Application of multidisciplinary team-based integrated traditional Chinese medicine and Western medicine in rotator cuff injury patients undergoing arthroscopic surgery

BACKGROUND Arthroscopic rotator cuff repair is a common surgical treatment for rotator cuff injuries(RCIs).Although this procedure has certain clinical advantages,it requires rehabilitation management interventions to ensure therapeutic efficacy.AIM To investigate the effect of integrated traditional Chinese medicine and Western medicine(TCM-WM)under the multidisciplinary team(MDT)model on the postoperative recovery of patients undergoing arthroscopic surgery for RCIs.METHODS This study enrolled 100 patients who underwent arthroscopic rotator cuff repair for RCIs at the Seventh People’s Hospital of Shanghai University of Traditional Chinese Medicine between June 2021 and May 2024.They were divided into a control group(n=48)that received routine rehabilitation treatment and an experimental group(n=52)that received TCM-WM under the MDT model(e.g.,acupuncture,TCM traumatology and orthopedics,and rehabilitation).The results of the Constant–Murley Shoulder Score(CMS),Visual Analogue Scale(VAS),Shoulder Pain and Disability Index(SPADI),muscular strength evaluation,and shoulder range of motion(ROM)assessments were analyzed.RESULTS After treatment,the experimental group showed significantly higher CMS scores in terms of pain,functional activity,shoulder joint mobility,and muscular strength than the baseline and those of the control group.The experimental group also exhibited significantly lower VAS and SPADI scores than the baseline and those of the control group.In addition,the experimental group showed significantly enhanced muscular strength(forward flexor and external and internal rotator muscles)and shoulder ROM(forward flexion,abduction,and lateral abduction)after treatment compared with the control group.CONCLUSION TCM-WM under the MDT model improved shoulder joint function,relieved postoperative pain,promoted postoperative functional recovery,and facilitated the recovery of muscular strength and shoulder ROM in patients with RCIs who underwent arthroscopic rotator cuff repair. 展开更多

Objective:To systematically explore the effect and mechanism of traditional Chinese herbs in the treatment of allergic rhinitis.Methods:Magnolia flower,xanthium fruit,astragalus root,paniculate cynanchum,scutellaria root,cicada molting,stephania root,small centipeda herb,and licorice root as traditional Chinese herbs are widely used to treat allergic rhinitis.With multiple database integration and entity grammar systems,a molecular interaction network of traditional Chinese herbs for treating allergic rhinitis was established.Bioinformatics approaches were adopted to integrate relevant data and biological information by multiple databases.Molecular interaction network of Chinese herbs for treating allergic rhinitis was constructed based on entity grammar systems model.The network was then used for elucidating the antiallergic rhinitis mechanism of the Chinese herbs on the molecular level.The subnetworks were also extracted to explicitly display the pathway and targets where effective components acted on.Results:Traditional Chinese herbs could influence various pathological aspects in allergic rhinitis including the production of pro-inflammatory substances,such as histamine and cytokine interleukin-4(IL-4),interleukin-13(IL-13),lowering immunoglobulin E(IgE)level or blocking antigen binding,altering the biological processes of IgE,modulating the balance of T helper(Th)or other cells by cell proliferation and differentiation,mediating cellecell signaling and second-messenger-mediated signaling,stabilizing the cell membrane,and affecting regulation of cellular defense response.Conclusion:The research theoretically confirms the mechanism of anti-allergic rhinitis by traditional Chinese herbs,provides important fundamental research information for treatment of allergic rhinitis,and may serve as a reference for new drug development and effective ingredients of compatibility. 展开更多

Allergic rhinitis(AR)and is a type of allergic diseases caused by an inflammation of the nasal mucosa caused by allergens.Clinically,AR is a common allergic disease and is difficult to treat.Due to differences in individual constitutions also known as body type,the severity of the disease is also different and it may even affect the daily life of the patient if it is severe.The effective treatment of AR has become a hot topic in current allergic disease research.Traditional Chinese Medicine’s understanding of constitution is based on the“Yellow Emperor’s Internal Classic”book,and Professor Wang Qi put forward the“physical constitution theory of traditional Chinese medicine”.The constitution is a key internal factor of the occurrence and development of diseases.It is believed in Traditional Chinese medicine(TCM)that the constitution can be regulated,and rectifying the patient’s imbalanced constitution has a positive effect on the diagnosis,treatment,and prognosis of the disease.Therefore,based on the“physical constitution theory of TCM”,this study considers the idea of treating body differently applying TCM constitution in the treatment of AR as the breakthrough point,and explores the correlation between AR and TCM constitution.The results show that TCM constitution is an intrinsic factor affecting the pathogenesis of AR.The use of TCM constitution theory in the treatment of AR has a positive effect and should be popularized in application. 展开更多

