Carpal tunnel syndrome(CTS)is a multifactorial compression neuropathy.It is reported to be very common and rising globally.CTS’s treatment varies from conservative measures to surgical treatments.Surgery has shown to...Carpal tunnel syndrome(CTS)is a multifactorial compression neuropathy.It is reported to be very common and rising globally.CTS’s treatment varies from conservative measures to surgical treatments.Surgery has shown to be an effective method for more severe cases.However few unclear aspects and room for further research and improvements still remains.We performed a narrative literature review on the most up to date progress and innovation in terms of surgical treatments for CTS.The simple algorithm of leaving the choice of the surgical method to surgeons’preference and experience(together with consideration of patients’related factors)seem to be the best available option,which is supported by the most recent metanalysis and systematic reviews.We suggest that surgeons(unless in presence of precise indications towards endoscopic release)should tend to perform a minimally invasive open approach release,favoring the advantage of a better neurovascular structures visualization(and a consequent higher chance to perform a complete release with long term relief of symptoms)instead of favoring an early reduction(in the first postoperative days)of immobilization and pain.Research towards a universally accepted standardization should be aimed for by the researchers,who have failed to date to sufficiently limit bias and limitations.展开更多

Therefore, according to the type of building structures, the airflow also has different forms. Studies have been conducted on the relation between airflow and pedestrian comfort; however, only few of them have focused on comprehensive urban planning that considers different weather conditions and people's ability to adapt. This research focuses on the differences in wind conditions caused by different spatial forms in different seasons. On the basis of a field survey in both summer and winter in a public open space, evaluation standards developed from environmental meteorological data and public feedback were used to evaluate simulation results. Next, several assumptions about canyon orientation and building types were proposed. Wind tunnel and CFD (computational fluid dynamics) simulations were conducted to evaluate the assumptions. The results showed that the canyon orientation significantly affected overall wind conditions and different building structures affected airflow. This research also provides a method to evaluate urban areas that have complicated wind environments.展开更多
This paper presents a new analytical solution for assessing the longitudinal deformation of shield tunnel associated with overcrossing tunnelling in consideration of circumferential joints.A simplified longitudinal be...This paper presents a new analytical solution for assessing the longitudinal deformation of shield tunnel associated with overcrossing tunnelling in consideration of circumferential joints.A simplified longitudinal beam-spring model(SLBSM)is established to model the longitudinal behaviours of shield tunnel,which can consider the opening and dislocation between segmental rings simultaneously.Then,the existing tunnel is treated as the SLBSM resting on the elastic foundation.The state equations including tunnel displacements and internal forces are constructed to solve the discontinuous deformation of circumferential joint-segmental ring.The feasibility of the proposed solution is verified through three well-documented cases.The predictions from the proposed method are also compared with other analytical methods.It is found that the proposed method can well capture the deformation of tunnel segmental rings and joints,where the rigid displacement mainly occurs in the segmental rings while the rotation and dislocation occur in the circumferential joints.Some dominant parameters are also analysed to explore the effects on existing tunnel deformation,including the rotation stiffness and shearing stiffness of joints,the skew angle and the clearance between new and old tunnels.展开更多
