Argon laser photocoagulation versus intrastromal voriconazole injection in treatment of mycotic keratitis 被引量:2

AIM: To compare argon laser photocoagulation and intrastromal injection of voriconazole as adjunctive treatment modalities in cases of resistant mycotic corneal ulcers. METHODS: Two groups each of them included 20 cases of resistant mycotic corneal ulcers. Both groups treated with local and systemic specific antimicrobial drugs guided with culture and sensitivity results. In one group argon laser photocoagulation was used as an adjunctive therapy to the specific antifungal drugs and in the other group, intrastromal injection of voriconazole was done besides the specific antifungal drugs. The 40 cases included in the study were proven according to culture and sensitivity to be 28 cases with pure fungal results and 12 cases with mixed (fungal and bacterial). In argon laser group, argon laser irradiation of the corneal ulcer was performed using argon laser 532 nm wavelength (Carl Zeiss LSL 532s AG; Meditec, Inc.) after fluorescein staining. In the other group, voriconazole solution (500 pg/mL) was prepared and injected in the corneal stroma. All cases were followed up for 3mo after healing was achieved. RESULTS: Complete healing of the epithelial defect and resolution of stromal infiltration with no adverse effects were achieved in argon laser group in duration ranged from 2-4wk in 90% of cases. In voriconazole group 4 cases needed amniotic membrane graft due to thinning and 16 cases healed in duration ranged from 2- 6wk (80% of cases). CONCLUSION: Argon superior to intrastromal treatment of resistant fungal laser photocoagulation is voriconazole injection in corneal ulcers. 展开更多

Densities of Ar metastable states 1s5 and 1s3 are measured by using the tunable diode laser absorption spectroscopy(TDLAS) in Ar and Ar/O2 mixture dual-frequency capacitively coupled plasma(DF-CCP). We investigate the effects of high-frequency(HF, 60 MHz) power, low-frequency(LF, 2 MHz) power, and working pressure on the density of Ar metastable states for three different gas components(0%, 5%, and 10% oxygen mixed in argon). The dependence of Ar metastable state density on the oxygen content is also studied at different working pressures. It is found that densities of Ar metastable states in discharges with different gas components exhibit different behaviors as HF power increases. With the increase of HF power, the metastable density increases rapidly at the initial stage, and then tends to be saturated at a higher HF power. With a small fraction(5% or 10%) of oxygen added in argon plasma, a similar change of the Ar metastable density with HF power can be observed, but the metastable density is saturated at a higher HF power than in the pure argon discharge. In the DF-CCP, the metastable density is found to be higher than in a single frequency discharge, and has weak dependence on LF power. As working pressure increases, the metastable state density first increases and then decreases,and the pressure value, at which the density maximum occurs, decreases with oxygen content increasing. Besides, adding a small fraction of oxygen into argon plasma will significantly dwindle the metastable state density as a result of quenching loss by oxygen molecules. 展开更多

Purpose: To evaluate the therapeutic effect of combind argon and Nd: YAG laser peripheral iridectomy.Methods: 151 cases (200 eyes) of primary angle-closure glaucoma and combined glaucoma were treated by combined procedure. Argon laser was the first used to create partial iridectomy in 2/3-3/4 thickness without penetrating pigmented epithelium, and then Nd: YAG laser was required to complete a patent iridectomy. Results: A successful iridectomy was achieved in all patients independent of any type of irides (100%). Iris penetration rate in one session presented in 95. 0%. Complications appeared to be less common in our patients: iris bleeding was only seen in 5. 0% cases; the postoperative inflammation was mild; corneal burns, lecalized lenticular opacities and closure of iridectomies were less frequent. The follow-up ranged from 3m to 6. 5 years. The success rate in controling IOP with/ without medications were 99. 0% except 2 eyes failur. No attack occured in 129 eyes with prophylatic laser therapy 展开更多

The incoming and out going states of electron are described by the Volkov wave functions, and the electron-target interaction is treated as a screening potential. 目的:探讨Argon激光治疗糖尿病视网膜病变(DR)的疗效和相关影响因素。方法:对1995/2002在我院实施Argon激光治疗糖尿病视网膜病变患者504例(613眼)就疗效和有关影响因素加以回顾性总结与分析。结果:单纯性和增殖性DR视力不变和增进者分... 目的:探讨Argon激光治疗糖尿病视网膜病变(DR)的疗效和相关影响因素。方法:对1995/2002在我院实施Argon激光治疗糖尿病视网膜病变患者504例(613眼)就疗效和有关影响因素加以回顾性总结与分析。结果:单纯性和增殖性DR视力不变和增进者分别为84%和79%,下降者分别为16%和21%。单纯性DR视力下降以黄斑出血、渗出水肿和晶状体混浊加重为主;增殖性DR视力下降以玻璃体积血、持续黄斑水肿、晶状体混浊加重、新生血管性青光眼和牵引性视网膜脱离为主。伴有高血压、糖尿病性肾病者和Ⅰ型糖尿病患者光凝术后视力下降者居多(33%~85%)。眼底和荧光血管造影随访,单纯性DR83%视网膜水肿、渗出和出血吸收;增殖性DR51%视网膜水肿、渗出和出血吸收,新生血管退缩。16%单纯性DR和34%增殖性DR需要再次补充光凝。结论:DR光凝术应根据眼底和FFA改变确定光凝治疗范围,选择合适波长激光和灵活合理应用各项激光参数,以减少并发症和获得高比例的视网膜有效光斑,同时平稳控制血糖,积极改善患者全身状况,重视光凝术后定期的随访观察,必要时补充光凝。 展开更多

