In this paper, the pulsatile flow of blood through an inclined catheterized stenosed artery is analyzed. Perturbation method is used to solve the implicit system of partial differential equations with suitable boundary conditions. Various analytical expressions axial velocity, flow rate, wall shear stress and effective viscosity have been derived with the help of MATLAB for understanding the fluid flow phenomena. The combined effect of catheterization, body acceleration, slip and inclination has been seen by plotting the graph and observed that axial velocity and flow rate increases with the increase in body acceleration, inclination angle and slip velocity while axial velocity diminishes on increasing the catheter radius. Wall shear stress increases with the increase in catheter radius and body acceleration but presence of slip velocity reduces the wall shear stress. Effective viscosity diminishes on increasing body acceleration and inclination angle, whereas slightly augmented in non-inclined stenosed artery. 展开更多

The blood flow through a catheterized artery having a mild stenosis at the wall together with a blood clot at the centre is studied in the current investigation.Stenosis can occur in vessels carrying blood to brain(i.e.,Carotid arteries),Renal arteries that supply blood to kidneys etc.The flow is refined in such vessels by application of catheter. We have used a Newtonian viscous fluid model and also distinct shapes of stenosis,(i.e.,symmetric and non-symmetric shapes)are considered for this study. The entropy generation togetherwith viscous dissipation is also taken into account for a complete description of heat transfer mechanism.Exact solutions are calculated for the problem subject to given“no slip conditions”.The results are discussed graphically. The velocity quickly increases for a non-symmetric stenosis as compared to a symmetric stenosis. When the height of mild stenosis increases and the channel becomes narrow then the velocity increases in the centre but it decreases with the stenosed wall. However,as the height of blood clotσincreases then the velocity of blood flow reduces with the wall having clot but it remains almost same with the stenosed wall.Streamlines are plotted to visualize the flow pattern. The trapping is symmetric for a symmetric stenosis shape but it changes to non-symmetric trapping when we have a non-symmetric shape of stenosis. 展开更多

Although the device is useful medically to treat urinary retention and incontinence, its uses have also been associated with infections such as catheter-associated urinary tract infections. <strong>Objectives:</strong> To evaluate catheter-associated urinary tract infection in terms of its prevalence, pathogens causing the infection and the resistance pattern to some commonly used antibiotics. <strong>Methods: </strong>Urine and biofilm specimen were collected from 105 catheterized patients. Bacteria pathogens were identified based on colony morphology, Gram staining and other biochemical reactions. Kirby-Bauer disc diffusion method was employed to determine the antibiotic resistance pattern of the isolated pathogens. <strong>Results: </strong>The study recorded a 74.29% prevalence of catheter-associated urinary tract infection among the rarely symptomatic patients. <em>Escherichia coli</em> were significantly isolated (p < 0.000) in both samples compared to Klebsiella pneumoniae and <em>Staphylococcus aureus</em>. Most isolated pathogens from both samples were resistant to ciprofloxacin with the biofilm pathogens being highly resistant than the urine pathogens. K. pneumoniae from both samples showed higher resistance levels to antibiotics than <em>E. coli</em> and <em>S. aureus</em>. <strong>Conclusion:</strong> Ciprofloxacin, a commonly used antibiotic by catheterized patients turn out to be less effective against the pathogens. The use of ciprofloxacin in catheter-associated urinary tract infection treatment should therefore be given a second thought. 展开更多

Catheter is commonly used by the surgeons for various reasons in the treatment of a patient suffering with cardiovascular diseases. Catheterization increases the mean flow resistance in the arterial blood flow and many other complications are associated with the presence of catheter in the artery. Effects of catheter in stenosed artery can be estimated non-invasively by means of hemo-dynamic indicator-WSS, WSSG, volume flow rate and impedance. The effect of slip at the arterial wall, inclination of the artery and magnetic field on the hemodynamic indicators and flow profiles are computed, presented and discussed through graphs. 展开更多

BACKGROUND Peripherally inserted central catheters(PICCs)are an essential infusion route for oncology patients receiving intravenous treatments, but lower extremity veni-puncture is the preferred technique for patients with superior vena cava syndrome(SVCS).We report the case of a patient with a lower extremity PICC ectopic to the ascending lumbar vein,to indicate and verify PICC catheterisation in the lower extremity is safe and feasible. And hope to provide different per-spectives for clinical PICC venipuncture to get the attention of peers.CASE SUMMARY On 24 August 2022,a 58-year-old male was admitted to our department due to an intermittent cough persisting for over a month, which worsened 10 d prior.Imaging and laboratory investigations suggested the patient with pulmonary malignancy and SVCS.Chemotherapy was not an absolute contraindication in this patient.Lower extremity venipuncture is the preferred technique because administering upper extremity venous transfusion to patients with SVCS can exacerbate oedema in the head, neck, and upper extremities. The patient and his family were informed about the procedure, and informed consent was obtained. After successful puncture and prompt treatment, the patient was discharged, experiencing some relief from symptoms.CONCLUSION Inferior vena cava catheterisation is rare and important for cancer patients with SVCS,particularly in complex situations involving ectopic placement. 展开更多

