Objective:To detect common chromosomal aneuploidy variations in embryos from couples undergoing assisted reproductive technology and preimplantation genetic screening and their possible associations with embryo quality. Methods:In this study,359 embryos from 62 couples were screened for chromosomes 13,21,18,X,and Y by fluorescence insitu hybridization. For biopsy of blastomere,a laser was used to remove a significantly smaller portion of the zona pellucida. One blastomere was gently biopsied by an aspiration pipette through the hole. After biopsy, the embryo was immediately returned to the embryo scope until transfer.Embryo integrity and blastocyst formation were assessed on day 5.Results:Totally,282 embryos from 62 couples were evaluated. The chromosomes were normal in 199(70.57%)embryos and abnormal in 83(29.43%)embryos.There was no significant association between the quality of embryos and numerical chromosomal abnormality(P=0.67).Conclusions:Embryo quality is not significantly correlated with its genetic status.Hence, the quality of embryos determined by morphological parameters is not an appropriate method for choosing embryos without these abnormalities. 展开更多

Background and Purpose: A growing number of couples/partners have been undergoing assisted reproductive technology (ART) in Japan. The purpose of this study was to clarify the support from healthcare professionals based on the support vectors in the Family Care/Caring Theory proposed by Hohashi (2015) through a literature review. Methods: Using Ichushi-Web, we searched for original articles using the keywords “assisted reproductive technology”, “infertility”, “family”, “couple”, “nursing”, “care”, and “support”. Thirteen articles suitable for the purpose of this study were subjected to content analysis. Family support was encoded and grouped into subcategories and categories, and classified according to support vectors. Results: A total of 21 categories of support from healthcare professionals was extracted. Intervention for family internal environment included seven categories, such as “Nursing professionals stay close to females”. Intervention for family system unit included five categories, such as “Nursing professionals adjust couple/partner relationships”. Intervention for micro system only included “Nursing professionals provide opportunities for peer support to the couples/partners” and intervention for macro system only included “Nursing professionals encourage medical doctors to relate to females”. Intervention for family chrono environment included seven categories, such as “Healthcare professionals resolve female’s anxieties”. Conclusion: The support from healthcare professionals could be organized by the support vectors of Family Care/Caring Theory, but intervention for supra system (culture, religion, etc.) was lacking. Moreover, because most support was directed toward females or couples/partners, male-focused direct and/or indirect support are also needed. . 展开更多

Introduction: Infertilityaffects one in six couples, and it is an important public health issue largely due to thepervasive effects on the emotional and psychological wellbeing of affected couples. In many developing nations emphasis is placed on childbirth and inability to fulfill this role can be very distressing. There is an unmet need for assisted reproductive technology (ART) in many developing countries and where facilities exist, they are mostly privately owned, expensive and concentrated in urban areas. To bridge this gap, public fertility clinics have been established to provide subsidized care. Evaluating the characteristics and peculiarities of clientele presenting at these public facilities will aid planning and prioritization of care. Methodology: A descriptive retrospective study of 116 infertile patients presenting to the fertility clinic of the University College Hospital, Ibadan, Nigeria from inception on the 14th of February 2019 and 31st of December 2022.Data was analyzed using the Statical Package for Social Sciences (IBM, SPSS, New York) version 23. Descriptive statistics were used to summarize the results which were presented with the aid of bar charts and frequency tables. Result: The mean age of the patients was 40.70 ± 6.62 years. Post-menopausal patients accounted for about one-fifth of the study population while 80.2% (93 women) were older than 35 years. The mean duration of infertility was 9.39 ± 6.11years and nine patients (7.8%) had a duration greater than 2 decades. Secondary infertility occurred in 67.2% of the women. Twenty-nine women (25%) had undergone myomectomy prior to presentation. Hypertension (11.2%) was the most prevalent comorbidity. Nineteen patients (16.4%) had used contraceptives in the past with the male condom (36.8%)being the most preponderant. Sixty-seven patients had experienced pregnancy losses before 28 weeks of gestation while just 16 patients (13.8%) had undergone ART, and none was successful. Conclusion: Secondary infertility was the prevalent type of infertility and may not be unconnected with the low contraceptive usage and high risk of sexually transmitted infection. Late presentation coupled with a large proportion of post-menopausal clientele suggests delayed health-seeking behavior most probably due to the prohibitive cost of ART. The need to streamline services offered in public fertility clinics is paramount in low-income countries grappling with scarce resources. A pragmatic approach will involve the provision of low-cost ART, while enhancing gamete donation programs through the implementation of gamete sharing policies. This will invariably bridge the unmet need and skewed access to ART in developing countries. 展开更多

