The two pathologies may influence each other. The aim was to study the correlation between glycemic control and blood pressure control and to determine the factors associated with blood pressure control. Methodology: This was a descriptive cross-sectional study with an analytical focus over 7 months. Patients were recruited as outpatients and all underwent ambulatory blood pressure measure, glycated hemoglobin and creatinine measurements, and assessment of compliance with treatment. Results: During this period 116 patients were collected. The predominance was female 69%. The mean age of the patients was 62 ± 7 years with a peak between 60 and 70 years. The average age of hypertension was 12 years and that of diabetes 6 1/2 years. The most frequently associated cardiovascular risk factor was a sedentary lifestyle (71.5%) after age. 57.8% of patients were not controlled at the office, with a predominance of systolic hypertension (58.2%). 61.6% of patients were controlled by ambulatory blood pressure measure, a rate of 47.8% of white coat hypertension. Glycemic control was observed in 42.2% of cases and 87% of patients had good renal function (glomerular filter rate ≥ 60 ml/mn). Therapeutic compliance was good in 53.4% of cases and dual therapy was the most used therapeutic modality 44.8% (52 patients) followed by triple therapy. The factors associated with poor blood pressure control were glycemic imbalance, non-compliance and monotherapy. Dual therapy had a protective effect. Conclusion: The association of hypertension and type 2 diabetes is frequent. The risk of occurrence increases with age. Ambulatory blood pressure measure is the best method to assess blood pressure control. Optimization of blood pressure control should also include optimization of glycemic control.展开更多

It constitutes a major public health problem, the leading chronic disease in the world. The objective was to determine the prevalence, treatment rate and control of hypertension. Methods: This was a cross-sectional and descriptive study which took place over a period of 6 months in a hospital environment and in the general population. Results: Of the 1000 participants, 637 had hypertension, giving a prevalence of 63.70% with a female predominance. Thirty-three percent (33%) were unaware of their high blood pressure. The age group 60 and more was the most represented (44%). A proportion of 33 and 23.20 were overweight and obese participants, respectively. Male subjects were more overweight than female, unlike obesity which was more common among female subjects. Sixty-two percent (62%) of hypertensives were treated, of whom 44% were non-compliant. The excessively high cost and consumption of medications as needed were the main factors in therapeutic non-compliance. Twenty-two percent (22%) of all hypertensive patients and 35% of treated hypertensive patients were controlled. Women were more treated but less observant and less controlled than men. Therapeutic coverage and combination therapy rates were lower in rural areas. Hypertensives who had a high level of education were better treated and controlled than those who had no level. Conclusion: High blood pressure remains a real public health problem in Mali. It is more common in people aged 60 and over and in females. One in three hypertensives were unaware of their hypertension. The majority received antihypertensive treatments, but only a minority of them had their hypertension controlled.展开更多

First of all, the IGC system model of the UCAV is built based on the three-channel independent design idea, which reduces the difficulties of designing the controller. Then, IGC control laws are designed using the trajectory linearization control (TLC). A nonlinear disturbance observer (NDO) is introduced to the IGC controller to reject various uncertainties, such as the aerodynamic parameter perturbation and the measurement error interference. The stability of the closed-loop system is proven by using the Lyapunov theorem. The performance of the proposed IGC design method is verified in a terminal attack mission of the suicide UCAV. Finally, simulation results demonstrate the superiority and effectiveness in the aspects of guidance accuracy and system robustness.展开更多

The desired pressure is determined by using the finite element method. A linear quadratic constant state tracking problem was considered over an infinite time interval. The optimal control law was derived by differentiating the Hamilton function with respect to system input. In order to verify the effectiveness of the proposed mathematical model and optimal control law, an experimental study on the pressure control of the soil chamber in shield tunneling was conducted in a laboratory. The experiment results show that soil pressure in the soil chamber in shield tunneling can be accurately controlled.展开更多

From Hamilton's principle nonlinear dynamic equations of the piezoelastic laminated cylindrical shell were derived. Based on which, the dynamic equations of a closed piezoelastic cylindrical shell under hydrostatic pressure are obtained. An analytical solution was presented for the case of vibration of a simply supported piezoelastic laminated cylindrical shell under hydrostatic pressure. Using veloctity feedback control, a model for active vibration control of the laminated cylindrical shell with piezoelastic sensor/actuator is established. Numerical results show that, the static deflection of the cylindrical shell can be changed when voltages with suitable value and direction are applied on the piezoelectric layers. For the dynamic response problem of the system, the larger the gain is, the more the vibration of the system is suppressed in the vicinity of the resonant zone. This presents a potential way to actively reduce the harmful effect of the resonance on the system and verify the feasibility of the active vibration control model.展开更多