BACKGROUND Long-term abdominal drains(LTAD)are a cost-effective palliative measure to manage malignant ascites in the community, but their use in patients with end-stage chronic liver disease and refractory ascites is not routine practice. The safety and cost-effectiveness of LTAD are currently being studied in this setting, with preliminary positive results. We hypothesised that palliative LTAD are as effective and safe as repeat palliative large volume paracentesis(LVP)in patients with cirrhosis and refractory ascites and may offer advantages in patients’quality of life.AIM To compare the effectiveness and safety of palliative LTAD and LVP in refractory ascites secondary to end-stage chronic liver disease.METHODS A retrospective, observational cohort study comparing the effectiveness and safety outcomes of palliative LTAD and regular palliative LVP as a treatment for refractory ascites in consecutive patients with end-stage chronic liver disease followed-up at our United Kingdom tertiary centre between 2018 and 2022 was conducted.Fisher’s exact tests and the Mann-Whitney U test were used to compare qualitative and quantitative variables,respectively.Kaplan-Meier survival estimates were generated to stratify time-related outcomes according to the type of drain.RESULTS Thirty patients had a total of 35 indwelling abdominal drains and nineteen patients underwent regular LVP.The baseline characteristics were similar between the groups.Prophylactic antibiotics were more frequently prescribed in patients with LTAD(P=0.012),while the incidence of peritonitis did not differ between the two groups(P=0.46).The incidence of acute kidney injury(P=0.014)and ascites/drain-related hospital admissions(P=0.004)were significantly higher in the LVP group. The overall survival was similar in the two groups(log-rank P=0.26),but the endpoint-free survival was significantly shorter in the LVP group(P=0.003,P<0.001,P=0.018 for first ascites/drain-related admission, acute kidney injury and drain-related complications, respectively).CONCLUSION The use of LTAD in the management of refractory ascites in palliated end-stage liver disease is effective, safe, and may reduce hospital admissions and utilisation of healthcare resources compared to LVP. 展开更多

BACKGROUND Severe acute pancreatitis(SAP),a condition with rapid onset, critical condition and unsatisfactory prognosis, poses a certain threat to human health, warranting optimization of relevant treatment plans to improve treatment efficacy.AIM To evaluate the efficacy and safety of computerized tomography-guided the-rapeutic percutaneous puncture catheter drainage(CT-TPPCD)combined with somatostatin(SS)in the treatment of SAP.METHODS Forty-two SAP patients admitted to The Second Affiliated Hospital of Fujian Medical University from June 2020 to June 2023 were selected. On the basis of routine treatment,20 patients received SS therapy(control group)and 22 patients were given CT-TPPCD plus SS intervention(research group). The efficacy, safety(pancreatic fistula,intra-abdominal hemorrhage, sepsis, and organ dysfunction syndrome),abdominal bloating and pain relief time, bowel recovery time, hospital stay, inflammatory indicators(C-reactive protein,interleukin-6,and pro-calcitonin),and Acute Physiology and Chronic Health Evaluation(APACHE)II score of both groups were evaluated for comparison.RESULTS Compared with the control group, the research group had a markedly higher total effective rate, faster abdominal bloating and pain relief and bowel recovery,INTRODUCTION Pancreatitis,an inflammatory disease occurring in the pancreatic tissue,is classified as either acute or chronic and is associated with high morbidity and mortality, imposing a socioeconomic burden[1,2].The pathogenesis of this disease involves early protease activation, activation of nuclear factor kappa-B-related inflammatory reactions, and infiltration of immune cells[3].Severe acute pancreatitis(SAP)is a serious condition involving systemic injury and subsequent possible organ failure, accounting for 20%of all acute pancreatitis cases[4].SAP is also characterized by rapid onset, critical illness and unsatisfactory prognosis and is correlated with serious adverse events such as systemic inflammatory response syn-drome and acute lung injury, threatening the health of patients[5,6].Therefore,timely and effective therapeutic inter-ventions are of great significance for improving patient prognosis and ensuring therapeutic effects.Somatostatin(SS),a peptide hormone that can be secreted by endocrine cells and the central nervous system,is in-volved in the regulatory mechanism of glucagon and insulin synthesis in the pancreas[7].It has complex and pleiotropic effects on the gastrointestinal tract, which can inhibit the release of gastrointestinal hormones and negatively modulate the exocrine function of the stomach, pancreas and bile, while exerting a certain influence on the absorption of the di-gestive system[8,9].SS has shown certain clinical effectiveness when applied to SAP patients and can regulate the severity of SAP and immune inflammatory responses, and this regulation is related to its influence on leukocyte apoptosis and adhesion[10,11].Computerized tomography-guided therapeutic percutaneous puncture catheter drainage(CT-TPPCD)is a surgical procedure to collect lesion fluid and pus samples from necrotic lesions and perform puncture and drainage by means of CT image examination and precise positioning[12].In the research of Liu et al[13],CT-TPPCD applied to pa-tients undergoing pancreatic surgery contributes to not only good curative effects but also a low surgical risk.Baudin et al[14]also reported that CT-TPPCD has a clinical success rate of 64.6%in patients with acute infectious necrotizing pan-creatitis,with nonfatal surgery-related complications found in only two cases, suggesting that this procedure is clinically effective and safe in the treatment of the disease. In light of the limited studies on the efficacy and safety of SS plus CT-TPPCD in SAP treatment, this study performed a relevant analysis to improve clinical outcomes in SAP patients. 展开更多