AIM:To document the prognosis of laser treatment in patients with exudative age-related macular degeneration(AMD).· METHODS:Efficacy of the intervention was evaluated using a before-after method.· RESULTS:A total of 392 eyes of 341 patients with exudative AMD were examined.77.6% had choroideal neovascularisation(CNV).Before the use of indocyanine green(ICG) angiography,occult CNV was detected in only 1.8% of the eyes,but after the use of ICG angiography,this increased to 19.5%(P<0.001).Of the 349 eyes which were followed up,visual acuity had remained stable in 68.2% of the eyes.There was a statistically significant relationship between localisation of lesion and visual acuity changes on pre-and post-laser treatment(P<0.001).Also there was a statistically significant relationship between localisation of lesion and recurrence(P<0.05).The recurrence was less in subfoveal lesions than that in juxtafoveal and extrafoveal lesions.· CONCLUSION:ICG angiography is highly important in the treatment of occult CNV. 展开更多

FITC-conjugated nanoferrofluid (FNFF) was synthesized and characterized to study the dynamic of laser-induced transport of NPs in water. The results confirmed a definite laser-induced enhanced velocity of NPs (100 &mu;m&sdot;s&minus;1) almost twice as much the without laser (i.e. Brownian motion). The diffusion coefficients of 17 × 10&minus;6 m2&sdot;s&minus;1 and 55 × 10&minus;6 m2&sdot;s&minus;1 were found for the cases without and with laser action respectively. The act of laser when switched on after NPs had reached the steady state was very prominent. The laser-induced heat and power generated by NPs were calculated 0.2&mu;W&sdot;cm&minus;3 and 0.4 pW&sdot;cm&minus;2 respectively. Our experiment condition was non-adiabatic and that the heat generated was diffused into the surrounding. We considered the Maxwell’s criteria (Kp/Kw &minus;1&sdot;K&minus;1. Based on the Brownian diffusion and DLVO theory, at earlier times where the NPs are more dispersed within the medium are displaced faster. However, at later stages they become less mobile as they are agglomerated. The mechanisms for the enhanced mobility and laser transport of NPs are thought to be due to e.m.w induced force (i.e. an oscillatory motion) and laser absorptive force (i.e., photothermophoresis). A beam divergence of about 5.24&deg;(or 91 mrad) was determined. A non-linear behaviour of laser beam was observed as a trajectory path within the water due to thermal heating hence causing the change of refractive index of medium and redistribution of NPs concentration. 展开更多

The HfO<sub>2</sub> amorphous single layers have porous surface morphologies. Different processes will cause differences in coatings absorption and surface morphology, which in turn will cause changes in the spectral transmittance curve. The ion beam sputtering HfO<sub>2</sub> single layers have high content of argon (4.5% - 8%). The laser damage of HfO<sub>2</sub> single layers is related to argon inclusions and non-stoichiometric defects. The changes of argon flux and beam voltage have a greater impact on argon content and O/Hf ratio. When the argon content in the coatings is lower and the O/Hf ratio is higher, the laser damage thresholds of the HfO<sub>2</sub> single layers are higher. </div> 展开更多

目的探讨康柏西普注射液联合氩激光视网膜光凝术对糖尿病视网膜病变(diabetic retinopathy,DR)患者最佳矫正视力及血液流变学指标的影响。方法选取2020年3月—2023年3月襄城县人民医院收治的182例DR患者作为研究对象,以随机数字表法分... 目的探讨康柏西普注射液联合氩激光视网膜光凝术对糖尿病视网膜病变(diabetic retinopathy,DR)患者最佳矫正视力及血液流变学指标的影响。方法选取2020年3月—2023年3月襄城县人民医院收治的182例DR患者作为研究对象,以随机数字表法分为观察组、对照组,各91例。对照组采取氩激光视网膜光凝术治疗,观察组在此基础上采取康柏西普注射液治疗。比较两组治疗前后患眼病变情况(视野灰度值、出血斑面积、血管瘤体积)、最佳矫正视力、视网膜厚度变化情况、血液流变学指标[血浆黏度(PV)、红细胞沉降率(ESR)、红细胞聚集指数(AE)、红细胞变形指数(DE)]、生化指标[血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、胰岛素样生长因子-1(IGF-1)]及并发症发生情况。结果治疗后观察组视野灰度值、出血斑面积、血斑体积、视网膜厚度小于对照组,最佳矫正视力高于对照组(P<0.05);治疗后观察组PV、ESR、AE、DE水平低于对照组(P<0.05);治疗后观察组VEGF、bFGF、IGF-1水平低于对照组(P<0.05);观察组并发症总发生率(5.49%)低于对照组(32.97%)(P<0.05)。结论康柏西普注射液、氩激光视网膜光凝术联合治疗效果显著,能有效抑制视网膜新生血管生成,改善其局部血液微环境、DR临床症状,提高最佳矫正视力,降低并发症发生率。 展开更多

