Objective To study the changing characteristics and trend of medical income structure in the government-run hospitals of traditional Chinese medicine(TCM),evaluate the effects of relevant reform measures,and to put fo...Objective To study the changing characteristics and trend of medical income structure in the government-run hospitals of traditional Chinese medicine(TCM),evaluate the effects of relevant reform measures,and to put forward corresponding suggestions for further optimizing their income structure.Methods The data related to the average medical income of government-run hospitals of TCM from 2012 to 2021 were sorted out.Then,descriptive analysis method was used to analyze the changes of related indicators.Besides,structural change method was applied to investigate the changes of outpatient income and inpatient income.Results and Conclusion From 2012 to 2021,the growth of medical income in government-run hospitals of TCM tended to be stable,and the proportion of medical service income increased from 22.62%(2012)to 29.38%(2021),but the average annual growth rate was only 0.68%.The main items that caused the change of outpatient income structure were medicine revenue,laboratory tests,diagnosis and treatment,and the cumulative contribution rate was 89.15%.The main items that caused the change of inpatient income structure were medicine revenue,sanitary materials,and auxiliary examinations income,with a cumulative contribution rate of 80.04%.However,the contribution rate of registration,diagnosis,treatment,surgery and nursing income reflecting the value of medical personnel’s technical labor was relatively small.The medical income structure of government-run hospitals of TCM underwent great changes and gradually became reasonable,but the medical service income increased slowly,and not all indicators achieved the expectations.To promote the sustainable development of public hospitals of TCM and enable them to provide high-quality and efficient TCM medical and health services,it is necessary to further improve the relevant policy mechanism.展开更多

First, TCM should strive for a consensus of TCM theory which focuses on the guiding principles of “diagnosis and treatment based on an overall analysis of the illness and the patient’s condition"and “the opposite principle of five internal organs".Among these principles, “keeping relative balance of three internal systems"is a valuable guidance of the diagnosis and treatment theory. Secondly, macroscopic should be integrated with microscopic, totality with parts,phenomenology with materialism.A dialectical law should be applied to guide analysis. Although traditional Chinese medicine(TCM)has been a part of the Malaya and the Malaysia people’s method of maintaining health and well-being for many centuries,it was never been enrolled in the public health-care sy...Although traditional Chinese medicine(TCM)has been a part of the Malaya and the Malaysia people’s method of maintaining health and well-being for many centuries,it was never been enrolled in the public health-care system.However,the rising cost of Western drugs and the increasing number of people frequenting TCM has driven the government to search for cheaper options and to look into its safe practice.In 1992,the government mandated all TCM herbal products to be registered with the National Pharmaceutical Control Bureau.All TCM products sold in Malaysia are now Good Manufacturing Practice certified.The government has also established a Traditional and Complementary Medicine Division within the Ministry of Health.Between 2008 and 2014,a traditional and complementary medicine unit was set up in major hospitals in all states.TCM is one of the main services offered by the unit.In 2016,the government enacted the Traditional and Complementary Medicine Act to regulate the TCM practice.Subsequently,a 10-vear blueprint has been drawn to support the development of all traditional and complementary medicines in Malaysia.This article aims to provide a brief overview on the regulatory development of TCM in Malaysia.展开更多