Aims:Although the application of ultrasound-guided vascular puncture and Z-stitch hemostasis to manage femoral access has been widely utilized, there is limited data on this combined application in adult congenital heart disease(ACHD)patients undergoing electrophysiological(EP)procedures. We sought to evaluate the safety and efficacy of ultrasound-guided puncture and postprocedural Z-stitch hemostasis for ACHD patients under-going EP procedures. Methods and Results:The population of ACHD patients undergoing transfemoral EP pro-cedures at the University of Zurich Heart Center between January 2019 and December 2022 was observed and analyzed.During the study period, femoral access(left/right,arterial/venous)was performed under real-time ultrasound guidance. At the end of the procedure,a single Z-stitch was performed at the puncture site.We eval-uated the incidence of in-hospital complications associated with femoral access puncture in this population.Among 101 patients who had a total of 147 previous ipsilateral vascular punctures(mean 1.5 per person),100 patients underwent successful femoral vascular access for EP procedures. The median age of the patients was 47±15 years and 34(34%)were male.Z-stitches were performed after the procedure in 100 patients with 303 femoral vascular accesses(mean 3 punctures per person). No patient developed vascular puncture relevant inguinal hematoma, pseudo aneurysm, arteriovenousfistula, venous or arterial thrombosis. Conclusion:In ACHD patients undergoing EP procedures, optimal femoral access management can be achieved with ultra-sound-guided puncture and postprocedural Z-stitch hemostasis. 展开更多

BACKGROUND Peritoneal dialysis(PD)is an important renal replacement therapy in patients with end-stage renal disease.PD catheters remain the lifeline for patients undergoing PD.The catheter technique survival rate is considered a core PD outcome domain.CASE SUMMARY The PD catheter spontaneously dislodged in a patient undergoing PD during regular fluid exchange without pain.Abdominal computed tomography showed a tunnel infection.A double-cuff straight Tenckhoff catheter had been inserted using the Seldinger technique. Before this incident, the patient had a history of tunnel infections. We speculate that recurrent tunnel infections and catheter insertion using the Seldinger technique may have led to catheter dislodgement.CONCLUSION The present case suggests that clinicians should more rigorously assess the persistence of catheter-related infections concerning the potential complications arising from catheter dislodgement associated with the Seldinger technique. 展开更多

BACKGROUND Enterocutaneous fistula(ECF)is an abnormal connection between the gastrointestinal tract and the skin.ECF can lead to massive body fluid loss, hypercatabolism, and malnutrition. Therefore, nutritional support plays a crucial role in managing ECFs and promoting the healing of fistulas. For nutritional support, enteral nutrition(EN)is the preferred method when gastrointestinal function is recovering.Currently, various EN approaches have been applied for different anatomical positions of the ECF.However, the effectiveness of administering EN support for treating lower ECFs still needs further exploration and improvement.CASE SUMMARY We present the case of a 46-year-old male who underwent gastrointestinal stromal tumour resection.Six days after the surgery, the patient presented with fever, fatigue, severe upper abdominal pain, and septic shock.Subsequently, lower ECFs were diagnosed through laboratory and imaging examinations. In addition to symptomatic treatment for homeostasis, total parenteral nutrition support was administered in the first 72 h due to dysfunction of the intestine. After that,we gradually provided EN support through the intestinal obstruction catheter in consideration of the specific anatomic position of the fistula instead of using the nasal jejunal tube.Ultimately, the patient could receive optimal EN support via the catheter, and no complications were found during the treatment.CONCLUSION Nutritional support is a crucial element in ECF management, and intestinal obstruction catheters could be used for early EN administration. 展开更多
