METHODS We enrolled 174 chronic hepatitis B-related liver cirrhosis patients treated with 300 mg/d TDF at six Korean centers. Of the 174 cirrhosis patients, 57 were assigned to the decompensated cirrhosis group and 117 were assigned to the compensated cirrhosis group. We followed the patients for 12 mo and evaluated clinical outcomes, including biochemical, virological, and serological responses. We also evaluated changes in hepatic and renal function and compared the decompensated and compensated cirrhosis groups. RESULTS The 1-year complete virological response(CVR) and Hepatitis B e antigen(HBe Ag) seroconversion were seen in 70.2% and 14.2% in the decompensated cirrhosis group, respectively. The rates of HBe Ag seroconversion/loss and ALT normalization at month 12 were similar in both groups. TDF treatment was also effective for decreasing the level of hepatitis B virus(HBV) DNA in both groups, but CVR was higher in the compensated group(88.9% vs 70.2%, P = 0.005). Tenofovir treatment for 12 mo resulted in improved Child-Turcotte-Pugh(CTP) and model for end-stage liver disease(MELD) scores in decompensated group(P < 0.001). Of the 57 decompensated patients, 39(68.4%) achieved CTP class A and 27(49.1%) showed improvement in the CTP score of 2 points after 12 mo of TDF. The observed rate of confirmed 0.5 mg/d L increases in serum levels of creatinine in the decompensated and compensated cirrhosis group were 7.0% and 2.5%, respectively(P < 1.000).CONCLUSION TDF therapy in decompensated cirrhosis patients was effective for decreasing HBV DNA levels and improving hepatic function with relatively lower CVR than in compensated cirrhosis. Thus, physicians should carefully monitor not only renal function but also treatment responses when using TDF in decompensated cirrhosis patients.展开更多

The so-called compensated case is that there is no net magnetization on the AFM side of the interface. Our conclusion is that the EB in this case is primarily from the asymmetry of the interracial exchange coupling strengths between the FM and the two sublattices of the AFM. The effects of the layer thickness, temperature and the interracial coupling strength oi2 the exchange bias HE are investigated. The dependence of HE on the FM layer thickness and temperature is qualitatively in agreement with experimental results. HE is nearly inversely proportional to FM thickness. When temperature varies, both HE and He decrease with temperature increasing. The anisotropy of the FM layer only slightly influence He, but does not influence HE.展开更多

This proposed calibration technique compensating both TC and output value of BGR achieves fine adjustment step towards the reference voltage, while keeping optimal TC by utilizing large resistance to help layout match. The high-order curvature compensation realized by poly and p-diffusion resistors is introduced into the design to guarantee the temperature characteristic. Implemented in 180 nm technology, the proposed BGR has been simulated to have a power supply rejection ratio( PSRR) of 91 dB@100 Hz. The calibration technique covers output voltage scope of 0. 49 V-0. 56 Vwith TC of 9. 45 × 10^(-6)/℃-9. 56 × 10^(-6)/℃ over the temperature range of-40 ℃-120 ℃. To date, no satisfactory theoretical model has been able to explain precisely the variety of new experimental results and observations related to compensated-Si. In this study, a new approach considering equilibrium ionisation constants according to electrolyte theory was proposed, which reproduce, for single-doped Si, Thurber's curves of charge carrier's mobilities. When more than one doping species are involved, as in compensated-Si, a numerical algorithm has to be used for solving multiple equilibrium systems. The study of such systems demonstrates a particular behaviour known from buffered solutions. Equilibrium constants were calculated from thermodynamic properties of chemical compounds, and a new general theory was proposed using available knowledge of electrochemistry (Nernst equation, Butler-Volmer equation). Considering that the silicon/dopant systems constitute a weak electrolyte solid solution, it is concluded that the electrolyte solution theory provides a good physical model and mathematical framework to get a better understanding of solar cell's behaviour.展开更多