目的:分析LASIK术前检查发现的近视患者视网膜病变的特征,评价眼底氩激光光凝治疗的疗效与安全性。方法:回顾性研究,分析了施行LASIK术的连续性病例1 100例(2 190眼)的术前资料,在对所有患者术前扩瞳后以检眼镜、三面镜详细检查眼底周... 目的:分析LASIK术前检查发现的近视患者视网膜病变的特征,评价眼底氩激光光凝治疗的疗效与安全性。方法:回顾性研究,分析了施行LASIK术的连续性病例1 100例(2 190眼)的术前资料,在对所有患者术前扩瞳后以检眼镜、三面镜详细检查眼底周边视网膜情况并记录,对严重的视网膜病变如格变、囊样变、干性裂孔、视网膜浅脱离等予以氩离子激光光凝治疗,经随访稳定有效后再择期行LASIK手术。结果:视网膜病变及视网膜裂孔的发生率均随屈光度增加而提高,组间有显著性差异(P<0.05),颞侧病变比例高于鼻侧。在全部患眼的检查中,发现各类视网膜变性132眼,64眼发现格子样变性(48.48%)、37眼发现囊样变性(28.03%)、54眼发现霜样变性(40.91%)、34眼发现不压白变(25.76%)、12眼发现铺路石样变性(9.09%)、14眼发现干性裂孔(10.61%)、4眼发现裂孔伴局限性浅脱离(3.03%)。对出现较严重视网膜病变的43例患者(78眼)施行了眼底氩激光治疗。复诊时见激光斑反应明显、干性裂孔稳定,局限性浅脱离患者经2~4次激光,光凝后视网膜完全平复。对眼底经光凝后行LASIK的患者,在3~6mo复诊中无1例视网膜脱离发生。结论:LASIK术前对全部患者进行常规三面镜详查眼底是十分有必要,对发现的周边视网膜严重变性及时予以激光光凝能够有效防止视网膜脱离的发生。 展开更多

目的:观察氩激光视网膜光凝治疗视网膜静脉周围炎(Eales病)的效果。方法:对经荧光素眼底血管造影(FFA)检查确诊为Eales病患者28例52眼,按眼底病变区域行氩激光光凝,每周一次,分别为1~4次,3mo后FFA复查,按眼底病变好转与否决定是/否补... 目的:观察氩激光视网膜光凝治疗视网膜静脉周围炎(Eales病)的效果。方法:对经荧光素眼底血管造影(FFA)检查确诊为Eales病患者28例52眼,按眼底病变区域行氩激光光凝,每周一次,分别为1~4次,3mo后FFA复查,按眼底病变好转与否决定是/否补充激光光凝,6mo后再行FFA检查;追踪复查30例50眼药物治疗Eales病患者的治疗效果,并行FFA检查。比较氩激光视网膜光凝与药物治疗的治疗前、后视力、眼底变化,评价治疗效果。结果:氩激光视网膜光凝治疗Eales病28例52眼,有效49眼(94%),无效3眼(5.8%)。药物治疗Eales病30例50眼,有效30眼(60.0%),无效20眼(40.0%)。结论:氩激光视网膜光凝治疗Eales病效果优于药物治疗。FFA检查对Eales病的发病机制、诊断、鉴别诊断、氩激光视网膜光凝、早期治疗及疗效观察提供重要的指导依据。 展开更多

目的:探讨低能量氩离子激光在防止翼状胬肉术后复发中的作用。方法:将90例原发性翼状胬肉术后一周术区结膜或角膜缘出现新生血管胚芽的病人随机分为3组,A组为低能量氩离子激光组,以功率0.1~0.15W,时间0.2s,计数56~187点进行光凝,每次... 目的:探讨低能量氩离子激光在防止翼状胬肉术后复发中的作用。方法:将90例原发性翼状胬肉术后一周术区结膜或角膜缘出现新生血管胚芽的病人随机分为3组,A组为低能量氩离子激光组,以功率0.1~0.15W,时间0.2s,计数56~187点进行光凝,每次间隔1周,共1~3次;B组为常规能量氩离子激光组,以功率0.2~0.3W,时间0.2s,计数47~166点进行光凝;C组为对照组不行光凝。结果:A组激光后并发症(结膜灼伤、结膜出血)较B组少(P<0.05),随访一年,复发率A组为6.67%,B组为10%,C组为33.33%。A组与C组比较差异有统计学意义(P<0.01);B组与C组比较差异有统计学意义(P<0.05);A组与B组比较差异无统计学意义(P>0.05)。结论:低能量氩离子激光封闭胬肉术后一周新生血管胚芽,能降低胬肉术后复发率,操作简单,病人痛苦少,可作为防止翼状胬肉术后复发的一种有效方法。 展开更多
