The association with a number of factors (maternal age, low level of education, low socio-economic status and multiple sexual partners, etc.) that can be found in our environment suggest its probably high prevalence amongst vaginal infections that are responsible for adverse obstetrical outcomes, but up-to-date estimates are lacking. Objective: To assess the obstetrical risk associated with Trichomonas vaginalis (T. vaginalis) infection in our environment. Methods: We designed a protocol for a prospective cohort study which will take place in four medical facilities in the city of Kinshasa, where all pregnant women with a pregnancy of at least 20 weeks and who will give written consent will be included. Vaginal swab specimens will be collected for T. vaginalis research by direct microscopy wet mount. Follow-up will consist of recording the process of the pregnancy and obstetrical outcomes. Conclusion: Results from this study will allow to enhance management and also bring updated estimates on T. vaginalis prevalence and its obstetrical outcomes for infected pregnant woman in our environment.展开更多
Background:Bacterial vaginosis(BV),a lower genital tract syndrome,has been linked to adverse pregnancy outcomes and is regarded as the prevalent type of vaginal infection in females of childbearing age.Objectives:To d...Background:Bacterial vaginosis(BV),a lower genital tract syndrome,has been linked to adverse pregnancy outcomes and is regarded as the prevalent type of vaginal infection in females of childbearing age.Objectives:To determine the accuracy of Nugent scoring and Gardnerella vaginalis culture in the diagnosis of bacterial vaginosis(BV)among pregnant women attending antenatal clinic in Nnamdi Azikiwe University Teaching Hospital,Nnewi,Nigeria.Method:This cross-sectional study evaluated biospecimen from 333 pregnant women enrolled through systematic random sampling technique.Biospecimens of vaginal discharge were tested for BV infection using Amsel’s criteria,Nugent’s score and culture of G.Vaginalis.Using Amsel’s criteria as a“gold standard”,the Nugent’s score and culture of G.vaginalis were estimated.Results:Prevalence of 26.12%,25.82% and 28.20% of BV was found using Amsel criteria,Nugent’s method and culture of G.vaginalis,respectively.No statistical relationship exists between socio-demographic characteristics and BV(P>0.05).Sexual exposure,and vaginal hygienic practices influences BV(P<0.05)infection and also with fishy odor during or after sexual intercourse,Gardnerella morphotypes,Bacteroides morphotypes and BV(P<0.05).An inverse relationship existed between lactobacilli morphotypes and BV.The prevalence of HIV was 5.41% and 16 out of 18 had BV diagnosed using Amsel criteria.Nugent method correlated strongly with Amsel criteria(P<0.05).In comparison with Amsel criteria,it had 78.16% sensitivity,92.68% specificity,79.07% positive predictive value,92.31% negative predictive value and 88.89% accuracy rate.This was in contrast distinction to the culture of G.vaginalis,which had 56.32%sensitivity,81.70% specificity,52.13% positive predictive value,84.10% negative predictive value and 75.08% accuracy rate.Conclusion:Nugent method correlated strongly with Amsel criteria(P<0.05)and had 78.16% sensitivity,92.68% specificity,79.07% positive predictive value,92.31% negative predictive value and 88.89% accuracy rate.Contrariwise,the culture of G.vaginalis had 56.32% sensitivity,81.70% specificity,52.13% positive predictive value,84.10% negative predictive value and 75.08% accuracy rate for BV diagnosis.展开更多
Objective:The surgical repair of hypospadias is done in two stages in a select group of patients with severe anomaly.The first stage(Ⅰ)procedure consists of correction of penile shaft curvature and second stage(Ⅱ)re...Objective:The surgical repair of hypospadias is done in two stages in a select group of patients with severe anomaly.The first stage(Ⅰ)procedure consists of correction of penile shaft curvature and second stage(Ⅱ)repair involves the creation of a neourethra.This neourethra needs a cover of an intermediate layer in order to have good functional and cosmetic results.Among the various local flaps,tunica vaginalis flap is a good option for the use as an intermediate layer.Methods:We have managed 22 patients of chordee with hypospadias by staged repair.In Stage I,chordee correction was done by dividing the urethral plate and covering the penile shaft with dorsal prepucial flaps.In Stage Ⅱ,a neourethra was created and covered with tunica vaginalis flap either through the same incision(14/22)or via a subcutaneous tunnel(8/22).An indwelling catheter was kept for 10 to 12 days.Results:Eighteen(81.8%)patients had successful functional and cosmetic repair.Two patients(9.1%)had urethrocutaneous fistula of which one healed on subsequent dilatation while the other one(4.5%)needed repair.Overall fistula formation rate was 4.5%.In two patients,the external urinary meatus could be made upto subglanular or coronal level.Conclusion:Staged repair of chordee with hypospadias is valuable in selected group of patients and tunica vaginalis flap is an excellent intermediate layer to cover the neourethra.However preoperative counseling is particularly essential in patients where the external urinary meatus can be created at coronal or subglanular level.展开更多