In Nigeria, there is a very slow rise in contraceptive use prevalence, resulting in high fertility rate. Generally contraceptive is either used for birth spacing, or for the purpose of terminal fertility control to limit the number of children, and the choice of method for this purpose varies. Aims and Objectives: To determine the contraceptive methods of choice among clients seeking terminal fertility control, and their reasons for the choice of such methods. Materials and Methods: A cross-sectional study of clients attending the family planning clinic of Ladoke Akintola University of Technology Teaching hospital over a period of twenty-four months, between July, 2009 and June, 2011. All clients who had completed their family, and wanted a contraceptive method to limit their family size filled the semi-structured questionnaire after giving their informed consent. Clients’ own reasons for the choice of the particular contraceptive method were grouped, entered into SPSS work sheet, and analyzed using SPSS version 17. Result: There were five hundred and ninety-two new contraceptive method acceptors during the study period, of which 264 (44.6%) were for terminal fertility control. Progestogen-only injectable contraceptive was the method of choice for terminal fertility control by 145 (54.9%) of the clients, while 85 (32.2%) made intrauterine contraceptive device (copper-T) their method of choice, and the least chosen method was female surgical sterilization (2, 0.8%). Ease of administration, satisfaction with previous use, long duration of action, and husband’s preference were the reasons for the choice of the methods. Conclusion: There is awareness of terminal fertility control in Nigeria, especially in the southwestern region of the country, and this may be responsible for the decreasing prevalence of grandmultiparity in the region. However, reversible contraceptive method is the preferred option for this purpose.展开更多

The two species are rich in vitamins A, B1, B3, B8. Germination promotes the increase in their concentrations. To ensure preservation and increased bioavailability of the vitamins, it is necessary to apply a DIC of 30s to 400 kPa making it possible to support the cellular expansion and the decontamination of germinated seeds. Vitamin A content in germinated carob seed increased by 82.54%, which is not the case for fenugreek. A decrease in vitamin A concentration of 4 μg/100 g of dry matter. DIC increase vitamin B1, B3 and B8 in germinated fenugreek seeds. A slight decrease of B vitamins in germinated carob seed after treatment with DIC has been noticed. This loss can be explained by the low bioavailability caused mainly by the botanical structures of the seeds.展开更多

Advanced means insensitive to patient breathing activity as well as to ventilator settings while improved-shape intends in comparison to conventional square waveform for a progressive approaching towards physiological transpulmonary pressure and respiratory airflow waveforms. Such functional features along with the best ventilation control for the specific therapeutic requirements of each patient can be achieved through the implementation of both diagnostic and compensation procedures effectively carried out by the Advance Lung Ventilation System (ALVS) already successfully tested for square waveform as airways pressure excitation. Triangular and trapezoidal waveforms have been considered as airways pressure excitation. The results shows that the latter fits completely the requirements for a physiological pattern of endoalveolar pressure and respiratory airflow waveforms, while the former exhibits a lower physiological behaviour but it is anyhow periodically recommended for performing adequately the powerful diagnostic procedure.展开更多

