However, despite advances in research to improve this disease, there is a high rate of late detection, leading to diagnosis at an advanced stage and a reduced chance of survival. Objective: The aim of this study is to identify the factors associated with late detection of breast cancer in women in Sub-Saharan Africa from 2014 to 2020.Setting: This systematic review focuses on sub-Saharan Africa. Methods: We searched for articles in four databases (PubMed, Embase, Global-Health and CINAHL) between 2014 and 2020 and performed a narrative synthesis to organize and group the different factors associated with late breast cancer detection. Result: After reviewing 583 publications, 6 studies were selected, highlighting factors such as lack of awareness, knowledge gaps, difficulties in accessing health services and financial constraints associated with late breast cancer screening. The participants, who ranged in number from 20 to 1776, were mainly aged between 18 and 25, with a mean age of 25 years and 6 months. Conclusion: The analysis enabled us to identify various factors associated with late breast cancer screening. Collaboration between health professionals, community organizations and policy-makers is essential to foster an environment conducive to the prevention and early detection of breast cancer.展开更多

BACKGROUND Non-invasive fibrosis scores are not yet validated in the newly defined metabolic associated fatty liver disease(MAFLD).AIM To evaluate the diagnostic performance of four non-invasive scores including aspar...BACKGROUND Non-invasive fibrosis scores are not yet validated in the newly defined metabolic associated fatty liver disease(MAFLD).AIM To evaluate the diagnostic performance of four non-invasive scores including aspartate aminotransferase to platelet ratio index(APRI),fibrosis-4 index(FIB-4),body mass index,aspartate aminotransferase/alanine aminotransferase ratio,diabetes score(BARD),and nonalcoholic fatty liver disease fibrosis score(NFS)in patients with MAFLD.METHODS Consecutive patients with histologically confirmed MAFLD were included.The discrimination ability of different non-invasive scores was compared.RESULTS A total of 417 patients were included;156(37.4%)of them had advanced fibrosis(Metavir≥F3).The area under receiver operating characteristic curve of FIB-4,NFS,APRI,and BARD for predicting advanced fibrosis was 0.736,0.724,0.671,and 0.609,respectively.The area under receiver operating characteristic curve of FIB-4 and NFS was similar(P=0.523),while the difference between FIB-4 and APRI(P=0.001)and FIB-4 and BARD(P<0.001)was statistically significant.The best thresholds of FIB-4,NFS,APRI,and BARD for diagnosis of advanced fibrosis in MAFLD were 1.05,-2.1,0.42,and 2.A subgroup analysis showed that FIB-4,APRI,and NFS performed worse in the pure MAFLD group than in the hepatitis B virus-MAFLD group.CONCLUSION APRI and BARD scores do not perform well in MAFLD.The FIB-4 and NFS could be more useful,but a new threshold is needed.Novel non-invasive scoring systems for fibrosis are required for MAFLD.展开更多
Long noncoding RNA(IncRNA)urothelial carcinoma associated 1(UCA1)has been reported to be highly expressed in many kinds of cancers.This meta-analysis summarized its potential prognostic value in digestive system malig...Long noncoding RNA(IncRNA)urothelial carcinoma associated 1(UCA1)has been reported to be highly expressed in many kinds of cancers.This meta-analysis summarized its potential prognostic value in digestive system malignancies.A meta-analysis was performed through a comprehensive search in PubMed,EMBASE,the Cochrane Library,Web of Science and Chinese National Knowledge Infrastructure(CNKI)for suitable articles on the prognostic impact of UCA1 in digestive system malignancies from inception to June 27,2019.Pooled hazard ratios(HRs)with 95% confidence interval(95%CI)were calculated to summarize the effect.Sixteen studies were included in the study,with a total of 1504 patients.A significant association was observed between UCA1 abundance and poor overall survival(OS),and shorter disease-free survival(DFS)for patients with digestive system malignancies,with pooled HR of 2.07(95%CI:1.74-2.47),and of 2.50(95%CI:1.62-3.86).Subgroup analysis and sensitivity analysis suggested the reliability of our findings.It is suggested that UCA1 abundance may serve as a reliable predictive factor for poor prognosis in patients with digestive system malignancies.展开更多

To investigate blood lead levels (BLL) and possible risk factors for lead exposure among children in China A total of 388 articles met our inclusion criteria. The overall geometric mean (GM) BLL was 71 μg/L, and the prevalence of elevated BLL (EBLL, defined as BLL ≥100 μg/L) was 18.48% among children. The prevalence of EBLL remained significantly higher among boys. In children less than 6 years of age, there were significantly increasing trends in both BLL and prevalence of EBLL in an age-dependent manner. The ban on leaded gasoline significantly reduced the BLL as well as EBLL prevalence; however, children whose parents had lower educational levels or were exposed to lead in the workplace had a higher EBLL prevalence. Despite its decline over time, the average BLL among children in China remains higher than the average level most recently reported in the United States. Childhood lead poisoning remains a public health problem in China.展开更多