Chlamydia trachomatis (CT) and Trichomonas vaginalis (TV) are the most frequent curable STI in adolescents, causing serious consequences for their reproductive health. Therefore, we aimed to determine the prevalence and incidence of CT and TV, as well as their risk factors in pregnant adolescents from Belém, northern Brazilian Amazon. Methods: This prospective study enrolled 199 adolescents up to 20 weeks of pregnancy. They were scheduled for follow-up visit between 28 and 29 weeks of pregnancy. Sociodemographic and behavioral data were obtained by interview. Cervicovaginal samples were taken to test for TV, CT, Neisseria gonorrhoeae and bacterial vaginosis. Univariate and multivariate analyses were performed to test the association of prevalent/incident CT and TV with the variables. Results: Prevalence of cervical CT infection was 33.7% (n = 67/ 199), and for trichomoniasis it was 4.0% (n = 8/199). Cervical ectopy increased the risk for prevalent CT (OR, 1.93;95% CI, 1.01 - 3.70), while having treated vaginal discharge in the past (OR, 0.51;95% CI, 0.26 - 0.98) and being married (OR, 0.10;95% CI, 0.01 - 0.83) were protective against current CT and TV, respectively. Among the 95 (47.7%) adolescents who completed follow-up, 15 cases of incident CT were identified. Incident CT was associated with having a formal or informal job (OR, 28.4;95% CI, 2.1 - 391.6) and bacterial vaginosis treatment at the baseline (OR, 0.08;95% CI, 0.01 - 0.69). Conclusion: Prevalence and incidence rates of TV and CT are high in this population devoid of STI routine screening. Treatment of bacterial vaginosis may benefit this population by reducing risk for CT acquisition.展开更多

Conclusions:This is the first description of biological variability in clonal stock of Trichomonas vaginalis.Different degrees of drug susceptibility were determined among clonal strains tested. Further studies will be necessary to ascertain the importance of this variability in clinical infection.展开更多

Vaginal swabs collected from the patients suspected for BV were subjected to clinical BV diagnosis, isolation and identification of G. vaginalis, metronidazole susceptibility testing, vaginolysin and sialidase gene PCR, Piot's biotyping and amplified ribosomal DNA restriction analysis genotyping.Results: Among the 207 patients suspected for BV, G. vaginalis was isolated from 47 subjects. G. vaginalis coexisted with Trichomonas vaginalis and Candida albicans in 26 samples. Three G. vaginalis isolates were resistant to metronidazole. Biotyping revealed 1 and 7 as the common types. Amplified ribosomal DNA restriction analysis genotype II was found to be more common(n = 22; 46%) than I(n = 12; 25.53%) and III(n = 13;27.6%). All genotype I and III isolates carried the sialidase gene, while 91.6% and 84.6%contained the vaginolysin gene. Genotype I was significantly associated with postgynaecological surgical complications and abortions(P = 0.002).Conclusions: The existence of pathogenic G. vaginalis clones in Malaysia including drug resistant strains should not be taken lightly and needs to be monitored as these may bring more complications especially among women of child bearing age and pregnant women.展开更多
BACKGROUND Gardnerella vaginalis(G.vaginalis)is a facultative anaerobic bacteria known to cause bloodstream infections.However,cases are very rare in clinics.There is very limited clinical experience in the treatment ...BACKGROUND Gardnerella vaginalis(G.vaginalis)is a facultative anaerobic bacteria known to cause bloodstream infections.However,cases are very rare in clinics.There is very limited clinical experience in the treatment of bloodstream infections caused by G.vaginalis.Therefore,there is an urgent need for effective antibacterial drugs to treat patients with bloodstream infections caused by G.vaginalis.CASE SUMMARY A woman who underwent a cesarean section presented with a sudden onset of high fever 1-d post-surgery.The blood cultures suggested an infection due to G.vaginalis,and treatment with cefoperazone-sulbactam was started.After 5 d of treatment,there was a decrease in the hemogram;however,the temperature and C-reactive protein levels remained high.Based on clinical experience and a review of literature,the treatment was modified to include ornidazole in combination with cefoperazone-sulbactam.Following a week of treatment,the temperature,hemogram and C-reactive protein levels returned to normal,and blood cultures turned negative,suggesting a therapeutic effect of the combination treatment.CONCLUSION This case highlighted the effective use of cefoperazone-sulbactam combined with ornidazole for bloodstream infection caused by G.vaginalis following a cesarean section.展开更多
BACKGROUND Metastases from pancreas or ampullary malignancies are common,but the spread to testicle and paratesticular tissue is exceedingly rare.To the best of our knowledge,fewer than 30 cases have been reported in ...BACKGROUND Metastases from pancreas or ampullary malignancies are common,but the spread to testicle and paratesticular tissue is exceedingly rare.To the best of our knowledge,fewer than 30 cases have been reported in the literature.More rarely,metastasis to tunica vaginalis testis occurs without involvement of the testes and epididymis.CASE SUMMARY A 65-year-old male who complained of painless swelling of the left scrotum for over 1 wk was referred to the Department of Urology.Scrotal ultrasound showed left testicular hydrocele with paratesticular masses.Chest computed tomography revealed lung metastasis and enlarged left supraclavicular lymph node.The blood tumor markersalpha-fetoprotein,human chorionic gonadotropin,and serum lactate dehydrogenase were withinnormal limits.The preoperative diagnosis was left testicular tumor with lung metastasis.Then radical orchidectomy of the left testicle and high ligation of the spermatic cord were performed,and postoperative histopathology suggested metastatic tumors that was confirmed by an abdominal computed tomographic scan.The positive computed tomography findings,in conjunction with the expression of cytokeratin 7(CK7),CK20,CK5/6,and absence of expression of Wilms’tumor suppressor gene 1,calretinin,melanocyte,prostate-specific antigen,thyroid transcription factor-1,GATA binding protein 3,caudal type homeobox 2,and napsinA supported the diagnosis of pancreatic adenocarcinoma.The outcome of this patient was unsatisfactory,and he died 3 mo later.CONCLUSION This case suggests that pancreatic metastatic carcinoma must be considered in the differential diagnosis of scrotal enlargement.The advanced age of the patient wassuggestive of a secondary testicular tumor.In addition,careful physical examination and ultrasonography as well as radiological examination have become a standard modality.展开更多