In Togo, that affection constitutes a more and more pre-occupying cause of morbidity and mortality. This study is a prospective one which intended to identify the antihypertensive regimens prescribed and evaluate their effect on patients’ blood pressure (BP) control. Out of the 204 patients enrolled (mean: 55.01 ± 12.55 years;sex ratio: 1.3), 112/176 placed on antihypertensive therapy have controlled their BP (38.39% outpatients vs 61.61% inpatients). Related to the sex factor, we didn’t observe any significant difference in the BP control. Whereas, the mean median value of BP reduction of outpatients (30.00/15.00 mmHg) (p = 0.001) was half lower than that of inpatients (60.00/30.00 mmHg (p = 0.004)). Thirty five outpatients (81.40%) vs 64 inpatients (92.75%) were placed on combination therapy. The bitherapy was prescribed to 23 outpatients (53.49%) against 27 inpatients (39.13%) while the quadritherapy and more than 4 drugs combination were prescribed exclusively to inpatients (20.29%, n = 14). That quadritherapy induced a significant mean reduction of inpatients’ SBP compared to monotherapy (p = 0.043) and to bitherapy (p = 0.004). The favorite combinations were D + CCA, D + ACEI, D + CCA + ACEI and D + CCA + ACEI + CAAD of which the quadruple therapy showed a significant inpatients’ DBP control (p = 0.015) compared to D + CCA combination. The combinations including at least one diuretic induced a significant difference between outpatients (median value: 30.000/10.000 mmHg) (p < 0.001) and inpatients (median value: 60.000 mmHg/30 mmHg) (p < 0.001). The first-line molecules and fixe combinations prescribed in decreasing frequency were among others: hydrochlorothiazide + captopril, nicardipine, α methyldopa for outpatients;furosemide, nicardipine, captopril, α methyldopa, hydrochlorothiazide + captopril for inpatients. Diuretics, CCAs and ACEIs were the 3 favorite pharmacological groups for essential hypertension management in our African resource limited context. Combined to CAAD, they represented the best quadruple combination among inpatients having showed a significant difference in DBP control compared to D + CCA combination.展开更多

Due to anatomical challenges and a diverse bacterial flora in this area, surgical site infections (SSI) represent a common, debilitating and sometimes life-threatening complication. The INVIPS-Trial evaluates the role of Negative Pressure Wound Therapy (NPWT) on closed inguinal incisions in elective vascular surgery to prevent SSI and other wound complications. Methods: This randomized controlled trial (RCT) registered at ClinicalTrials.gov (Identifier: NCT01913132) compares the effects of a NPWT dressing (PICO, Smith & Nephew, UK) and the center’s standard wound dressing (Vitri Pad, ViTri Medical, Sweden) on postoperative wound complications, especially SSI. The study includes two distinct vascular procedures with different SSI risk profiles: endovascular aortic repair (EVAR) and open surgical approaches involving the common femoral artery (OPEN). Results: Four hundred ninety-five groin incisions in both treatment arms are anticipated to be included in the EVAR group and 147 inguinal incisions in both treatment arms in the OPEN group. Since a large percentage of inguinal vascular procedures in both groups but especially in the EVAR group are performed bilaterally, many patients can serve as their own control by randomly receiving NPWT on one and the standard dressing on the contralateral inguinal incision. Conclusions: This ongoing RCT attempts to elucidate the potential benefit of NPWT on closed inguinal incisions after different vascular procedures. Outcome and conclusions of this trial could have implications on postoperative wound care of patients in both vascular surgery and other surgical specialties.展开更多

The dynamic terminal sliding mode controller is formulated based on Lyapunov theory such that the existence of the sliding phase of the closed-loop control system can be guaranteed, chattering phenomenon caused by the switching control action can be eliminated, and high precision performance is realized. Moreover, by designing terminal equation, the output tracking error converges to zero in finite time, the reaching phase of DSMC is eliminated and global robustness is obtained. The simulation results for an inverted pendulum are given to demonstrate the properties of the proposed method.展开更多

文摘Water-assisted injection molding(WAIM),a newly developed fluid-assisted injection molding technology has drawn more and more attentions for the energy saving, short cooling circle time and high quality of products.Existing research for the process of WAIM has shown that the pressure control of the injecting water is mostly important for the WAIM.However, the proportional pressure control for the WAIM system is quite complex due to the existence of nonlinearities in the water hydraulic system. In order to achieve better pressure control performance of the injecting water to meet the requirements of the WAIM,the proportional pressure control of the WAIM system is investigated both numerically and experimentally.A newly designed water hydraulic system for WAIM is first modeled in AMEsim environment, the load characteristics and the nonlinearities of water hydraulic system are both considered, then the main factors affecting the injecting pressure and load flow rate are extensively studied.Meanwhile,an open-loop model-based compensation control strategy is employed to regulate the water injection pressure and a feedback proportional integrator controller is further adopted to achieve better control performance. In order to verify the AMEsim simulation results WAIM experiment for particular Acrylonitrile Butadiene Styrene(ABS) parts is implemented and the measured experimental data including injecting pressure and flow rate results are compared with the simulation. The good coincidence between experiment and simulation shows that the AMEsim model is accurate, and the tracking performance of the load pressure indicates that the proposed control strategy is effective for the proportional pressure control of the nonlinear WAIM system. The proposed proportional pressure control strategy and the conclusions drawn from simulation and experiment contribute to the application of water hydraulic proportional control and WAIM technology.

