BACKGROUND Stroke frequently results in oropharyngeal dysfunction(OD),leading to difficulties in swallowing and eating,as well as triggering negative emotions, malnutrition, and aspiration pneumonia, which can be detrimental to patients. However, routine nursing interventions often fail to address these issues adequately.Systemic and psychological interventions can improve dysphagia symptoms, relieve negative emotions, and improve quality of life. However, there are few clinical reports of systemic interventions combined with psychological interventions for stroke patients with OD.AIM To explore the effects of combining systemic and psychological interventions in stroke patients with OD.METHODS This retrospective study included 90 stroke patients with OD,admitted to the Second Affiliated Hospital of Qiqihar Medical College(January 2022–December 2023),who were divided into two groups:regular and coalition.Swallowing function grading(using a water swallow test),swallowing function[using the standardized swallowing assessment(SSA)],negative emotions[using the selfrating anxiety scale(SAS)and self-rating depression scale(SDS)],and quality of life(SWAL-QOL)were compared between groups before and after the intervention;aspiration pneumonia incidence was recorded.RESULTS Post-intervention,the coalition group had a greater number of patients with grade 1 swallowing function compared to the regular group, while the number of patients with grade 5 swallowing function was lower than that in the regular group(P<0.05).Post-intervention,the SSA,SAS,and SDS scores of both groups decreased, with a more significant decrease observed in the coalition group(P<0.05).Additionally,the total SWAL-QOL score in both groups increased, with a more significant increase observed in the coalition group(P<0.05).During the intervention period, the total incidence of aspiration and aspiration pneumonia in the coalition group was lower than that in the control group(4.44%vs 20.00%;P<0.05).CONCLUSION Systemic intervention combined with psychological intervention can improve dysphagia symptoms, alleviate negative emotions, enhance quality of life, and reduce the incidence of aspiration pneumonia in patients with OD. 展开更多

How does psychological distress evolve over time? This study utilizes cross-sectional data from the China General Social Survey from 2010 to 2017 to explore the differences in psychological distress among Chinese residents at various ages, periods, and cohorts. The dummy variable method and random effects hierarchical age-period-cohort model were employed to isolate the age, period, and cohort effects that impacted the psychological distress of Chinese urban residents.First,in terms of the age effect, the psychological distress experienced by residents tends to increase initially and then decrease with age.Middle-aged individuals, around 40 years old, often face a particularly severe form of psychological distress known as the“midlife crisis.”Additionally,middle-aged people who have larger families may experience more significant psychological distress due to increased family burdens.Second,in terms of the period effect, the psychological distress experienced by residents has been on the rise due to the continued development of society. Those who are at a disadvantage in market competition and work longer hours may be particularly vulnerable to increased psychological distress.Thirdly, the cohort effect refers to the different social behaviors and attitudes exhibited by individuals of different generations. In terms of this effect, psychological distress initially decreased and then increased among those born between 1945 and 1990.Individuals born in the 1980s and 1990s experienced higher levels of psychological distress, with internet use having a greater negative impact.Additionally, the positive effect of income on their psychological distress was reduced. The psychological distress of Chinese urban residents is affected by a variety of time-related factors, which are closely related to Chinese clan culture, market competition and social changes. The heavy family burden imposed by traditional clan culture, the different working conditions created by market competition, and the emergence of new lifestyles, such as internet use during social changes, have led to diverse psychological distress experiences among residents across different ages, time periods, and cohorts. In the future, longitudinal studies are needed to strengthen the robustness of the conclusions and consider the long-term impact of the COVID-19 pandemic.Mental health support should pay specific attention to the psychological distress of middle-aged individuals,long-time workers, and internet users. 展开更多

