Although effective school-based alcohol prevention programs do exist, the overall efficacy of these programs has been compromised by implementation failure. The CLIMATE Schools: Alcohol Module was developed to overcome some of the obstacles to high fidelity program implementation. This paper details this development of the CLIMATE Schools: Alcohol Module. The development involved two stages, both of which were considered essential. The first stage, involved reviewing the literature to ensure the program was based on the most effecttive pedagogy and health promotion practice and the second stage involved collaborating with teachers, students and specialists in the area of alcohol and other drugs, to ensure these goals were realised. The final CLIMATE Schools: Alcohol Module consists of computer-driven harm minimisation program which is based on a social influence approach. The program consists of six lessons, each with two components. The first component involves students completing an interactive computer-based program, with the second consisting of a variety of individual, small group and class-based activities. This program was developed to provide an innovative new platform for the delivery of drug education and has proven to be both feasible and effective in the school environment. The success of this program is considered to be testament to this collaborative development approach. 展开更多

By the determination of the metal residues in the samples, the calculated actual daily exposure and concentration of the metal Pd, Ir, Rh, Ru, Mo, Ni, Cr, V, Cu, Mn, Fe and Zn that were less than the permitted daily exposures (PDE) and the limited concentration permitted in the EMEA guideline on the specification limits for residues of metal catalysts or metal reagents [1] . The metal residues can de adequately removed from the active pharmaceutical ingredients and the corresponding drugs. Introduction: Nursing students’ experiences during the pandemic provoked social isolation, the way to learn and every context increasing their stress and anxiety leading to drug use and abuse, among others. Problem-based learning (PBL) is a pedagogic strategy to strengthen significant learning;then the objective was to establish PBL influence in nursing students’ experiences on drug use and abuse during COVID-19 contingency. Methods: Qualitative, phenomenological and descriptive paradigm, 12 female and male nursing students aged 20 - 24 years old from the 5<sup>th</sup> and 6<sup>th</sup> semesters participated. Information collection was through semi-structured interview and a deep one in four cases. A guide of questions about: How the pandemic impacted your life? How did you face it? And what did you learn during this process? Those questions were used. Qualitative data analysis was based on De Souza Minayo, and signed informed consent was obtained from participants. Results: Students’ experiences allowed four categories to emerge, with six sub-categories. Category I. Students’ experiences on drug use and abuse facing the sanitary contingency;Category II. Students’ skills development to identify a problem and design of appropriate solutions;Category III. Developing skills to favor interpersonal relationships;Category IV. Influence of PBL in nursing students’ experiences on drug use and abuse during the COVID-19 contingency. Conclusion: PBL favored analysis and thoughts in nursing students’ experiences on drug use and abuse during the COVID-19 contingency, they worked collaboratively, developed resilience to daily life situations, and implemented stress coping strategies with their significant learning, which diminished their risk behavior. 展开更多

Intraperitoneal carcinomatosis(PC)may occur with several tumor entities.The prognosis of patients suffering from PC is usually poor.Present treatment depends on the cancer entity and includes systemic chemotherapy,radiation therapy,hormonal therapy and surgical resection.Only few patients may also benefit from hyperthermic intraperitoneal chemotherapy with a complete tumor remission.These therapies are often accompanied by severe systemic side-effects.One approach to reduce side effects is to target chemotherapeutic agents to the tumor with carrier devices.Promising experimental results have been achieved using drug-eluting beads(DEBs).A series of in vitro and in vitro experiments has been conducted to determine the suitability of their extravascular use.These encapsulation devices were able to harbor CYP2B1producing cells and to shield them from the hosts immune system when injected intratumorally.In this way ifosfamide-which is transformed into its active metabolites by CYP2B1-could be successfully targeted into pancreatic tumor growths.Furthermore DEBs can be used to target chemotherapeutics into the abdominal cavity for treatment of PC.If CYP2B1 producing cells are proven to be save for usage in man and if local toxic effects of chemotherapeutics can be controlled,DEBs will become promising tools in compartmentbased anticancer treatment. 展开更多

The study states the development and characterization of Calcium-based biocomposites: Hydroxyapatite (HAP), and PVA-Gelatin-HAP films. For the preparation of Calcium-based biocomposites, an unconventional source, the waste material calcite stone, was used as calcium raw material, and by the process of calcination, calcium oxide was synthesized. From calcium oxide, HAP was prepared by chemical precipitation method, which was later added in different proportions to PVA-Gelatin solution and finally dried to form biocomposite films. Then the different properties of PVA/Gelatin/HAP composite, for instance, chemical, mechanical, thermal, and swelling properties due to the incorporation of various proportions of HAP in PVA-Gelatin solution, were investigated. The characterization of the HAP was conducted by X-ray Diffraction Analysis, and the characterization of HAP-PVA-Gelatin composites was done by Fourier Transform Infrared Spectroscopy, Thermomechanical Analysis, Tensile test, Thermogravimetric Differential Thermal Analysis, and Swelling Test. The produced biocomposite films might have applications in orthopedic implants, drug delivery, bone tissue engineering, and wound healing. 展开更多