Hospital-based health technology assessment(HB-HTA)is highly valuable in the application of hospital medical technology,cost control,improvement of medical quality,and protection of medical safety,and its use is incre...Hospital-based health technology assessment(HB-HTA)is highly valuable in the application of hospital medical technology,cost control,improvement of medical quality,and protection of medical safety,and its use is increasing daily globally.However,in China,HB-HTA is still in its infancy,and it is yet to be adopted in traditional Chinese medicine(TCM).Therefore,this article introduces the application and development of HB-HTA,as well as discusses the current situation of TCM and related future development strategies to provide ideas and references for the development of HB-HTA in TCM.We conclude and recommend that all major stakeholders,including the state,health departments,and hospital leaders,support and promote the exploration and development of HB-HTA in TCM hospitals and integrative medicine hospitals.We also suggest the utilization of existing talents and introduction of other necessary talents to promote better and faster organizational development,as well as suggest the formation of a national and internationally recognized HB-HTA guidance manual and toolkit for TCM hospitals based on both the national HTA guidelines and the evaluation of TCM hospitals pilot projects.Moreover,we recommend that HB-HTA pilot study should be executed with Chinese patent medicine as the starting point to form the implementation process and framework in the TCM arena.展开更多
BACKGROUND Nonalcoholic fatty liver disease(NAFLD)affects more than one-quarter of the global population.Due to the lack of approved chemical agents,many patients seek treatment from traditional Chinese medicine(TCM)f...BACKGROUND Nonalcoholic fatty liver disease(NAFLD)affects more than one-quarter of the global population.Due to the lack of approved chemical agents,many patients seek treatment from traditional Chinese medicine(TCM)formulas.A variety of systematic reviews have been published regarding the effectiveness and safety of TCM formulas for NAFLD.AIM To critically appraise available systematic reviews and sort out the high-quality evidence on TCM formulas for the management of NAFLD.METHODS Seven databases were systematically searched from their inception to 28 February 2020.The search terms included“non-alcoholic fatty liver disease,”“Chinese medicines,”“systematic review,”and their synonyms.Systematic reviews involving TCM formulas alone or in combination with conventional medications were included.The methodological quality and risk of bias of eligible systematic reviews were evaluated by using A Measure Tool to Assess Systematic Reviews 2(AMSTAR 2)and Risk of Bias in Systematic Review(ROBIS).The quality of outcomes was assessed by the Grading of Recommendations Assessment,Development and Evaluation(GRADE)system.RESULTS Seven systematic reviews were ultimately included.All systematic reviews were conducted based on randomized controlled trials and published in the last decade.According to the AMSTAR 2 tool,one systematic review was judged as having a moderate confidence level,whereas the other studies were rated as having a low or extremely low level of confidence.The ROBIS tool showed that the included systematic reviews all had a high risk of bias due to insufficient consideration of identified concerns.According to the GRADE system,only two outcomes were determined as high quality;namely,TCM formulas with the HuoXueHuaYu principle were better than conventional medications in ultrasound improvement,and TCM formulas were superior to antioxidants in alanine aminotransferase normalization.Other outcomes were downgraded to lower levels,mainly because of heterogeneity among studies,not meeting optimal information sample size,and inclusion of excessive numbers of small sample studies.Nevertheless,the evidence quality of extracted outcomes should be further downgraded when applying to clinical practice due to indirectness.CONCLUSION The quality of available systematic reviews was not satisfactory.Researchers should avoid repeatedly conducting systematic reviews in this area and focus on designing rigorous randomized controlled trials to support TCM formula applications.展开更多

Method...Objective: This review introduced the origin, developing course and modern application of Chinese food therapy (CFT) in traditional Chinese medicine to enrich our understanding of relevant concepts and ways. Methods: PubMed, EMBASE, CBM, CNKI, VIP and Wanfang Databases were searched for papers on a range of terms relating to CFT. 41 references were finally selected for this review. Results: The development of CFT is comprised of five important periods in Chinese history. They axe as following: the infancy period represented by the idea of homogeny of medicine and food; theoretical development period represented by Inner Canon of Yellow Emperor, Shennong 1' Classic of Materia Medica and Treatise on Febrile and Miscellaneous Diseases; specialized volumes on CFT represented by Dietotherapy Worth a Thousand Gold; representative monographs on CFT represented by Dietetic Materia Medica and Principles of Correct Diet and the perfect period represented by integration of ancient food therapy practice and modern nutriology. Conclusion: CFT harbors a long and profound history, making great contribution to people's health maintenance and illness prevention. With the further development of integrative medicine, CFT may play a more important role in clinics and family health care based on integration of syndrome differentiation and disease differentiation.展开更多