Thus, physicians should carefully monitor not only renal function but also treatment responses when using TDF in decompensated cirrhosis patients.
文摘BACKGROUND The albumin-bilirubin(ALBI)score is an index of liver function recently developed to assess prognosis in patients with hepatocellular carcinoma(HCC).It can detect small changes in liver dysfunction and has been successfully applied to the prediction of survival in patients with non-malignant liver diseases of various etiologies.AIM To investigate the ALBI score for identifying decompensation risk at the 3-year follow-up in patients with compensated cirrhosis.METHODS One-hundred and twenty-three patients with compensated cirrhosis without HCC in King Chulalongkorn Memorial Hospital diagnosed by imaging were retrospectively enrolled from January 2016 to December 2020.A total of 113 patients(91.9%)had Child A cirrhosis with a median model for end-stage liver disease(MELD)score of less than 9.Baseline clinical and laboratory variables and decompensation events were collected. The ALBI score was calculated and validated to classify decompensation risk into low-,middle-,and high-risk groups using three ALBI grade ranges(ALBI grade 1:≤-2.60;grade 2:>-2.60 but≤-1.39;grade 3:>-1.39).Decompensation events were defined as ascites development, variceal bleeding,or grade 3 or 4 hepatic encephalopathy.RESULTS Among 123 cirrhotic patients enrolled,13.8%(n=17)developed decompensating events at a median time of 25[95%confidence interval(CI):17-31]mo.Median baseline ALBI score in compensated cirrhosis was significantly lower than that of patients who developed decompensation events[-2.768(-2.956 to-2.453)vs-2.007(-2.533 to-1.537);P=0.01].Analysis of decompensation risk at 3 years showed that ALBI score had a time-dependent area under the curve(tAUC)of 0.86(95%CI:0.78-0.92),which was significantly better than that of ALBI-Fibrosis-4(ALBI-FIB4)score(tAUC=0.77),MELD score(tAUC=0.66),Child-Pugh score(tAUC=0.65),and FIB-4 score(tAUC=0.48)(P<0.05 for all). The 3-year cumulative incidence of decompensation was 3.1%,22.6%,and 50%in the low-,middle-,and high-risk groups,respectively(P<0.001).The odds ratio for decompensation in patients of the high-risk group was 23.33(95%CI:3.88-140.12,P=0.001).CONCLUSION The ALBI score accurately identifies decompensation risk at the 3-year follow-up in patients with compensated cirrhosis. Those cirrhotic patients with a high-risk grade of ALBI score showed a 23 times greater odds of decompensation.

文摘BACKGROUND Cognitive impairment(CI)is common in patients with heart failure(HF),but the association between CI and biomarkers related to HF or cognitive decline in patients with HF remains unclear.METHODS This prospective observational study investigated the incidence of CI,subsequent cognitive changes, and the asso-ciation between CI and novel biomarkers in patients with left ventricular ejection fraction<40%who were hospitalized for acute decompensated HF.Patients were evaluated for CI,depressive symptoms, and quality of life with the Mini-Mental State Examin-ation(MMSE)and the Mini-Cog,Beck Depression Inventory(BDI)-II,and Kansas City Cardiomyopathy Questionnaire(KCCQ),respectively. The primary endpoint was a composite of all-cause mortality or hospitalization for HF at one year.RESULTS Among the 145 patients enrolled in this study,54 had CI(37.2%)at baseline. The mean MMSE increased signific-antly at the 3-month and 1-year follow-up,accompanied by decreased BDI-II and increased KCCQ scores. The improvement in the MMSE scores mainly occurred in patients with CI.Among the biomarkers assayed, only growth/differentiation factor(GDF)-15>1621.1 pg/mL was significantly associated with CI(area under the curve=0.64;P=0.003).An increase in GDF-15 per 1000 units was associated with an increased risk of the primary endpoint(hazard ratio=1.42;95%confidence interval:1.17-1.73;P<0.001).CONCLUSIONS In patients with HF with CI,cognitive function, depression, and quality of life measures improved at the 3-month and 1-year follow-up.GDF-15 predicted CI with moderate discrimination capacity and was associated with worse HF out-comes.

文摘To reduce the difficulty of the epitaxy caused by multiple quantum well infrared photodetector(QWIP)with tunnel compensation structure,an improved structure is proposed. In the new structure, the superlattices are located between the tunnel junction and the barrier as the infrared absorption region, eliminating the effect of doping concentration on the well width in the original structure.Theoretical analysis and experimental verification of the new structure are carried out.The experimental sample is a two-cycle device, each cycle contains a tunnel junction,a superlattice infrared absorption region and a thick barrier. The photosurface of the detector is 200×200μm^2 and the light is optically coupled by 45°oblique incidence. The results show that the optimal operating voltage of the sample is-1.1 V,the dark current is 2.99×10^-8A,and the blackbody detectivity is1.352×10^8 cm·Hz^1/2·W^-1at 77 K.Our experiments show that the new structure can work normally.