It is found that the predicted meso-r-scale convective phenomena arc basically unsteady under the situation of strong shear at low-levels, while the meso-β-scale convective system is maintained up to 3 hours or more. The meso -β- scale cloud system exhibits characteristics of a multi-celled convective storm in which the meso-r-scale convective cells have lifetime of about 30 min. Pressure perturbation depicts a meso-low after a half hour in the low levels. As the cloud system evolves, the meso-low intensifies and extends to the upshear side and covers the entire domain in the mid-lower levels with the peak values of 5-8 hPa. Temperature perturbation depicts a warm region in the middle levels through the entire simulation period. The meso-r-scale warm cores with peak values of 4-8 ℃ are associated with strong convective cells. The cloud top evaporation causes a stronger cold layer around the cloud top levels.Simulation of microphysics exhibits that graupel is primarily concentrated in the strong convective cells forming the main source of convective rainfall after one hour of simulation time. Aggregates are mainly located in the stratiform region and decaying convective cells which produce the stratiform rainfall. Riming of the ice crystals is the predominant precipitation formation mechanism in the convection region, whereas aggregation of ice crystals is the predominant one in the stratiform region, which is consistent with observations. Sensitivity experiments of ice-phase mierophysical processes show that the microphysical structures of the convective cloud system can be simulated better with the diagnosed aggregation collection efficiencies.展开更多

In this editorial,we comment on the article by Chen et al recently published in 2024.We focus the debate on whether reducing the upper limit of normal of alanine aminotransferase(ALT)would effectively identify cases o...In this editorial,we comment on the article by Chen et al recently published in 2024.We focus the debate on whether reducing the upper limit of normal of alanine aminotransferase(ALT)would effectively identify cases of fibrosis in metabolic-dysfunction associated fatty liver disease(MAFLD).This is important given the increasing prevalence of MAFLD and obesity globally.Currently,a suitable screening test to identify patients in the general population does not exist and most patients are screened after the finding of an abnormal ALT.The authors of this paper challenge the idea of what a normal ALT is and whether that threshold should be lowered,particularly as their study found that 83.12%of their study population with a diagnosis of MAFLD had a normal ALT.The main advantages of screening would be to identify patients and provide intervention early,the mainstay of this being changing modifiable risk factors and monitoring for liver fibrosis.However,there is not enough suitable therapeutic options available as of yet although this is likely to change in the coming years with more targets for therapy being discovered.Semaglutide is one example of this which has demonstrated benefit with an acceptable side effect profile for those patients with MAFLD and obesity,although studies have not yet shown a significant improvement in fibrosis regression.It would also require a huge amount of resource if a reduced ALT level alone was used as criteria;it is more likely that current scoring systems such as fibrosis-4 may be amended to represent this additional risk.Currently,there is not a good argument to recommend wide-spread screening with a reduced ALT level as this is unlikely to be cost-effective.This is compounded by the fact that there is a significant heterogeneity in what is considered a normal ALT between laboratories.Although studies previously have suggested a more pragmatic approach in screening those over the age of 60,this is likely to change with the increasing incidence of obesity within the younger age groups.The main message from this study is that those who have hypercholesterolemia and high body metabolic index should have these risk factors modified to maintain a lower level of ALT to reduce the risk of progression to fibrosis and cirrhosis.展开更多

Despite significant advances in our understanding of the molecular pathogenesis of liver cancer and the availability of novel pharmacotherapies,liver cancer remains the fourth leading cause of cancer-related mortality...Despite significant advances in our understanding of the molecular pathogenesis of liver cancer and the availability of novel pharmacotherapies,liver cancer remains the fourth leading cause of cancer-related mortality worldwide.Tumor relapse,resistance to current anti-cancer drugs,metastasis,and organ toxicity are the major challenges that prevent considerable improvements in patient survival and quality of life.Calculus bovis(CB),an ancient Chinese medicinal drug,has been used to treat various pathologies,including stroke,convulsion,epilepsy,pain,and cancer.In this editorial,we discuss the research findings recently published by Huang et al on the therapeutic effects of CB in inhibiting the development of liver cancer.Utilizing the comprehensive transcriptomic analyses,in vitro experiments,and in vivo studies,the authors demonstrated that CB treatment inhibits the tumor-promoting M2 phenotype of tumor-associated macrophages via downregulating Wnt pathway.While multiple studies have been performed to explore the molecular mechanisms regulated by CB,this study uniquely shows its role in modulating the M2 phenotype of macrophages present within the tumor microenvironment.This study opens new avenues of future investigations aimed at investigating this drug’s efficacy in various mouse models including the effects of combination therapy,and against drug-resistant tumors.展开更多
BACKGROUND Multisystem inflammatory syndrome in children(MIS-C) has emerged as a new disease associated with COVID-19 that presents in acute critically ill children with acute cardiovascular dysfunction.AIM To determi...BACKGROUND Multisystem inflammatory syndrome in children(MIS-C) has emerged as a new disease associated with COVID-19 that presents in acute critically ill children with acute cardiovascular dysfunction.AIM To determine whether the age-adjusted N-terminal pro-brain natriuretic peptide(NT-proBNP) value(Z-log-NT-proBNP) is associated with severe MIS-C and myocardial dysfunction.METHODS A retrospective study was conducted which included children with MIS-C managed at our institution between April 1,2020,and February 28,2022.We divided the population into groups depending on severity based on pediatric intensive care unit(PICU) admission.We compared Z-log-NT-proBNP values across these groups and analyzed Z-log-NT-proBNP dynamics during the onemonth follow-up.RESULTS We included 17 participants [median age 3(2-9) years] and seven(41%) required PICU admission.All(100%) of these cases presented very high(Z-log > 4) levels of NT-proBNP at the time of admission compared to only 5(50%) patients with non-severe MIS-C(P = 0.025).NT-proBNP was significantly correlated with highsensitive Troponin I levels(P = 0.045),Ross modified score(P = 0.003) and left ventricle ejection fraction(P = 0.021).CONCLUSION Raised NT-proBNP,specifically very high values(Z-log-NT-proBNP > 4) could help in the early identification of MIS-C patients with myocardial dysfunction requiring inotropic support and PICU admission.展开更多