AIM: To assess the effects of poor nutritional and psychological status on tolerance of cancer treatment and the recovery of physical performance status in patients with gastrointestinal cancer. METHODS: An epidemiological survey with respect to nutritional and psychological status in patients with gastrointestinal cancer was conducted among 182 operated patients in four provincial-level hospitals from December 2005 to June 2006. The food frequency survey method, state-trait anxiety inventory (STAI) and depression status inventory (DSI) were used to obtain information about the diet and psychological status in the patients. Nutritional status in the participants was reflected by serum albumin (Alb), hemoglobin (HB) and body mass index (BMI). RESULTS: Alb, protein intake and anxiety were associated with the severity of side effects of treatment. The adjusted relative risk (RR) for Alb, protein intake and anxiety was 3.30 (95% CI: 1.08, 10.10, P = 0.03), 3.25 (95% CI: 1.06, 9.90, P = 0.04) and 1.48 (95% CI: 1.29, 1.70, P < 0.0001), respectively. Moreover, calorie intake, HB and depression were associated with the recovery of physical performance status in the patients. Adjusted relative risk was 2.12 (95% CI: 1.09, 4.03, P = 0.028), 2.05 (95% CI: 1.08, 3.88, P =0.026) and 1.07 (95% CI: 1.02, 1.12, P = 0.007), respectively. CONCLUSION: Both poor nutrition status and psychological status are independent risk factors for severe side effects of cancer treatment, and have impact on the recovery of physical performance status in patients after treatment. 展开更多

Construction employees could experience occupational psychological disorders, such as workaholism and burnout due to their work, personality characteristics or lifestyle. This study sought to explore the effects of psychological disorders on construction employees and the construction industry. To achieve this aim, both the methods of focus group discussions and survey questionnaire were employed. The focus group discussions revealed 17 potential effects and 12 potential effects of psychological disorders on the construction employees and the construction industry respectively. A quantitative study was then employed to determine the key effects and to test the reliability of the findings from the focus group study. The results revealed that the highly perceived effects of psychological disorders on construction employees were accident-prone, chronic pain, insomnia or sleep disturbances, as these had the highest mean scores. The key effects also identified as perceived effects of construction employees’ psychological health conditions on the construction industry were: absenteeism/sick leave, errors in work, job dissatisfaction and increased medical costs. Exploratory factor analysis was employed, and the 17 effects on construction employees were categorized under behavioural effects and physiological effects. The 12 effects on the construction industry were also categorized under direct costs and indirect costs. The results from this study confirm the need for strategic interventions to mitigate the effects of occupational psychological disorders on construction employees and the construction industry of Ghana and to some extent globally. The exploratory nature of the study using preliminary findings from focus group discussions contributes to the literature on occupational health psychology. 展开更多

AIM To determine the placebo response rate associated with different types of placebo interventions used in psychological intervention studies for irritable bowel syndrome. METHODS Randomized controlled trials comparing psychological interventions(stress management/relaxation therapy(cognitive) behavioral therapy, short-term psychodynamic therapy, and hypnotherapy) for the treatment of adult patients with irritable bowel syndrome(IBS) diagnosed with the Manning or Rome criteria with an adequate placebo control treatment and reporting data on IBS symptom severity were identified by searching Pub Med, Embase, the Cochrane Library, CINAHL and Psyc INFO databases. Full-text articles that were written in English and published between 1966 and February 2016 in peer-reviewed journals were selected for the present review. Placebo interventions were considered to be adequate if the number of sessions and the amount of time spent with the therapist were the same as in the active treatment. The placebo response rate(PRR) was computed for IBS symptom severity(primary outcome measure) as well as for anxiety, depression and quality of life(secondary outcome measures). RESULTS Six studies, with a total of 555 patients met the inclusion criteria. Four studies used an educational intervention, whereas two studies used a form of supportive therapy as the placebo intervention. The PRR for IBS symptom severity ranged from 25% to 59%, with a pooled mean of 41.4%. The relative PRR for the secondary outcome measures ranged from 0% to 267% for anxiety, 6% to 52% for depression 20% to 125% for quality of life. The PRR associated with pharmacological treatments, treatment with dietary bran and complementary medicine ranged from 37.5% to 47%. Contrary to our expectations, the PRR in studies on psychological interventions was comparable to that in studies on pharmacological, dietary and alternative medical interventions.CONCLUSION The PRR is probably determined to a larger extent by patient-related factors, such as expectations and desire for the treatment to be effective, than the content of the placebo intervention. 展开更多
