International study of the Complex Stress Reaction Syndrome:Implications for transdiagnostic clinical practice

BACKGROUND The debate regarding diagnostic classification systems in psychiatry(categorial vs dimensional systems)has essential implications for the diagnosis,prevention and treatment of stress reactions.We previously found a unique pattern of stress reaction in a study executed during the coronavirus disease 2019 pandemic using large representative samples in two countries,and termed it the Complex Stress Reaction Syndrome(CSRS).AIM To investigate CSRS,Type A(psychiatric symptoms,spanning anxiety,depression,stress symptoms,and posttraumatic stress disorder(PTSD)),with or without long-coronavirus disease(COVID)residuals(CSRS,Type B,neuropsychiatric symptoms spanning cognitive deficits and fatigue,excluding systemic symptoms).Our two-tailed hypothesis was that CSRS is a condition related to an unrecognized type of stress reaction in daily life in the general population(Type A)or that it is related to the severe acute respiratory syndrome coronavirus 2 infection and its long-COVID residuals(Type B).METHODS 977 individuals in four continents(North America,Europe,Australia and the Middle East)completed the online study questionnaire in six languages using the Qualtrics platform.The study was managed by six teams in six countries that promoted the study on social media.The questionnaire assessed anxiety,depression,stress symptoms and PTSD(CSRS,Type A),cognitive deficits and fatigue(CSRS,Type B).The data were analyzed using Proportion Analyses,Multivariate Analysis of Co-Variance(MANCOVA),linear regression analyses and validated clinical cutoff points.RESULTS The results of the Proportion Analyses showed that the prevalence of 4 symptoms spanning anxiety,depression,stress symptoms,and PTSD was significantly higher than the most prevalent combinations of fewer symptoms across 4 continents,age groups,and gender.This supports the transdiagnostic argument embedded in the CSRS(Type A).The same pattern of results was found in infected/recovered individuals.The prevalence of the 4 psychiatric symptoms combination was significantly greater than that of 5 and 6 symptoms,when adding cognitive deficits and fatigue,respectively.MANCOVA showed a significant three-way interaction(age×gender×continent).Further analyses showed that the sources of this three-way interaction were threefold relating to two sub-populations at-risk:(1)Individuals that self-identified as non-binary gender scored significantly higher on all 4 psychiatric symptoms of the CSRS,Type A at young age groups(<50 years old)in North America compared to(self-identified)women and men located in the 4 continents studied,and to other ages across the adult life span;and(2)This pattern of results(CSRS,Type A)was found also in women at young ages(<40 years old)in North America who scored higher compared to men and women in other continents and other ages.Linear regression analyses confirmed the MANCOVA results.CONCLUSION These results show a combined mental health risk factor related to stress reactivity,suggesting that the CSRS is sensitive to populations at risk and may be applied to future identification of other vulnerable sub-populations.It also supports the transdiagnostic approach for more accurate prevention and treatment.Time will tell if such transdiagnostic syndromes will be part of the discussions on the next revisions of the traditional classification systems or whether the crisis in psychiatry further evolves.

Research on Mao Zedong’s Early Physical Education Thought and Practice

This paper explores Mao Zedong’s early views on physical education and practice,focusing on the“Six-Section Exercise”he created.The exercise,which integrates elements from traditional health-preserving techniques,gymnastics,boxing,and military training,was designed to promote balanced physical development,be practical and effective,and require less time per session.The paper argues that Mao Zedong’s views on physical education were grounded in the belief that it could effectively transform individuals and contribute to the creation of a strong sports nation.The paper also discusses the historical context and social trends behind the creation of the“Six-Section Exercise”,including the New Culture Movement and debates between Chinese and Western,static and dynamic,and new and old approaches to physical education.The paper concludes by suggesting that Mao Zedong’s early sports thinking has significant academic value and can provide guidance for the construction of a healthy China today.

