Another 70 students waiting to undergo the colloquium were interviewed as a control group. Two parallel series of drawings depicting personal stress situations were collected and, after evaluation by three expert judges in “double-blind” conditions, led to establishing five forms of development of the graphic language from the first to second drawing, defined as follows: a) distension, with a reduction in stress indicators and increase in comfort indicators;b) explicit representation of conflict resolution;c) appearance of new elements, with persisting conflict;d) increase in stress indicators;e) invariant or equivalent repetition of the stress drawing. Results showed that students who underwent the clinical colloquium (experimental group) changed their stress drawings mainly in the direction of distension and conflict resolution, while the control group had more invariant or equivalent repetition of the stress situation (?= 62.77;p < 0.001). The events depicted in the stress drawings were divided into three categories: a) limited short-term stressful situations due to mainly external agents;b) externally induced stressful situations with intense emotional resonance;c) wide-ranging long-lasting stressful situations with great involvement of the self. The participants were divided into: a) very anxious subjects;b) averagely anxious subjects;c) not very anxious subjects, on the basis of their scores on two questionnaires: the MPI (1959) and the STAI (1970). There is a correspondence between the subjects’ level of conflict and extension of the depicted stress. This confirms the usefulness of the Stress Drawing Recall Technique in psychological counselling.展开更多

Although considerable strides have been made in enhancing English handwritten character recognitionthrough various techniques, deciphering Arabic handwritten characters is particularly intricate. This complexityarises from the diverse array of writing styles among individuals, coupled with the various shapes that a singlecharacter can take when positioned differently within document images, rendering the task more perplexing. Inthis study, a novel segmentation method for Arabic handwritten scripts is suggested. This work aims to locatethe local minima of the vertical and diagonal word image densities to precisely identify the segmentation pointsbetween the cursive letters. The proposed method starts with pre-processing the word image without affectingits main features, then calculates the directions pixel density of the word image by scanning it vertically and fromangles 30° to 90° to count the pixel density fromall directions and address the problem of overlapping letters, whichis a commonly attitude in writing Arabic texts by many people. Local minima and thresholds are also determinedto identify the ideal segmentation area. The proposed technique is tested on samples obtained fromtwo datasets: Aself-curated image dataset and the IFN/ENIT dataset. The results demonstrate that the proposed method achievesa significant improvement in the proportions of cursive segmentation of 92.96% on our dataset, as well as 89.37%on the IFN/ENIT dataset.展开更多

A hallmark of PD is the accumulation of alpha-synuclein, a presynaptic neuronal protein that aggregates to form Lewy bodies, leading to neuronal dysfunction and cell death. The study of alpha-synuclein and its pathological forms is crucial for understanding the etiology of PD and developing effective diagnostic and therapeutic strategies. Analytical techniques play a pivotal role in elucidating the structure, function, and aggregation mechanisms of alpha-synuclein. Biochemical methods such as Western blotting and enzyme-linked immunosorbent assay (ELISA) are employed to detect and quantify alpha-synuclein in biological samples, offering insights into its expression levels and post-translational modifications. Imaging techniques like immunohistochemistry and positron emission tomography (PET) allow for the visualization of alpha-synuclein aggregates in tissue samples and in vivo, respectively, facilitating the study of its spatial distribution and progression in PD Spectroscopic methods, including nuclear magnetic resonance (NMR) spectroscopy and mass spectrometry, provide detailed structural information on alpha-synuclein and its isoforms, aiding in the identification of conformational changes associated with aggregation. Emerging techniques such as cryo-electron microscopy (Cryo-EM) and single-molecule fluorescence enable high-resolution structural analysis and real-time monitoring of alpha-synuclein aggregation dynamics, respectively. The application of these analytical techniques has significantly advanced our understanding of the pathophysiological role of alpha-synuclein in PD. They have contributed to the identification of potential biomarkers for early diagnosis and the evaluation of therapeutic interventions targeting alpha-synuclein aggregation. Despite technical limitations and challenges in clinical translation, ongoing advancements in analytical methodologies hold promise for improving the diagnosis, monitoring, and treatment of Parkinson’s disease through a deeper understanding of alpha-synuclein pathology.展开更多

DGE represents impaired gastric motility without significant mechanical obstruction and is associated with an increased length of hospital stay, increased healthcare costs, and a high readmission rate. We reviewed published studies on various technical modifications to reduce the incidence of DGE. Data sources: Studies were identified by searching Pub Med for relevant articles published up to December 2022. The following search terms were used: “pancreaticoduodenectomy”, “pancreaticojejunostomy”, “pancreaticogastrostomy”, “gastric emptying”, “gastroparesis” and “postoperative complications”. The search was limited to English publications. Additional articles were identified by a manual search of references from key articles. Results: In recent years, various surgical procedures and techniques have been explored to reduce the incidence of DGE. Pyloric resection, Billroth II reconstruction, Braun's enteroenterostomy, and antecolic reconstruction may be associated with a decreased incidence of DGE, but more high-powered studies are needed in the future. Neither laparoscopic nor robotic surgery has demonstrated superiority in preventing DGE, and the use of staplers is controversial regarding whether they can reduce the incidence of DGE. Conclusions: Despite many innovations in surgical techniques, there is no surgical procedure that is superior to others to reduce DGE. Further larger prospective randomized studies are needed.展开更多

This confirms the usefulness of the Stress Drawing Recall Technique in psychological counselling.
文摘The development of intestinal anastomosis techniques, including hand suturing, stapling, and compression anastomoses, has been a significant advancement in surgical practice. These methods aim to prevent leakage and minimize tissue fibrosis, which can lead to stricture formation. The healing process involves various phases:hemostasis and inflammation, proliferation, and remodeling.Mechanical staplers and sutures can cause inflammation and fibrosis due to the release of profibrotic chemokines.Compression anastomosis devices, including those made of nickel-titanium alloy, offer a minimally invasive option for various surgical challenges and have shown safety and efficacy. However, despite advancements, anastomotic techniques are evaluated based on leakage risk, with complications being a primary concern.Newer devices like Magnamosis use magnetic rings for compression anastomosis, demonstrating greater strength and patency compared to stapling.Magnetic technology is also being explored for other medical treatments. While there are promising results, particularly in animal models, the realworld application in humans is limited, and further research is needed to assess their safety and practicality.