Objective:To study the clinical effect of traditional Chinese medicine fumigation on treating allergic rhinitis.Methods:80 cases of patients with allergic rhinitis in our hospital from August 2020 to August 2021 were selected and then randomly divided into two groups.In this research,the combination of montelukast sodium tablets and cetirizine was used in the control group.Traditional Chinese medicine fumigation was given in addition to oral medication in the treatment group.Results:The total effective rate of the clinical treatment in the treatment group was higher than that in the control group,and the difference was statistically significant(p<.05).It took less time to relieve nasal itching,runny nose,nasal congestion,and sneezing in the treatment group than in the control group,and the difference was statistically significant(p<.05).Besides,the symptoms were much more relieved in the treatment group than in the control group,and the difference was statistically significant.Conclusions:Traditional Chinese medicine fumigation shows a remarkable clinical effect on the treatment of allergic rhinitis,and it is worthy of clinical application. 展开更多

Objective:TMeta-analysis was performed to evaluate the clinical efficacy of the combination of traditional Chinese medicine and western medicine in treating patients with retinal vein occlusion(RVO)complicated by macular edema(ME).Methods:We searched The CNKI,Wanfang Data,PubMed and Cochrane Library from database.All of randomized controlled trials(RCTs)of the combination of traditional Chinese medicine and western medicine in treating patients with RVO complicated by ME were searched.The Revman5.3 software were used for meta-analysis.Results:A total of 30 trials involving 1943 patients and 1976 eyes were included.Meta-analysis showed that compared with western medicine,the combination of traditional Chinese medicine and western medicine can significantly improve the best corrected visual acuity[(MD=0.08,95%CI=0.07~0.10,P<0.00001)(MD=-0.16,95%CI=-0.24^-0.08,P=0.0002)(MD=5.78,95%CI=1.30~10.25,P=0.01)],reduce the central macular thickness(MD=-54.6,95%CI=-73.76^-35.43,P<0.00001),and improve the clinical effective rate(RR=1.32,95%CI=1.22~1.43,P<0.00001).Conclusion:The combination of traditional Chinese and western medicine in treating patients with RVO complicated by ME was superior to western medicine in improving the best corrected visual acuity,reducing the thickness of macular fovea and improving the clinical efficacy. 展开更多

Aimed at solving the difficult problems in metastasis of tumor, many TOM practitioners on the basis of syndrome differentiation have raised theories about the cause of tumor metastasis. On the basis of these theories, some TOM recipes against tumor metastasis have been developed to serve as an effective supplement to surgical operation, radio- and chemotherapy. Objective With the increasing publication of clinical practice guidelines(CPG)for Traditional Chinese Medicine(TCM)and Integrated Traditional Chinese and Western Medicine(IM),the standardization and scientifiction of its formulation have gradually attracted many people’s attention.To offer an overview of TCM and IM CPGs published over the past decade and analyze their general characteristics and methodological quality.Methods The China National Knowledge Infrastructure(CNKI)and WANFANG databases were searched for clinical practice guidelines and expert consensus papers from January 2010 to June 2021.Two researchers independently completed the literature screening and cross-checking according to the inclusion and exclusion criteria of CPGs and extracted information on general characteristics and methodological quality of CPGs.Results According to the selection criteria,231 CPGs(EB-CPGs=119,CBCPGs=112)were selected and the number of CPGs published in the 11 years showed an overall upward trend.The vast majority of CPGs used the Western naming system for the diseases,and only 11 CPGs were named of TCM diseases or symptoms.TCM treatments were recommended in 223 CPGs.There were 156 ancient Chinese Medicine literature sources cited in 231 CPGs and opinions and experiences of 62 TCM experts cited in 37 CPGs.The methodological quality of EB-CPGs for TCM and IM were significantly better than CB-CPGs in 11 items.Only 60 EB-CPGs and 7 CB-CPGs designated clear criteria for grading quality of evidence and strength of the recommendations and 74 CPGs presented both the level of evidence and the strength of recommendations.We classified all CPGs according to whether or not they used GRADE,and the results showed that the CPGs using GRADE had higher methodological quality and more standardized reports.Conclusion This research has shown that the quantity and quality of CPGs in both TCM and IM have improved over the time span,but the methodological quality,especially evidence citation,and the use of criteria for grading quality of evidence and strength of the recommendations,still needs to further improvement in the future. 展开更多