文摘This article examines the overall characteristics of historical medical exchanges between China and foreign countries,analyzing their contribution to the development of medicines in the world.It presents a summary of historical experiences of the development of medicines through mutual exchanges.It also provides an objective analysis of the effects of the eastward dissemination of Western medicine on traditional Chinese medicine.The history of their exchanges can be used as a mirror for each to learn from the other’s strengths to offset their own weakness,and achieve the coordinated and balanced development of both traditional Chinese medicine and Western medicine.Such balanced development would enable the world’s medicinal systems to thrive and protect the health of people worldwide.
文摘Although the traditional Chinese medicine (TCM) has developed a profound theory system and a fame of outstanding advantage in curative effect, it seems that the TCM is not quite adaptable to the deveiopment and requirement of the modern society.A principle of “Adopting the advanced part of Western medicine to serve the TCM" should be adhered to for the traditional Chinese medicine. First, TCM should strive for a consensus of TCM theory which focuses on the guiding principles of “diagnosis and treatment based on an overall analysis of the illness and the patient’s condition"and “the opposite principle of five internal organs".Among these principles, “keeping relative balance of three internal systems"is a valuable guidance of the diagnosis and treatment theory. Secondly, macroscopic should be integrated with microscopic, totality with parts,phenomenology with materialism.A dialectical law should be applied to guide analysis. In treatment, adjustment and balance are basic principles.It is necessary not only prepare the medicine in advance accoding to the predicted disease of the year,but also adopt methods to restrict disease in advance.Basides, TCM dose type should be ameliorated in order to meet the needs of society.

文摘In this coronavirus disease 2019(COVID-19)epidemic prevention and control battle,Chinese medicine has demonstrated the unique advantages of Chinese medicine and has made important contributions to global epidemic prevention and control.At the same time,the epidemic is also a concentrated test of the medical education system of Chinese medicine.There are insufficient public health and integrated Chinese and western medicine talents in the discipline development of Chinese medicine colleges,and the ability of Chinese medicine to prevent and treat infectious diseases needs to be improved.Infectious disease discipline system is not perfect and many other problems.This article puts forward new thoughts and suggestions on the problems and challenges existing in the discipline development of traditional Chinese medicine schools,and provides countermeasures and references for the next discipline development of traditional Chinese medicine schools in the“post-epidemic era”.
文摘Malignant tumor has become a major threat affecting human health,and is one of the main causes of human death.Recent studies have shown that many traditional Chinese medicines(TCM)have good anti-tumor activity,which may improve the therapeutic effect of routine treatment and quality of life with lower toxicity.However,the efficacy of TCM alone for the treatment of tumors is limited.Metal ions are essential substances for maintaining normal physiological activities.This article summarized the multiple mechanisms in which metal ions are involved in the prevention and treatment of tumors in TCM.
文摘Although traditional Chinese medicine(TCM)has been a part of the Malaya and the Malaysia people’s method of maintaining health and well-being for many centuries,it was never been enrolled in the public health-care system.However,the rising cost of Western drugs and the increasing number of people frequenting TCM has driven the government to search for cheaper options and to look into its safe practice.In 1992,the government mandated all TCM herbal products to be registered with the National Pharmaceutical Control Bureau.All TCM products sold in Malaysia are now Good Manufacturing Practice certified.The government has also established a Traditional and Complementary Medicine Division within the Ministry of Health.Between 2008 and 2014,a traditional and complementary medicine unit was set up in major hospitals in all states.TCM is one of the main services offered by the unit.In 2016,the government enacted the Traditional and Complementary Medicine Act to regulate the TCM practice.Subsequently,a 10-vear blueprint has been drawn to support the development of all traditional and complementary medicines in Malaysia.This article aims to provide a brief overview on the regulatory development of TCM in Malaysia.
文摘The increasing number of TCM practitioners and herbal suppliers,both of which accelerated by the formation of various TCM institutions,substantiated the creation of regional traditional Chinese medicine(TCM)associations in post-World War II Malaya.In response to the restrictions and levies imposed by the British colonial government,these regional associations united and formed a national organisation now known as the Federation of Chinese Physicians and Medicine Dealers Association of Malaya(FCPMDAM).The current designated TCM practitioner body,the Malaysian Chinese Medical Association(MCMA)were also originally established to nurture local talents in the face of import restrictions.Owing to difference with MCMA,a separate association named the Federation of Chinese Physicians and Acupuncturists Associations of Malaysia(FCPAAM)was setup in 2003 to absorb self-studied and patrimonial-educated TCM practitioners.
文摘Traditional Chinese Medicine(TCM)in British Malaya developed concurrently with the influx of Chinese immigrants.To cater for tins growing community,Chinese medical halls which sell Chinese herbs were established in major townships.Consultation and various TCM treatments were also offered by contract TCM practitioners in some of these medical halls.As the needs for TCM services continued to grow,dedicated TCM institutions were set up subsequently.The establishment of these institutions marked the beginning of professional TCM services in the history of Malaysia.
文摘Chinese Medicine was introduced to the Malay Peninsula during the colonial era circa 18th to 19th century.The British imported Chinese immigrants mainly for tin mining.The early Chinese settlers had brought with them the knowledge of Traditional Chinese Medicine(TCM)to Malaya.As time goes by,TCM becomes one of the popular traditional medicines that are accepted by other ethnicities.TCM has thus contributed to the welfare of Malaysians.In 2016,a Traditional and Complementary Medicine(T and CM)Act was passed by the Malaysia Parliament.TCM is one of the T and CM practices that fall under the regulation.TCM services are now offered in Malaysia’s public hospitals in addition to private practices.This article aims to provide a brief overview on the development of TCM in the precolonial and postindependent Malaysia.