Especially, in human-robot interaction research field, some researchers attempt to make robots talk with human in daily life. The key challenges for making robots talk naturally in conversation are to need to consider multi-modal non-verbal information same as human, and to learn with small amount of labeled multi-modal data. Previous multi-modal learning needs a large amount of labeled data while labeled multi-modal data are shortage and difficult to be collected. In this research, we address these challenges by integrating single-modal classifiers which trained each modal information respectively. Our architecture utilized knowledge by using bi-directional associative memory. Furthermore, we conducted the conversation experiment for collecting multi-modal non-verbal information. We verify our approach by comparing accuracies between our system and conventional system which trained multi-modal information.展开更多
The Pacific oyster Crassostrea gigas,one of the most exploited molluscs in the world,has suffered from massive mortality in recent decades,and the occurrence mechanisms have not been well characterized.In this study,t...The Pacific oyster Crassostrea gigas,one of the most exploited molluscs in the world,has suffered from massive mortality in recent decades,and the occurrence mechanisms have not been well characterized.In this study,to reveal the relationship of associated microbiota to the fitness of oysters,temporal dynamics of microbiota in the gill,hemolymph,and hepatopancreas of C.gigas during April 2018-January 2019 were investigated by 16 S rRNA gene sequencing.The microbiota in C.gigas exhibited tissue heterogeneity,of which Spirochaetaceae was dominant in the gill and hemolymph while Mycoplasmataceae enriched in the hepatopancreas.Co-occurrence network demonstrated that the gill microbiota exhibited higher inter-taxon connectivity while the hemolymph microbiota had more modules.The richness(Chao 1 index)and diversity(Shannon index)of microbial community in each tissue showed no significant seasonal variations,except for the hepatopancreas having a higher richness in the autumn.Similarly,beta diversity analysis indicated a relatively stable microbiota in each tissue during the sampling period,showing relative abundance of the dominant taxa exhibiting temporal dynamics.Results indicate that the microbial community in C.gigas showed a tissue-specific stability with temporal dynamics in the composition,which might be essential for the tissue functioning and environmental adaption in oysters.This work provides a baseline microbiota in C.gigas and is helpful for the understanding of host-microbiota interaction in oysters.展开更多
Background Community-based health insurance(CBHI)schemes are crucial for households to avoid financial hardship,improve healthcare quality,and engage in health policies.Household satisfaction is a key indicator for as...Background Community-based health insurance(CBHI)schemes are crucial for households to avoid financial hardship,improve healthcare quality,and engage in health policies.Household satisfaction is a key indicator for assessing healthcare quality and identifying service gaps.However,research on household satisfaction with CBHI in Ethiopia is limited.Therefore,this study aimed to evaluate household satisfaction and associated factors with CBHI schemes in Ethiopia.Methods A comprehensive search of relevant literature was conducted using multiple databases,including Pub-Med,Google Scholar,Africa Journal Online,and Ethiopian Universities’institutional open-access online repositories.The search was carried out between January 25,2023,and February 28,2023.Twelve primary studies,including eight published and four unpublished,were identified and included in the analysis with a total sample size of 5311 participants.A protocol with the registration number CRD20531345698 is recorded on the Prospero database.Two authors,DT and MK,independently extracted the required data using a standardized form.The extracted data were then analyzed using STATA version 17 software.Heterogeneity was assessed using the Cochrane Q-test and I2 tests.Finally,a random-effect model was employed to calculate the overall household satisfaction with CBHI and to determine the associated factors.Results The meta-analysis showed that the overall household satisfaction with CBHI in Ethiopia was 62.26%(95%CI 53.25-71.21%).The study found regional variations in household satisfaction,with 63.40%in Oromia,64.01%in Amhara,49.58%in Addis Ababa,and 66.76%in SNNPs.The study identified several factors associated with household satisfaction and the CBHI scheme,including the availability of drugs(OR 2.13,95%CI 1.47-2.78),friendly services(OR 3.85,95%CI 1.60-6.10),affordability of premium(OR 2.80,95%CI 1.97-3.63),and knowledge/awareness of CBHI(OR 2.52,95%CI 1.73-3.33).Conclusions The study provides valuable insights into household satisfaction with CBHI in Ethiopia,with a considerable proportion of enrolees being satisfied.The finding highlights regional variations in household satisfaction and underscores the need for tailored interventions and monitoring to enhance CBHI sustainability and effectiveness.The results suggest that healthcare providers and policymakers should prioritize the availability of drugs,friendly services,affordable premiums,and education to improve household satisfaction with CBHI schemes.展开更多

