To now, clarithromycin was the best effective, well-tolerated and safe antibiotic used in treatment of the bacterium, but, increasing trend of resistance reduced efficacy of recommended regimens. Indeed, gastroenterologists are mostly unable to start appropriate therapyaccording to the sensitivity profile-due to the certain difficulties in routine H. pylori culture procedure and being time consuming method. This announcement by World Health Organization(WHO) was an onset to reconsider current challenging dilemma about H. pylori clarithromycin resistant isolates. Therefore, investigating of various factors affecting this nomination by WHO is highly welcomed. In fact, WHO enumerated more than 16 pathogens which seriously threats human life and public health, thus better management or effective guidelines are necessary. Here for the first time, we nominated this phenomenon as ‘‘gastroenterologist's persistence'' which should be equally investigated as antibiotic resistance. The ability of gastroenterologists to win the game against H. pylori infections is highly influenced by their collaboration with diagnostic laboratories to apply susceptibility patterns before any prescription. Two questions are addressed. Firstly, by using international organizations as the subjects and quantitative analysis of the spatial distribution of their offices, the overall position of Chinese cities in the global distribution of international organizations is estimated. The results show that 22 Chi- nese cities are involved in international political affairs. Beijing, Shanghai and Guangzhou dominate these activities and have high po- litical global-city status. Secondly, through qualitative analysis of Guangzhou City, the authors explore how its status as a center for intemational organizations has been shaped in recent years. The locational decision-making process is analyzed and the factors are demonstrated at both national and city scales. The definition of international organizations as used in this study refers not only to those supranational bodies that have the ability to manage relations across states (such as the European Union), but also those in- ter-governmental organizations (IGOs), international non-governmental organizations (INGOs) and diplomatic representatives and delegations that today also manage and regulate relations between and among states.展开更多

Storage c...A descriptive study on assessment of medicines cold chain storage conformity with World Health Organization (WHO) requirements in public health facilities was carried out in Dar es Salaam and Dodoma regions. Storage conformity in selected health facilities was assessed by monitoring temperature using temperature data loggers mounted in the refrigerators for a period of 30 days. Results indicated almost half of the health facilities 48.5% visited, did not significantly (P = 0.031) comply with storage temperature (+2°C to +8°C) as per WHO requirement because all recorded Mean Kinetic temperature (MKT) > 8°C. In rural areas, 59.2% of visited health facilities adhered to the WHO recommended storage temperature while in urban areas only 31.6% complied. The study has established electricity failure in urban and lack of gas in rural areas coupled with absence of contingency plan as major challenges to WHO temperature conformity in storage of cold chain medicines in health facilities in Tanzania.展开更多

After Vietnam's accession to the World Trade Organization (WTO), a large amount of FDI capital flowed into the country, up to 143,950.3 million USD. Wherein, there was a switching of FDI capital from the manufacturing sector to the service one in tandem with a downward trend in the agriculture. Like the previous duration, Vietnam's FDI capital sources came mostly from Asia-Pacific region and European economies (net capital, technology exporters) probably caused by Vietnam's integration emphases on dynamic Asia-Pacific region. Geographical location of FDI was characterized by a concentration on the three main key economic regions: the Red River Delta (surrounding Ha Noi, Hai Phong, and Quang Ninh), the Central region (surrounding Da Nang), and the Southeast of the country (surrounding Ho Chi Minh City) owing to good infrastructure, abundance of skillful labor force, and large market size. In attempts to improve patient safety, the World Health...Background: The incidence of in-hospital adverse events is about 10%, with a majority of these related to surgery, and nearly half considered preventable events. In attempts to improve patient safety, the World Health Organization (WHO) developed a checklist to be used at critical perioperative moments. This meta-analysis examines the impact of the WHO surgical safety checklist (SSC) on various patient outcomes. Methods: A comprehensive search of all published studies assessing the use of the WHO SSC in patients undergoing surgery was conducted. Studies using the WHO SSC in any surgical setting, with pre-implementation and post-implementation outcome data were included. The incidence of patient outcomes (total complications, surgical site infections, unplanned return to the operating room (OR) within 30 days, and overall mortality) and adherence to safety measures were analyzed. Results: 10 studies involving 51,125 patients (27,490 prior to implementation and 23,635 after implementation of the WHO SSC) were analyzed. The implementation of the WHO SSC significantly reduced the risk of total complications by 37.9%, surgical site infections by 45.5%, unplanned return to OR by 32.1%, and mortality by 15.3%. Increased adherence to safety measures including airway evaluation, use of pulse oximetry, prophylactic antibiotics when necessary, confirmation of patient name and surgical site, and sponge count was also observed. Conclusions: The use of the WHO SSC is associated with a significant reduction in post-operative complication rates and mortality. The WHO SSC is a valuable tool that should be universally implemented in all surgical centers and utilized in all surgical patients.展开更多