文摘BACKGROUND Postoperative delirium, particularly prevalent in elderly patients after abdominal cancer surgery, presents significant challenges in clinical management.AIM To develop a synthetic minority oversampling technique(SMOTE)-based model for predicting postoperative delirium in elderly abdominal cancer patients.METHODS In this retrospective cohort study,we analyzed data from 611 elderly patients who underwent abdominal malignant tumor surgery at our hospital between September 2020 and October 2022.The incidence of postoperative delirium was recorded for 7 d post-surgery.Patients were divided into delirium and non-delirium groups based on the occurrence of postoperative delirium or not.A multivariate logistic regression model was used to identify risk factors and develop a predictive model for postoperative delirium. The SMOTE technique was applied to enhance the model by oversampling the delirium cases. The model’s predictive accuracy was then validated.RESULTS In our study involving 611 elderly patients with abdominal malignant tumors, multivariate logistic regression analysis identified significant risk factors for postoperative delirium. These included the Charlson comorbidity index,American Society of Anesthesiologists classification, history of cerebrovascular disease, surgical duration, perioperative blood transfusion, and postoperative pain score. The incidence rate of postoperative delirium in our study was 22.91%.The original predictive model(P1)exhibited an area under the receiver operating characteristic curve of 0.862.In comparison, the SMOTE-based logistic early warning model(P2),which utilized the SMOTE oversampling algorithm, showed a slightly lower but comparable area under the curve of 0.856,suggesting no significant difference in performance between the two predictive approaches.CONCLUSION This study confirms that the SMOTE-enhanced predictive model for postoperative delirium in elderly abdominal tumor patients shows performance equivalent to that of traditional methods, effectively addressing data imbalance.

文摘Recognizing handwritten characters remains a critical and formidable challenge within the realm of computervision. The results demonstrate that the proposed method achievesa significant improvement in the proportions of cursive segmentation of 92.96% on our dataset, as well as 89.37%on the IFN/ENIT dataset.

文摘Parkinson’s disease (PD) is a progressive neurodegenerative disorder characterized by motor symptoms such as tremors, rigidity, and bradykinesia, as well as non-motor symptoms including cognitive impairment and mood disorders. Despite technical limitations and challenges in clinical translation, ongoing advancements in analytical methodologies hold promise for improving the diagnosis, monitoring, and treatment of Parkinson’s disease through a deeper understanding of alpha-synuclein pathology.

文摘A crystal plasticity finite element model was developed for the drawing deformation of pure copper micro wire, based on rate-dependent crystal plasticity theory. The impact of wire diameter compression ratio on the micro-mechanical deformation behavior during the wire drawing process was investigated. Results indicate that the internal deformation and slip of the drawn wire are unevenly distributed, forming distinct slip and non-slip zones.Additionally, horizontal strain concentration bands develop within the drawn wire.As the wire diameter compression ratio increases, the strength of the slip systems and the extent of slip zones inside the deformation zone also increase. However, the fluctuating stress state, induced by contact pressure and frictional stress, results in a rough and uneven wire surface and diminishes the stability of the drawing process.

文摘In this study,a numerical analysis was conducted on the ductile fracture of a 2-mm diameter Mg-1Zn-0.5Mn-0.5Sr-0.1Ca alloy wire during drawing. The hexagonally close-packed crystal structure of Mg alloys causes asymmetric fracture behavior, especially in the compression region. The aim of this study is to develop a comprehensive damage model for Mg alloy wire that accurately predicts ductile fracture, with a focus on the compression region.A novel experimental method was introduced to measure the ductile fracture of Mg alloy wires under different stress states. The wire drawing process was simulated using the Generalized Incremental Stress-State dependent damage(GISSMO)Model and the Semi-Analytical Model for Polymers(SAMP)model. The damage model's prediction and the experimental results were found to be in excellent agreement, especially in determining crack initiation.Computational analysis established a safe zone diagram for die angle and reduction ratio, and experimental validation confirmed the feasibility of this approach. The proposed damage model can provide a practical and reliable analysis for optimizing the drawing process of Mg alloy wire.

文摘Background: Delayed gastric emptying(DGE) is one of the most common complications after pancreaticoduodenectomy(PD). Further larger prospective randomized studies are needed.
文摘Procidentia is one of complications of pregnancy and childbirth. The challenge of treatment is to preserve fertility if needed and to find effective methods with rapid learning curve. Our technique—Isam technique—fulfills these needs with a short hospital stay, very low risk of complications and relapse, a rapid learning curve as well, and no need for long contraceptive methods after operation.
文摘Congenital aortic stenosis(cAS)frequently requires intervention during the neonatal or infantile period. However, surgical repair is challenging because of the narrow surgical space. We performed bicuspidization using the open-sleeve technique for cAS with a unicuspid aortic valve in two patients.Postoperatively, the patients were doing well without reintervention for the aortic valve for 8 and 6 years, respectively.Their aortic annular diameter increased along with somatic growth.Bicuspidization for neonates or infancy can be performed safely using the open-sleeve technique as its midterm results have been satisfactory.