However, current technologies have barely explored the correlation between perturbation removal and background restoration, consequently struggling to generate high-naturalness content in challenging scenarios. In this paper, we rethink the image enhancement task from the perspective of joint optimization: Perturbation removal and texture reconstruction. To this end, we advise an efficient yet effective image enhancement model, termed the perturbation-guided texture reconstruction network(PerTeRNet). It contains two subnetworks designed for the perturbation elimination and texture reconstruction tasks, respectively. To facilitate texture recovery,we develop a novel perturbation-guided texture enhancement module(PerTEM) to connect these two tasks, where informative background features are extracted from the input with the guidance of predicted perturbation priors. To alleviate the learning burden and computational cost, we suggest performing perturbation removal in a sub-space and exploiting super-resolution to infer high-frequency background details. Our PerTeRNet has demonstrated significant superiority over typical methods in both quantitative and qualitative measures, as evidenced by extensive experimental results on popular image enhancement and joint detection tasks. Objective To systematically summarize the published literature on the genetic variants associated with nonalcoholic fatty liver disease(NAFLD).Methods Literature from Web of Science,PubMed,and Embase between January 1...Objective To systematically summarize the published literature on the genetic variants associated with nonalcoholic fatty liver disease(NAFLD).Methods Literature from Web of Science,PubMed,and Embase between January 1980 and September 2022 was systematically searched.Meta-analyses of the genetic variants were conducted using at least five data sources.The epidemiologic credibility of the significant associations was graded using the Venice criteria.Results Based on literature screening,399 eligible studies were included,comprising 381 candidate gene association,16 genome-wide association,and 2 whole-exome sequencing studies.We identified 465 genetic variants in 173 genes in candidate gene association studies,and 25 genetic variants in 17 genes were included in the meta-analysis.The meta-analysis identified 11 variants in 10 genes that were significantly associated with NAFLD,with cumulative epidemiological evidence of an association graded as strong for two variants in two genes(HFE,TNF),moderate for four variants in three genes(TM6SF2,GCKR,and ADIPOQ),and weak for five variants in five genes(MBOAT7,PEMT,PNPLA3,LEPR,and MTHFR).Conclusion This study identified six variants in five genes that had moderate to strong evidence of an association with NAFLD,which may help understand the genetic architecture of NAFLD risk.展开更多

文摘Research background: Breast cancer remains a major public health problem, with a high number of new cases and deaths each year. However, despite advances in research to improve this disease, there is a high rate of late detection, leading to diagnosis at an advanced stage and a reduced chance of survival. Objective: The aim of this study is to identify the factors associated with late detection of breast cancer in women in Sub-Saharan Africa from 2014 to 2020.Setting: This systematic review focuses on sub-Saharan Africa. Methods: We searched for articles in four databases (PubMed, Embase, Global-Health and CINAHL) between 2014 and 2020 and performed a narrative synthesis to organize and group the different factors associated with late breast cancer detection. Result: After reviewing 583 publications, 6 studies were selected, highlighting factors such as lack of awareness, knowledge gaps, difficulties in accessing health services and financial constraints associated with late breast cancer screening. The participants, who ranged in number from 20 to 1776, were mainly aged between 18 and 25, with a mean age of 25 years and 6 months. Conclusion: The analysis enabled us to identify various factors associated with late breast cancer screening. Collaboration between health professionals, community organizations and policy-makers is essential to foster an environment conducive to the prevention and early detection of breast cancer.
文摘This article reviews the study,“Fanlian huazhuo formula alleviates high-fat-dietinduced nonalcoholic fatty liver disease by modulating autophagy and lipid synthesis signaling pathway”published in the World Journal of Gastroenterology.The study explores the therapeutic potential of Fanlian Huazhuo formula(FLHZF)in treating metabolic-associated steatotic liver disease(MASLD),demonstrating that FLHZF reduces lipid accumulation,oxidative stress,and liver injury in MASLD models by modulating key signaling pathways involved in lipid metabolism and autophagy.This editorial emphasizes the potential of FLHZF as a treatment for MASLD and calls for further research to verify its clinical efficacy.