The bioclimatic regularities between the average annual precipitation,average annual temperatures and the density of organic carbon in the soil layer of 0-30 cm of the steppes in the regions of the world are given.The...The bioclimatic regularities between the average annual precipitation,average annual temperatures and the density of organic carbon in the soil layer of 0-30 cm of the steppes in the regions of the world are given.They are distinguished by a high certainty of quantization by asymmetric wave equations.It turned out that,due to the vibrational adaptation of organic carbon,precipitation and temperature are dependent on each other.For example,the model of the influence of precipitation on temperature includes the first term in the form of Laplace’s law(in mathematics),Mandelbrot’law(in physics),Zipf-Perl(in biology),and Pareto(in econometrics).The second term is the biotechnical law of the author of the article,which gives the maximum change in the indicator.Both components form a trend that makes it possible to divide the precipitation interval into three stages:(1)with an increase in precipitation from 0 to 60 mm,the temperature decreases according to Mandelbrot’s law from 23.25 to 0.50С;(2)from 60 to 2100 mm,the temperature rises to 24℃;(3)with a further increase in precipitation over 2100 mm,a slow decrease in temperature occurs.The third term is an asymmetric wavelet with a constant half-period of 367.8 mm.A positive sign shows that in the steppes there is a positive oscillatory adaptation of temperature to changes in precipitation.In the interval of precipitation 0-350 mm,an oscillatory decrease in temperature occurs.It turns out that the first oscillation at 0 mm precipitation begins with a very high temperature gradient of thermal energy.The first interval includes Mongolia and Inner Mongolia.In the second interval of 350-750 mm,an oscillatory increase in temperature occurs.Then,in the third interval 750-1050 mm,the temperature drops again.The second oscillation with a correlation coefficient of 0.9685 has clear precipitation boundaries in the range of 200-2000 mm.Due to the negative sign,the fluctuation is a crisis,inhibiting the rise in temperature.And the third fluctuation has a positive effect on the temperature.The mechanism of oscillatory adaptation in the steppe soil is so perfect that it changes for itself the conditions of the place where the grass grows.An amplitude-frequency analysis of each oscillation will make it possible to determine the specific particular effects of precipitation and temperature on each other and on the density of organic carbon.It was found that two-factor modeling of the change in the soil organic carbon density makes it possible to achieve an identification error even less than the absolute measurement error.展开更多

Its safety and efficacy has been demonstrated extensively in the literature, leading to increased use around the world over the last two decades. This has been demonstrated by the increase in Ponseti related Pub Med publications from many countries. We found evidence of Ponseti activity in 113 of 193 United Nations members. The contribution of many organizations which provide resources to healthcare practitioners in low and middle income countries, as well as Ponseti champions and modern communication technology, have helped to spread the Ponseti method around the world. Despite this, there are many countries where the Ponseti method is not being used, as well as many large countries in which the extent of activity is unknown. With its low rate of complication, low cost, and high effectiveness,this method has unlimited potential to treat clubfoot in both developed and undeveloped countries. Our listing of countries who have not yet shown presence of Ponseti activity will help non-governmental organizations to target those countries which still need the most help.展开更多

In neural systems, their macroscopic states are represented by the activity states of neuron populations, and phase transitions between different activity states are closely related to corresponding functions in the brain. In particular, phase transitions to some rhythmic synchronous firing states play significant roles on diverse brain functions and disfunctions, such as encoding rhythmical external stimuli, epileptic seizure, etc. However, in previous studies, phase transitions in neuronal networks are almost driven by network parameters (e.g., external stimuli), and there has been no investigation about the transitions between typical activity states of neuronal networks in a self-organized way by applying plastic connection weights. In this paper, we discuss phase transitions in electrically coupled and lattice-based small-world neuronal networks (LBSW networks) under spike-timing-dependent plasticity (STDP). By applying STDP on all electrical synapses, various known and novel phase transitions could emerge in LBSW networks, particularly, the phenomenon of self-organized phase transitions (SOPTs): repeated transitions between synchronous and asynchronous firing states. We further explore the mechanics generating SOPTs on the basis of synaptic weight dynamics.展开更多