Neoadjuvant chemoradiotherapy for locally advanced rectal cancer:The debate continues

Rectal carcinoma represents the 30% of all colorectal cancers, with about 40000 new cases/years. In the past two decades, the management of rectal cancer has made important progress, highlighting the main role of a multimodality strategy approach, combining surgery, radiation therapy and chemotherapy. Nowadays, surgery remains the primary treatment and neoadjuvant chemoradiotherapy, based on fluoropyrimidine(5-FU) continuous infusion, is considered the standard in locally advanced rectal carcinoma. The aim is to reduce the incidence of local recurrence and to perform a conservative surgery. To improve these purposes different drugs combination have been tested in the neo-adjuvant setting. At American Society of Clinical Oncology 2014 an important abstract was presented focusing on the role of adding oxaliplatin to concomitant treatment, in patients with locally advanced rectal carcinoma. Rodel et al reported on the CAO/ARO/AIO-04 randomized phase Ⅲ trial that compared standard treatment with 5-FU and radiation therapy, to oxaliplatin plus 5-FU inassociation with radiation therapy. The addition of oxaliplatin to the neo-adjuvant treatment has been shown to improve disease-free survival from 71.2% to 75.9%(P = 0.03). This editorial was planned to clarify the optimal treatment in patients with locally advanced rectal cancer, considering the results from CAO/ARO/AIO-04 study.

Remarkable carbon dioxide catalytic capture (CDCC) leading to solid-form carbon material via a new CVD integrated process (CVD-IP): An alternative route for CO_2 sequestration

Through our newly-developed ＂chemical vapor deposition integrated process （ISVD-IP）＇＂ using carbon OlOXlae （t..u2） as me raw matenal and only carbon source introduced, CO2 could be catalytically activated and converted to a new solid-form product, i.e., carbon nanotubes （CO2-derived） at a quite high yield （the single-pass carbon yield in the solid-form carbon-product produced from CO2 catalytic capture and conversion was more than 30% at a single-pass carbon-base）. For comparison, when only pure carbon dioxide was introduced using the conventional CVD method without integrated process, no solid-form carbon-material product could be formed. In the addition of saturated steam at room temperature in the feed for CVD, there were much more end-opening carbon nano-tubes produced, at a slightly higher carbon yield. These inspiring works opened a remarkable and alternative new approach for carbon dioxide catalytic capture to solid-form product, comparing with that of CO2 sequestration （CCS） or CO2 mineralization （solidification）, etc. As a result, there was much less body volume and almost no greenhouse effect for this solid-form carbon-material than those of primitive carbon dioxide.

Methylprednisolone for acute spinal cord injury: an increasingly philosophical debate

Following publication of NASCIS Ⅱ, methylprednisolone sodium succinate（MPSS） was hailed as a breakthrough for patients with acute spinal cord injury（SCI）. MPSS use for SCI has since become very controversial and it is our opinion that additional evidence is unlikely to break the stalemate amongst clinicians. Patient opinion has the potential to break this stalemate and we review our recent findings which reported that spinal cord injured patients informed of the risks and benefits of MPSS reported a preference for MPSS administration. We discuss the implications of the current MPSS debate on translational research and seek to address some misconceptions which have evolved. As science has failed to resolve the MPSS debate we argue that the debate is an increasingly philosophical one. We question whether SCI might be viewed as a serious condition like cancer where serious side effects of therapeutics are tolerated even when benefits may be small. We also draw attention to the similarity between the side effects of MPSS and isotretinoin which is prescribed for the cosmetic disorder acne vulgaris. Ultimately we question how patient autonomy should be weighed in the context of current SCI guidelines and MPSS＇s status as a historical standard of care.

Economic Impact of GM Hysteria on EU Feed Market

A growing world population coupled with changing diet is projected to increase demand for food production by 70% by 2050. Helping farmers lose less of their crops will be a key factor in promoting food security. In addition to pesticides GM crops will continue to be a vital tool in the diverse range of technologies that can maintain and improve living standards for the people of the world. The EU is dependent for 65% on imports of protein-rich feedstuffs for which there are no substitutes in the short term. The EU livestock sector uses imported soybean, soybean meal and maize by-products as animal feed. Without an adequate supply of these feed ingredients, the EU’s livestock production will loose competitiveness. However, demands from the EU differ to those from third countries with respect to the GM varieties grown, and what are authorised for import into the EU. The risk that supplies of soya products and maize by-products could be affected by the low-level presence of non-EU approved GM material has not been fully resolved as the EU has allowed just a 0.1% tolerance for this plus 0.05% tolerance for measuring uncertainty. This could cause supply problems for the animal feed industry, and ultimately supply of food to consumers. The importance of the EU market and EU requirements for the major soybean and maize exporter countries is declining over time and it is becoming increasingly difficult and costly to maintain a non-GM supply chain in the EU. Food companies and supermarkets will struggle to stay GM-free. EU member states dependent on imports will be forced to deal with more GM presence in their chain. Market forces are forcing governments to authorize products more efficiently, develop tolerance policies or tolerate unapproved varieties in their imports.
