Gastric cancer,a prevalent malignancy worldwide,ranks sixth in terms of frequency and third in fatality,causing over a million new cases and 769000 annual deaths.Predominant in Eastern Europe and Eastern Asia,risk factors include family medical history,dietary habits,tobacco use,Helicobacter pylori,and Epstein-Barr virus infections.Unfortunately,gastric cancer is often diagnosed at an advanced stage,leading to a grim prognosis,with a 5-year overall survival rate below 5%.Surgical intervention,particularly with D2 Lymphadenectomy,is the mainstay for early-stage cases but offers limited success.For advanced cases,the National Comprehensive Cancer Network recommends chemotherapy,radiation,and targeted therapy.Emerging immunotherapy presents promise,especially for unresectable or metastatic cases,with strategies like immune checkpoint inhibitors,tumor vaccines,adoptive immunotherapy,and nonspecific immunomodulators.In this Editorial,with regards to the article“Advances and key focus areas in gastric cancer immunotherapy:A comprehensive scientometric and clinical trial review”,we address the advances in the field of immunotherapy in gastric cancer and its future prospects. 展开更多

Randomized control trial of a culturally adapted behavioral activation therapy for Muslim patients with depression in Pakistan

BACKGROUND Behavioral activation therapy(BA)is as effective as cognitive behavior therapy(CBT)in treating depression and can be delivered by practitioners with much less psychological training,making it particularly suitable for low resource settings.BA that is culturally adapted for Muslims(BA-M)is a culturally adapted form of BA that has been found acceptable and feasible for Muslims with depression in the United Kingdom and Turkey;however,this is the first time that its efficacy has been determined through a definitive randomized controlled trial.AIM To compare the effectiveness of BA-M with CBT for Muslim patients with depression in Pakistan.METHODS One hundred and eight patients were randomized 1:1 to treatment arms in a parallel-group randomized controlled trial in hospital or community sites in Lahore,Pakistan.Recruitment followed self-referral or referrals from clinicians,consultants or relevant professionals at each site.Four measures were recorded by blinded assessors:The patient health questionnaire-9(PHQ-9);the BA for depression scale short form(BADS-SF);symptom checklist-revised and the World Health Organization Quality-of-Life Brief Scale.All measures were recorded at baseline and post treatment;PHQ-9 and BADS-SF were also recorded at each session and at three month follow up.The primary analysis was to regress the PHQ-9 score after therapy upon the PHQ-9 score before therapy(baseline)and the type of therapy given,that is,analysis of covariance.In addition,analysis using PHQ-9 scores collected at each therapy session was employed in a 2-level regression model.RESULTS Patients in the BA-M arm experienced greater improvement in PHQ-9 score of 1.95 units compared to the CBT arm after adjusting for baseline values(P=0.006)The key reason behind this improvement was that patients were retained in therapy longer under BA-M,in which patients were retained for an average 0.75 sessions more than CBT patients(P=0.013).Patients also showed significant differences on physical(P<0.001),psychological(P=0.004)and social(P=0.047)domains of Quality of Life(QoL)at post treatment level,indicating an increased QoL in the BA-M group as compared to the treatment as usual group.Some baseline differences were noted in both groups for BA scores and two domains of QoL scale:Physical and environment,which might have influenced the results,though the BA-M group showed more improvement at completion of therapy.CONCLUSION Results proved the efficacy of BA-M in reducing symptoms for depressed patients in Pakistan,indicating BA-M is a promising treatment modality for depression in future,particularly in low resource settings. 展开更多

Breast cancer is the most frequent cancer among females and also a leading cause of cancer related mortality worldwide.A multimodality treatment approach may be utilized for optimal management of patients with combinations of surgery,radiation therapy(RT)and systemic treatment.RT composes an integral part of breast conserving treatment,and is typically used after breast conserving surgery to improve local control.Recent years have witnessed significant improvements in the discipline of radiation oncology which allow for more focused and precise treatment delivery.Adaptive radiation therapy(ART)is among the most important RT techniques which may be utilized for redesigning of treatment plans to account for dynamic changes in tumor size and anatomy during the course of irradiation.In the context of breast cancer,ART may serve as an excellent tool for patients receiving breast irradiation followed by a sequential boost to the tumor bed.Primary benefits of ART include more precise boost localization and potential for improved normal tissue sparing with adapted boost target volumes particularly in the setting of seroma reduction during the course of irradiation.Herein,we provide a concise review of ART for breast cancer in light of the literature. 展开更多