A person diagnosed with Alzheimer’s slowly loses cognitive function and ultimately becomes entirely dependent upon the caregivers. Caregivers must help the patient in everyday activities ranging from walking to cooking, eating, and so on. In most caregiving facilities, a single caregiver often handles more than one patient, which results in caregiver burnout. Researchers are developing useful technologies to prevent caregiver burnouts and facilitate families in the best possible manner. The goal of this purposive short review of literature is to study the modern tools, devices, and gadgets available to Alzheimer’s patients and caregivers and understand the focus areas for future research. The review identified a range of products and technologies that help in monitoring to diagnosis, aid therapy, and reduce the burden on caregivers. These technologies play a vital role in improving the quality of life for both the patients and the caregivers. The study identified reducing cost of the devices, increasing robustness and dependability of the devices, and various aspects of the assistive technologies, including ethical and privacy issues, as the focus areas for future research. 展开更多

The participants were 50 primiparas who had undergone ART at two fertility treatment facilities in a metropolitan area in Japan. For the infertility reflection, they conducted an online reflection. Data were collected three times: at the 5th week of pregnancy (Time 1), the 8th week of pregnancy as the final consultation at the clinic (Time 2), and the 16th week of pregnancy as the final point of early pregnancy (Time 3). <strong>Results: </strong>The data from 40 participants were analyzed. More than 80% of the users of the online reflection positively evaluated the appropriateness and usefulness of the methods and contents. Organized thoughts and feelings by reflection were shown as the reasons for the usefulness. The evaluation of the online reflection showed a relatively strong correlation with the Care Need Satisfaction Scale (CNSS) for both Time 2 and Time 3, but the online reflection did not show a significant correlation with the other outcome variables. There were no significant differences in outcome variables between users and non-users of online reflection between Time 2 and Time 3. <strong>Conclusions:</strong> Attempts at the reflection in early pregnancy require modified methods that do not have a negative impact and lead to the fulfillment of needs. 展开更多

In the field of philosophy of technology, the concept of mediation is central to understanding how technology shapes human experience and behavior. Our aim in this paper is to contribute to the understanding of technological mediation,in particular how and why it is possible.Technological mediation occurs within a mediation space between the technological realm and the user realm. In the technological realm, technology regularizes events and actions, while in the user realm, the user interprets the significative potential of the technology. This interpretation process is identical with use and involves the formation of user habits, which are constrained by technological, cognitive, and sociocultural factors. To provide a theoretical framework for our analysis,we propose the Semiotic Model of Technological Mediation(SMTM),which draws on Charles Peirceʼs semiotics. To illustrate our argument,we focus on assistive technology and provide a recurring example of the Medimi®Smart,a digital, comprehensive system for medication handling. 展开更多

Reproduction technologies(RTs)can provide for the reliable reproduction of amphibians,as well as perpetuation of species genetic variation with the use of biobanks. In 1982,in anticipation of the biodiversity conservation crisis, major Russian institutions collaborated in a dynamic program to develop and implement RTs for the sustainable management of amphibian biodiversity. An initial primary focus was the captive breeding of threatened Russian endemic anuran and caudate species, using RTs that varied from environmental manipulation to the use of exogenous gonadotropic hormones to stimulate reproduction. These species were mostly from Palearctic or cool mountain regions, but also included a wide range of species from warm regions.Other early achievements included the successful cryopreservation of anuran spermatozoa and anuran diploid pluripotent cell nuclei,in order to store both the matrilineal and patrilineal genomes in biobanks, with their subsequent development to the blastula stage after implantation into enucleated oocytes. After the turn of the 21st Century,in support of the priorities of the Amphibian Conservation Action Plan(2007),we developed RTs for the refrigerated storage of testicular or urinary spermatozoa for days to weeks at 4℃,the cryopreservation of urinary spermatozoa using anovel cryoprotectant, the in vitro fertilisation of hormonally induced oocytes either fresh or after refrigerated ex situ or in situ storage, and the artificial insemination of salamanders with fresh spermatozoa. In this article,we describe previously unpublished techniques and techniques from obscure Russian sources. 展开更多