文摘An active temperature compensated fiber Bragg grating(FBG)vibration sensor with a constant section cantilever beam is proposed for the simultaneous measurement of temperature and vibration, and the sensor is verified by a temperature compensation feedback system. The high-temperature vibration sensor is composed of a quartz cantilever beam and a femtosecond Bragg grating. The feedback control demodulation system of active temperature compensation can adjust the laser wavelength to stabilize the grating offset point and realize simultaneous measurement of temperature and vibration. On this basis, the performance of the sensor is tested and analyzed within the range of 20-400℃by setting up a high-temperature vibration test system. The experimental results show that the sensitivity of the sensor is about 132.33 mV/g,and the nonlinearity is about 3.33%.The sensitivity between the laser wavelength and temperature is about 0.01307 nm/℃.In addition, the active temperature compensated fiber Bragg grating vibration sensor has the advantages of a simple structure, stable performance, easy demodulation and high sensitivity. Moreover, the sensor can achieve high temperature vibration signal monitoring and has good practical application value.
文摘Compensated liver cirrhosis(CLC)is defined as cirrhosis with one or more decompensating events, such as ascites, variceal haemorrhage,or hepatic encephalopathy.Patients with CLC are largely asymptomatic with preserved hepatic function. The transition from CLC to decompensated cirrhosis occurs as a result of a complex interaction between multiple predisposing and precipitating factors. The first decompensation event in CLC patients is considered a significant turning point in the progression of cirrhosis,as it signals a drastic decline in median survival rates from 10-12 years to only 1-2 years.Furthermore, early cirrhosis has the potential to regress as liver fibrosis is a dynamic condition. With the advent of effective non-invasive tools for detecting hepatic fibrosis, more and more patients with CLC are currently being recognised. This offers clinicians a unique opportunity to properly manage such patients in order to achieve cirrhosis regression or,at the very least, prevent its progression. There are numerous emerging approaches for preventing or delaying decompensation in CLC patients.A growing body of evidence indicates that treating the underlying cause can lead to cirrhosis regression, and the use of non-selective beta-blockers can prevent decompensation by lowering portal hypertension.Additionally,address-ing various cofactors(such as obesity, diabetes, dyslipidaemia, and alcoholism)and precipitating factors(such as infection, viral hepatitis, and hepatotoxic drugs)that have a detrimental impact on the natural course of cirrhosis may benefit patients with CLC.However,high-quality data must be generated through well-designed and adequately powered randomised clinical trials to validate these diseasemodifying techniques for CLC patients. This article discussed the natural history of CLC,risk factors for its progression, and therapeutic approaches that could alter the trajectory of CLC evolution and improve outcomes.

文摘The exchange bias (EB) of the ferromagnetic (FM)/antiferromagnetic (AFM) bilayers in a compensated case is studied by use of the many-body Green's function method of quantum statistical theory. The anisotropy of the FM layer only slightly influence He, but does not influence HE.

文摘Based on the analysis of the-state-of-the-art of pressure compensation of underwater hydraulic systems (UHSs), a new method of pressure compensation of UHSs, whose hydraulic power unit is in the atmospheric circumstance, is proposed. And a pilot-operated relief valve with pressure compensation is realized. The pressure compensation precision is guaranteed by direct detection. Its dynamic performance and stability are improved by a dynamic feedback. 文摘A lowtemperature coefficient( TC) bandgap reference( BGR) with novel process variation calibration technique is proposed in this paper. The designed BGR provides a reference voltage of 500 mV,with measured TC of 10. 1 × 10^(-6)/℃.

文摘When wind farms, which is based on double fed induction generator(DFIG),are connected to the series compensation power system, the phenomenon of sub-synchronous oscillation(SSO)may occur. In order to study the sub-synchronous oscillation in wind power connected to series compensated power system and its influencing factors,we used RT-LAB to establish a simulation model concerning wind power connected to series-compensation power system. This model take a wind power connected to series compensation power system in north China as prototype. All influence factors of wind power SSO are simulated and analyzed by using time domain analysis method. The simulation results show that the effects of wind speed, series compensation degree and proportional control coefficient of rotor side converter(RSC)are most obvious.
文摘Models establishing relationships between electrical resistivity and dopant densities of silicon wafers/bricks are not applicable for compensated-Si, such as upgraded metallurgical grade silicon UMG-Si. Considering that the silicon/dopant systems constitute a weak electrolyte solid solution, it is concluded that the electrolyte solution theory provides a good physical model and mathematical framework to get a better understanding of solar cell's behaviour.
文摘To solve the symmetric positive definite linear system Ax = b on parallel and vector machines, multisplitting methods are considered. Here the s.p.d. (symmetric positive definite) matrix A need not be assumed in a special form (e.g. the dissection form [11]). The main tool for deriving our methods is the diagonally compensated reduction (cf. [1]). The convergence of such methods is also discussed by using this tool. [WT5,5”HZ]

文摘The electronic structures of Ti_2NbSb with Hg_2CuTi structure and TiZrNbSb with LiMgPdSn structure are investigated using first-principles calculations. The results indicate that Ti_2NbSb is a fully compensated ferrimagnetic spin-gapless semiconductor with an energy gap of 0.13 e V,and TiZrNbSb is a half-metallic fully compensated ferrimagnet with a half-metallic gap of 0.17 e V.For Ti_2NbSb,the total energy of the Hg_2CuTi structure is0.62 e V/f.u.higher than that of the L2_(1) structure, which is the ground state, and for TiZrNbSb,the total energy of the structure considered in this work is only 0.15 e V/f.u.larger than that of the ground state. Thus both of them may be good candidates for spintronic applications.