Deformable image registration (DIR) has been an important component in adaptive radiotherapy (ART). Our goal was to examine the accuracy of ART using the dice similarity coefficient (DSC) and to determine the optimal ... Deformable image registration (DIR) has been an important component in adaptive radiotherapy (ART). Our goal was to examine the accuracy of ART using the dice similarity coefficient (DSC) and to determine the optimal timing of replanning. A total of 22 patients who underwent volume modulated arc therapy (VMAT) for head and neck (H&N) cancers were prospectively analyzed. The planning target volume (PTV) was to receive a total of 70 Gy in 33 fractions. A second planning CT scan (rescan) was performed at the 15th fraction. The DSC was calculated for each structure on both CT scans. The continuous variables to predict the need for replanning were assessed. The optimal cut-off value was determined using receiver operating characteristic (ROC) curve analysis. In the correlation between body weight loss and DSC of each structure, weight loss correlated negatively with DSC of the whole face (rs = -0.45) and the face surface (rs = -0.51). Patients who required replanning tended to have experienced rapid weight loss. The threshold DSC was 0.98 and 0.60 in the whole face and the face surface, respectively. Patients who showed low DSC in the whole face and the face surface required replanning at a significantly high rate (P < 0.05 and P < 0.01). Weight loss correlated with DSC in both the whole face and the face surface (P < 0.05 and P < 0.05). The DSC values in the face predicted the need for replanning. In addition, weight loss tended to correlate with DSC. DIR during ART was found to be a useful tool for replanning. 展开更多

BACKGROUND Infiltration of the breast by leukemic cells is uncommon but may manifest as an oncological emergency requiring prompt management.Extramedullary relapse of T-cell acute lymphoblastic leukemia(T-ALL)within the breast is exceedingly rare and there is paucity of data in the literature regarding this entity.No consensus exists on management of isolated extramedullary breast relapses of T-ALL.Herein,we report a case of isolated extramedullary breast relapse of T-ALL treated with breathing adapted radiation therapy(BART)using the active breathing control(ABC)system.CASE SUMMARY The patient was a 33-year-old female with diagnosis of T-ALL.She received intensive systemic chemotherapy that resulted in complete remission of her disease,and then underwent allogeneic hematopoietic stem cell transplantation.After a 15 mo period without symptoms and signs of progression,the patient presented with palpable masses in both breasts.She complained from severe pain and swelling of the breasts.Imaging workup showed bilateral breast lesions,and diagnosis of breast infiltration by leukemic cells was confirmed after immunohistopathological evaluation.The patient suffering from severe pain,discomfort,and swelling of both breasts due to leukemic infiltration was referred to the Radiation Oncology Department for symptomatic palliation.Whole breast irradiation was delivered to both breasts of the patient with BART using the ABC system.The patient had complete resolution of her symptoms after treatment with BART.CONCLUSION BART with the ABC system resulted in complete resolution of the patient’s symptoms due to leukemic infiltration of both breasts with T-ALL.This contemporary treatment technique should be preferred for radiotherapeutic management of patients with leukemic infiltration of the breasts to achieve effective symptomatic palliation. 展开更多

