AIM:Gallstone disease is increasing in sub-saharan Africa (SSA).In the west, the majority of stones can be dissolved with bile salts,since the major component is cholesterol.This medical therapy is expensive and not r...AIM:Gallstone disease is increasing in sub-saharan Africa (SSA).In the west, the majority of stones can be dissolved with bile salts,since the major component is cholesterol.This medical therapy is expensive and not readily accessible to poor populations of SSA.It was therefore necessary to analyze the chemical composition of biliary stones in a group of patients,so as to make the case for introducing bile salt therapy in SSA.METHODS: All patients with symptomatic gallstones were recruited in the study. All stones removed during cholecystectomy were sent to Houston for x-ray diffraction analysis.Data on age, sex, serum cholesterol,and thepercentage by weight of cholesterol, calcium carbonate, and amorphous material in each stone was entered into a preestablished proforma. Frequencies of the major components of the stones were determined.RESULTS:Sixteen women and ten men aged between 27 and 73 (mean 44.9) years provided stones for the study.The majority of patients (65.38%) had stones with less than 25% of cholesterol.Amorphous material made up more than 50% and 100% of stones from 16 (61.53%) and 9 (34.61%) patients respectively.CONCLUSION:Cholesterol is present in small amounts in a minority of gallstones in Yaounde. Dissolution of gallstones with bile salts is unlikely to be successful.展开更多

The acupoints Xiaguan (ST 7), Jiache (ST 6), Yifeng (TE 17) and Hegu (LI 4) on the affected side were punctured first, after removal of needles, 5-7 layers of cotton cloth was placed on the above acupoints, then moxibustion with moxa-roll made of Chinese medicine was performed. Finally Dian-finger pressing, An-pressing and Rou stationary circular pressing with finger belly of the thumb were applied to the local region around the mandible joint for 3-5 min. Methods: Sixty-two patients with different types of Le Fort fractures underwent CT scanning and 3D-CT reconstruction. The data were analyzed by multiplanar reconstruction (MPR), surface shaded display (SSD) and volume rendering (VR) respectively. Results: The patients with Le Fort Ⅰ, Le Fort Ⅱ fracture and Le Fort Ⅲ fracture accounted for 16.1%, 14.5% and 12.9% respectively. The compound fractures were the most common type and accounted for 56.5% (n=35, 18 cases with Le FortⅠ+Ⅱ fracture, 10 cases with Le Fort Ⅱ+Ⅲ fracture and 7 cases with Le FortⅠ+ Ⅱ+Ⅲ fracture). Fifty-five cases coexisted with other fractures in maxillofacial region. 2D-CT could be used to define the tiny fractures and the deep-structure fractures more accurately compared with 3D-CT, but the real impression of Le Fort type fractures could not be correctly evaluated on 2D-CT. 3D-CT could clearly demonstrate the whole shape of Le Fort type fractures and identify the classification of Le Fort fractures. Conclusions: 3D-CT is the best imaging method for the diagnosis of Le Fort type fractures and can provide valuable information of space relationship, especially for the design of treatment plan before operation.展开更多