文摘BACKGROUND Non-invasive fibrosis scores are not yet validated in the newly defined metabolic associated fatty liver disease(MAFLD).AIM To evaluate the diagnostic performance of four non-invasive scores including aspartate aminotransferase to platelet ratio index(APRI),fibrosis-4 index(FIB-4),body mass index,aspartate aminotransferase/alanine aminotransferase ratio,diabetes score(BARD),and nonalcoholic fatty liver disease fibrosis score(NFS)in patients with MAFLD.METHODS Consecutive patients with histologically confirmed MAFLD were included.The discrimination ability of different non-invasive scores was compared.RESULTS A total of 417 patients were included;156(37.4%)of them had advanced fibrosis(Metavir≥F3).The area under receiver operating characteristic curve of FIB-4,NFS,APRI,and BARD for predicting advanced fibrosis was 0.736,0.724,0.671,and 0.609,respectively.The area under receiver operating characteristic curve of FIB-4 and NFS was similar(P=0.523),while the difference between FIB-4 and APRI(P=0.001)and FIB-4 and BARD(P<0.001)was statistically significant.The best thresholds of FIB-4,NFS,APRI,and BARD for diagnosis of advanced fibrosis in MAFLD were 1.05,-2.1,0.42,and 2.A subgroup analysis showed that FIB-4,APRI,and NFS performed worse in the pure MAFLD group than in the hepatitis B virus-MAFLD group.CONCLUSION APRI and BARD scores do not perform well in MAFLD.The FIB-4 and NFS could be more useful,but a new threshold is needed.Novel non-invasive scoring systems for fibrosis are required for MAFLD.
文摘Long noncoding RNA(IncRNA)urothelial carcinoma associated 1(UCA1)has been reported to be highly expressed in many kinds of cancers.This meta-analysis summarized its potential prognostic value in digestive system malignancies.A meta-analysis was performed through a comprehensive search in PubMed,EMBASE,the Cochrane Library,Web of Science and Chinese National Knowledge Infrastructure(CNKI)for suitable articles on the prognostic impact of UCA1 in digestive system malignancies from inception to June 27,2019.Pooled hazard ratios(HRs)with 95% confidence interval(95%CI)were calculated to summarize the effect.Sixteen studies were included in the study,with a total of 1504 patients.A significant association was observed between UCA1 abundance and poor overall survival(OS),and shorter disease-free survival(DFS)for patients with digestive system malignancies,with pooled HR of 2.07(95%CI:1.74-2.47),and of 2.50(95%CI:1.62-3.86).Subgroup analysis and sensitivity analysis suggested the reliability of our findings.It is suggested that UCA1 abundance may serve as a reliable predictive factor for poor prognosis in patients with digestive system malignancies.

文摘In this study, we searched multiple databases for all relevant original articles (1996-2013). To investigate blood lead levels (BLL) and possible risk factors for lead exposure among children in China A total of 388 articles met our inclusion criteria. The overall geometric mean (GM) BLL was 71 μg/L, and the prevalence of elevated BLL (EBLL, defined as BLL ≥100 μg/L) was 18.48% among children. The prevalence of EBLL remained significantly higher among boys. In children less than 6 years of age, there were significantly increasing trends in both BLL and prevalence of EBLL in an age-dependent manner. The ban on leaded gasoline significantly reduced the BLL as well as EBLL prevalence; however, children whose parents had lower educational levels or were exposed to lead in the workplace had a higher EBLL prevalence. Despite its decline over time, the average BLL among children in China remains higher than the average level most recently reported in the United States. Childhood lead poisoning remains a public health problem in China.
文摘Numerical simulation of meso-β-scale convective cloud systems associated with a PRE-STORM MCC case has been carried out using a 2-D version of the CSU Regional Atmospheric Modeling System (RAMS) nonhydrostatic model with parameterized microphysics. It is found that the predicted meso-r-scale convective phenomena arc basically unsteady under the situation of strong shear at low-levels, while the meso-β-scale convective system is maintained up to 3 hours or more. The meso -β- scale cloud system exhibits characteristics of a multi-celled convective storm in which the meso-r-scale convective cells have lifetime of about 30 min. Pressure perturbation depicts a meso-low after a half hour in the low levels. As the cloud system evolves, the meso-low intensifies and extends to the upshear side and covers the entire domain in the mid-lower levels with the peak values of 5-8 hPa. Temperature perturbation depicts a warm region in the middle levels through the entire simulation period. The meso-r-scale warm cores with peak values of 4-8 ℃ are associated with strong convective cells. The cloud top evaporation causes a stronger cold layer around the cloud top levels.Simulation of microphysics exhibits that graupel is primarily concentrated in the strong convective cells forming the main source of convective rainfall after one hour of simulation time. Aggregates are mainly located in the stratiform region and decaying convective cells which produce the stratiform rainfall. Riming of the ice crystals is the predominant precipitation formation mechanism in the convection region, whereas aggregation of ice crystals is the predominant one in the stratiform region, which is consistent with observations. Sensitivity experiments of ice-phase mierophysical processes show that the microphysical structures of the convective cloud system can be simulated better with the diagnosed aggregation collection efficiencies.
文摘To evaluate and predict liver fibrosis in patients with nonalcoholic fatty liver disease(NAFLD),several non-invasive scoring systems were built and widely used in the progress of diagnosis and treatment,which showed great diagnostic efficiency,such as aspartate aminotransferase to platelet ratio index,fibrosis-4 index,body mass index,aspartate aminotransferase to alanine aminotransferase ratio,diabetes score and NAFLD fibrosis score.Since the new concept of metabolic associated fatty liver disease(MAFLD)was proposed,the clinical application value of the non-invasive scoring systems mentioned above has not been assessed in MAFLD.The evaluation of the diagnostic performance of these non-invasive scoring systems will provide references for clinicians in the diagnosis of MAFLD.