目的:通过检验公立医院药品集中带量采购政策实施效果,为药品集采工作高质量发展提供理论依据和实证证据。方法:首先运用德尔菲专家咨询法构建公立医院药品集中带量采购政策实施效果评价体系,再采用统计学方法分析14所三级公立医院第5... 目的:通过检验公立医院药品集中带量采购政策实施效果,为药品集采工作高质量发展提供理论依据和实证证据。方法:首先运用德尔菲专家咨询法构建公立医院药品集中带量采购政策实施效果评价体系,再采用统计学方法分析14所三级公立医院第5批国家组织药品集采情况,对其进行评价。结果:评价体系包含集采工作报量质量、集采任务完成质量和集采工作保障力度等3个一级指标,以及14个二级指标。评价体系构建科学合理全面,其中集采任务完成质量、约定采购量是否按时完成、违规采购行为、集采药品供应情况以及非中选产品采购量占比是评价重点。14所三级公立医院第5批国家组织药品集采执行效果评价得分偏低,有较大提升空间。结论:建议加强集采药品的供应保障,畅通医保、医疗和医药间的沟通渠道,提高药品集采工作精细化管理水平,开展药品集采工作常态化监管等。 展开更多

目的建立骨科Ⅰ类切口手术预防使用抗菌药物评价标准,评价药品带量采购对该类手术合理性、经济性的影响。方法以《抗菌药物临床用指导原则(2015版)》、相关药品说明书、临床指南及专家共识等为依据制定评价标准。以2021年10月抗菌药物... 目的建立骨科Ⅰ类切口手术预防使用抗菌药物评价标准,评价药品带量采购对该类手术合理性、经济性的影响。方法以《抗菌药物临床用指导原则(2015版)》、相关药品说明书、临床指南及专家共识等为依据制定评价标准。以2021年10月抗菌药物带量采购为政策执行点,选取安徽省某三甲医院2019年5月—2024年2月的9个骨科270份(带量采购前135份,带量采购后135份)Ⅰ类切口手术归档病例采用加权TOPSIS法进行数据分析。结果评价指标权重占比较高的有适应证、药品选择、术前、术中、术后给药时机、遴选药物经济性(分别为0.181、0.170、0.144、0.076、0.072、0.066)。政策执行后平均病例抗菌药物使用费用降幅为77.67%,带量采购前用药合理占70.37%(C_(i)≥0.6),带量采购后用药合理占84.45%(C_(i)≥0.6),差异具有统计学意义(P<0.05)。结论应用加权TOPSIS法评价骨科Ⅰ类切口手术预防使用抗菌药物合理性、经济性的方法可行,带量采购减轻患者用药负担的同时间接提高了用药的合理性,针对存在不合理现象医院需加强监管,促进临床合理用药。 展开更多

国家集中带量采购政策下样本医院良性前列腺增生治疗药物使用情况分析

目的探讨国家药品集中带量采购等政策对良性前列腺增生(BPH)治疗药物使用的影响,为临床合理使用BPH治疗药物提供数据支撑。方法从全国医药信息网中提取2019至2023年样本医院良性前列腺增生治疗药物的使用数据(涉及892家医院,包括三级医... 目的探讨国家药品集中带量采购等政策对良性前列腺增生(BPH)治疗药物使用的影响,为临床合理使用BPH治疗药物提供数据支撑。方法从全国医药信息网中提取2019至2023年样本医院良性前列腺增生治疗药物的使用数据(涉及892家医院,包括三级医院645家、二级医院247家),分析并比较该类药物的使用金额、用药频度(DDDs)、限定日费用(DDDc)、排序比(B/A)等指标的变化情况。结果受相关政策影响,样本医院BPH治疗药物的使用金额呈先大幅下降后平稳上升的趋势、DDDs总体呈逐年上升趋势,而DDDc总体呈逐年降低趋势;坦索罗辛和非那雄胺的DDDs在该类药物中持续排名前2位;坦索罗辛的B/A值显著高于其他BPH治疗药物。结论2019至2023年,因国家药品集采等政策的实施,切实减轻了良性前列腺增生患者的经济负担。在α受体阻滞剂和5α还原酶抑制剂两种分类中B/A最高的坦索罗辛和非那雄胺占据BPH治疗药物的市场主导地位,从经济性角度看,良性前列腺增生治疗药物的临床使用相对合理。 展开更多