In this work anticancer activity of three live protozoan parasites;Toxoplasma gondii, Trichomonas vaginalis and Leishmania major on fibrosarcoma cells growth has been investigated in cell culture medium. In this experimental study, WEHI-164 fibrosarcoma cells treated with alive Toxoplasma gondii tachyzoite, Trichomonas vaginalis trophozoite or Leishmania major promastigote as case groups or left intact as control groups. Following 24 hours incubation the number of cells, lactate dehydrogenase (LDH) and apoptosis were determined in case and control groups. Toxoplasma gondii tachyzoite decreased cell proliferation and increased cell lyses’ but it did not induce apoptosis. Trichomonas vaginalis or Leishmania major didn’t show any effects on cell proliferation, cell lyses or apoptosis. Therefore Toxoplasma gondii may have anticancer activity and further works is recommended to understand the mechanisms of action.展开更多

Direct wet mount microscopy and Giemsa staining techniques were used on High Vaginal Swab (HVS) specimens collected on sterile swab sticks. A prevalence of 20 (57.1%) was recorded using both methods;12 (7.2%) in PHC and 08 (6.2%) in EHC Mbo local government area, Akwa Ibom State respectively. Differences among pregnant and non-pregnant women used were statistically significant (P < 0.05). Women in the age group 15 - 19 years had the highest prevalence of infection 03 (10.5%), closely followed by those of age group 20 - 24 years 6 (6.8%). A total number of 14 (6.7%) infections were recorded among married women followed by single women with 06 (10%). A higher prevalence rate of 01 (24%) was recorded among women who attended Quranic level of education and 04 (16.7%) with uneducated women, this may be due to small sample size within this group of women. 12 (6.5%) in women with primary school education and 03 (4.7%) in women with secondary school level of education. The need for improved personal hygiene in IDP camps and other effective intervention programmes among these vulnerable groups of women is advocated.展开更多
This study aimed to investigate the incidence of patent processus vaginalis(PPV)in pediatric patients with a unilateral nonpalpable testis and explore the associated factors.From May 2014 to April 2017,152 boys who we...This study aimed to investigate the incidence of patent processus vaginalis(PPV)in pediatric patients with a unilateral nonpalpable testis and explore the associated factors.From May 2014 to April 2017,152 boys who were diagnosed with a unilateral nonpalpable testis and underwent laparoscopy in Shanghai Children’s Hospital(Shanghai,China)were included in this study.The data were collected and reviewed,and the results were analyzed regarding the age at operation,side,development,and position of the nonpalpable testis.The mean age of the patients was 2.6(standard deviation:2.3)years.The testis was absent in 14 cases,nonviable in 81 cases,and viable in 57 cases.The incidence of PPV was 37.5%(57 of 152)on the ipsilateral side and 16.4%(25 of 152)on the contralateral side.The ipsilateral PPV was more prevalent when the nonpalpable testis occurred on the right side(P<0.01).Besides,patients with a viable testis had a greater incidence of ipsilateral PPV than those with a nonviable or absent testis(P<0.01).Moreover,this rate was the highest when the testis was in the abdominal cavity and the lowest when the testis was in the scrotum(both P<0.01).However,the incidence of contralateral PPV was independent of all the factors.In conclusion,in children with a nonpalpable testis,the incidence of an ipsilateral PPV was significantly related to the side,development,and position of the testis,while it was independent of these factors on the contralateral side.展开更多

Trichomonas vaginalis is an excellent model system for transposon study since its genome (- 160 Mb) has been sequenced and is composed of - 65% transposons and other repetitive elements. In this study, we primarily report the identification of Kolobok-type transposons (termed tvBac) in T. vaginalis and the results of transposase sequence analysis. We categorized 24 novel subfamilies of the Kolobok element, including one autonomous subfamily and 23 non-autonomous subfamilies. We also identified a novel H2CH motif in tvBac transposases based on multiple sequence alignment. In addition, we supposed that tvBac and Mutator transposons may have evolved independently from a common ancestor according to our phylogenetic analysis. Our results provide basic information for the understanding of the function and evolution of tvBac transposons in particular and other related transposon families in general.展开更多

