1.5%).Conclusions:Both the mifepristone-misoprostol and the EL regimens were effective and safe for the termination of mid-trimester pregnancy in women with prior cesarean.A thinner LUS is associated with a relatively high risk of uterine rupture. 展开更多