目的:探讨基于聚焦解决模式的健康教育对初产妇非药物性分娩镇痛效果。方法:将本院2022年4-12月给予常规健康教育的非药物性分娩镇痛初产妇设为对照组,2023年4-12月实施聚焦解决模式下健康教育的非药物性分娩镇痛初产妇设为观察组,每组... 目的:探讨基于聚焦解决模式的健康教育对初产妇非药物性分娩镇痛效果。方法:将本院2022年4-12月给予常规健康教育的非药物性分娩镇痛初产妇设为对照组,2023年4-12月实施聚焦解决模式下健康教育的非药物性分娩镇痛初产妇设为观察组,每组200例,比较两组分娩疼痛程度、产程及产后出血情况。结果:观察组第一产程、第二产程时的视觉模拟法评分(3.92±0.85分、5.66±1.15分)均低于对照组(5.49±1.03分、8.04±1.53分),简易应对方式量表中消极应对评分(13.02±1.59分)低于对照组(15.10±1.96分),积极应对评分(30.85±4.05分)高于对照组(27.69±3.72分),第一产程(6.38±1.95h)、第二产程(53.46±9.78min)及总产程(7.57±2.30h)时间均短于对照组(8.21±2.63h、61.14±10.51min、9.43±3.11h),产后出血量(121.65±38.26ml)少于对照组(164.72±47.69ml)(均P<0.05)。结论:基于聚焦解决模式的健康教育可显著缓解初产妇非药物性分娩镇痛分娩疼痛程度,改善心理状态,缩短产程,减少产后出血量。 展开更多

目的探索抗菌药物带量采购和分级管理政策对碳青霉烯类抗生素利用的影响。方法采用Mann-Kendall趋势检验分析各级医疗机构碳青霉烯类抗生素用药频度(DDDs)、采购金额(Cost)、药品日均费用(DDDc)、每天每1000居民用药频度(DID)的变化趋势... 目的探索抗菌药物带量采购和分级管理政策对碳青霉烯类抗生素利用的影响。方法采用Mann-Kendall趋势检验分析各级医疗机构碳青霉烯类抗生素用药频度(DDDs)、采购金额(Cost)、药品日均费用(DDDc)、每天每1000居民用药频度(DID)的变化趋势,以2020年5月1日为带量采购政策的干预分界点,以2021年9月作为分级管理目录的干预分界点,采用间断时间序列分析法研究带量采购和分级管理政策对碳青霉烯类抗生素临床用药的影响。结果在带量采购政策执行后,碳青霉烯类抗生素DDDs、DID明显上升,但长期趋势变化不显著;与政策执行前相比,碳青霉烯类抗生素的Cost和DDDc瞬时下降,DDDc长期趋势变化显著,但Cost长期趋势变化不显著。分级管理目录更新后,碳青霉烯类抗生素DDDs、Cost瞬时下降,但长期下降趋势不显著,并且DDDc呈长期上升趋势。结论带量采购政策降低了碳青霉烯类抗生素DDDc,短期内降低了Cost;但对DDDs、Cost、DID的长期影响有限;分级管理对医疗机构碳青霉烯类抗生素的利用影响有限。 展开更多

Purpose: To establish a HPLC testing method of the content of bulk picoplatin and its impurities. Method: the separation was perform on a C18 column(4.6 mm×250 mm, 5 m) with potassium dihydrogen phosphate-aceton-itrile as the mobile phase at a flow rate of 1.0 mL/min. The detecting wavelength was set at 210 nm, and the column temperature was set at 30℃. Result: in the method validation, the linear relationship modulus of picoplatin is 0.9999, the systemic precision is 0.44%, the method precision is 0.74%, the average recovery rate is 99.62%, the LOD and LOQ of picoplatin is 0.2 ng and 1.0 ng. The average resolution of picoplatin and its impurities is more than 2. Conclusion: The established method is good specificity, high sensitivity, and good repeatability which could provide scientific evidence for the quality control of picoplatin and its impurities. 展开更多