Objective:To investigate the effects of female body mass index on embryo development and assisted reproductive technology outcomes, aiming to provide better treatment for patients with different body mass index and provide reference for clinical treatment. Methods:The study retrospectively collected data of 3783 patients who received their first fresh embryo transfer and were ovulated by a long protocol at the Reproductive Medicine Center of the First Affiliated Hospital of Hainan Medical University from January 2015 to October 2021.Patients were divided into four groups based on body mass index(BMI):low weight group, normal weight group, overweight group and obese group. The normal weight group was used as a control to compare the basic information, assisted reproductive technology process, embryo development and assisted reproductive technology outcomes between different groups. Results:Analyzing patients'basic information,we found that the duration of infertility was significantly longer in obese women(P=0.007).Basal hormone levels in the overweight and obese groups were lower than those in the normal group(P<0.05).Basal Follicle-stimulating hormone(FSH),basal Luteinizing hormone(LH),basal Estradiol(E2),basal Progesterone(P),and anti-Mullerian hormone(AMH)in the obese group were lower than the normal weight group(P<0.05),and the number of antral follicle counting(AFC)was reduced in the obese group(P=0.011).The overweight group only showed a decrease in E2 and P levels(P<0.05).During the ART,there was a significant difference in Gonadotropin(Gn)dosage among the four groups, with the obese group was the most, followed by the overweight group, and the low weight group was the least(P<0.001).Gn days were increased in the obese group(P<0.001).LH,E2,and P on trigger day were all lower in the overweight and obese groups than in the normal weight group(P<0.05).Comparing the embryo development process,we found that the blastocysts of the obese group showed delayed development at the stages of pronuclei disappearance,four-cell and blastocyst formation(P<0.05).The ART outcomes were worse in the obese group, the clinical pregnancy rate(P=0.044)and live birth rate(P=0.036)were reduced in the obese group. After logistic regression, obesity was found to be a risk factor for clinical pregnancy(OR=0.683,95%CI:0.479-0.973,P=0.035)and live birth(OR=0.662,95%CI:0.459-0.954,P=0.027).Female age was a risk factor for biochemical pregnancy, clinical pregnancy and live birth(P<0.05).Conclusion:Female obesity prolongs the duration of infertility, causes endocrine disorders, increases Gn dosage and days, and leads to poorer assisted reproductive technology outcomes.Female obesity delays the blastocyst development process and presents as a risk factor for clinical pregnancy and live birth. 展开更多

Through a series of duly cited, and quoted (“–”), excerpts, originating mostly from two Technology Trends reports, WIPO (2019), WIPO (2021), an attempt to establish a pseudo dialog is proposed, in order to surface a few questions accessible to a larger public of innovators, who may have an interest in the IP accessibility dimension using Frontier Technologies (such as AI, IoT, 3D…). AT, Assistive Technology, is instrumental as a uniquely faceted example. The illustrations’ section extends the consideration and integration of additional “traits” of the field of interest and related. Intuitive answers are “reachable” and further literature is provided on the relevant matters and connected. The emphasis is on educational, illustrative and demonstrative value. 展开更多

Objective:To investigate the influence of season on live birth and clinical pregnancy rates,as well as assisted reproductive technology(ART)outcomes,in the Hainan region. Methods:Patients were categorized into four groups based on the dates of artificial insemination and transplantation:spring, summer, autumn,or winter. The main outcome measures were clinical pregnancy rates and live birth rates.Secondary outcomes included body mass index(BMI),oocyte number, two pronuclei(2PN)cleavage rate, total gonadotropin(Gn)dosage and days,age,2PN fertilization rate, sperm concentration, sperm PR rate,anti-Müllerian hormone(AMH),and endometrial thickness.Outpatient semen quality indicators included sperm PR rate, total sperm count, sperm concentration, and total sperm motility. Results:This retrospective cohort study analyzed 2,016 artificial insemination cycles and 1,783 ovarian retrieval cycles from January 2017 to October 2022,and assessed the semen quality of 6,651 outpatients from May 2017 to October 2022.In artificial insemination cycles, sperm PR rate and clinical pregnancy rate were highest in winter, with a statistically significant difference between groups(P<0.05).Clinical pregnancy rate was influenced by both age and sperm PR rate(P<0.05).In ovarian retrieval cycles, the winter group had significantly higher clinical pregnancy,2PN fertilization, and 2PN cleavage rates than the other groups. The autumn group had higher live birth rates, though not significantly different.Additionally, winter months showed higher total sperm concentration and total sperm number compared to other seasons. Conclusion:Seasonality affected clinical pregnancy and live birth rates in artificial insemination cycles but not in ovarian retrieval cycles in the Hainan region. These findings suggest that while there is no need to choose a specific season for ovarian retrieval cycles, artificial insemination in winter may be preferable for patients. 展开更多

