Operating room black box:Scrutinizer of theatre practices

Objective:Adverse surgical events are a major cause of morbidity,mortality,and disability worldw ide.The cause of many such events can be attributed to interruptions in the operating room(OR),muli-tasking by surgeons,etc.The objective of this study was to observe the types and frequency of intra-operative wor kflow interruptions in our ORs.Method:This ccoss-sectional study was conducted from March Do April of 2023.An observational approach using an audio-video recording device was employed to record OR flow disr uptions.One elective OR and one emergency OR under the Department of General Surgery were selected for the study.All open and laparoscopic surger ies conducted in the selected ORs were included.An Internet Protocol camera'was installed in the selec ted ORS with a view of the entire room,including the anesthesia station.Audio-video recording was started after the first indsion and stopped after closure of the surgical site.Result:Of the 51 cases that were studied,45(88.2%)were elective,and 18(35.3%)were laparoscopic cases.They could be classified into 8 types of open procedures and 4 types of lapar oscopic procedures.The mean maximum headcount inside the OR was 15.5土3.6 and doors opened on average of 15.8土6.0 times during a procedure.Other interruptions were surgeons attending phone calls(24,47.1%),leaving the sterile area(21,41.2%),technical disturbances(32,62.7%),anesthetic interruptions(18,35.3%),and faulty instruments(29,56.9%)Elective procedures had a signifcandy higher average number of in-terruptions per operating hour than emergency procedures(175±8.6vs.7.1±2.9,p<0.01).Condusion:Preventable factors such as faulty instruments,anesthetic interruption,and attending phone calls by the surgeon are commonly observed in ORs.They need to be addressed by timely surgical audits or the adoption of continued sureillance methods that can help take measures to minimize their occurrence. 展开更多

This study develops an Enhanced Threshold Based Energy Detection approach(ETBED)for spectrum sensing in a cognitive radio network.The threshold identification method is implemented in the received signal at the secondary user based on the square law.The proposed method is implemented with the signal transmission of multiple outputs-orthogonal frequency division multiplexing.Additionally,the proposed method is considered the dynamic detection threshold adjustments and energy identification spectrum sensing technique in cognitive radio systems.In the dynamic threshold,the signal ratio-based threshold is fixed.The threshold is computed by considering the Modified Black Widow Optimization Algorithm(MBWO).So,the proposed methodology is a combination of dynamic threshold detection and MBWO.The general threshold-based detection technique has different limitations such as the inability optimal signal threshold for determining the presence of the primary user signal.These limitations undermine the sensing accuracy of the energy identification technique.Hence,the ETBED technique is developed to enhance the energy efficiency of cognitive radio networks.The projected approach is executed and analyzed with performance and comparison analysis.The proposed method is contrasted with the conventional techniques of theWhale Optimization Algorithm(WOA)and GreyWolf Optimization(GWO).It indicated superior results,achieving a high average throughput of 2.2 Mbps and an energy efficiency of 3.8,outperforming conventional techniques. 展开更多

Digital Disparities:How Artificial Intelligence Can Facilitate Anti-Black Racism in the U.S.Healthcare Sector