The incidence of contralateral PPV on laparoscopy in previous studies was reported as 20%-50%, but it is believed to approach 100% in premature infants. It has been reported that 4%-34% patients have the risk of developing subsequent metachronous contralateral hernia after a repair of an ipsilateral hernia.Iu If this is true, then the identification and closure of a PPV at the time of ipsilateral hernia repair will obviate the need for a second operation in thousands of children every year. With recent advances in minimally invasive surgery, transinguinal diagnostic laparoscopy has emerged as a safe and effective method for evaluating a pediatric patient with PPV. Meanwhile, the laparoscopy has been widely used to manage the infantile hypertrophic pyloric stenosis (IHPS), due to equally high success rate, minimal complications, and a shorter hospital stay.展开更多

These people were referred to one of the laboratories in town by gynecologist. After preparing the checklist of underlying data, vaginal wet smear samples was prepared to assess andChi-square in terms ofTrichomonas vaginalis(T. vaginalis) infection was examine. And results were analyzed with statistical tests. Results: 11.1% of referred casese were 30-40 years old, and about 54.8% of referred were illiterate and primitive. The frequency of trichomoniasis was 11.1% and statistically there was no significant difference. There was no significant difference among the age, education level, method of contraception, marriage age, location, employment status and the number of delivery withT. vaginalis.Conclusion:T. vaginalis compared to other studies have a moderate prevalence among women of Urmia. Because of the concordance of this infection with other sexual transmitted diseases, it is recommended that further studies of the prevalence of these diseases in Urmia be done.展开更多

Objective: To verify whether Thymus vulgaris also has antiprotozoal activity against Trichomonas vaginalis, Giardia lamblia and Entamoeba histolytica trophozoites. Materials and methods: Conventional cultures of parasites were measured on the third day during the logarithmic growth phase. The antiprotozoal activity of the methanol extract and its fractions were evaluated comparing growth in cultures with and without extracts. Next, the extract was fractionated by polarity-based partitioning. Then, the purity of each fraction was determined by thin layer chromatography (TLC). The percentage of growth inhibition was calculated with respect to untreated controls. The 50% inhibitory concentration (IC50) of each extract was calculated by PROBIT analysis. Results: We found that a methanol extract of the aerial parts of Thymus vulgaris, at 300 μg/mL, inhibited the in vitro growth of G. lamblia and T. vaginalis, while E. histolytica growth was poorly inhibited. The methanol extract was further separated into mixtures of ursolic, oleanolic, and betulinic acids. The IC50 values of ursolic acid against G. lamblia and T. vaginalis were 8.12 μg/mL and 5.51 μg/mL, respectively. Conclusions: The methanol extract fraction containing ursolic acid obtained from Thymus vulgaris has antiprotozoal activity against G. lamblia and T. vaginalis trophozoites.展开更多

O...The authors report a cross-sectional descriptive study over 6 years, from December 8th, 2003 to October 27th, 2009, involving 911 women involved in the sex trade within the Yèrêlon group in Bobo-Dioulasso. Objectives: To describe the epidemiological, clinical and etiological aspects of vaginal discharge in women in the Yèrêlon group of the city Bobo-Dioulasso. Results: Frequency of vaginal discharge was 48.89%. The mean age was 28.4 years with extremes of 16 and 54 years. Single, divorced and widowed women accounted for 78.8% of the study population. Women who never attended school and those with primary education accounted for 74.3% of the study population. HIV serology was positive in 38.4% of cases. The main symptoms were genital itching, dyspareunia and urinary signs. The main germs identified in the laboratory were Candida albicans, Trichomonas vaginalis and Gardenerella vaginalis. Conclusion: vaginal discharge was found in 48.89% of women in the Yèrêlon group of Bobo-Dioulasso. The clinical study of the discharge and the laboratory results allowed a diagnosis and a better management of the leucorrhea in the group Yèrêlon.展开更多

