Kikuchi-Fujimoto Disease: A Case of Lymphadenitis Following COVID-19 Infection

Introduction: Kikuchi-Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a rare disease of uncertain origin proposed to be secondary to viral or autoimmune causes. It presents as unilateral cervical lymphadenopathy and fever, typically resolving within a few months, however, it mimics malignant lymphoma, leading to extensive work-up fearing malignancy. Case History: A 33-year-old female with a history of Sjögren’s syndrome, rheumatoid arthritis (not on immunosuppressive therapy) and recent COVID-19 infection two months ago presented with episodes of flu-like symptoms;fever (101˚F), chills, and myalgias for two months. Upon evaluation, she had leukopenia (WBC 1.8 k/uL), neutropenia (1.0 k/uL), elevated CRP (134 mg/L), and CT neck demonstrated multiple enlarged cervical lymph nodes with necrosis. The patient was started on Cefdinir and doxycycline for a possible atypical infection given neutropenic fevers and the patient continued to have fevers. A biopsy showed focal necrosis with no evidence of malignancy, suggesting KFD that was likely triggered by a recent COVID infection. She was treated with Prednisone and naproxen, which led to an improvement in symptoms and recovery of her pancytopenia. Discussion: This case is unique since the patient’s recent infection with COVID-19 may have triggered the clinical manifestations of KFD. There have been a few case reports of children who were diagnosed with KFD after a COVID-19 infection and adults diagnosed with KFD following COVID vaccinations;however, this is the first case report involving a young adult in her thirties who was diagnosed with KFD two months after COVID-19. 展开更多

Background: The COVID-19 pandemic emerged unexpectedly, impacting millions worldwide and, though waning in many regions, remains a significant concern. This study focused on understanding COVID-19’s clinical presentation, disease progression, and outcomes among hospitalized pediatric patients. Additionally, it sought to investigate the impact of concurrent chronic medical conditions on disease severity. Methods: Conducted retrospectively between September 2020 and December 2021, this study observed pediatric patients (below 18 years) hospitalized with confirmed COVID-19 at an Irbid, Jordan tertiary healthcare center. Patients meeting inclusion criteria, including a positive nasal swab PCR test and hospitalization need, were analyzed. Epidemiological and clinical data were collected and segregated into two groups (A and B) based on the presence of chronic conditions. Comparative analysis encompassed presentation, disease trajectory, and outcomes between these groups. Results: Among 451 tested positive patients, 112 were included in the study. Significant differences emerged between the two pediatric groups hospitalized for COVID-19. Those with concurrent chronic illnesses exhibited heightened symptom severity—fever, dyspnea, fatigue—and a higher incidence of abnormal chest radiographs, often requiring intensive care and experiencing elevated mortality rates. Conclusion: This study underscores the criticality of prioritizing COVID-19 management strategies for pediatric patients with pre-existing chronic medical conditions. The findings highlight the increased vulnerability and poorer outcomes experienced by this subgroup, emphasizing the necessity for tailored interventions and focused care approaches. 展开更多

As the pandemic unfolded, it became evident that certain groups of individuals were at an elevated risk of experiencing severe disease outcomes. Among these high-risk groups, individuals with pre-existing cardiac conditions emerged as particularly vulnerable. Objective: This study aimed to investigate the relationship between the length of stay, mortality, and costs of COVID-19 patients with and without a history of cardiac disease. Design: This retrospective study was conducted in Jam Hospital in Tehran, Iran, from March 21, 2021, to March 21, 2022. All patients with laboratory-confirmed COVID-19 who were hospitalized during this period were included. Results: A total of 500 COVID-19 patients were hospitalized, with 31.6% having a history of cardiac disease and 68.4% without any cardiac disease. Patients with cardiac disease were significantly older (median [range] age, 69.35 [37 - 94] years) compared to non-cardiac patients (54.95 [13 - 97] years) (p Conclusion: Patients with cardiac disease who are hospitalized with COVID-19 have a higher mortality rate, longer hospital stays, greater disease severity, ICU admission, and higher costs. 新型冠状病毒疫苗接种对维持性血液透析合并COVID-19患者的价值

