The loss of a toothhas lifelong consequences for children and teenagers, but the maintenance of pulphealth and the calcific healing of multiple root fractures are rarely reported in theliterature.CASE SUMMARY This case reports healing of a permanent tooth with complicated crown–root andadditional root fractures, in which pulp health was maintained. A 10-year-old girlfell and fractured the root of her maxillary left central incisor at the cervical level. After the coronal fragment was repositioned, the tooth was splinted until thetooth was no longer mobile, 2 years later. Eight years after treatment, the toothhas remained asymptomatic with vital pulp and localized gingival overgrowth.Cone-beam computed tomography revealed not only calcified healing of the CRFbut also spontaneous healing in an additional undiagnosed root fracture. Thefracture line on the enamel could not be healed by hard tissue and formed agroove in the cervical crown. It was speculated that the groove was related to thelocalized gingival overgrowth.CONCLUSION This case provides a clinical perspective of the treatment of a tooth with acomplicated CRF and an additional root fracture.展开更多

Increasing laparoscopic experience and techniques have made laparoscopic subtotal cholecystectomy (LSC) a feasible option in more complex procedures. In recent years, few studies with a few cases of LSC have reported good results in patients with various types of cholecystitis. This study was designed to evaluate the feasibility, indications, characteristics and benefits of LSC in patients with complicated cholecystitis. METHODS: Altogether, 3485 patients were scheduled to receive LC during the past 4 years at our institute. Among them, 168 patients with various complicated forms of cholecystitis were treated by LSC. Meanwhile, the other 3317 patients who received standard LC were enrolled as the control group. Perioperative data from the two groups were collected and retrospectively analyzed. RESULTS: In the LSC group, 135 patients suffered from acute calculic cholecystitis, 18 from chronic calculic cholecystitis with cirrhotic portal hypertention, and 15 from chronic calculic atrophy cholecystitis with severe fibrosis. These patients constituted 4.8% of the total patients who underwent LC (168/3485) in the same period at our institute. In 122 patients, the cystic duct and artery were clipped before division. In another 46 patients, the gallbladder was initially incised at Hartmann's pouch. Five patients (3.0%) were converted to open subtotal cholecystectomy. The median operation time for LSC was 65.5±15.2 minutes, estimated operative blood loss was 71.5±15.5 ml, and the time to resume diet was 20.4±6.3 hours. Thirteen patients (7.7%) had local complications. The mean postoperative hospital stay was 4.2±2.6 days. In the LC group, 2887 had chronic calculic cholecystitis, 312had acute calculic cholecystitis, 47 had chronic calculic atrophy cholecystitis, and 71 had polypus. Seventeen patients (0.5%) were converted to open cholecystectomy. The median operation time was 32.6±10.2 minutes, the estimated operative blood loss was 24.5±8.5 ml, and the time to resume diet was 18.3±4.5 hours. Thirty- nine patients (1.2%) had local complications. Mean postoperative hospital stay was 3.8±1.4 days. There was no bile duct injury or mortality in either group. CONCLUSIONS: LSC for patients with complicated cholecystitis is difficult, with a longer operation time, more operative blood loss and higher conversion and complication rates than LC. However, it is feasible and relatively safe. LSC is advantageous over open surgery, but it remains a non-routine choice. It is important to know the technical characteristics of LSC, and pay attention to perioperative bleeding and bile leak.展开更多