Then, it studied agricultural intellectual property resources such as geographical indications, trademarks, patents, new plant varieties, biological genetic resources, and traditional knowledge related to C. morifolium . Finally, it summarized poverty-stricken areas ecologically suitable for planting C. morifolium , analyzed the GAP certification of C. morifolium , pollution-free agricultural product certification, green food certification, and organic food certification, and discussed the strategies for C. morifolium intellectual property protection and authenticity protection in the context of TCM development strategy and rural revitalization strategy.

文摘Hospital-based health technology assessment(HB-HTA)is highly valuable in the application of hospital medical technology,cost control,improvement of medical quality,and protection of medical safety,and its use is increasing daily globally.However,in China,HB-HTA is still in its infancy,and it is yet to be adopted in traditional Chinese medicine(TCM).Therefore,this article introduces the application and development of HB-HTA,as well as discusses the current situation of TCM and related future development strategies to provide ideas and references for the development of HB-HTA in TCM.We conclude and recommend that all major stakeholders,including the state,health departments,and hospital leaders,support and promote the exploration and development of HB-HTA in TCM hospitals and integrative medicine hospitals.We also suggest the utilization of existing talents and introduction of other necessary talents to promote better and faster organizational development,as well as suggest the formation of a national and internationally recognized HB-HTA guidance manual and toolkit for TCM hospitals based on both the national HTA guidelines and the evaluation of TCM hospitals pilot projects.Moreover,we recommend that HB-HTA pilot study should be executed with Chinese patent medicine as the starting point to form the implementation process and framework in the TCM arena.

文摘BACKGROUND Nonalcoholic fatty liver disease(NAFLD)affects more than one-quarter of the global population.Due to the lack of approved chemical agents,many patients seek treatment from traditional Chinese medicine(TCM)formulas.A variety of systematic reviews have been published regarding the effectiveness and safety of TCM formulas for NAFLD.AIM To critically appraise available systematic reviews and sort out the high-quality evidence on TCM formulas for the management of NAFLD.METHODS Seven databases were systematically searched from their inception to 28 February 2020.The search terms included“non-alcoholic fatty liver disease,”“Chinese medicines,”“systematic review,”and their synonyms.Systematic reviews involving TCM formulas alone or in combination with conventional medications were included.The methodological quality and risk of bias of eligible systematic reviews were evaluated by using A Measure Tool to Assess Systematic Reviews 2(AMSTAR 2)and Risk of Bias in Systematic Review(ROBIS).The quality of outcomes was assessed by the Grading of Recommendations Assessment,Development and Evaluation(GRADE)system.RESULTS Seven systematic reviews were ultimately included.All systematic reviews were conducted based on randomized controlled trials and published in the last decade.According to the AMSTAR 2 tool,one systematic review was judged as having a moderate confidence level,whereas the other studies were rated as having a low or extremely low level of confidence.The ROBIS tool showed that the included systematic reviews all had a high risk of bias due to insufficient consideration of identified concerns.According to the GRADE system,only two outcomes were determined as high quality;namely,TCM formulas with the HuoXueHuaYu principle were better than conventional medications in ultrasound improvement,and TCM formulas were superior to antioxidants in alanine aminotransferase normalization.Other outcomes were downgraded to lower levels,mainly because of heterogeneity among studies,not meeting optimal information sample size,and inclusion of excessive numbers of small sample studies.Nevertheless,the evidence quality of extracted outcomes should be further downgraded when applying to clinical practice due to indirectness.CONCLUSION The quality of available systematic reviews was not satisfactory.Researchers should avoid repeatedly conducting systematic reviews in this area and focus on designing rigorous randomized controlled trials to support TCM formula applications.