文摘This study aimed to investigate bacterial community in an urban drinking water distribution system (DWDS) during an occurrence of colored water. Variation in the bacterial community diversity and structure was observed among the different waters, with the predominance of Proteobacteria. While Verrucomicrobia was also a major phylum group in colored water. Limnobacter was the major genus group in colored water, but Undibacterium predominated in normal tap water. The coexistence of Limnobacter as well as Sediminibacterium and Aquobocterium might contribute to the formation of colored water.
文摘In this editorial,we comment on the article by Chen et al recently published in 2024.We focus the debate on whether reducing the upper limit of normal of alanine aminotransferase(ALT)would effectively identify cases of fibrosis in metabolic-dysfunction associated fatty liver disease(MAFLD).This is important given the increasing prevalence of MAFLD and obesity globally.Currently,a suitable screening test to identify patients in the general population does not exist and most patients are screened after the finding of an abnormal ALT.The authors of this paper challenge the idea of what a normal ALT is and whether that threshold should be lowered,particularly as their study found that 83.12%of their study population with a diagnosis of MAFLD had a normal ALT.The main advantages of screening would be to identify patients and provide intervention early,the mainstay of this being changing modifiable risk factors and monitoring for liver fibrosis.However,there is not enough suitable therapeutic options available as of yet although this is likely to change in the coming years with more targets for therapy being discovered.Semaglutide is one example of this which has demonstrated benefit with an acceptable side effect profile for those patients with MAFLD and obesity,although studies have not yet shown a significant improvement in fibrosis regression.It would also require a huge amount of resource if a reduced ALT level alone was used as criteria;it is more likely that current scoring systems such as fibrosis-4 may be amended to represent this additional risk.Currently,there is not a good argument to recommend wide-spread screening with a reduced ALT level as this is unlikely to be cost-effective.This is compounded by the fact that there is a significant heterogeneity in what is considered a normal ALT between laboratories.Although studies previously have suggested a more pragmatic approach in screening those over the age of 60,this is likely to change with the increasing incidence of obesity within the younger age groups.The main message from this study is that those who have hypercholesterolemia and high body metabolic index should have these risk factors modified to maintain a lower level of ALT to reduce the risk of progression to fibrosis and cirrhosis.
文摘Over recent years,the nomenclature of non-alcoholic fatty liver disease has undergone significant changes.Indeed,in 2020,an expert consensus panel proposed the term“Metabolic(dysfunction)associated fatty liver disease”(MAFLD)to underscore the close association of fatty liver with metabolic abnormalities,thereby highlighting the cardiometabolic risks(such as metabolic syndrome,type 2 diabetes,insulin resistance,and cardiovascular disease)faced by these patients since childhood.More recently,this term has been further replaced with metabolic associated steatotic liver disease.It is worth noting that emerging evidence not only supports a close and independent association of MAFLD with chronic kidney disease in adults but also indicates its interplay with metabolic impairments.However,comparable pediatric data remain limited.Given the progressive and chronic nature of both diseases and their prognostic cardiometabolic implications,this editorial aims to provide a pediatric perspective on the intriguing relationship between MAFLD and renal function in childhood.