Human Adaptive Mechatronics(HAM)includes human and computer system in a closed loop.Elderly person with disabilities,normally carry out their daily routines with some assistance to move their limbs.With the short fall of human care takers,mechatronics devices are used with the likes of exoskeleton and exosuits to assist them.The rehabilitation and occupational therapy equipments utilize the electromyography(EMG)signals to measure the muscle activity potential.This paper focuses on optimizing the HAM model in prediction of intended motion of upper limb with high accuracy and to increase the response time of the system.Limb characteristics extraction from EMG signal and prediction of optimal controller parameters are modeled.Time and frequency based approach of EMG signal are considered for feature extraction.The models used for estimating motion and muscle parameters from EMG signal for carrying out limb movement predictions are validated.Based on the extracted features,optimal parameters are selected by Modified Lion Optimization(MLO)for controlling the HAM system.Finally,supervised machine learning makes predictions at different points in time for individual sensing using Support Vector Neural Network(SVNN).This model is also evaluated based on optimal parameters of motion estimation and the accuracy level along with different optimization models for various upper limb movements.The proposed model of human adaptive controller predicts the limb movement by 96%accuracy. 展开更多

目的评估基于Velocity软件,不同锥形束CT(Cone Beam CT,CBCT)扫描模式下对应的合成CT(synthetic CT,sCT)的图像质量。方法通过设置CIRS肺运动模体常规的呼吸运动状态,对其进行4D-CT模拟定位。在平均密度投影CT即计划CT(planning CT,pCT... 目的评估基于Velocity软件,不同锥形束CT(Cone Beam CT,CBCT)扫描模式下对应的合成CT(synthetic CT,sCT)的图像质量。方法通过设置CIRS肺运动模体常规的呼吸运动状态,对其进行4D-CT模拟定位。在平均密度投影CT即计划CT(planning CT,pCT)上完成计划设计后,将其传至TrueBeam直线加速器,分别实现3D-CBCT和4D-CBCT的扫描和图像重建,再基于Velocity软件得到相应的sCT。结果sCT图像保留了基准pCT图像的精细结构,且二者图像轮廓之间有较强的相关性,所有Dice相似系数都超过0.85。通过直观对比可看出,相比对应的CBCT图像,sCT图像伪影明显减少。sCT3D/sCT4D图像中不同感兴趣区的CT值均明显更低,sCT的噪声更小。对sCT而言,sCT3D图像的CT值的平均百分差异为7.68%,sCT4D图像的CT值的平均百分差异为4.65%。相比3D-CBCT,基于4D-CBCT得到的sCT图像的CT值精度得到了改善。结论在固定呼吸状态下,基于Velocity得到的对应不同模式CBCT的sCT图像质量与基准pCT可以相比拟,与呼吸相关的图像引导在肺癌患者放疗时更有临床价值。 展开更多

目的探讨运动疗法联合语言康复训练对语言发育迟缓患儿的临床效果及对语言发育能力和社会生活能力的影响。方法选取2022年5月至2023年1月在余姚市妇幼保健院就诊的语言发育迟缓患儿70例为研究对象,采用随机数字表法分为观察组35例和对照... 目的探讨运动疗法联合语言康复训练对语言发育迟缓患儿的临床效果及对语言发育能力和社会生活能力的影响。方法选取2022年5月至2023年1月在余姚市妇幼保健院就诊的语言发育迟缓患儿70例为研究对象,采用随机数字表法分为观察组35例和对照组35例。对照组患儿采用语言康复训练,观察组患儿在语言康复训练基础上联合运动疗法。两组干预时间均为8周。比较两组干预疗效、治疗前后Gesell发育量表评分和社会生活能力变化。结果观察组患儿总有效率为94.29%,高于对照组的74.29%,差异有统计学意义(P<0.05)。两组患儿治疗后精细动作、适应能力、语言能力、个人-社交行为和大运动评分均高于治疗前,差异均有统计学意义(均P<0.05);观察组患儿治疗后上述评分均高于对照组,差异均有统计学意义(均P<0.01)。两组患儿治疗后运动、交往、作业操作和参加集体活动评分均高于治疗前,差异均有统计学意义(均P<0.05);观察组患儿治疗后上述评分均高于对照组,差异均有统计学意义(均P<0.01)。结论运动疗法联合语言康复训练对语言发育迟缓患儿的临床效果显著,且可改善患儿语言发育能力,提高患儿社会生活能力。 展开更多