We...Old world bluestems (Bothriochloa spp.) have been successfully introduced as grasses for livestock forage in the semiarid Texas High Plains. Questions remain, however, on effects of these grasses on soil resources. We tested the hypothesis that differences in grass species produce differences in soil properties important to crop growth and useful in selecting the optimum species for the Southern High Plains of Texas. Three old world bluestem (Bothriochloa) species [C.E. Hubbard ‘Caucasian’, B. caucasica (Trin.);‘WW Spar’, B. ischaemum (L.) Keng.var ischaemum (Hack.);and S.T. Blake ‘WW-B Dahl’, B. bladhii (Retz)] were grown in a randomized complete block design, with three replications, for nine years on a clay loam soil near Lubbock, Texas. Soil samples were collected in the ninth year to determine soil texture, wet aggregate stability, bulk density (BD), soil organic carbon (SOC), particulate organic carbon (POC), and soil strength as measured by the cone pentrometer. The grass species differed in their above-ground biomass and below- ground root production. In the ninth year of production, Bothriochloa caucasica and B. bladhii produced about twice the above-ground biomass with about 25% fewer roots than B. ischaemum. Soils where B. caucasica was grown had the highest BD (1.36 Mg m–3) and B. ischaemum had the lowest (1.31 Mg m–3). The soil in which B. ischaemum was growing had a lower BD, greatest root biomass, organic matter content, and aggregate stability suggesting superior soil quality for agricultural production. The species B. bladhii, however, often exhibited soil properties that were similar to both other species tested. Since Bothriochloa bladhii had superior or similar soil properties for plant growth among the species tested and has been shown to be higher in forage quality, animal performance, and carrying capacity than the other species, it appears to be the best choice among these three species to optimize both animal performance and desirable soil properties.展开更多

The implementation of the WHO SSC significantly reduced the risk of total complications by 37.9%, surgical site infections by 45.5%, unplanned return to OR by 32.1%, and mortality by 15.3%. Increased adherence to safety measures including airway evaluation, use of pulse oximetry, prophylactic antibiotics when necessary, confirmation of patient name and surgical site, and sponge count was also observed. Conclusions: The use of the WHO SSC is associated with a significant reduction in post-operative complication rates and mortality. The WHO SSC is a valuable tool that should be universally implemented in all surgical centers and utilized in all surgical patients.

文摘The bioclimatic regularities between the average annual precipitation,average annual temperatures and the density of organic carbon in the soil layer of 0-30 cm of the steppes in the regions of the world are given.They are distinguished by a high certainty of quantization by asymmetric wave equations.It turned out that,due to the vibrational adaptation of organic carbon,precipitation and temperature are dependent on each other.For example,the model of the influence of precipitation on temperature includes the first term in the form of Laplace’s law(in mathematics),Mandelbrot’law(in physics),Zipf-Perl(in biology),and Pareto(in econometrics).The second term is the biotechnical law of the author of the article,which gives the maximum change in the indicator.Both components form a trend that makes it possible to divide the precipitation interval into three stages:(1)with an increase in precipitation from 0 to 60 mm,the temperature decreases according to Mandelbrot’s law from 23.25 to 0.50С;(2)from 60 to 2100 mm,the temperature rises to 24℃;(3)with a further increase in precipitation over 2100 mm,a slow decrease in temperature occurs.The third term is an asymmetric wavelet with a constant half-period of 367.8 mm.A positive sign shows that in the steppes there is a positive oscillatory adaptation of temperature to changes in precipitation.In the interval of precipitation 0-350 mm,an oscillatory decrease in temperature occurs.It turns out that the first oscillation at 0 mm precipitation begins with a very high temperature gradient of thermal energy.The first interval includes Mongolia and Inner Mongolia.In the second interval of 350-750 mm,an oscillatory increase in temperature occurs.Then,in the third interval 750-1050 mm,the temperature drops again.The second oscillation with a correlation coefficient of 0.9685 has clear precipitation boundaries in the range of 200-2000 mm.Due to the negative sign,the fluctuation is a crisis,inhibiting the rise in temperature.And the third fluctuation has a positive effect on the temperature.The mechanism of oscillatory adaptation in the steppe soil is so perfect that it changes for itself the conditions of the place where the grass grows.An amplitude-frequency analysis of each oscillation will make it possible to determine the specific particular effects of precipitation and temperature on each other and on the density of organic carbon.It was found that two-factor modeling of the change in the soil organic carbon density makes it possible to achieve an identification error even less than the absolute measurement error.