文摘A novel integrated guidance and control (IGC) design method is proposed to solve problems of low control accuracy for a suicide unmanned combat aerial vehicle (UCAV) in the terminal attack stage. Finally, simulation results demonstrate the superiority and effectiveness in the aspects of guidance accuracy and system robustness.

文摘The possibility to enhance the stability and robustness of electrohydraulic brake(EHB)systems is considered a subject of great importance in the automotive field. In such a context, the present study focuses on an actuator with a four-way sliding valve and a hydraulic cylinder.A 4-order nonlinear mathematical model is introduced accordingly.Through the linearization of the feedback law of the high order EHB model,a sliding mode control method is proposed for the hydraulic pressure. The hydraulic pressure tracking controls are simulated and analyzed by MATLAB/Simulink soft considering separately different conditions,i.e.,a sine wave,a square wave and a square wave with superimposed sine disturbance. The results show that the proposed strategy can track the target within 0.25 s,and the mean observed error is less than 1.2 bar. Moreover, with such a strategy, faster response and less overshoot are possible, which should be regarded as significant advantages.

文摘In-situ pressure-preserved coring(IPP-Coring)is considered to be the most reliable and efficient method for the identification of the scale of oil and gas resources.During IPP-Coring,because the rotation behavior of the pressure controller valve cover in different medium environments is unclear, interference between the valve cover and inner pipe may occur and negatively affect the IPP-Coring success rate.To address this issue,we conducted a series of indoor experiments employing a high-speed camera to gain greater insights into the valve cover rotation behavior in different medium environments,e.g.,air,water,and simulated drilling fluids. The results indicated that the variation in the valve cover rotation angle in the air and fluid environments can be described by a one-phase exponential decay function with a constant time parameter and by biphasic dose response function, respectively. The rotation behavior in the fluid environments exhibited distinct elastic and gravitational acceleration zones. In the fluid environments, the density clearly impacted the valve cover closing time and rotation behavior, whereas the effect of viscosity was very slight. This can be attributed to the negligible influence of the fluid viscosity on the drag coefficient found in this study;meanwhile, the density can increase the buoyancy and the time period during which the valve cover experienced a high drag coefficient.Considering these results, control schemes for the valve cover rotation behavior during IPP-Coring were proposed for different layers and geological conditions in which the different drilling fluids should be used,e.g.,the use of a high-density valve cover in high-pore pressure layers.

文摘A mathematical model of the soil pressure system in shield tunneling was proposed to optimize soil pressure control in the soil chamber, based on the constitutive relationship between strain and stress. The experiment results show that soil pressure in the soil chamber in shield tunneling can be accurately controlled.

文摘The control of the piezoelastic laminated cylindrical shell's vibration under hydrostatic pressure was discussed. This presents a potential way to actively reduce the harmful effect of the resonance on the system and verify the feasibility of the active vibration control model.