目的探讨肺炎患儿雾化治疗中采用知信行健康教育联合三维适性支持护理的效果。方法选取2021年5月至2023年2月在郑州市妇幼保健院接受治疗的肺炎患儿共计106例,以随机数字表法分为干预组(n=53,知信行健康教育联合三维适性支持护理)与对照... 目的探讨肺炎患儿雾化治疗中采用知信行健康教育联合三维适性支持护理的效果。方法选取2021年5月至2023年2月在郑州市妇幼保健院接受治疗的肺炎患儿共计106例,以随机数字表法分为干预组(n=53,知信行健康教育联合三维适性支持护理)与对照组(n=53,常规护理干预),对两组症状改善时间、家长疾病认知度、治疗舒适度[kolcaba舒适状态量表(GCQ)]及并发症进行比较。结果干预组发热消失时间、咳嗽消失时间、气促消失时间、肺部啰音消失时间均短于对照组,差异有统计学意义(P<0.05)。干预后,两组家长疾病认知度(发生机制、检查方法、治疗方法、饮食要点)均有所提高,且干预组高于对照组,差异有统计学意义(P<0.05)。干预后,两组GCQ(心理精神、生理、环境、社会文化)评分均有所提高,且干预组高于对照组,差异有统计学意义(P<0.05)。干预组并发症发生率(3.77%)低于对照组(20.75%),差异有统计学意义(P<0.05)。结论知信行健康教育联合三维适性支持护理应用于肺炎患儿雾化治疗中,能够缩短症状消失时间,提高家长疾病认知度,改善舒适状态,减少并发症发生。 展开更多

目的:观察作业治疗对学龄前期脑瘫儿童接受普通教育应用适应性的影响。方法:将60例脑瘫患儿随机分为对照组(n=30)和观察组(n=30),对照组接受普通学前教育(幼儿园),观察组在此基础上给予作业治疗,分别于治疗前、治疗3个月后、治疗6个月... 目的:观察作业治疗对学龄前期脑瘫儿童接受普通教育应用适应性的影响。方法:将60例脑瘫患儿随机分为对照组(n=30)和观察组(n=30),对照组接受普通学前教育(幼儿园),观察组在此基础上给予作业治疗,分别于治疗前、治疗3个月后、治疗6个月后采用精细运动功能评定量表(FMFM)、婴儿-初中生社会适应能力量表(S-M)从上肢精细运动能力值、独立生活、运动能力、作业操作能力、交往能力、参加集体活动、自我管理7个方面评价脑瘫儿童的学龄前期教育适应程度,同时评价作业治疗对提高学龄前期脑瘫儿童接受普通教育适应行为的可行性。结果:两组治疗后3、6个月FMFM总分及各分区评分显著提高(P<0.05),且观察组改善优于对照组(P<0.05);两组治疗后3、6个月S-M总分、独立生活、作业操作能力、参加集体活动、自我管理均显著提高(P<0.05),且观察组优于对照组(P<0.05);但在运动能力、交往能力两组治疗前与治疗后3个月、6个月分值对比有统计学意义(P<0.05),组间差异无统计学意义(P>0.05)。结论:学龄前期脑瘫儿童早期接受普通教育有利于改善其适应行为,结合作业治疗可效果更加显著,值得临床推广应用。 展开更多