In view of the association of BV with obstetric/gynaecological complications and high rate of HIV transmission, it is desirable to know the prevalence of BV in pregnant women. Knowledge of risk factors will also help to design preventive measures. Methodology: A prospective study was performed to determine the prevalence of BV among pregnant women. High vaginal swabs (HVS) were collected and smears made were Gram stained and scored by the Nugent method. A predesigned questionnaire was also used to obtain information on possible risk factors. Associations between categorical variables were assessed using chi square test and fisher’s exact test as appropriate. Result: Three hundred married women aged between 18 and 45 years were studied. The prevalence of BV was 64.3%, 25% had intermediate and 10.7% had negative Nugent scores. BV rates were high across all age groups, educational background, ethnicity, parity, HIV status, and occupation. The occurrence of BV was higher (75.5%) among gestational age group ≤ 30 weeks (p 0.05) and in women who engaged in unskilled labour (p 0.05). Conclusion: Bacterial vaginosis is a very common condition among the studied population. This high prevalence may necessitate routine screening and treatment during antenatal care in preventing the associated adverse effects of BV during pregnancy and childbirth.展开更多
文摘Background: Trichomoniasis is the most common non-viral sexually transmitted infection (STI) in the world, and the occurrence of this infection during pregnancy is responsible for adverse obstetrical outcomes like premature labor, premature rupture of membranes (PROM) and low birth weight (birth weight < 2500 g). The association with a number of factors (maternal age, low level of education, low socio-economic status and multiple sexual partners, etc.) that can be found in our environment suggest its probably high prevalence amongst vaginal infections that are responsible for adverse obstetrical outcomes, but up-to-date estimates are lacking. Objective: To assess the obstetrical risk associated with Trichomonas vaginalis (T. vaginalis) infection in our environment. Methods: We designed a protocol for a prospective cohort study which will take place in four medical facilities in the city of Kinshasa, where all pregnant women with a pregnancy of at least 20 weeks and who will give written consent will be included. Vaginal swab specimens will be collected for T. vaginalis research by direct microscopy wet mount. Follow-up will consist of recording the process of the pregnancy and obstetrical outcomes. Conclusion: Results from this study will allow to enhance management and also bring updated estimates on T. vaginalis prevalence and its obstetrical outcomes for infected pregnant woman in our environment.
文摘Background:Bacterial vaginosis(BV),a lower genital tract syndrome,has been linked to adverse pregnancy outcomes and is regarded as the prevalent type of vaginal infection in females of childbearing age.Objectives:To determine the accuracy of Nugent scoring and Gardnerella vaginalis culture in the diagnosis of bacterial vaginosis(BV)among pregnant women attending antenatal clinic in Nnamdi Azikiwe University Teaching Hospital,Nnewi,Nigeria.Method:This cross-sectional study evaluated biospecimen from 333 pregnant women enrolled through systematic random sampling technique.Biospecimens of vaginal discharge were tested for BV infection using Amsel’s criteria,Nugent’s score and culture of G.Vaginalis.Using Amsel’s criteria as a“gold standard”,the Nugent’s score and culture of G.vaginalis were estimated.Results:Prevalence of 26.12%,25.82% and 28.20% of BV was found using Amsel criteria,Nugent’s method and culture of G.vaginalis,respectively.No statistical relationship exists between socio-demographic characteristics and BV(P>0.05).Sexual exposure,and vaginal hygienic practices influences BV(P<0.05)infection and also with fishy odor during or after sexual intercourse,Gardnerella morphotypes,Bacteroides morphotypes and BV(P<0.05).An inverse relationship existed between lactobacilli morphotypes and BV.The prevalence of HIV was 5.41% and 16 out of 18 had BV diagnosed using Amsel criteria.Nugent method correlated strongly with Amsel criteria(P<0.05).In comparison with Amsel criteria,it had 78.16% sensitivity,92.68% specificity,79.07% positive predictive value,92.31% negative predictive value and 88.89% accuracy rate.This was in contrast distinction to the culture of G.vaginalis,which had 56.32%sensitivity,81.70% specificity,52.13% positive predictive value,84.10% negative predictive value and 75.08% accuracy rate.Conclusion:Nugent method correlated strongly with Amsel criteria(P<0.05)and had 78.16% sensitivity,92.68% specificity,79.07% positive predictive value,92.31% negative predictive value and 88.89% accuracy rate.Contrariwise,the culture of G.vaginalis had 56.32% sensitivity,81.70% specificity,52.13% positive predictive value,84.10% negative predictive value and 75.08% accuracy rate for BV diagnosis.
文摘Objective:The surgical repair of hypospadias is done in two stages in a select group of patients with severe anomaly.The first stage(Ⅰ)procedure consists of correction of penile shaft curvature and second stage(Ⅱ)repair involves the creation of a neourethra.This neourethra needs a cover of an intermediate layer in order to have good functional and cosmetic results.Among the various local flaps,tunica vaginalis flap is a good option for the use as an intermediate layer.Methods:We have managed 22 patients of chordee with hypospadias by staged repair.In Stage I,chordee correction was done by dividing the urethral plate and covering the penile shaft with dorsal prepucial flaps.In Stage Ⅱ,a neourethra was created and covered with tunica vaginalis flap either through the same incision(14/22)or via a subcutaneous tunnel(8/22).An indwelling catheter was kept for 10 to 12 days.Results:Eighteen(81.8%)patients had successful functional and cosmetic repair.Two patients(9.1%)had urethrocutaneous fistula of which one healed on subsequent dilatation while the other one(4.5%)needed repair.Overall fistula formation rate was 4.5%.In two patients,the external urinary meatus could be made upto subglanular or coronal level.Conclusion:Staged repair of chordee with hypospadias is valuable in selected group of patients and tunica vaginalis flap is an excellent intermediate layer to cover the neourethra.However preoperative counseling is particularly essential in patients where the external urinary meatus can be created at coronal or subglanular level.

Finally, the biomembrane loosed and fractured.The cell nuclei presented the karyorrhexis.