目的探讨接种新型冠状病毒疫苗(新冠疫苗)对维持性血液透析(MHD)患者新型冠状病毒感染(COVID-19)严重程度的影响。方法回顾性分析2022年12月至2023年1月安徽医科大学附属阜阳人民医院肾内科收治的166例终末期肾脏病行MHD合并COVID-19患... 目的探讨接种新型冠状病毒疫苗(新冠疫苗)对维持性血液透析(MHD)患者新型冠状病毒感染(COVID-19)严重程度的影响。方法回顾性分析2022年12月至2023年1月安徽医科大学附属阜阳人民医院肾内科收治的166例终末期肾脏病行MHD合并COVID-19患者的临床资料。依据疫苗接种情况分为未接种组(接种0针)及接种组(接种≥2针)。比较两组患者临床特征及短期预后,并应用logistic回归模型分析患者住院及重症风险增加的独立危险因素。结果166例MHD合并COVID-19患者中,未接种组91例(54.8%),接种组75例(45.2%)。未接种组年龄与吸烟比例高于接种组(P<0.05)。感染新型冠状病毒肺炎后,患者临床表现多样,发热(73.1%)、乏力(65.6%)、食欲减退(57.5%)最常见。接种组使用激素、中心吸氧、住院、重症比例明显减少,治愈患者比例显著增加(P<0.05)。接种新冠疫苗可减少MHD患者住院(OR=0.364,95%CI:0.147~0.901)及重症(OR=0.182,95%CI:0.037~0.901)发生风险。合并糖尿病、冠状动脉粥样硬化性心脏病(冠心病)是MHD合并COVID-19患者住院及重症风险增加的独立危险因素,年龄是其重症风险增加的独立危险因素。结论终末期肾脏病接受MHD合并COVID-19患者临床表现多样,合并糖尿病、冠心病患者住院及重症风险均增加,年龄增加患者重症风险增加,接种新冠疫苗可减少住院及重症风险。 展开更多

目的探讨血细胞比值及炎症指标对自身炎症性疾病(AIDs)合并新型冠状病毒肺炎(COVID-19)严重程度的预测价值。方法148例AIDs合并COVID-19患者根据COVID-19严重程度分为轻症组100例和重症组48例。比较2组患者感染后24~72 h内的红细胞沉降... 目的探讨血细胞比值及炎症指标对自身炎症性疾病(AIDs)合并新型冠状病毒肺炎(COVID-19)严重程度的预测价值。方法148例AIDs合并COVID-19患者根据COVID-19严重程度分为轻症组100例和重症组48例。比较2组患者感染后24~72 h内的红细胞沉降率(ESR)、C反应蛋白(CRP)、系统免疫炎症指数(SII)和白细胞-C反应蛋白比值(LeCR)等指标差异;采用Logistic回归分析AIDs合并COVID-19严重程度的危险因素;受试者工作特征(ROC)曲线评估各指标对AIDs合并COVID-19严重程度的预测价值。结果重症组ESR、CRP、血小板计数(PLT)、中性粒细胞-淋巴细胞比值(NLR)、血小板-淋巴细胞比值(PLR)、C反应蛋白-淋巴细胞比值(CLR)、C反应蛋白-白蛋白比值(CAR)、SII和血小板-白蛋白比值(PAR)高于轻症组,平均血小板体积(MPV)、淋巴细胞计数(LYM)、白蛋白(Alb)和LeCR低于轻症组(P<0.05)。Logistic回归分析显示,ESR升高是AIDs合并COVID-19严重程度的独立危险因素,LeCR和SII升高是保护因素。ROC曲线分析显示,LeCR对AIDs合并重症COVID-19的预测效能优于ESR和SII。结论ESR、SII和LeCR是预测AIDs并发COVID-19疾病严重程度的有效指标。 展开更多