An Evolutionary Approach for Personalized Therapy in Multiple Myeloma

Most patients with multiple myeloma (MM) respond well to initial therapy, but invariably relapse due to evolution of resistant phenotypes. Here we examine the evolutionary dynamics of proliferation of resistant MM phenotypes during therapy. By applying computational models to data from three clinical trials for newly diagnosed MM patients, we have quantified the size and level of chemoresistance of subpopulations within the tumor burden in 124 patients, prior to and during therapy. Subsequently, we used the computational models to explore an alternative strategy of “adaptive therapy” (AT), which includes defined treatment holidays, to improve the duration of “controlled disease” (CD). Simulations showed that AT could prolong CD in all three trials: 50.0% vs. 11.1% 50-month CD for a single agent approach in older adults (P = 0.0123), 80.4% vs. 58.8% 60-month CD for a multi-agent bortezomib based therapy (P = 0.0082), and 54.0% vs. 24.0% 60-month CD for a multi-agent lenalidomide based therapy (P < 0.0001). Increases in duration of CD resulted from the stabilization of tumor burden, which in turn would delay the growth of chemoresistant sub-populations in patients with partial (PR), or very good partial response (VGPR). 目的基于千伏级锥形束CT(Kilovolt Cone-Beam Computed Tomography,KV-CBCT)非小细胞肺癌(Non-Small Cell Lung Cancer,NSCLC)患者第1周放疗前扫描的锥形束CT(Cone-Beam Computed Tomography,CBCT)图像设计的自适应放疗计划,比较自适应... 目的基于千伏级锥形束CT(Kilovolt Cone-Beam Computed Tomography,KV-CBCT)非小细胞肺癌(Non-Small Cell Lung Cancer,NSCLC)患者第1周放疗前扫描的锥形束CT(Cone-Beam Computed Tomography,CBCT)图像设计的自适应放疗计划,比较自适应放疗前后放疗计划的剂量学差异,以及探讨自适应放疗应用于NSCLC患者放疗的可行性。方法选取行调强放疗的NSCLC患者15例,将首周每天1次和后续每周1次获得的CBCT验证图像传入Eclipse计划系统与定位CT图像融合配准后,在CBCT图像上重新勾画大体肿瘤体积(Gross Tumor Volume,GTV)、临床靶区体积(Clinical Target Volume,CTV),将首周CBCT图像勾画的CTV拷贝到定位CT中合成并命名为临床靶区体积(Clinical Target Volume,CTVa),并依据首周测量的摆位误差外扩生成计划靶体积(Plan Target Volume,PTVa),同理获得后续几周的计划靶体积(Plan Target Volume,PTVb)。保持最初治疗计划的射束和优化条件等参数不变,针对PTVa重新设计出自适应放疗计划(Adaptive Radiation Therapy,ARTa),进一步比较自适应放疗前后放疗计划的剂量学差异,同时在ARTa中评估PTVb的剂量覆盖率。结果PTVa的体积小于初始靶区PTV,差异具有统计学意义(P<0.05);ARTa处方剂量的体积覆盖率为96.2%±1.7%,优于批准计划(Planapproved)的95.1%±2.3%,差异有统计学意义(P<0.05),且在ARTa中,PTVa的处方剂量线能使PTVb体积覆盖率达到96.8%±1.7%;ARTa中肺组织(V_(5)、V_(10)、V_(20)、V_(30)、D_(mean))、心脏(V_(30)、V_(40)、D_(mean))、脊髓(D_(max)、D_(mean))均较Planapproved有所降低,且两两比较差异有统计学意义(P<0.05)。结论使用患者首周CBCT提供的反馈进程(空间位移、解剖形变)重新设计的ARTa可以用于患者后续的放疗中,并能够减少重要器官的照射剂量和提高靶区剂量,进而有利于提高非小细胞肺癌肿瘤增益比。 展开更多

目的:探讨前列腺癌内分泌治疗病人心理社会适应的影响因素,为制定科学、有效的护理干预方案提供依据。方法:基于社会-细胞理论,通过目的性抽样于2022年1月—3月选取上海市某三级甲等医院的前列腺癌内分泌治疗病人12例,采用半结构化访谈... 目的:探讨前列腺癌内分泌治疗病人心理社会适应的影响因素,为制定科学、有效的护理干预方案提供依据。方法:基于社会-细胞理论,通过目的性抽样于2022年1月—3月选取上海市某三级甲等医院的前列腺癌内分泌治疗病人12例,采用半结构化访谈,运用内容分析法分析资料。结果:共提炼出4个主题、11个亚主题,分别为疾病因素(症状困扰、基因问题、有效治疗)、个体因素(认知差异、不确定感和复发恐惧、应对方式)、家庭因素(家庭角色、家庭影响)、社会因素(社会歧视、社会支持、经济负担)。结论:前列腺癌内分泌治疗病人心理社会适应不良问题突出,且受到疾病诊治、个人、家庭及社会诸多因素的影响。医护人员在今后工作中需多维度、系统化评估各因素对病人心理社会适应的影响,根据病人不同特质,针对性地提供专业支持,提高病人生活质量。 展开更多