This paper delves into the intricate interplay between artificial intelligence(AI)systems and the perpetuation of Anti-Black racism within the United States medical industry.Despite the promising potential of AI to enhance healthcare outcomes and reduce disparities,there is a growing concern that these technologies may inadvertently/advertently exacerbate existing racial inequalities.Focusing specifically on the experiences of Black patients,this research investigates how the following AI components:medical algorithms,machine learning,and natural learning processes are contributing to the unequal distribution of medical resources,diagnosis,and health care treatment of those classified as Black.Furthermore,this review employs a multidisciplinary approach,combining insights from computer science,medical ethics,and social justice theory to analyze the mechanisms through which AI systems may encode and reinforce racial biases.By dissecting the three primary components of AI,this paper aims to present a clear understanding of how these technologies work,how they intersect,and how they may inherently perpetuate harmful stereotypes resulting in negligent outcomes for Black patients.Furthermore,this paper explores the ethical implications of deploying AI in healthcare settings and calls for increased transparency,accountability,and diversity in the development and implementation of these technologies.Finally,it is important that I prefer the following paper with a clear and concise definition of what I refer to as Anti-Black racism throughout the text.Therefore,I assert the following:Anti-Black racism refers to prejudice,discrimination,or antagonism directed against individuals or communities of African descent based on their race.It involves the belief in the inherent superiority of one race over another and the systemic and institutional practices that perpetuate inequality and disadvantage for Black people.Furthermore,I proclaim that this form of racism can be manifested in various ways,such as unequal access to opportunities,resources,education,employment,and fair treatment within social,economic,and political systems.It is also pertinent to acknowledge that Anti-Black racism is deeply rooted in historical and societal structures throughout the U.S.borders and beyond,leading to systemic disadvantages and disparities that impact the well-being and life chances of Black individuals and communities.Addressing Anti-Black racism involves recognizing and challenging both individual attitudes and systemic structures that contribute to discrimination and inequality.Efforts to combat Anti-Black racism include promoting awareness,education,advocacy for policy changes,and fostering a culture of inclusivity and equality. 展开更多

目的采用Box-Behnken响应面法结合BP神经网络优化黑胡椒脐贴处方。方法在前期预试验的基础上,以载药量、促渗剂用量、聚乙二醇(polyethylene glycol,PEG)6000与微粉硅胶的比例为影响因素,进行单因素试验。结合体外透皮试验,利用高效液... 目的采用Box-Behnken响应面法结合BP神经网络优化黑胡椒脐贴处方。方法在前期预试验的基础上,以载药量、促渗剂用量、聚乙二醇(polyethylene glycol,PEG)6000与微粉硅胶的比例为影响因素,进行单因素试验。结合体外透皮试验,利用高效液相色谱法测定胡椒碱的含量,对胡椒碱峰面积、指纹图谱的总峰面积和脐贴的成型性、剥离残留度和基质填充情况赋以权重,所得结果为综合评价指标。在单因素试验的基础上利用Box-Behnken响应面法结合BP神经网络优化脐贴处方。结果黑胡椒脐贴的最佳处方为载药量7%、氮酮4.5%、PEG6000与微粉硅胶的质量比为43∶1。结论黑胡椒脐贴的处方合理,可为后续贴剂的药效学研究提供基础。 展开更多

为研究多特征组合在国产高分卫星影像土地利用分类中的应用效果,本文采用GF6-WFV多光谱影像数据,构建基于光谱波段、植被指数、纹理特征的多特征组合,并采用ReliefF算法进行特征优选获取信息量冗余较小的优选特征集合,结合EnMap-BOX工... 为研究多特征组合在国产高分卫星影像土地利用分类中的应用效果,本文采用GF6-WFV多光谱影像数据,构建基于光谱波段、植被指数、纹理特征的多特征组合,并采用ReliefF算法进行特征优选获取信息量冗余较小的优选特征集合,结合EnMap-BOX工具包寻优改进的SVM算法中惩罚参数C和核函数系数g获取最优分类模型,对研究区进行分类。结果表明:1)特征选择能够较好地降低多特征集合的信息量冗余。2)基于特征选择的改进SVM算法模型能获取较高的土地利用分类精度,总体精度达到82.89%,Kappa系数达到0.78,可以为土地利用分类提供一种具有较高应用价值的方法。 展开更多

In many engineering applications, it is necessary to calculate the min-area encasing box of a circumscription. In this paper, an algorithm for generating the min-area rectangle encasing box, based on revolving angle, is investigated and hence put forward. The algorithm computes the areas of the outer rectangular bounds of a closed contour in different revolving angles θ by dispersing approach where 0< θ < π/2 because of the axial symmetry. It is very simple, straight forward and highly efficient. The complexity of its computing time reaches O(n·k ). Practical applications suggest its usefulness and efficiency. 展开更多