文摘Background: Adolescents are disproportionally affected by sexually transmitted infections (STI). Chlamydia trachomatis (CT) and Trichomonas vaginalis (TV) are the most frequent curable STI in adolescents, causing serious consequences for their reproductive health. Therefore, we aimed to determine the prevalence and incidence of CT and TV, as well as their risk factors in pregnant adolescents from Belém, northern Brazilian Amazon. Methods: This prospective study enrolled 199 adolescents up to 20 weeks of pregnancy. They were scheduled for follow-up visit between 28 and 29 weeks of pregnancy. Sociodemographic and behavioral data were obtained by interview. Cervicovaginal samples were taken to test for TV, CT, Neisseria gonorrhoeae and bacterial vaginosis. Univariate and multivariate analyses were performed to test the association of prevalent/incident CT and TV with the variables. Results: Prevalence of cervical CT infection was 33.7% (n = 67/ 199), and for trichomoniasis it was 4.0% (n = 8/199). Cervical ectopy increased the risk for prevalent CT (OR, 1.93;95% CI, 1.01 - 3.70), while having treated vaginal discharge in the past (OR, 0.51;95% CI, 0.26 - 0.98) and being married (OR, 0.10;95% CI, 0.01 - 0.83) were protective against current CT and TV, respectively. Among the 95 (47.7%) adolescents who completed follow-up, 15 cases of incident CT were identified. Incident CT was associated with having a formal or informal job (OR, 28.4;95% CI, 2.1 - 391.6) and bacterial vaginosis treatment at the baseline (OR, 0.08;95% CI, 0.01 - 0.69). Conclusion: Prevalence and incidence rates of TV and CT are high in this population devoid of STI routine screening. Treatment of bacterial vaginosis may benefit this population by reducing risk for CT acquisition.

文摘Objective:To produce comparative data on a group of Trichomonas vaginalis clonal strains with varied drug responses using identical methods and materials.Methods:Five clonal strains of Trichomonas vaginalis were isolated from reference strain using agar plate technique.The variability of growth kinetic and susceptibility of clonal strain to metronidazole,tinidazole, satranidazole and nitazoxanide were observed in 96 well microlilre plate.Results:Among these clonal strains there was a good correlation between rates of growth with the relative susceptibility of the strains to drugs in vitro.Regarding metronidazole,tinidazole and satranidazole susceptibility,different degrees of susceptibility were determined.However,no difference in nitazoxanide susceptibility was found between the clonal strain tested and a reference strain. Conclusions:This is the first description of biological variability in clonal stock of Trichomonas vaginalis.Different degrees of drug susceptibility were determined among clonal strains tested. Further studies will be necessary to ascertain the importance of this variability in clinical infection.

文摘Objective: To update the status of Gardnerella vaginalis(G. vaginalis) as a causative agent of bacterial vaginosis(BV) in Malaysia and to define its epidemiology, metronidazole resistance and virulence properties.Methods: It is a single-centre(Gynaecology clinic at the Hospital Kuala Lumpur,Malaysia) prospective study with laboratory-based microbiological follow up and analyses. Vaginal swabs collected from the patients suspected for BV were subjected to clinical BV diagnosis, isolation and identification of G. vaginalis, metronidazole susceptibility testing, vaginolysin and sialidase gene PCR, Piot's biotyping and amplified ribosomal DNA restriction analysis genotyping.Results: Among the 207 patients suspected for BV, G. vaginalis was isolated from 47 subjects. G. vaginalis coexisted with Trichomonas vaginalis and Candida albicans in 26 samples. Three G. vaginalis isolates were resistant to metronidazole. Biotyping revealed 1 and 7 as the common types. Amplified ribosomal DNA restriction analysis genotype II was found to be more common(n = 22; 46%) than I(n = 12; 25.53%) and III(n = 13;27.6%). All genotype I and III isolates carried the sialidase gene, while 91.6% and 84.6%contained the vaginolysin gene. Genotype I was significantly associated with postgynaecological surgical complications and abortions(P = 0.002).Conclusions: The existence of pathogenic G. vaginalis clones in Malaysia including drug resistant strains should not be taken lightly and needs to be monitored as these may bring more complications especially among women of child bearing age and pregnant women.
文摘BACKGROUND Gardnerella vaginalis(G.vaginalis)is a facultative anaerobic bacteria known to cause bloodstream infections.However,cases are very rare in clinics.There is very limited clinical experience in the treatment of bloodstream infections caused by G.vaginalis.Therefore,there is an urgent need for effective antibacterial drugs to treat patients with bloodstream infections caused by G.vaginalis.CASE SUMMARY A woman who underwent a cesarean section presented with a sudden onset of high fever 1-d post-surgery.The blood cultures suggested an infection due to G.vaginalis,and treatment with cefoperazone-sulbactam was started.After 5 d of treatment,there was a decrease in the hemogram;however,the temperature and C-reactive protein levels remained high.Based on clinical experience and a review of literature,the treatment was modified to include ornidazole in combination with cefoperazone-sulbactam.Following a week of treatment,the temperature,hemogram and C-reactive protein levels returned to normal,and blood cultures turned negative,suggesting a therapeutic effect of the combination treatment.CONCLUSION This case highlighted the effective use of cefoperazone-sulbactam combined with ornidazole for bloodstream infection caused by G.vaginalis following a cesarean section.