核酸检测Ct值在COVID-19重症风险预测模型构建的应用价值研究

目的验证荧光PCR测量的循环阈值(Ct值)对COVID-19患者重症风险评估的应用价值,并结合其它生化指标构建一个预测模型。方法通过对山东大学附属威海市立医院236例COVID-19住院患者的回顾性分析(2022年12月至2023年5月),收集病毒核酸检测C... 目的验证荧光PCR测量的循环阈值(Ct值)对COVID-19患者重症风险评估的应用价值,并结合其它生化指标构建一个预测模型。方法通过对山东大学附属威海市立医院236例COVID-19住院患者的回顾性分析(2022年12月至2023年5月),收集病毒核酸检测Ct值及常规检验指标。利用Logistic回归分析筛选影响重症风险因素,并构建预测模型。采用受试者工作特征曲线(ROC)和临床决策曲线(DCA)评估模型性能,并量化Ct值的贡献度。结果年龄、白细胞计数、血小板、血红蛋白升高与重症风险正相关;较高的首次核酸检测阳性Ct值与重症风险负相关。COVID-19重症肺炎风险预测模型提示,年龄较大、PLT和WBC水平较高,以及Ct值和HGB较低的患者,重症化的风险显著增加。模型拟合度良好,AIC、C-index、R2和霍斯默检验的P值分别为126.00、0.90、0.51和0.53。相较仅含临床指标的模型,含Ct值的模型的ROC下面积(AUC)值在训练集和验证集分别从0.84和0.89提升至0.93和0.97,灵敏度从0.71和0.78增加为0.86和1.00,特异度从0.82和0.87上升到0.87和0.92。DCA结果验证了组合模型的临床应用价值。结论Ct值是评估COVID-19患者重症风险的重要指标,本研究所构建的预测模型为患者的早期诊治提供了创新且有效的工具。 展开更多

目的探讨新型冠状病毒感染(COVID-19)型急性呼吸窘迫综合征(ARDS)与非COVID-19型ARDS患者炎症反应和细胞免疫的差异性。方法选取2022年1月至2023年12月郑州市第一人民医院收治的COVID-19型ARDS患者120例纳入观察组(中度89例,重度31例),... 目的探讨新型冠状病毒感染(COVID-19)型急性呼吸窘迫综合征(ARDS)与非COVID-19型ARDS患者炎症反应和细胞免疫的差异性。方法选取2022年1月至2023年12月郑州市第一人民医院收治的COVID-19型ARDS患者120例纳入观察组(中度89例,重度31例),根据1∶1配对原则,另选取同期其他肺部致病菌导致ARDS患者120例纳入对照组,同期健康体检者120例纳入健康组。比较三组受检者及不同病情程度COVID-19型ARDS患者血清炎性相关因子[白介素-6(IL-6)、白介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]及细胞免疫相关因子[T细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、自然杀伤细胞(NK细胞)]水平,采用Spearman分析炎性及细胞免疫相关因子与COVID-19感染及COVID-19型ARDS患者病情严重程度的相关性,采用偏回归分析COVID-19型患者ARDS发生的影响因素。结果观察组患者血清IL-6、IL-4、TNF-α、CRP水平[(47.52±6.25)ng/L、(21.53±3.59)ng/L、(1.55±0.36)ng/L、(20.69±3.81)mg/L]明显高于对照组[(15.83±2.67)ng/L、(13.68±2.46)ng/L、(1.27±0.30)ng/L、(12.35±2.67)mg/L],且对照组明显高于健康组[(3.12±0.79)ng/L、(2.76±0.54)ng/L、(0.88±0.25)ng/L、(3.98±1.12)mg/L],差异均有统计学意义(P<0.05);观察组患者的CD3^(+)、CD4^(+)、CD8^(+)、NK细胞水平[(59.03±3.03)%、(19.56±1.04)%、(27.81±3.97)%、(22.59±3.40)%]明显低于对照组[(64.42±3.26)%、(28.10±1.91)%、(40.11±4.05)%、(35.31±3.97)%],且对照组明显低于健康组[(70.35±4.18)%、(43.38±3.06)%、(50.88±4.67)%、(40.64±4.06)%],差异均有统计学意义(P<0.05);COVID-19型ARDS重度患者的血清IL-6、IL-4、TNF-α、CRP水平[(66.58±6.10)ng/L、(30.66±5.47)ng/L、(1.92±0.45)ng/L、(28.96±3.70)mg/L]明显高于中度患者[(40.88±4.62)ng/L、(18.35±3.07)ng/L、(1.42±0.33)ng/L、(17.81±2.93)mg/L],而CD3^(+)、CD4^(+)、CD8^(+)、NK细胞水平[(52.54±4.07)%、(15.20±1.09)%、(22.13±1.81)%、(18.03±1.46)%]明显低于中度患者[(61.29±3.62)%、(21.08±1.35)%、(29.79±2.02)%、(24.18±2.25)%],差异均有统计学意义(P<0.05);Spearman分析结果显示,IL-6(r=0.675、0.609)、IL-4(r=0.632、0.614)、TNF-α(r=0.585、0.566)、CRP(r=0.619、0.587)水平与COVID-19感染及COVID-19型ARDS患者病情严重程度均呈正相关(P<0.05),CD3^(+)(r=-0.539、-0.505)、CD4^(+)(r=-0.592、-0.554)、CD8^(+)(r=-0.588、-0.570)、NK细胞(r=-0.601、-0.592)水平与COVID-19感染及COVID-19型ARDS患者病情严重程度均呈负相关(P<0.05);偏回归分析结果显示,血清IL-6、IL-4、TNF-α、CRP、CD3^(+)、CD4^(+)、CD8^(+)、NK细胞水平均是COVID-19型患者发生ARDS的危险因素(P<0.05)。结论炎症及细胞免疫相关因子表达在COVID-19型及非COVID-19型ARDS患者中存在差异性,且与COVID-19感染及COVID-19型ARDS病情严重程度均具有一定相关性,可辅助临床识别COVID-19感染,评估病情严重程度。 展开更多