文摘The Ponseti method has become the gold standard for the treatment of idiopathic clubfoot. Its safety and efficacy has been demonstrated extensively in the literature, leading to increased use around the world over the last two decades. This has been demonstrated by the increase in Ponseti related Pub Med publications from many countries. We found evidence of Ponseti activity in 113 of 193 United Nations members. The contribution of many organizations which provide resources to healthcare practitioners in low and middle income countries, as well as Ponseti champions and modern communication technology, have helped to spread the Ponseti method around the world. Despite this, there are many countries where the Ponseti method is not being used, as well as many large countries in which the extent of activity is unknown. With its low rate of complication, low cost, and high effectiveness,this method has unlimited potential to treat clubfoot in both developed and undeveloped countries. Our listing of countries who have not yet shown presence of Ponseti activity will help non-governmental organizations to target those countries which still need the most help.

文摘Phase transitions widely exist in nature and occur when some control parameters are changed. In neural systems, their macroscopic states are represented by the activity states of neuron populations, and phase transitions between different activity states are closely related to corresponding functions in the brain. In particular, phase transitions to some rhythmic synchronous firing states play significant roles on diverse brain functions and disfunctions, such as encoding rhythmical external stimuli, epileptic seizure, etc. However, in previous studies, phase transitions in neuronal networks are almost driven by network parameters (e.g., external stimuli), and there has been no investigation about the transitions between typical activity states of neuronal networks in a self-organized way by applying plastic connection weights. In this paper, we discuss phase transitions in electrically coupled and lattice-based small-world neuronal networks (LBSW networks) under spike-timing-dependent plasticity (STDP). By applying STDP on all electrical synapses, various known and novel phase transitions could emerge in LBSW networks, particularly, the phenomenon of self-organized phase transitions (SOPTs): repeated transitions between synchronous and asynchronous firing states. We further explore the mechanics generating SOPTs on the basis of synaptic weight dynamics.
文摘Old world bluestems (Bothriochloa spp.) have been successfully introduced as grasses for livestock forage in the semiarid Texas High Plains. Questions remain, however, on effects of these grasses on soil resources. We tested the hypothesis that differences in grass species produce differences in soil properties important to crop growth and useful in selecting the optimum species for the Southern High Plains of Texas. Three old world bluestem (Bothriochloa) species [C.E. Hubbard ‘Caucasian’, B. caucasica (Trin.);‘WW Spar’, B. ischaemum (L.) Keng.var ischaemum (Hack.);and S.T. Blake ‘WW-B Dahl’, B. bladhii (Retz)] were grown in a randomized complete block design, with three replications, for nine years on a clay loam soil near Lubbock, Texas. Soil samples were collected in the ninth year to determine soil texture, wet aggregate stability, bulk density (BD), soil organic carbon (SOC), particulate organic carbon (POC), and soil strength as measured by the cone pentrometer. The grass species differed in their above-ground biomass and below- ground root production. In the ninth year of production, Bothriochloa caucasica and B. bladhii produced about twice the above-ground biomass with about 25% fewer roots than B. ischaemum. Soils where B. caucasica was grown had the highest BD (1.36 Mg m–3) and B. ischaemum had the lowest (1.31 Mg m–3). The soil in which B. ischaemum was growing had a lower BD, greatest root biomass, organic matter content, and aggregate stability suggesting superior soil quality for agricultural production. The species B. bladhii, however, often exhibited soil properties that were similar to both other species tested. Since Bothriochloa bladhii had superior or similar soil properties for plant growth among the species tested and has been shown to be higher in forage quality, animal performance, and carrying capacity than the other species, it appears to be the best choice among these three species to optimize both animal performance and desirable soil properties.