This is mainly shown by multi-phase structural evolution, multiple sets of source-reservoir-cap rock combinations, multiple stages of hydrocarbon generation and expulsion from source rocks, multi-cycle hydrocarbon enrichment and accumulation, and multi-phase reservoir adjustment and reconstruction. The enrichment, accumulation and distribution of hydrocarbon is mainly controlled by the source rock kitchen, paleo- anticline, regional cap rock and intensity of tectonic movement. In this paper, the T-BCMS model has been developed to predict favorable areas of hydrocarbon accumulation in complicated superimposed basins according to time and spatial relationships among five key factors. The five factors include unconformity surface representing tectonic balancing (B), regional cap rock representing hydrocarbon protection (C), paleo-anticline representing hydrocarbon migration and accumulation (M), source rock kitchen representing hydrocarbon generation and expulsion (S) and geological time (T). There are three necessary conditions to form favorable areas of hydrocarbon accumulation. First, four key factors BCMS should be strictly in the order of BCMS from top to bottom. Second, superimposition of four key factors BCMS in the same area is the most favorable for hydrocarbon accumulation. Third, vertically ordered combination and superimposition in the same area of BCMS should occur at the same geological time. The model has been used to predict the most favorable exploration areas in Ordovician in the Tarim Basin in the main hydrocarbon accumulation periods. The result shows that 95% of the discovered Ordovician hydrocarbon reservoirs are located in the predicted areas, which indicates the feasibility and reliability of the key factor matching T-BCMS model for hydrocarbon accumulation and enrichment.展开更多

文摘To the Editor:Liver transplantation(LT)is the most effective method for endstage liver disease.Split liver transplantation(SLT)is an effective method to enlarge the number of liver grafts. However, because of the existence of portal vein variations, right hemi-grafts splitting and recipients’portal vein(PV)reconstruction might be more challenging[1,2].According to the origins of intrahepatic PV branches,Nakamura and his coworkers[3]classified PV variants into five classes(Fig.S1):type A to E.It is reported that the standard anatomy in PV branching pattern accounts for only 65%of investigated population, and that the most common anatomic variation of main portal vein(MPV)is trifurcation variation followed by right posterior portal vein(RpPV)as a first branch of MPV[4].Type C and D variations are the two most technique highly demanding types. The variation of PV in donated liver graft still challenges surgeons, especially in the field of SLT.There is still a lack of a common sense about the modality of complicated variations in PV reconstruction. Here,we presented two complicated SLT cases, existence of type C and D PV variations in grafts, respectively.
文摘In order to improve the detection accuracy of Doppler asymmetric spatial heterodyne(DASH)interferometer in harsh temperatures,an opto-mechanical-thermal integration analysis is carried out.Firstly, the correlation between the interference phase and temperature is established according to the working principle and the phase algorithm of the interferometer.Secondly, the optical mechanical thermal analysis model and thermal deformation data acquisition model are designed. The deformation data of the interference module and the imaging optical system at different temperatures are given by temperature load simulation analysis, and the phase error caused by thermal deformation is obtained by fitting. Finally, based on the wind speed error caused by thermal deformation of each component,a reasonable temperature control scheme is proposed. The results show that the interference module occupies the main cause, the temperature must be controlled within(20±0.05)℃,and the temperature control should be carried out for the temperature sensitive parts, and the wind speed error caused by the part is 3.8 m/s.The thermal drift between the magnification of the imaging optical system and the thermal drift of the relative position between the imaging optical system and the detector should occupy the secondary cause, which should be controlled within(20±2)℃,and the wind speed error caused by the part is 3.05 m/s.In summary, the wind measurement error caused by interference module, imaging optical system, and the relative position between the imaging optical system and the detector can be controlled within 6.85 m/s.The analysis and temperature control schemes presented in this paper can provide theoretical basis for DASH interferometer engineering applications.

文摘BACKGROUND In rhinoplasty, calcification around silicone implants is frequently observed in the tip dorsum(TD)area.Additionally, based on a review of various literature,it is presumed that calcification in silicone implants occurs due to both inflammatory chemical reactions and physical friction against the tissue. The calcification of nasal silicone implants not only results in the functional loss of the implants, but also leads to material deformation. However, there is a lack of research on calcification of nasal silicone implants in the current literature.AIM To elucidate various clinical characteristics of calcification around nasal silicone implants, using histological and radiological analysis.METHODS This study analyzed data from 16 patients of calcified nasal implants, who underwent revision rhinoplasty for various reasons after undergoing augmentation rhinoplasty with silicone implants. The collected data included information on implant duration, implant types, location of calcification, presence of inflammatory reactions, and computed tomography(CT)scans.RESULTS The most common location of calcification,as visually analyzed, was in the TD area, accounting for 56%.Additionally,the analysis of CT scans revealed a trend of increasing Hounsfield Unit values for calcification with the duration of implantation, although this trend was not statistically significant(P=0.139).CONCLUSION Our study shows that reducing the frequency of calcification may be achievable by using softer silicone implants and by minimizing the damage to perioperative tissues.