文摘Background: The prevalence of contraceptive use has increased worldwide due to the development and introduction of modern contraceptives and the establishment of organized family planning programs. However, reversible contraceptive method is the preferred option for this purpose.
文摘Fenugreek and carob seed were germinated in the dark for 4 days and 15 days respectively. This loss can be explained by the low bioavailability caused mainly by the botanical structures of the seeds.
文摘The present paper describes the theoretical treatment performed for the geometrical optimization of advanced and improved-shape waveforms as airways pressure excitation for controlled breathings in dual-controlled ventilation applied to anaesthetized or severe brain injured patients, the respiratory mechanics of which can be assumed linear. The results shows that the latter fits completely the requirements for a physiological pattern of endoalveolar pressure and respiratory airflow waveforms, while the former exhibits a lower physiological behaviour but it is anyhow periodically recommended for performing adequately the powerful diagnostic procedure.
文摘Noise pollution is one of the most significant harmful physical factors in the industrial and occupational environments.Due to the high costs of exposure to excessive noise;continuous sound evaluation, propose and implement noise control plans in occupational environments is necessary. Thus, the present study aimed to review environmental sound measurements, drawing of noise maps, and prioritizing the engineering noise control methods using the Analytic Hierarchy Process(AHP).This study was a descriptive-analytical study that aimed to assess occupational noises and present a control plan in the City Gas Stations(CGSs)of Kerman,Iran in 2021.The present study was done in two phases. In the first phase, six CGSs were investigated to measure and evaluate the noise. In addition, the noise map of a CGS was drawn using the Surfer software. Finally, the AHP was used in the second phase of the research to prioritize the control measures. In this phase, four criteria and ten alternatives were identified.According to first phase results, the sound pressure level(SPL)of the stations varied from 76 to 98 dBA.Besides, the majority of the studied stations had a sound level higher than 85 dBA(danger zone). The second phase of the study showed that out of the four evaluated criteria, the executability criterion had the highest impact and the cost criterion had the lowest impact on the selection of control measures with a weight of 0.587 and 0.052,respectively.Based on the results of prioritization of the alternatives, using a silenced regulator(weight of 0.223)and increasing the thickness of the tube(weight of 0.023)had the highest and lowest priorities among the alternatives, respectively. The use of engineering noise control methods such as using silenced regulators was the best way to control the noises of CGSs.Additionally;it is noteworthy that AHP is a practical method for prioritizing alternatives to achieve the most accurate decision-making.The results of AHP can be of great help to health and safety experts and managers in choosing the sound engineering control measures more precisely.
文摘High blood pressure (HBP) is a health problem world—wide. Combined to CAAD, they represented the best quadruple combination among inpatients having showed a significant difference in DBP control compared to D + CCA combination.

文摘This paper presents a novel non-singular fast terminal sliding mode control(NFTSMC)based on the deep flux weakening switching point tracking method in order to improve the control performance of permanent interior magnet synchronous motor(IPMSM)drive systems. The mathematical model of flux weakening(FW)control is established, and the deep flux weakening switching point is calculated accurately by analyzing the relationship between the torque curve and voltage decline curve.Next,a second-order NFTSMC is designed for the speed loop controller to ensure that the system converges to the equilibrium state in finite time. Then,an extended sliding mode disturbance observer(ESMDO)is designed to estimate the uncertainty of the system. Finally, compared with both the PI control and sliding mode control(SMC)by simulations and experiments with different working conditions, the method proposed has the merits of accelerating convergence, improving steady-state accuracy, and minimizing the current and torque pulsation.

文摘Background/Aims: Inguinal incisions are a common route of access in vascular surgery. Outcome and conclusions of this trial could have implications on postoperative wound care of patients in both vascular surgery and other surgical specialties.
文摘BACKGROUND Hypertension is a major risk factor for cardiovascular disease and stroke, and its prevalence is increasing worldwide.Health education interventions based on the health belief model(HBM)can improve the knowledge, attitudes, and behaviors of patients with hypertension and help them control their blood pressure.AIM To evaluate the effects of health education interventions based on the HBM in patients with hypertension in China.METHODS Between 2021 and 2023,140 patients with hypertension were randomly assigned to either the intervention or control group. The intervention group received health education based on the HBM,including lectures, brochures, videos, and counseling sessions, whereas the control group received routine care.Outcomes were measured at baseline, three months, and six months after the intervention and included blood pressure, medication adherence,self-efficacy,and perceived benefits, barriers, susceptibility, and severity.RESULTS The intervention group had significantly lower systolic blood pressure[mean difference(MD):-8.2 mmHg,P<0.001]and diastolic blood pressure(MD:-5.1 mmHg,P=0.002)compared to the control group at six months. The intervention group also had higher medication adherence(MD:1.8,P<0.001),self-efficacy(MD:12.4,P<0.001),perceived benefits(MD:3.2,P<0.001),lower perceived barriers(MD:-2.6,P=0.001),higher perceived susceptibility(MD:2.8,P=0.002),and higher perceived severity(MD:3.1,P<0.001)than the control group at six months.CONCLUSION Health education interventions based on the HBM effectively improve blood pressure control and health beliefs in patients with hypertension and should be implemented in clinical practice and community settings.

文摘A new dynamic terminal sliding mode control (DTSMC) technique is proposed for a class of single-input and single-output (SISO) uncertain nonlinear systems. The simulation results for an inverted pendulum are given to demonstrate the properties of the proposed method.