产后循证康复方案联合音乐疗法应用于产妇的效果

目的探讨产后循证康复方案联合音乐疗法应用于产妇的效果。方法按照随机数字表法将我院2020年8月至2022年12月期间分娩的124例产妇分为两组,每组各62例,对照组接受产后循证康复方案管理,观察组在其基础上加以音乐疗法干预,观察两组产后... 目的探讨产后循证康复方案联合音乐疗法应用于产妇的效果。方法按照随机数字表法将我院2020年8月至2022年12月期间分娩的124例产妇分为两组,每组各62例,对照组接受产后循证康复方案管理,观察组在其基础上加以音乐疗法干预,观察两组产后恢复情况、负性情绪、盆底功能以及母亲角色适应能力。结果观察组泌乳、子宫复旧时间均短于对照组,泌乳量高于对照组(P<0.05);干预1、3个月后,观察组爱丁堡产后抑郁量表(EPDS)得分均低于对照组(P<0.05);干预3个月后,观察组I、Ⅱ类肌纤维电压均高于对照组(P<0.05);干预1、3个月后,观察组母亲角色适应能力得分均高于对照组(P<0.05)。结论产后循证康复方案联合音乐疗法可有效改善产妇负性情绪,提升其角色转换适应能力,改善盆底功能,加快产后康复进程。 展开更多

目的:探讨^(125)I放射性粒子植入治疗肿瘤病人的心理弹性、家庭亲密度与适应性和孤独感的相关性。方法:采取便利抽样法,选取106例行^(125)I放射性粒子植入治疗的肿瘤病人作为研究对象,调查病人的一般资料,采用心理弹性量表中文版(CD-RI... 目的:探讨^(125)I放射性粒子植入治疗肿瘤病人的心理弹性、家庭亲密度与适应性和孤独感的相关性。方法:采取便利抽样法,选取106例行^(125)I放射性粒子植入治疗的肿瘤病人作为研究对象,调查病人的一般资料,采用心理弹性量表中文版(CD-RISC)评价病人的心理弹性水平,并使用家庭亲密度与适应性量表中文版(FACESIICV)及孤独感自评量表(UCLA)进行调查问卷。结果:^(125)I放射性粒子植入治疗肿瘤病人孤独感量表得分为(40.01±8.63)分,心理弹性量表得分为(61.79±14.54)分,家庭亲密度与适应性量表得分为(116.32±18.80)分;不同性别、职业、文化程度、家庭人均月收入的孤独感得分差异均有统计学意义(P<0.01),不同性别、家庭人均月收入心理弹性得分和家庭亲密度与适应性得分差异有统计学意义(P<0.01)。106例^(125)I放射性粒子植入治疗的肿瘤病人的孤独感量表得分和心理弹性量表得分呈显著负相关(r=-0.912,P<0.01),心理弹性量表得分和家庭亲密度与适应性量表得分呈显著正相关关系(r=0.884,P<0.01),孤独感量表得分和家庭亲密度与适应性量表得分呈显著负相关关系(r=-0.907,P<0.01);纳入性别、职业、文化程度、家庭人均月收入、心理弹性得分、家庭亲密度与适应性得分进行孤独感影响因素多元线性逐步回归分析,家庭亲密度与适应性得分和家庭人均月收入均有统计学意义(P<0.01)。结论:不同性别、职业、文化程度、家庭人均月收入是^(125)I放射性粒子植入治疗的肿瘤病人孤独感的影响因素,及早对孤独感程度进行评估并提前进行有效干预,改善病人家庭亲密度与适应性情况,对于降低其孤独感程度具有显著效果,对促进病人早日回归家庭与社会具有重大的意义。 展开更多