文摘Despite significant advances in our understanding of the molecular pathogenesis of liver cancer and the availability of novel pharmacotherapies,liver cancer remains the fourth leading cause of cancer-related mortality worldwide.Tumor relapse,resistance to current anti-cancer drugs,metastasis,and organ toxicity are the major challenges that prevent considerable improvements in patient survival and quality of life.Calculus bovis(CB),an ancient Chinese medicinal drug,has been used to treat various pathologies,including stroke,convulsion,epilepsy,pain,and cancer.In this editorial,we discuss the research findings recently published by Huang et al on the therapeutic effects of CB in inhibiting the development of liver cancer.Utilizing the comprehensive transcriptomic analyses,in vitro experiments,and in vivo studies,the authors demonstrated that CB treatment inhibits the tumor-promoting M2 phenotype of tumor-associated macrophages via downregulating Wnt pathway.While multiple studies have been performed to explore the molecular mechanisms regulated by CB,this study uniquely shows its role in modulating the M2 phenotype of macrophages present within the tumor microenvironment.This study opens new avenues of future investigations aimed at investigating this drug’s efficacy in various mouse models including the effects of combination therapy,and against drug-resistant tumors.
文摘BACKGROUND Multisystem inflammatory syndrome in children(MIS-C) has emerged as a new disease associated with COVID-19 that presents in acute critically ill children with acute cardiovascular dysfunction.AIM To determine whether the age-adjusted N-terminal pro-brain natriuretic peptide(NT-proBNP) value(Z-log-NT-proBNP) is associated with severe MIS-C and myocardial dysfunction.METHODS A retrospective study was conducted which included children with MIS-C managed at our institution between April 1,2020,and February 28,2022.We divided the population into groups depending on severity based on pediatric intensive care unit(PICU) admission.We compared Z-log-NT-proBNP values across these groups and analyzed Z-log-NT-proBNP dynamics during the onemonth follow-up.RESULTS We included 17 participants [median age 3(2-9) years] and seven(41%) required PICU admission.All(100%) of these cases presented very high(Z-log > 4) levels of NT-proBNP at the time of admission compared to only 5(50%) patients with non-severe MIS-C(P = 0.025).NT-proBNP was significantly correlated with highsensitive Troponin I levels(P = 0.045),Ross modified score(P = 0.003) and left ventricle ejection fraction(P = 0.021).CONCLUSION Raised NT-proBNP,specifically very high values(Z-log-NT-proBNP > 4) could help in the early identification of MIS-C patients with myocardial dysfunction requiring inotropic support and PICU admission.
文摘We propose a learning architecture for integrating multi-modal information e.g., vision, audio information. In recent years, artificial intelligence (AI) is making major progress in key tasks like a language, vision, voice recognition tasks. Most studies focus on how AI could achieve human-like abilities. Especially, in human-robot interaction research field, some researchers attempt to make robots talk with human in daily life. The key challenges for making robots talk naturally in conversation are to need to consider multi-modal non-verbal information same as human, and to learn with small amount of labeled multi-modal data. Previous multi-modal learning needs a large amount of labeled data while labeled multi-modal data are shortage and difficult to be collected. In this research, we address these challenges by integrating single-modal classifiers which trained each modal information respectively. Our architecture utilized knowledge by using bi-directional associative memory. Furthermore, we conducted the conversation experiment for collecting multi-modal non-verbal information. We verify our approach by comparing accuracies between our system and conventional system which trained multi-modal information.

文摘The Pacific oyster Crassostrea gigas,one of the most exploited molluscs in the world,has suffered from massive mortality in recent decades,and the occurrence mechanisms have not been well characterized.In this study,to reveal the relationship of associated microbiota to the fitness of oysters,temporal dynamics of microbiota in the gill,hemolymph,and hepatopancreas of C.gigas during April 2018-January 2019 were investigated by 16 S rRNA gene sequencing.The microbiota in C.gigas exhibited tissue heterogeneity,of which Spirochaetaceae was dominant in the gill and hemolymph while Mycoplasmataceae enriched in the hepatopancreas.Co-occurrence network demonstrated that the gill microbiota exhibited higher inter-taxon connectivity while the hemolymph microbiota had more modules.The richness(Chao 1 index)and diversity(Shannon index)of microbial community in each tissue showed no significant seasonal variations,except for the hepatopancreas having a higher richness in the autumn.Similarly,beta diversity analysis indicated a relatively stable microbiota in each tissue during the sampling period,showing relative abundance of the dominant taxa exhibiting temporal dynamics.Results indicate that the microbial community in C.gigas showed a tissue-specific stability with temporal dynamics in the composition,which might be essential for the tissue functioning and environmental adaption in oysters.This work provides a baseline microbiota in C.gigas and is helpful for the understanding of host-microbiota interaction in oysters.
文摘Background Community-based health insurance(CBHI)schemes are crucial for households to avoid financial hardship,improve healthcare quality,and engage in health policies.Household satisfaction is a key indicator for assessing healthcare quality and identifying service gaps.However,research on household satisfaction with CBHI in Ethiopia is limited.Therefore,this study aimed to evaluate household satisfaction and associated factors with CBHI schemes in Ethiopia.Methods A comprehensive search of relevant literature was conducted using multiple databases,including Pub-Med,Google Scholar,Africa Journal Online,and Ethiopian Universities’institutional open-access online repositories.The search was carried out between January 25,2023,and February 28,2023.Twelve primary studies,including eight published and four unpublished,were identified and included in the analysis with a total sample size of 5311 participants.A protocol with the registration number CRD20531345698 is recorded on the Prospero database.Two authors,DT and MK,independently extracted the required data using a standardized form.The extracted data were then analyzed using STATA version 17 software.Heterogeneity was assessed using the Cochrane Q-test and I2 tests.Finally,a random-effect model was employed to calculate the overall household satisfaction with CBHI and to determine the associated factors.Results The meta-analysis showed that the overall household satisfaction with CBHI in Ethiopia was 62.26%(95%CI 53.25-71.21%).The study found regional variations in household satisfaction,with 63.40%in Oromia,64.01%in Amhara,49.58%in Addis Ababa,and 66.76%in SNNPs.The study identified several factors associated with household satisfaction and the CBHI scheme,including the availability of drugs(OR 2.13,95%CI 1.47-2.78),friendly services(OR 3.85,95%CI 1.60-6.10),affordability of premium(OR 2.80,95%CI 1.97-3.63),and knowledge/awareness of CBHI(OR 2.52,95%CI 1.73-3.33).Conclusions The study provides valuable insights into household satisfaction with CBHI in Ethiopia,with a considerable proportion of enrolees being satisfied.The finding highlights regional variations in household satisfaction and underscores the need for tailored interventions and monitoring to enhance CBHI sustainability and effectiveness.The results suggest that healthcare providers and policymakers should prioritize the availability of drugs,friendly services,affordable premiums,and education to improve household satisfaction with CBHI schemes.

