目的:应用高分辨率光学相干断层扫描仪(Cirrus-HD OCT)测量服用乙胺丁醇(EMB)的结核病患者视盘周围视网膜神经纤维层(p-RNFL)厚度和黄斑区神经节细胞-内丛状层(GCIPL)的厚度,探讨EMB对视网膜神经节细胞的损害特点,评估高分辨率OCT在早... 目的:应用高分辨率光学相干断层扫描仪(Cirrus-HD OCT)测量服用乙胺丁醇(EMB)的结核病患者视盘周围视网膜神经纤维层(p-RNFL)厚度和黄斑区神经节细胞-内丛状层(GCIPL)的厚度,探讨EMB对视网膜神经节细胞的损害特点,评估高分辨率OCT在早期诊断中毒性视神经病变(EON)中的价值。方法:病例对照研究。收集2018年1月至2019年3月就诊的服用EMB等抗结核药物的结核患者60例,剔除2例发病半年以上视神经萎缩者和9例合并其他眼底病变者,其中6例(12眼)确诊为EON纳入EON组,另43例(85眼)服药后视功能正常者纳入服药组,选择41例(82眼)与服药组年龄、性别匹配的正常人群纳入对照组1,另选择13例(26眼)与EON组年龄、性别匹配的正常人群纳入对照组2。对服药组和对照组1,EON组和对照组2,分别进行p-RNFL平均值及鼻、下、颞、上4个象限厚度的比较,以及黄斑GCIPL层平均厚度,GCIPL最小厚度及GCIPL鼻上、鼻下、下方、颞下、颞上、上方6个区域厚度的比较,分别进行独立样本t检验。结果:与对照组1相比,服药组p-RNFL的平均厚度和鼻、下、颞、上4个象限的厚度差异均无统计学意义(P>0.05),而GCIPL层除了颞下区差异无统计学意义(P>0.05),GCIPL的平均厚度、最小厚度、鼻上、鼻下、下方、颞上、上方区域均明显变薄,差异有统计学意义(t=-3.149、-2.880、-3.816、-3.697、-2.646、-2.231、-2.323,P<0.05)。与对照组2相比,EON组的p-RNFL在鼻侧和下方增厚,差异有统计学意义(t=2.452、2.314,P<0.05),GCIPL层在鼻上、鼻下、下方、颞下、颞上和上方均变薄,差异有统计学意义(t=-2.809、-2.622、-2.806、-2.461、-2.887、-3.478,P<0.05)。结论:高分辨率OCT能精确测量GCIPL厚度,更直接观察到视网膜神经节细胞的损害及其程度,结合p-RNFL和黄斑GCIPL厚度的测量,可帮助早期诊断EON。 展开更多

新型频域相关断层扫描对健康年轻人视网膜各参数的测量分析(英文)

目的:采用新型频域相关断层扫描测量分析健康年轻人中视网膜各参数生理的不对称性。方法:对146名健康年轻人进行横断面观察研究,采用新型频域相关断层扫描对其双眼视网膜参数(视网膜神经纤维层厚度、视盘、黄斑)进行扫描。通过右眼减去... 目的:采用新型频域相关断层扫描测量分析健康年轻人中视网膜各参数生理的不对称性。方法:对146名健康年轻人进行横断面观察研究,采用新型频域相关断层扫描对其双眼视网膜参数(视网膜神经纤维层厚度、视盘、黄斑)进行扫描。通过右眼减去左眼所得参数值的平均差进行配对t检验。两眼间的差异采用2.5%~97.5%之间进行评估。视网膜参数相关。通过回归分析,评价性别和血压对视网膜参数的影响。结果:两眼的视网膜神经纤维层平均厚度无统计学差异(+0.7μm,P=0.09)。两眼视网膜颞侧象限(+2.51μm,P=0.02),鼻侧象限(+2.50μm,P=0.04),钟表位右眼2点与左眼10点(+5.85μm,P=0.002),右眼3点与左眼9点(+3.20μm,P=0.005),右眼11点与左眼1点(+3.80μm,P=0.024),平均厚度(-0.034μm,P=0.013)和垂直方向杯盘比(-0.028μm,P=0.008),黄斑部鼻侧象限(+7.76μm,P=0.003)。左眼上方视网膜明显较薄(-2.40μm,P=0.03)。平均视网膜神经纤维层厚度和黄斑厚度分别为11μm和18μm。视网膜参数在双眼有中度的相关性(0.41~0.6),而两眼视网膜参数与性别、血压不相关(P<0.05)。研究表明通过新型频域相关断层扫描发现健康年轻人的视网膜神经纤维层厚度和黄斑厚度两眼间的差异分别不应超过11μm和18μm。结论:视网膜参数的不对称性存在于健康视网膜,并对早期视网膜病变的诊断提供有价值的评估。 展开更多

Assessments of tear meniscus height, tear film thickness, and corneal epithelial thickness after deep anterior lamellar keratoplasty 被引量:2

Methods: This was... Objective: To assess the lower tear meniscus height(LTMH), central tear film thickness(CTFT), and central corneal epithelial thickness(CCET) after deep anterior lamellar keratoplasty(DALK). Methods: This was a retrospective cross-sectional study of 20 patients who had DALK in one eye over a three-month period. LTMH, CTFT, and CCET of the operated eyes and the unoperated fellow eyes were measured using high-definition optical coherence tomography(HD-OCT). Correlations between three OCT assessments and age, time following surgery, graft size, bed size, and the number of residual sutures were analyzed. Results: Compared to patients with keratoconus, patients with other corneal conditions had significantly higher CCET in the fellow eye(P=0.024). For all patients, CCET in the operated eye was significantly negatively correlated with the number of residual sutures(R=-0.579, P=0.008), and was significantly positively correlated with time following surgery(R=0.636, P=0.003). In the fellow eye, a significant positive correlation was found between age and CCET(R=0.551, P=0.012), and a significant negative correlation between age and CTFT(R=-0.491, P=0.028). LTMH was found to be significantly correlated between operated and fellow eyes(R=0.554, P=0.011). There was no significant correlation between LTMH and age, bed/graft size, time following surgery, or residual sutures(all possible correlations, P0.05). Conclusions: Patients with keratoconus tend to have a thinner central corneal epithelium. Corneal epithelium keeps regenerating over time after DALK. DALK did not induce a significant change in tear volume compared with the fellow eye. Postoperative tear function might depend on an individual's general condition, rather than on age, gender, bed/graft size, time following surgery, or residual sutures. 展开更多