Summary: Evidence-based medicine is advocated by WHO and adopted by developed countries for many years. In China, however, the selection of essential medicine and various medical insurance reimbursement schemes medicine is usually based on experts' experience of prescription practice which is under heavy critics resulting from the lack of related comparative efficacy and evidence-based research. The efficacy of Jian'ganle in prevention of drug-induced liver injury (DILI) caused by antituberculotics was evaluated in this study by comparison with Hugan Pian, glucuronolactone and reduced glutathione. Evidence was provided for relevant sectors such as Ministry for Human Resources and Social Security of the People's Republic of China and National Health and Family Planning Commission of the Peo- ple's Republic of China to select and renew the Essential Medicine List (EML), the new rural cooperative medical scheme in China (NRCMS) list or the reimbursement list of industrial injury insurance. A total of 189 patients with initial pulmonary tuberculosis were divided into four groups who took antituberculotics combined with Jian'ganle, Hugan Pian, glucuronolactone and reduced glutathione respectively. Their liver function profile including alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), direct bilirubin (DBIL), total protein (TP), albumin (A) and globulin (G) were detected at admission as baseline and after treatment. The Jian'ganle group was compared with the three others by chi-square tests. In an aspect of maintaining bilirubin indexes normal, Jian'ganle was more efficacious than glucuronolactone. And Jian'ganle had a little more efficacy than reduced glutathione to maintain protein indexes normal as well. And the therapeutic regimen of antituberculotics combined with Jian'ganle was the best in treating tuberculosis and preventing DILI at the same time. The study showed that among the four hepatinicas which demonstrated similar prevention of DILI caused by antituberculotics, Jian'ganle has more advantages over the three others to some extent, which provides a reliable basis for health sectors to select and renew the EML, NRCMS List or the reimbursement list of industrial injury insurance. 展开更多

There are two general approaches to drug discovery. The oldest is the empirically-driven in vivo identification of a drug candidate, with little or no consideration given to identifying the active constituent. The alternative is mechanism-based, a process that entails the in vitro screening of purified chemical compounds to identify those that interact specifically with a selected biological target, after which they are tested for therapeutic potential. A major difference between these approaches is the extent to which the principles of pharmacology are employed to demonstrate safety and efficacy and to enable improvements in the therapeutic properties of the product. As a thorough pharmacological analysis of the pharmacokinetics and pharmacodynamics of a test agent requires that it be a stable, single, purified substance, such testing is more difficult with unpurified samples containing multiple compounds as compared to single agents. A lack of pharmacological information compromises the clinical utility of a test substance by leaving open questions about its bioavailability, metabolism, and mechanisms of therapeutic actions and toxicities. Although drug discovery success has be achieved with both the empirically-driven and mechanism-based approaches, the proper application of pharmacological techniques in the drug discovery process maximizes efficacy, safety and the chance for regulatory approval. In addition, pharmacological data provides information needed for improving the therapeutic properties of an agent, enhancing its clinical utility, and extending the product lifespan. 展开更多

目的探讨国家药品集中带量采购政策(后文简称“国家集采政策”)在广西壮族自治区的实施效果,为医疗机构后续开展集采药品相关工作提供参考。方法选取广西壮族自治区医疗机构国家集采政策实施前后的药品采购数据,以政策目标、利益相关方... 目的探讨国家药品集中带量采购政策(后文简称“国家集采政策”)在广西壮族自治区的实施效果,为医疗机构后续开展集采药品相关工作提供参考。方法选取广西壮族自治区医疗机构国家集采政策实施前后的药品采购数据,以政策目标、利益相关方关注的问题等为导向,引入并研究6个二级指标(如可获得性、可负担性、药品安全性等)、18个三级指标(如约定采购量完成率、负担水平、药占比等)。采用描述性统计方法对国家集采政策实施前后(2019年和2020年)的数据进行差异性、变化趋势等分析。结果在可获得性方面:广西壮族自治区医疗机构参与度为92.55%,疾病覆盖率为40.16%,整体约定采购量完成率中值为287.82%,总体配送率为97.20%。在可负担性方面:2019-2022年药品价格总体的降幅为74.80%,人均住院药品费用、门诊药占比、住院药占比降幅依次为17.61%、10.22%、20.10%,除慢性病、抗肿瘤用药外,药品可负担水平均低于1。对医药的影响:2022年药品不良反应事件占比为66.00%,相比执行国家集采政策前增幅为1.29%,广西壮族自治区通过一致性评价的药品共12个品规,前8名的制药企业的市场集中度小于20.00%。对医疗医保的影响:直至2022年基本实现仿制药替代原研药,同类非中选药品使用比约为9.12%,未提出二次换药的人数占63.39%;药品费用支出减少共24.59亿元。结论国家集采政策在广西壮族自治区取得了显著的效果,但同时还需重点关注扩大专科病种用药覆盖面、中选药品的临床综合评价、优化待分配量选择方案等问题。 展开更多