文摘Chronic enteropathy associated with the SLCO2A1 gene(CEAS)is a complex gastroenterological condition characterized by multiple ulcers in the small intestine with chronic bleeding and protein loss.This review explores the potential mechanisms underlying the pathogenesis of CEAS,focusing on the role of SLCO2A1-encoded prostaglandin transporter OATP2A1 and its impact on prostaglandin E2(PGE2)levels.Studies have suggested that elevated PGE2 levels contribute to mucosal damage,inflammation,and disruption of the intestinal barrier.The effects of PGE2 on macrophage activation and Maxi-Cl channel functionality,as well as its interaction with nonsteroidal anti-inflammatory drugs play crucial roles in the progression of CEAS.Understanding the balance between its protective and pro-inflammatory effects and the complex interactions within the gastrointestinal tract can shed light on potential therapeutic targets for CEAS and guide the development of novel,targeted therapies.

文摘Degradation under challenging conditions such as rain, haze, and low light not only diminishes content visibility, but also results in additional degradation side effects, including detail occlusion and color distortion. However, current technologies have barely explored the correlation between perturbation removal and background restoration, consequently struggling to generate high-naturalness content in challenging scenarios. In this paper, we rethink the image enhancement task from the perspective of joint optimization: Perturbation removal and texture reconstruction. To this end, we advise an efficient yet effective image enhancement model, termed the perturbation-guided texture reconstruction network(PerTeRNet). It contains two subnetworks designed for the perturbation elimination and texture reconstruction tasks, respectively. To facilitate texture recovery,we develop a novel perturbation-guided texture enhancement module(PerTEM) to connect these two tasks, where informative background features are extracted from the input with the guidance of predicted perturbation priors. To alleviate the learning burden and computational cost, we suggest performing perturbation removal in a sub-space and exploiting super-resolution to infer high-frequency background details. Our PerTeRNet has demonstrated significant superiority over typical methods in both quantitative and qualitative measures, as evidenced by extensive experimental results on popular image enhancement and joint detection tasks. 文摘The diagnosis of non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease only on the basis of laboratory parameter score such as Hepatic Steatosis Index which includes liver enzymes,gender,basal metabolic index,and presence of diabetic mellitus is not sufficient to exclude other causes of deranged liver enzymes especially medications and autoimmune related liver diseases.As the guideline suggests ultrasound is the preferred first-line diagnostic procedure for imaging of NAFLD,as it provides additional diagnostic information and the combination of biomarkers/scores and transient elastography might confer additional diagnostic accuracy and evident from previous similar studies too.

文摘Objective To systematically summarize the published literature on the genetic variants associated with nonalcoholic fatty liver disease(NAFLD).Methods Literature from Web of Science,PubMed,and Embase between January 1980 and September 2022 was systematically searched.Meta-analyses of the genetic variants were conducted using at least five data sources.The epidemiologic credibility of the significant associations was graded using the Venice criteria.Results Based on literature screening,399 eligible studies were included,comprising 381 candidate gene association,16 genome-wide association,and 2 whole-exome sequencing studies.We identified 465 genetic variants in 173 genes in candidate gene association studies,and 25 genetic variants in 17 genes were included in the meta-analysis.The meta-analysis identified 11 variants in 10 genes that were significantly associated with NAFLD,with cumulative epidemiological evidence of an association graded as strong for two variants in two genes(HFE,TNF),moderate for four variants in three genes(TM6SF2,GCKR,and ADIPOQ),and weak for five variants in five genes(MBOAT7,PEMT,PNPLA3,LEPR,and MTHFR).Conclusion This study identified six variants in five genes that had moderate to strong evidence of an association with NAFLD,which may help understand the genetic architecture of NAFLD risk.
