AIM:To explore the brain mechanism of acupuncture for children with anisometropic amblyopia using the voxelmirror homotopic connectivity(VMHC)analysis method of resting functional magnetic resonance imaging(rs-fMRI)te...AIM:To explore the brain mechanism of acupuncture for children with anisometropic amblyopia using the voxelmirror homotopic connectivity(VMHC)analysis method of resting functional magnetic resonance imaging(rs-fMRI)technology based on clinical effectiveness.METHODS:Eighty children with anisometropic monocular amblyopia were randomly divided into two groups:control(40 cases,1 case of shedding)and acupuncture(40 cases,1 case of shedding)groups.The control group was treated with glasses,red flash,grating,and visual stimulations,with each procedure conducted for 5min per time.Based on routine treatment,the acupuncture group underwent acupuncture of“regulating qi and unblocking meridians to bright eyes”,Jingming(BL1),Cuanzhu(BL2),Guangming(GB37),Fengchi(GB20)acupoints were taken on both sides,with the needle kept for 30min each time.Both groups were treated once every other day,three times per week,for a total of 4wk.After the treatment,the overall curative effect of the two groups and the latency and amplitude changes of P100 wave of pattern visual-evoked potential were counted.At the same time,nine children with left eye amblyopia were randomly selected from the two groups and were scanned with rsfMRI before and after treatment.The differences in the brain regions between the two groups were compared and analyzed with VMHC.RESULTS:Chi-square test showed a notable difference in the total efficiency rate between the acupuncture(94.87%)and control groups(79.49%).Regarding the P100 wave latency and amplitude,the acupuncture group had significantly shorter latency and higher amplitude of P100 wave than the control group.Moreover,the VMHC values of the bilateral temporal lobe,superior temporal gyrus,and middle temporal gyrus were notably increased in the acupuncture group after treatment.CONCLUSION:Acupuncture combined with conventional treatment can significantly improve the corrected visual acuity and optic nerve conduction in children with anisometropic amblyopia.Compared with the conventional treatment,the regulation of acupuncture on the functional activities of the relevant brain areas in the anterior cerebellum may be an effective acupuncture mechanism for anisometropic amblyopia.展开更多

The accuracy of area under curve was excellent indicated by the ROC curve analysis of each brain regions.CONCLUSION: Our study demonstrates preliminary evidence of disrupted interhemispheric rsFC in acute EP in sensorimotor and limbic system and somatosensory cortex, which might give some useful information for understanding the neurological mechanisms in acute EP individuals.展开更多

Totally, 16 male and 2 female acute OGI patients and 18 sex, age, and education-matched healthy volunteers were enrolled in the study. All subjects were scanned through functional magnetic resonance imaging (fMRI). Receiver operating characteristic (ROC) curves analyses had been used to identify the VMHC in these brain areas could be used as biomarkers to distinguish OGI and from healthy control (HC). The mean VMHC values in multiple brain areas and clinical OGI manifestations were evaluated with a Pearson correlation analysis. OGI patients had significantly decreased VMHC in the bilateral calcarine/lingual/cuneus (BA18, 19, 30) and middle occipital gyrus (BA18, 19). The OGI patients had abnormal interhemispheric FC in the dorsal visual pathway, which may represent the pathophysiological mechanism that underlies acute vision loss after OGI.展开更多
目的探讨体素镜像同伦连接(voxel-mirrored homotopic connectivity,VMHC)与局部一致性(regional homogeneity,ReHo)在评价经颅直流电刺激(transcranial direct current stimulation,tDCS)改善脑卒中后认知障碍中的应用价值。材料与方...目的探讨体素镜像同伦连接(voxel-mirrored homotopic connectivity,VMHC)与局部一致性(regional homogeneity,ReHo)在评价经颅直流电刺激(transcranial direct current stimulation,tDCS)改善脑卒中后认知障碍中的应用价值。材料与方法前瞻性纳入47名脑卒中后认知障碍(post-stroke cognitive impairment,PSCI)患者,随机分配到tDCS组与假刺激组,其中tDCS组23人,假刺激组24人,应用Mann-Whitney U检验比较两组患者在基线期与15天tDCS或假刺激治疗后认知量表评分变化值的差异,应用配对t检验比较两组患者在基线期与15天tDCS或假刺激治疗后VMHC与ReHo指标的差异,并提取差异脑区的VMHC与ReHo值与治疗前后认知量表评分的变化值进行相关性分析。结果tDCS组与假刺激组在治疗后的简易智能精神状态量表(Mini-Mental State Examination,MMSE)与蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)评分均优于治疗前,但tDCS组具有更好的治疗效果,治疗后相较于治疗前MMSE与MoCA量表评分改善值更大,差异具有统计学意义(P<0.05);VMHC指标结果:tDCS组患者在治疗后,双侧岛叶、楔前叶的VMHC值升高(P<0.05,FDR校正),假刺激组患者在治疗后,在双侧枕上回的VMHC值升高(P<0.05,FDR校正);ReHo指标结果:tDCS组患者在治疗后,前扣带回和顶下缘角回的ReHo值升高(P<0.05,FDR校正),假刺激组患者在治疗后相较于治疗前,未见明显差异脑区;相关性分析:tDCS组患者在治疗后,双侧楔前叶VMHC增加的值与MMSE、MoCA量表评分的变化值呈现正相关,MMSE(r=0.47,P=0.02);MoCA(r=0.43,P=0.04);其他差异脑区与MMSE与MoCA量表评分变化值无明显相关性。结论tDCS联合常规康复治疗对PSCI患者的认知康复效果优于仅进行常规康复治疗手段,应用VMHC与ReHo发现tDCS的治疗机制可能与改善默认模式网络(default mode network,DMN)与突显网络(salience network,SN)中部分脑区在双侧大脑半球之间的功能连接与自发活动有关。展开更多
There is considerable interest in the significance of structural and functional connections between the two brain hemispheres in terms of both normal function and in relation to psychiatric disorders.In recent years,m...There is considerable interest in the significance of structural and functional connections between the two brain hemispheres in terms of both normal function and in relation to psychiatric disorders.In recent years,many studies have used voxel mirrored homotopic connectivity analysis of resting state data to investigate the importance of connectivity between homotopic regions in the brain hemispheres in a range of neuropsychiatric disorders.The current review summarizes findings fromthese voxel mirrored homotopic connectivity studies in individuals with autism spectrum disorder,addiction,attention deficit hyperactivity disorder,anxiety and depression disorders,and schizophrenia,aswell as disorders such as Alzheimer’s disease,mild cognitive impairment,epilepsy,and insomnia.Overall,other than attention deficit hyperactivity disorder,studies across psychiatric disorders report decreased homotopic resting state functional connectivity in the default mode,attention,salience,sensorimotor,social cognition,visual recognition,primary visual processing,and reward networks,which are often associated with symptom severity and/or illness onset/duration.Decreased homotopic resting state functional connectivity may therefore represent a transdiagnostic marker for general psychopathology.In terms of disorder specificity,the extensive decreases in homotopic resting state functional connectivity in autism differ markedly from attention deficit hyperactivity disorder,despite both occurring during early childhood and showing extensive co-morbidity.A pattern of more posterior than anterior regions showing reductions in schizophrenia is also distinctive.Going forward,more studies are needed to elucidate the functions of these homotopic functional connections in both health and disorder and focusing on associations with general psychopathology,and not only on disorder specific symptoms.展开更多