BACKGROUND Chronic liver disease(CLD)was associated with adverse clinical outcomes among people with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection.AIM To determine the effects of SARS-CoV-2 infection on the incidence and treatment strategy of hepatocellular carcinoma(HCC)among patients with CLD.METHODS A retrospective,territory-wide cohort of CLD patients was identified from an electronic health database in Hong Kong.Patients with confirmed SARS-CoV-2 infection[coronavirus disease 2019(COVID-19)+CLD]between January 1,2020 and October 25,2022 were identified and matched 1:1 by propensity-score with those without(COVID-19-CLD).Each patient was followed up until death,outcome event,or November 15,2022.Primary outcome was incidence of HCC.Secondary outcomes included all-cause mortality,adverse hepatic outcomes,and different treatment strategies to HCC(curative,non-curative treatment,and palliative care).Analyses were further stratified by acute(within 20 d)and post-acute(21 d or beyond)phases of SARS-CoV-2 infection.Incidence rate ratios(IRRs)were estimated by Poisson regression models.RESULTS Of 193589 CLD patients(>95%non-cirrhotic)in the cohort,55163 patients with COVID-19+CLD and 55163 patients with COVID-19-CLD were included after 1:1 propensity-score matching.Upon 249-d median follow-up,COVID-19+CLD was not associated with increased risk of incident HCC(IRR:1.19,95%CI:0.99-1.42,P=0.06),but higher risks of receiving palliative care for HCC(IRR:1.60,95%CI:1.46-1.75,P<0.001),compared to COVID-19-CLD.In both acute and post-acute phases of infection,COVID-19+CLD were associated with increased risks of allcause mortality(acute:IRR:7.06,95%CI:5.78-8.63,P<0.001;post-acute:IRR:1.24,95%CI:1.14-1.36,P<0.001)and adverse hepatic outcomes(acute:IRR:1.98,95%CI:1.79-2.18,P<0.001;post-acute:IRR:1.24,95%CI:1.13-1.35,P<0.001),compared to COVID-19-CLD.CONCLUSION Although CLD patients with SARS-CoV-2 infection were not associated with increased risk of HCC,they were more likely to receive palliative treatment than those without.The detrimental effects of SARS-CoV-2 infection persisted in post-acute phase. 展开更多