Aids are only one of the instruments with which the disabled person can often reach a good level of autonomy,he can improve his conditions of life and prevent the aggravating of his situation. Aids serve also to facilitate care on the part of the family. The rights confirmed by Law 104 can also be achieved through the use of aids. The Italian state has planned a system of care with regards to aids so that some of them can be purchased with different tax concessions but essential aids, those necessary for the reaching of autonomy goals and prevention defined by an individual rehabilitative project, are supplied free of charge. In Italy essential aids are part therefore of a rehabilitation plan that is identified for each person with disabilities by a team made up of different professionals. 展开更多

Background:Dementia is a syndrome that affects a person’s ability to understand and express information. The higher prevalence of vision and/or hearing losses among persons with dementia in long-term care(LTC)facilities interferes with the ability of nurses to provide optimal care because communication is compromised. Therefore, the detection and screening for sensory impairment is of the utmost importance in LTC facilities;however, there is currently no agreement among nursing professionals on how to best identify such losses for the purpose of further referral, and the need for a validated screening measure suitable for nurses in LTC facilities is clear. The present project aims to close this gap by investigating the screening recommendations of vision-and hearing-care professionals working with clients affected by dementia. Methods:Eleven experts in audiology, optometry, deafblindness, and technology participated in individual semi-structured interviews on the topic of tools and strategies that can be used to screen individuals with dementia for sensory loss.Interview transcripts were coded by two evaluators using verbal agreement and consensus building. Results:Three main themes emerged from the interviews with experts:barriers, facilitators, and strategies.Barriers to sensory screening were often mentioned, particularly impaired communication and lack of staff cooperation.Facilitators consisted uniquely of people, such as family members, intervenors, and nurses.Strategies for sensory screening in this population consisted of improving communication through repetition and encouragements;considerations based on familiarity;and inferring an impairment on the basis of patient behaviour.Few of our interviewees were knowledgeable on the topic of screening apps. Conclusions:Our findings,to be integrated with a similar environmental scan conducted among LTC nurses, can inform the administration of sensory impairment screening tests among a population with dementia in order to optimize care. 展开更多

Background:In recent decades, several studies have been performed in the field of(non-)use of assistive technology devices(ATs)and have found that many variables are likely to be involved in the decision-making process of whether a person with a disability uses or abandons their aids.In low vision rehabilitation, high variability rates and a variety of reasons for non-use of low vision aids has been reported.Determinants of non-use of ATs have previously been categorized into personal factors,ATs factors, environmental factors of the user, and intervention factors. It is probable that some of these factors are also associated with the use of optical low vision aids(OLVAs).Thus, using a framework about adherence with medical interventions, the aim of this scoping review was to explore barriers and facilitators related to OLVAs(non-)use.Methods:Existing guidelines of the scoping review Methods were used to examine and summarize the extent, range, and nature of findings regarding factor categories associated to OLVAs usage.Searches were conducted using the following online databases:Embase,MedLine, and ERIC without limitation on publication dates.A combination of key words and MeSH terms was used based upon the identified core concepts of the research question:(I)low vision;(II)assistive technology;and(III)adherence.A charting form,a flow chart of the study selection process, and a combination of a descriptive numerical analysis and a thematic analysis of 24 studies were performed. Results:The results of this review indicate high variability rates(range:13-50%,M=24%,SD=10%)of people possessing OLVAs but not using them. Most authors do not use a precise definition of“non-use”,and when a definition is provided, the terminology is inconsistent. As expected, the four categories of variables identified are likely to be involved in OLVAs’(non-)use:(I)the largest number of concerns were personal factors.Several aspects regarding personal characteristics were identified, such as demographic,physical,psychological,social-emotional,and occupational factors. Some variables, such as age, diagnosis and visual acuity were reported as contradictorily influencing the OLVAs’usage;(II)ATs factors were associated with quality, appearance, practical aspects of use and the very type of OLVAs;(III)environmental factors referred to social support, physical barriers, expectations of the social environment, presence of a helper, and stigmatization;(IV)finally, intervention factors consisted of taking into account user’s opinions, positive health care experiences, instruction and training, provision processes, delivery periods, and follow-up services. Conclusions:This scoping review provides the preliminary evidence that factors related to OLVAs non-use could be classified into the same four typical categories described in the theory of non-adherence.These results suggest that strategies intended to enhance adherence might be for OLVAs should focus on these four aspects of device use. 展开更多