In the study of reservoirs, it is vital that we have a realistic physical model of the reservoir fluid that accurately describes the hydrocarbon system and its properties. The available equations of state (EOS) to model the fluid phase behavior have some inherent deficiencies that may cause erroneous predictions for real reservoir fluids, so these models should be tuned against experimental data by adjusting some parameters. Since there are many matching parameters, tuning the EOS against experimental data is a tedious and difficult work. In this study, a genetic algorithm as an optimization technique is used to solve this regression problem. This study presents a new method that uses a specially designed genetic algorithm to search for suitable regression parameters to match the EOS against measured data. The proposed method has been tested on three real black oil samples. The results show the surprising performance of the developed genetic algorithm to match the experimental data of the selected fluid samples. The main advantage of the used method is its high speed in finding a solution. Also, finding more than one solution, working automatically, confining the role of experts to the last stage, reducing costs and having the possibility of evaluating the different situations are the other advantages of this method to match ordinary black oil PVT data and makes it an ideal method to implement as an automatic EOS tuning algorithm for black oils. 展开更多

Reliability-based design optimization (RBDO) is intrinsically a double-loop procedure since it involves an overall optimization and an iterative reliability assessment at each search point. Due to the double-loop procedure, the computational expense of RBDO is normally very high. Current RBDO research focuses on problems with explicitly expressed performance functions and readily available gradients. This paper addresses a more challenging type of RBDO problem in which the performance functions are computation intensive. These computation intensive functions are often considered as a "black-box" and their gradients are not available or not reliable. On the basis of the reliable design space (RDS) concept proposed earlier by the authors, this paper proposes a Reliable Space Pursuing (RSP) approach, in which RDS is first identified and then gradually refined while optimization is performed. It fundamentally avoids the nested optimization and probabilistic assessment loop. Three well known RBDO problems from the literature are used for testing and demonstrating the effectiveness of the proposed RSP method. 展开更多

A merit function for the KKT-conditions problem is proposed, and some desirable properties of the merit function are obtained. Through the merit function, the original problem is reformulated as minimization with simple constraints. Then, the authors show that any stationary point of the optimization problem is a solution of the original problem. Based on the“three box”exergy analysis model,a black box-gray box hierarchical exergy analysis and evaluation method is put forward in this paper,which is applied to evaluate the power generation technology of differential pressure produced by natural gas expansion.By using the exergy analysis theory,the black box-gray box hierarchical exergy analysis models of three differential pressure power generation technologies are established respectively.Firstly,the“black box”analysis models of main energy consuming equipment are established,and then the“gray box”analysis model of the total system is established.Based on the calculation results of exergy analysis indexes,the weak energy consumption equipment in the whole power generation process is accurately located.Taking a gas field in southwest China as an example,the comprehensive energy consumption evaluation of the three power generation technologies is carried out,and the technology with the best energy consumption condition among the three technologies is determined.Finally,the rationalization improvement measures are put forward from improving the air tightness,replacing the deflector and reducing the flow loss. 展开更多

Internet of Things is an ecosystem of interconnected devices that are accessible through the internet.The recent research focuses on adding more smartness and intelligence to these edge devices.This makes them susceptible to various kinds of security threats.These edge devices rely on cryptographic techniques to encrypt the pre-processed data collected from the sensors deployed in the field.In this regard,block cipher has been one of the most reliable options through which data security is accomplished.The strength of block encryption algorithms against different attacks is dependent on its nonlinear primitive which is called Substitution Boxes.For the design of S-boxes mainly algebraic and chaos-based techniques are used but researchers also found various weaknesses in these techniques.On the other side,literature endorse the true random numbers for information security due to the reason that,true random numbers are purely non-deterministic.In this paper firstly a natural dynamical phenomenon is utilized for the generation of true random numbers based S-boxes.Secondly,a systematic literature review was conducted to know which metaheuristic optimization technique is highly adopted in the current decade for the optimization of S-boxes.Based on the outcome of Systematic Literature Review(SLR),genetic algorithm is chosen for the optimization of s-boxes.The results of our method validate that the proposed dynamic S-boxes are effective for the block ciphers.Moreover,our results showed that the proposed substitution boxes achieve better cryptographic strength as compared with state-of-the-art techniques. 展开更多