Objective:To explore the clinical effect of combination of traditional Chinese medicine and western medicine in treatment of vascular aging in patients with type 2 diabetes complicated by hypertension.Methods:Ninety patients with type 2 diabetes complicated by hypertension admitted to our hospital from May 2016 to August 2019 were selected as research objects.They were randomly divided into control group and observational group,with 45 cases each.Control group was given amlodipine besylate combined with metformin hydrochloride.On the basis of control group,observational group was given combination of TCM syndrome differentiation.Blood glucose,blood pressure and blood lipids before and after 14 days of treatment were compared between two groups.Results:Blood glucose,blood pressure and lipid indexes after treatment were lower than before treatment in both groups;observational group was lower than control group and the difference was statistically significant(P<0.05).Conclusion:Combination of traditional Chinese medicine and Western medicine could lower blood glucose and blood pressure indexes,control blood lipids and delay blood vessel aging in patients with type 2 diabetes complicated by hypertension,it is worthy of clinical popularization. 展开更多

Therapeutic effect of Western Medicine plus Traditional Chinese Medicine in the treatment of upper gastrointestinal tract perforation

OBJECTIVE: To explore the clinical efficacy of Traditional Chinese Medicine(TCM) combined with Western Medicine in the treatment of upper gastrointestinal tract perforation.METHODS: Totally 100 patients with upper gastrointestinal tract perforation hospitalized between January 2010 and January2015 were included and randomly divided into the control group and the observation group, 50 patients in each group. The patients in the control group received the conventional nonsurgical treatment of Western Medicine, whereas those in the observation group were treated by TCM plus the treatment given to the control group. The first period was defined as the closed perforation period, during which electro-acupuncture was performed at acupoints of Zusanli(ST36) and Zhongwan(Ren 12); the second period as the absorption period, during which Dachengqi Decoction was administered to restore normal bowel movement; the third period as the ulcer healing period,during which the TCM treatment was admini-stered based on the formula of Pingweisan. Patients in both groups were reexamined by endoscopy after 2 weeks of treatment.RESULTS: The time to relieve abdominal pain, the time to pass gas and the length of stay in the observation group were significantly improved compared with those in the control group(P<0.05). The cure rate of the observation group was significantly higher than that of the control group(P<0.05).After two weeks of treatment, in the observation group, the cured patients showed closed perforation, with disappearing mucosal congestion and edema, less white fur on the ulcers and shallower ulcers in the size of 3 to 5 mm; the ulcer was in the healing period and none of the patients showed acute perforation again. While in the control group, two patients were given endoscopy after 10 days of treatment, after which ulcer and acute perforation recurred.CONCLUSIONS: The treatment of upper gastrointestinal tract perforation by Western Medicine and TCM in combination could, at different stages, significantly shorten the time to relieve abdominal pain, the time to pass gas, and the length of stay, thus improving the cure rate. 展开更多

Objective:To summarize the current treatments of traditional Chinese medicine and Western medicine and to explore ideas for clinical treatments,rational use of traditional Chinese medicine,avoid medication errors,and make full use of the respective advantages of traditional Chinese medicine and Western medicine to achieve good results.Methods:We selected recent articles on pulmonary sarcoidosis and clinical observation at home and abroad,and summarized relevant traditional Chinese medicine books.Results:At present,hormone therapy is preferred in western medicine,followed by antimetabolic drugs,Anti-TNF,and also potential drug therapeutic strategies,such as"CLEAR"regimen and new target therapy.Traditional Chinese medicine through syndrome differentiation,from the whole,flexible use of dispel evil,phlegm,blood circulation,dredging collaterals,qi and other methods to achieve the effect of dissolution.Conclusion:The treatment research results of pulmonary sarcoidosis are considerable,but there are still many risk factors in Western medicine,and the benefits and treatment indications of individual drugs are still under study.Traditional Chinese medicine has certain safety and efficacy in the prevention and treatment of pulmonary sarcoidosis,but it needs modern medical technology examination.Therefore,the advantages of traditional Chinese and Western medicine should be fully combined to achieve the maximum treatment effects. 展开更多