文摘BACKGROUND Postoperative enterostomy is increasing in patients with colorectal cancer, but there is a lack of a model that can predict the probability of early complications.AIM To explore the factors influencing early postoperative stoma complications in colorectal cancer patients and to construct a nomogram prediction model for predicting the probability of these complications.METHODS A retrospective study of 462 patients who underwent postoperative ostomy for colorectal cancer in the Gastrointestinal Department of the Anhui Provincial Cancer Hospital. The patients’basic information, surgical details, pathological results, and preoperative inflammatory and nutritional indicators were reviewed. We used univariate and multivariate logistic regression to analyze the risk factors for early postoperative stoma complications in colorectal cancer patients and constructed a nomogram prediction model to predict the probability of these complications.RESULTS Binary logistic regression analysis revealed that diabetes[odds ratio(OR)=3.088,95%confidence interval(CI):1.419-6.719],preoperative radiotherapy and chemotherapy(OR=6.822,95%CI:2.171-21.433),stoma type(OR=2.118,95%CI:1.151-3.898),Nutritional risk screening 2002 score(OR=2.034,95%CI:1.082-3.822)and prognostic nutritional index(OR=0.486,95%CI:0.254-0.927)were risk factors for early stoma complications after colorectal cancer surgery(P<0.05).On the basis of these results,a prediction model was constructed and the area under the re-ceiver operating characteristic curve was 0.740(95%CI:0.669-0.811).After internal validation, the area under the receiver operating characteristic curve of the validation group was 0.725(95%CI:0.631-0.820).The calibration curves for the modeling group and validation group are displayed. The predicted results have a good degree of overlap with the actual results.CONCLUSION A previous history of diabetes, preoperative radiotherapy and chemotherapy, stoma type,Nutritional risk screening 2002 score and prognostic nutritional index are risk factors for early stoma complications after colorectal cancer surgery. The nomogram prediction model constructed on the basis of the results of logistic regression analysis in this study can effectively predict the probability of early stomal complications after colorectal cancer surgery.
文摘BACKGROUND Unilateral biportal endoscopic(UBE)surgery has developed rapidly during the past decade.Continuous epidural space irrigation is generally considered the principal reason for cerebral complications following UBE surgery. We present a case of mental symptoms during the general anesthesia awakening period due to pneumocephalus.CASE SUMMARY A 70-year-old woman with lumbar disc herniation underwent UBE surgery stably under general anesthesia.Uncontrollable hypertension occurred immediately after transfer to the postoperative care unit, accompanied by increased heart rate and tachypnea.During the recovery process, the patient responded to external stimuli but was confused and unable to complete command actions.Neck stiff-ness and significantly increased muscle strength on the left side indicated the presence of de-cerebrate rigidity. An urgent brain computed tomography scan showed pneumocephalus compressing the brainstem. After receiving analgesia and sedation treatment, the patient was conscious three hours later and recovered rapidly. She was discharged on the fifth postoperative day and followed up for 3 months with no surgical or brain complications.CONCLUSION Cerebral complications emerging during the general anesthesia awakening period following UBE surgery are not entirely due to increased intracranial pressure.Pneumocephalus induced by dural injury may also be a potential cause.