目的探讨可溶性生长刺激表达基因2蛋白(sST2)对合并新型冠状病毒感染(COVID-19)的终末期肾病(ESRD)患者预后的预测价值。方法回顾性分析合并COVID-19的ESRD患者的临床资料。采用单因素和多因素logistic回归探讨ESRD患者合并重型和危重型... 目的探讨可溶性生长刺激表达基因2蛋白(sST2)对合并新型冠状病毒感染(COVID-19)的终末期肾病(ESRD)患者预后的预测价值。方法回顾性分析合并COVID-19的ESRD患者的临床资料。采用单因素和多因素logistic回归探讨ESRD患者合并重型和危重型COVID-19及死亡的危险因素。分析细胞因子对患者预后的预测价值。结果共纳入100例合并COVID-19的ESRD患者,其中COVID-19普通型患者60例,重型患者22例,危重型患者18例。多因素logistic回归显示,入院脉搏氧饱和度、白蛋白、冠心病和连续性肾脏替代治疗是合并重型和危重型COVID-19的独立危险因素。冠心病、白蛋白、乳酸脱氢酶和机械通气是合并COVID-19的ESRD患者死亡的独立危险因素。入院时的sST2预测ESRD患者合并重型和危重型COVID-19的ROC曲线下面积(AUC)为0.706(95%CI:0.563~0.848,P=0.009),预测死亡的AUC为0.837(95%CI:0.729~0.944,P=0.007),白细胞介素-6(IL-6)和sST2联合预测死亡的AUC为0.923(95%CI:0.850~0.996,P=0.001)。结论sST2可作为预测合并COVID-19的ESRD患者预后的生物学标记物。 展开更多

目的 探讨中性粒细胞与淋巴细胞比值(NLR)联合三酰甘油-葡萄糖指数(TyG指数)与新冠病毒感染(COVID-19)组(n=56)和重度病例组(n=31),比较两组患者的一般情况、慢性病情况、血常规指标、生化指标、NLR和TyG指数。根据NLR和TyG指数的中位... 目的 探讨中性粒细胞与淋巴细胞比值(NLR)联合三酰甘油-葡萄糖指数(TyG指数)与新冠病毒感染(COVID-19)组(n=56)和重度病例组(n=31),比较两组患者的一般情况、慢性病情况、血常规指标、生化指标、NLR和TyG指数。根据NLR和TyG指数的中位数分组后,比较各组之间的结局、呼吸支持方式和病情严重程度。通过Logistics回归探索影响COVID-19严重程度的危险因素。通过受试者工作特征曲线分析NLR、TyG指数和二者联合后预测COVID-19进展为重度的价值。结果 重度病例组的NLR和TyG指数明显高于轻度病例组(NLR:P<0.001,TyG:P=0.028)。根据NLR和TyG指数中位数将研究对象分为4组后,高NLR,高TyG组的COVID-19患者病情更加严重(P<0.001),更容易使用呼吸机辅助呼吸(P=0.037)。在调整多种混杂因素之后,NLR仍是影响COVID-19严重程度的独立影响因素(P=0.001,OR=1.164,95%CI 1.066~1.271)。NLR和TyG指数预测新冠感染严重程度的曲线下面积(AUC)分别为0.759和0.643,两者联合预测的AUC高于单独预测(AUC=0.777)。结论 NLR、TyG指数与新冠感染严重程度关系密切,有望作为发展至重度病例的预测因子,两者联合后的预测价值更高,可能有助于早期制定个体化的治疗方案。 展开更多

Methods: The research literature from 2020 to 2022 was searched in the CNKI database, and CiteSpace software was used for visual analysis. Results: The papers on the prevention and treatment of COVID-19 by traditional Chinese medicine changed from cases, overviews, reports, and efficacy studies to more in-depth mechanism research, theoretical exploration, and social impact analysis, and finally formed a theory-clinical-society Influence-institutional change and other multi-dimensional achievement systems. Conclusion: Analyzing the changing trends of TCM hotspots in the prevention and treatment of COVID-19 can fully understand the important value of TCM, take the coordination of TCM and Western medicine as an important means to deal with public health security incidents, and promote the exploration of the potential efficacy of TCM, so as to enhance the role of TCM in Applications in social stability, emergency security, clinical practice, etc. 展开更多