文摘Traditional Chinese medicine(TCM)chiropractic massage is a kind of orthopedic therapy that corrects the abnormal anatomical position of bone or joint and corrects the dislocation of joint by manipulation so that the bone is soft and the Qi and blood are reconciled(Qi:The intangible,high-mobility nutritive substance that maintains vital activities).Manipulation treatment is the first of the four major orthopedic diseases.At the same time,TCM chiropractic massage is also an essential part of TCM.TCM orthopedic chiropractic practice originated from the struggle between humanity and nature,sprouted from the Warring States period to the two Han Dynasty,formed in the Sui,Tang,and Song Dynasties,matured in the Ming and Qing Dynasties,and flourished in the modern era.In more than 2,000 years of development,many practical techniques have been produced,which have profoundly influenced the inheritance and development of modern techniques,making the TCM orthopedic chiropractic massage in the contemporary era form the characteristics of“tendon and bone”and“point-line-face combination”.At present,the development of TCM chiropractic massage is facing the great challenges of modern medicine and modern advanced science and technology such as American chiropractic and the limitations of its development.Therefore,in the future,the development direction of Chinese medicine orthopedic chiropractic massage should be“hybrid to treat”.In the face of modern medicine,we should take its essence to its dross,eclectic,organically integrate its advantages into itself,enrich its theoretical framework,and strengthen the construction of disciplines and departments to enhance its competitiveness.It is believed that Chinese medicine orthopedic chiropractic massage can have better development in the future and can make more contributions to the health of the world’s people.

文摘The discovery of highly effective medicine"Qingfei Paidu Decoction"for COVID-19 is a new era gift of traditional Chinese medicine to the world.The traditional Chinese medicine Scutellaria baicalensis Georgi,as a medicinal plant in the family of Labiatae,is the dominant variety of genuine medicinal material in North China.It is the leading traditional Chinese medicine in the advantageous area of agricultural products with Chinese characteristics in Shangdang of Changzhi City,and it is one of the 21 herbal medicines in the new national prescription"Qingfei Paidu Decoction"for the rapid and effective treatment of COVID-19.This paper introduces S.baicalensis Georgi and its traditional Chinese medicine GAP base,ecologically suitable planting county and medicinal value in poor areas,and studies the intellectual property resources related to S.baicalensis Georgi,such as traditional knowledge,biological genetic resources,patents,new plant varieties,geographical indications,and trademarks.This paper mainly discusses the inheritance essence,innovation and high-quality development strategy of S.baicalensis Georgi under the rural revitalization strategy and traditional Chinese medicine development strategy from the aspects of intellectual property rights,important agricultural cultural heritage,advantageous areas of characteristic agricultural products and so on.
文摘Objective: This review introduced the origin, developing course and modern application of Chinese food therapy (CFT) in traditional Chinese medicine to enrich our understanding of relevant concepts and ways. Methods: PubMed, EMBASE, CBM, CNKI, VIP and Wanfang Databases were searched for papers on a range of terms relating to CFT. 41 references were finally selected for this review. Results: The development of CFT is comprised of five important periods in Chinese history. They axe as following: the infancy period represented by the idea of homogeny of medicine and food; theoretical development period represented by Inner Canon of Yellow Emperor, Shennong 1' Classic of Materia Medica and Treatise on Febrile and Miscellaneous Diseases; specialized volumes on CFT represented by Dietotherapy Worth a Thousand Gold; representative monographs on CFT represented by Dietetic Materia Medica and Principles of Correct Diet and the perfect period represented by integration of ancient food therapy practice and modern nutriology. Conclusion: CFT harbors a long and profound history, making great contribution to people's health maintenance and illness prevention. With the further development of integrative medicine, CFT may play a more important role in clinics and family health care based on integration of syndrome differentiation and disease differentiation.