文摘BACKGROUND Surgery is the gold standard for gallstone treatment.Nevertheless, the complications associated with the surgical procedure can exert diverse and adverse impacts on patients’health and quality of life to varying extents.Hence,it is essential to offer perioperative care to patients undergoing gallstone surgery.AIM To examine the impact of perioperative comprehensive nursing on pain intensity, complication rates, and patient comfort in individuals undergoing gallstone surgery.METHODS From February 2022 to February 2024,195 patients who underwent gallstone surgery at Sanmen People’s Hospital were selected and divided into two groups:A control group receiving routine nursing care(95 patients)and a research group receiving perioperative comprehensive nursing(100 patients).Key postoperative recovery indicators, including time to first postoperative anal exhaust, oral food intake, and ambulation, were observed, along with pain intensity(measured by the numeric rating scale),complication rate(bleeding, incision infection, recurrence),patient comfort(assessed using the visual analogue scale),and quality of life(measured by the World Health Organization Quality of Life-BREF).RESULTS The research group showed significantly shorter times to first postoperative anal exhaust, oral intake, and ambulation. Moreover, numeric rating scale pain scores in the research group were markedly lower post-nursing,and the total complication rate was significantly reduced compared to the control group.Furthermore, comfort levels improved considerably in the research group, and World Health Organization Quality of Life-BREF scores across the physical, psychological, social, and environmental domains were significantly higher compared to the control group following nursing care.CONCLUSION Perioperative comprehensive nursing effectively enhances postoperative recovery in patients undergoing gallstone surgery, reducing pain, lowering complications, and improving patient comfort and quality of life, which deserves clinical application.
文摘In this editorial,we discuss the article by Singh et al published in World Journal of Nephrology, stating the need for timely adjustments in inflammatory bowel disease(IBD)patients'long-term management plans.IBD is chronic and lifelong, with recurrence and remission cycles, including ulcerative colitis and Crohn's disease. It's exact etiology is unknown but likely multifactorial.Related to gut flora and immune issues.Besides intestinal symptoms,IBD can also affect various extrain-testinal manifestations such as those involving the skin, joints, eyes and urinary system. The anatomical proximity of urinary system waste disposal to that of the alimentary canal makes early detection and the differentiation of such symptoms very difficult.Various studies show that IBD and it's first-line drugs have nephro-toxicity,impacting the patients'life quality.Existing guidelines give very few references for kidney lesion monitoring.Singh et al's plan aims to improve treatment management for IBD patients with glomerular filtration rate decline, specifically those at risk. Most of IBD patients are young and they need lifelong therapy. So early therapy cessation, taking into account drug side effects, can be helpful.Artificial intelligence-driven diagnosis and treatment has a big potential for management improvements in IBD and other chronic diseases.

文摘This study evaluates the findings of Gu et al,who investigated the role of neutrophil gelatinase-associated lipocalin(NGAL)as a biomarker for predicting neuropsychiatric complications in acute ischemic stroke(AIS)patients. The results revealed that elevated serum NGAL levels at admission are associated with a higher risk of cognitive impairment, anxiety, and depressive symptoms at discharge. The study analyzed 150 AIS patients(mean age 65.4 years,58%male)using the Mini-Mental State Examination and the Hospital Anxiety and Depression Scale to assess neuropsychiatric outcomes.Multivariate analysis demonstrated that higher NGAL levels were independent predictors of cognitive impairment[odds ratio(OR)=1.42],anxiety(OR=1.28),and depression(OR=1.39).Notably,NGAL exhibited strong predictive power for cognitive impairment, with an area under the curve of 0.78.Despite these promising findings,NGAL’s clinical utility is limited by its non-specificity across various conditions.Nevertheless,NGAL levels could help identify AIS patients at risk for neuropsychiatric complications, enabling timely intervention and comprehensive neuropsychiatric evaluation. The study emphasizes the need for further research to validate NGAL’s predictive accuracy and specificity in diverse AIS populations and advocates for its integration with other diagnostic modalities to enhance clinical decision-making.
文摘BACKGROUND Gastric ulcer perforation is a critical condition that can lead to significant morbidity and mortality if not promptly addressed. It is often the result of chronic peptic ulcer disease, which is characterized by a breach in the gastric wall due to ulceration.Surgical intervention is essential for managing this life-threatening complication. However, the optimal surgical technique remains debatable among clinicians.Various methods have been employed, including simple closure, omental patch repair, and partial gastrectomy, each with distinct advantages and disadvantages.Understanding the comparative efficacy and postoperative outcomes of these techniques is crucial for improving patient care and surgical decision-making.This study addresses the need for a comprehensive analysis in this area.AIM To compare the efficacy and postoperative complications of different surgical methods for the treatment of gastric ulcer perforation.METHODS A retrospective analysis was conducted on 120 patients who underwent surgery for gastric ulcer perforation between September 2020 and June 2023.The patients were divided into three groups based on the surgical method:Simple closure, omental patch repair, and partial gastrectomy. The primary outcomes were the operative success rate and incidence of postoperative complications.Secondary outcomes included the length of hospital stay, recovery time, and long-term quality of life.RESULTS The operative success rates for simple closure, omental patch repair, and partial gastrectomy were 92.5%,95%,and 97.5%,respectively.Postoperative complications occurred in 20%,15%,and 17.5%of patients in each group, respectively. The partial gastrectomy group showed a significantly longer operative time(P<0.001)but the lowest rate of ulcer recurrence(2.5%,P<0.05).The omental patch repair group demonstrated the shortest hospital stay(mean 7.2 days,P<0.05)and fastest recovery time.CONCLUSION While all three surgical methods showed high success rates, omental patch repair demonstrated the best overall outcomes, with a balance of high efficacy, low complication rates, and shorter recovery time. However, the choice of the surgical method should be tailored to individual patient factors and the surgeon’s expertise.