文摘BACKGROUND Metastases from pancreas or ampullary malignancies are common,but the spread to testicle and paratesticular tissue is exceedingly rare.To the best of our knowledge,fewer than 30 cases have been reported in the literature.More rarely,metastasis to tunica vaginalis testis occurs without involvement of the testes and epididymis.CASE SUMMARY A 65-year-old male who complained of painless swelling of the left scrotum for over 1 wk was referred to the Department of Urology.Scrotal ultrasound showed left testicular hydrocele with paratesticular masses.Chest computed tomography revealed lung metastasis and enlarged left supraclavicular lymph node.The blood tumor markersalpha-fetoprotein,human chorionic gonadotropin,and serum lactate dehydrogenase were withinnormal limits.The preoperative diagnosis was left testicular tumor with lung metastasis.Then radical orchidectomy of the left testicle and high ligation of the spermatic cord were performed,and postoperative histopathology suggested metastatic tumors that was confirmed by an abdominal computed tomographic scan.The positive computed tomography findings,in conjunction with the expression of cytokeratin 7(CK7),CK20,CK5/6,and absence of expression of Wilms’tumor suppressor gene 1,calretinin,melanocyte,prostate-specific antigen,thyroid transcription factor-1,GATA binding protein 3,caudal type homeobox 2,and napsinA supported the diagnosis of pancreatic adenocarcinoma.The outcome of this patient was unsatisfactory,and he died 3 mo later.CONCLUSION This case suggests that pancreatic metastatic carcinoma must be considered in the differential diagnosis of scrotal enlargement.The advanced age of the patient wassuggestive of a secondary testicular tumor.In addition,careful physical examination and ultrasonography as well as radiological examination have become a standard modality.
文摘It has been shown that some parasites interfere with malignant tumor cells growth in vivo or in vitro. In this work anticancer activity of three live protozoan parasites;Toxoplasma gondii, Trichomonas vaginalis and Leishmania major on fibrosarcoma cells growth has been investigated in cell culture medium. In this experimental study, WEHI-164 fibrosarcoma cells treated with alive Toxoplasma gondii tachyzoite, Trichomonas vaginalis trophozoite or Leishmania major promastigote as case groups or left intact as control groups. Following 24 hours incubation the number of cells, lactate dehydrogenase (LDH) and apoptosis were determined in case and control groups. Toxoplasma gondii tachyzoite decreased cell proliferation and increased cell lyses’ but it did not induce apoptosis. Trichomonas vaginalis or Leishmania major didn’t show any effects on cell proliferation, cell lyses or apoptosis. Therefore Toxoplasma gondii may have anticancer activity and further works is recommended to understand the mechanisms of action.
文摘A total of 282 outpatient pregnant antenatal and non-pregnant women from internally displaced camps attending the Poly Health Clinic (PHC) and the Enwang Health Centre (EHC) were examined for Trichomonas vaginalis infection. Direct wet mount microscopy and Giemsa staining techniques were used on High Vaginal Swab (HVS) specimens collected on sterile swab sticks. A prevalence of 20 (57.1%) was recorded using both methods;12 (7.2%) in PHC and 08 (6.2%) in EHC Mbo local government area, Akwa Ibom State respectively. Differences among pregnant and non-pregnant women used were statistically significant (P < 0.05). Women in the age group 15 - 19 years had the highest prevalence of infection 03 (10.5%), closely followed by those of age group 20 - 24 years 6 (6.8%). A total number of 14 (6.7%) infections were recorded among married women followed by single women with 06 (10%). A higher prevalence rate of 01 (24%) was recorded among women who attended Quranic level of education and 04 (16.7%) with uneducated women, this may be due to small sample size within this group of women. 12 (6.5%) in women with primary school education and 03 (4.7%) in women with secondary school level of education. The need for improved personal hygiene in IDP camps and other effective intervention programmes among these vulnerable groups of women is advocated.

文摘This study aimed to investigate the incidence of patent processus vaginalis(PPV)in pediatric patients with a unilateral nonpalpable testis and explore the associated factors.From May 2014 to April 2017,152 boys who were diagnosed with a unilateral nonpalpable testis and underwent laparoscopy in Shanghai Children’s Hospital(Shanghai,China)were included in this study.The data were collected and reviewed,and the results were analyzed regarding the age at operation,side,development,and position of the nonpalpable testis.The mean age of the patients was 2.6(standard deviation:2.3)years.The testis was absent in 14 cases,nonviable in 81 cases,and viable in 57 cases.The incidence of PPV was 37.5%(57 of 152)on the ipsilateral side and 16.4%(25 of 152)on the contralateral side.The ipsilateral PPV was more prevalent when the nonpalpable testis occurred on the right side(P<0.01).Besides,patients with a viable testis had a greater incidence of ipsilateral PPV than those with a nonviable or absent testis(P<0.01).Moreover,this rate was the highest when the testis was in the abdominal cavity and the lowest when the testis was in the scrotum(both P<0.01).However,the incidence of contralateral PPV was independent of all the factors.In conclusion,in children with a nonpalpable testis,the incidence of an ipsilateral PPV was significantly related to the side,development,and position of the testis,while it was independent of these factors on the contralateral side.
文摘Trichomoniasis and HPV infection are common non-viral and viral sexually transmitted diseases worldwide.Emerging evidence shows that the female genital tract and its microbiota are greatly affected by these pathogens.However,the relationship between Trichomonas vaginalis,the vaginal microbiome,and High-risk(HR)-HPV infection is complex and multifaceted.Studies have proven that concurrent infections of HIV and HPV increase the risk of cervical cancer.With this basis,a question arises:How does the concurrent infection of trichomonas vaginalis and HPV affect genital tract health?Does this concurrent infection enhance or inhibit the development of cervical lesions?This review aims to bring light to these questions.This review also covers the association of trichomonas vaginalis and HPV with the microbiota of the genital tract.