Aim: To detect risk and preventive factors associated with the Omicron variant infection in university students, a combination of a web-based survey and multivariate logistic regression analysis was introduced as the front-line initiatives by the school health practitioners. Design: Questionnaire survey. Methods: The school-wide web-based questionnaire survey was conducted among our university students as a part of the annual health check-up in April, 2023. The positive outcome was confined to the first symptomatic COVID-19 onset during the Omicron variant outbreak. Results: In this self-administered survey, risk or protective associations were merely estimated statistically in university students (n = 5406). In measured factors, karaoke and club/group activities could maintain the statistical significance in adjusted odds ratios (ORs) as relative risk factors, and science course, measles/ rubella (MR) vaccination, and COVID-19 vaccination remained as relative protective factors in adjusted OR analyses. Club/group activities with member gathering and karaoke sing-along sessions in university students may frequently have WHO’s three Cs. These risk factors are still important topics for the infection control of COVID-19 in university students. Together with some recent reports from other researchers, the significant protective role of MR vaccine in our survey warrants further clinical investigation. If the breakthrough infection continuously constitutes the majority of infection, real data in test-negative case-control or web-based questionnaire design continue to be important for statistical analysis to determine the minimal requirement of our strategies which may be equivalent to or replace COVID-19 vaccines. 展开更多

Firstly, the properties o... The outbreak of COVID-19 in 2019 resulted in numerous infections and deaths. In order to better study the transmission of COVID-19, this article adopts an improved fractional-order SIR model. Firstly, the properties of the model are studied, including the feasible domain and bounded solutions of the system. Secondly, the stability of the system is discussed, among other things. Then, the GMMP method is introduced to obtain numerical solutions for the COVID-19 system and combined with the improved MH-NMSS-PSO parameter estimation method to fit the real data of Delhi, India from April 1, 2020 to June 30, 2020. The results show that the fitting effect is quite ideal. Finally, long-term predictions were made on the number of infections. We accurately estimate that the peak number of infections in Delhi, India, can reach around 2.1 million. This paper also compares the fitting performance of the integer-order COVID-19 model and the fractional-order COVID-19 model using the real data from Delhi. The results indicate that the fractional-order model with different orders, as we proposed, performs the best. 展开更多

目的目前尚不清楚新型冠状病毒感染(coronavirus disease 2019,COVID-19)疫情是否影响气象因素与流行性感冒(流感)的关联分析,本研究旨在探讨此问题及其影响特征,为今后精确评估气象因素对流感的风险效应提供依据。方法对厦门市气象因... 目的目前尚不清楚新型冠状病毒感染(coronavirus disease 2019,COVID-19)疫情是否影响气象因素与流行性感冒(流感)的关联分析,本研究旨在探讨此问题及其影响特征,为今后精确评估气象因素对流感的风险效应提供依据。方法对厦门市气象因素和流感发病的每日数据分为三部分:全年期(2010年1月1日—2021年12月31日)、非COVID-19疫情期(2010年1月1日—2020年1月21日)和COVID-19疫情期(2020年1月22日—2021年12月31日),采用R 4.2.1软件通过分布滞后非线性模型(distributed lag nonlinear model,DLNM)进行气象因素与流感的关联分析。结果非COVID-19疫情期低(<42%)和高(>85%)相对湿度(relative humidity,RH)是流感的危险因素,高RH风险累计效应随滞后时间的增加而增加,随RH的增加而先增加后减弱,RH达到93%滞后14 d时最显著(RR=1.41)。COVID-19疫情期RHU<75%对流感的风险累计效应随RH的减少和滞后时间的增加而增加,RH为35%滞后14 d时最显著(RR=1.86×10^(6))。非COVID-19疫情期降水量(precipitation,PRE)处于25~75 mm对流感的风险累计效应先增加后减少,随滞后时间的增加而增加,滞后14 d,PRE达到40 mm时最显著(RR=1.49)。COVID-19疫情期PRE对流感的风险累计效应随PRE的增加而增加。COVID-19疫情期流感发病数的大幅下降严重影响气象因素与流感发病关联分析,致研究结果与全年期和非COVID-19疫情期差别较大;对全年期的研究结果影响较小。结论COVID-19疫情封控政策导致了厦门市COVID-19疫情期流感数大幅下降,从而显著影响了气象因素与流感发病关联分析客观性和科学性,但对长期多年度的分析结果影响有限。此外,极端天气例如RHU、PRE等对流感发病关联的分析也产生一定影响。 展开更多