文摘This study highlights the importance of identifying and addressing risk factors associated with wound complications following transtibial amputation in diabetic patients. These amputations, often necessitated by severe diabetic foot ulcers, carry significant risks of postoperative complications such as infection and delayed wound healing.Elevated hemoglobin A1c levels, indicative of poor glycemic control, and a history of kidney transplantation, due to required immunosuppressive therapy, are key factors influencing these outcomes. This paper emphasizes the need for enhanced glycemic management and personalized postoperative care, particularly for immunocompromised individuals,to minimize complications and improve patient prognosis.Future research should focus on prospective studies to validate targeted interventions and optimize care strategies, ultimately aiming to reduce the healthcare burden associated with diabetic foot complications.

文摘BACKGROUND: Laparoscopic cholecystectomy (LC) has become the 'gold standard' in treating benign gallbladder diseases. It is important to know the technical characteristics of LSC, and pay attention to perioperative bleeding and bile leak.

文摘The geologic conditions of superimposed basins in China are very complicated. The result shows that 95% of the discovered Ordovician hydrocarbon reservoirs are located in the predicted areas, which indicates the feasibility and reliability of the key factor matching T-BCMS model for hydrocarbon accumulation and enrichment.
文摘AIM:To evaluate the safety and efficacy of Densiron 68 heavy silicone oil (HSO) tamponade for complicated retinal detachment(RD)in Chinese eyes.METHODS:Twenty-one eyes of 21 patients with complicated RD were included in this retrospective study. All patients underwent pars plana vitrectomy with an internal tamponade using Densiron 68 HSO.Anatomical and functional results and complications were evaluated, including retinal status, visual acuity(VA),intraocular pressure(IOP),intraocular inflammation, lens opacity, and HSO emulsification.RESULTS:Allthepatients were followed up for 3mo to1y(5.8±1.16mo).Retinal reattachment was achieved in 19of 21 patients(90.5%).VA improved in 18 of 21 patients(85.7%),from 1.93 logMAR(±0.48)to 1.52 logMAR(±0.45)(P=0.001).Postoperative complications included early dispersion of HSO in 7 eyes(38.8%),cataract in 10 of 18phakic eyes(55.5%),moderate postoperative inflammation reaction in 10 eyes(47.6%),and elevated IOP in 5 eyes(23.8%),all of which were controlled by medication or by surgery.CONCLUSION:Highanatomical and functional success rates can be achieved with primary vitrectomy for complicated RD by using Densiron 68 HSO;however,it should not be ignored that Densiron 68 HSO can cause some complications in the eye.