文摘Transposons are sequence elements widely distributed among genomes of all three kingdoms of life, providing genomic changes and playing significant roles in genome evolution. Trichomonas vaginalis is an excellent model system for transposon study since its genome (- 160 Mb) has been sequenced and is composed of - 65% transposons and other repetitive elements. In this study, we primarily report the identification of Kolobok-type transposons (termed tvBac) in T. vaginalis and the results of transposase sequence analysis. We categorized 24 novel subfamilies of the Kolobok element, including one autonomous subfamily and 23 non-autonomous subfamilies. We also identified a novel H2CH motif in tvBac transposases based on multiple sequence alignment. In addition, we supposed that tvBac and Mutator transposons may have evolved independently from a common ancestor according to our phylogenetic analysis. Our results provide basic information for the understanding of the function and evolution of tvBac transposons in particular and other related transposon families in general.
文摘INTRODUCTIONAlthough laparoscopic herniorrhaphy is the most common procedure performed by pediatric surgeons at present, herniorrhaphy with an asymptomatic patent processus vaginalis (PPV) remains controversial. The incidence of contralateral PPV on laparoscopy in previous studies was reported as 20%-50%, but it is believed to approach 100% in premature infants. It has been reported that 4%-34% patients have the risk of developing subsequent metachronous contralateral hernia after a repair of an ipsilateral hernia.Iu If this is true, then the identification and closure of a PPV at the time of ipsilateral hernia repair will obviate the need for a second operation in thousands of children every year. With recent advances in minimally invasive surgery, transinguinal diagnostic laparoscopy has emerged as a safe and effective method for evaluating a pediatric patient with PPV. Meanwhile, the laparoscopy has been widely used to manage the infantile hypertrophic pyloric stenosis (IHPS), due to equally high success rate, minimal complications, and a shorter hospital stay.
文摘Objective:To obtain the frequency of trichomoniasis and molecular diagnosis among women referred to a laboratory in Urmia (north west Iran).Methods: In the cross sectional study during the year 2014-2015, women with complaints of abnormal discharge, itching, irritation and unpleasant odor were studied using convenience sampling. These people were referred to one of the laboratories in town by gynecologist. After preparing the checklist of underlying data, vaginal wet smear samples was prepared to assess andChi-square in terms ofTrichomonas vaginalis(T. vaginalis) infection was examine. And results were analyzed with statistical tests. Results: 11.1% of referred casese were 30-40 years old, and about 54.8% of referred were illiterate and primitive. The frequency of trichomoniasis was 11.1% and statistically there was no significant difference. There was no significant difference among the age, education level, method of contraception, marriage age, location, employment status and the number of delivery withT. vaginalis.Conclusion:T. vaginalis compared to other studies have a moderate prevalence among women of Urmia. Because of the concordance of this infection with other sexual transmitted diseases, it is recommended that further studies of the prevalence of these diseases in Urmia be done.
文摘Introduction: Thymus vulgaris is used in traditional medicine to treat gastrointestinal diseases because of its antifungal, antibacterial, and antispasmodic activity. Objective: To verify whether Thymus vulgaris also has antiprotozoal activity against Trichomonas vaginalis, Giardia lamblia and Entamoeba histolytica trophozoites. Materials and methods: Conventional cultures of parasites were measured on the third day during the logarithmic growth phase. The antiprotozoal activity of the methanol extract and its fractions were evaluated comparing growth in cultures with and without extracts. Next, the extract was fractionated by polarity-based partitioning. Then, the purity of each fraction was determined by thin layer chromatography (TLC). The percentage of growth inhibition was calculated with respect to untreated controls. The 50% inhibitory concentration (IC50) of each extract was calculated by PROBIT analysis. Results: We found that a methanol extract of the aerial parts of Thymus vulgaris, at 300 μg/mL, inhibited the in vitro growth of G. lamblia and T. vaginalis, while E. histolytica growth was poorly inhibited. The methanol extract was further separated into mixtures of ursolic, oleanolic, and betulinic acids. The IC50 values of ursolic acid against G. lamblia and T. vaginalis were 8.12 μg/mL and 5.51 μg/mL, respectively. Conclusions: The methanol extract fraction containing ursolic acid obtained from Thymus vulgaris has antiprotozoal activity against G. lamblia and T. vaginalis trophozoites.
文摘The authors report a cross-sectional descriptive study over 6 years, from December 8th, 2003 to October 27th, 2009, involving 911 women involved in the sex trade within the Yèrêlon group in Bobo-Dioulasso. Objectives: To describe the epidemiological, clinical and etiological aspects of vaginal discharge in women in the Yèrêlon group of the city Bobo-Dioulasso. Results: Frequency of vaginal discharge was 48.89%. The mean age was 28.4 years with extremes of 16 and 54 years. Single, divorced and widowed women accounted for 78.8% of the study population. Women who never attended school and those with primary education accounted for 74.3% of the study population. HIV serology was positive in 38.4% of cases. The main symptoms were genital itching, dyspareunia and urinary signs. The main germs identified in the laboratory were Candida albicans, Trichomonas vaginalis and Gardenerella vaginalis. Conclusion: vaginal discharge was found in 48.89% of women in the Yèrêlon group of Bobo-Dioulasso. The clinical study of the discharge and the laboratory results allowed a diagnosis and a better management of the leucorrhea in the group Yèrêlon.
