Biocompatibility of light responsive materials prepared for accommodative intraocular lenses manufacturing

AIM:To investigate the biocompatibility and bacterial adhesion properties of light responsive materials(LRM)and analyze the feasibility and biosafety of employing LRM in the preparation of accommodative intraocular lenses(AIOLs).METHODS:Employing fundamental experimental research techniques,LRM with human lens epithelial cells(hLECs)and human retinal pigment epithelium cells(ARPE-19 cells)were co-cultured.Commercially available intraocular lenses(IOLs)were used as controls to perform cell counting kit-8(CCK-8),cell staining under varying light intensities,cell adhesion and bacterial adhesion experiments.RESULTS:LRM exhibited a stronger inhibitory effect on the proliferation of ARPE19 cells than commercially available IOLs when co-cultured with the undiluted extract for 96h(P<0.05).Under other culturing conditions,the effects on the proliferation of hLECs and ARPE-19 cells were not significantly different between the two materials.Under the influence of light irradiation at intensities of 200 and 300 mW/cm^(2),LRM demonstrated a markedly higher inhibitory effect on the survival of hLECs compared to commercially available IOLs(P<0.0001).They also showed a stronger suppressive effect on the survival rate of ARPE-19 cells,with significant differences observed at 200 mW/cm^(2)(P<0.001)and extremely significant differences at 300 mW/cm^(2)(P<0.0001).Additionally,compared to commercially available IOLs,LRM had a higher number of cells adhering to their surface(P<0.05),as well as a significantly greater number of adherent bacterium(P<0.0001).CONCLUSION:LRM,characterized by their excellent non-contact tunable deformability and low cytotoxicity to ocular tissues,show considerable potential for use in the fabrication of AIOLs.These materials demonstrate strong cell adhesion;however,during photothermal conversion processes involving shape deformation under various light intensities,the resultant temperature rise may harm surrounding cells.These factors suggest that while the material plays a positive role in reducing the incidence of posterior capsule opacification(PCO),it also poses potential risks for retinal damage.Additionally,the strong bacterial adhesion of these materials indicates an increased risk of endophthalmitis. 展开更多

AIM: To compare the visual functional outcomes with accommodating and multifocal intraocular lenses(IOLs).METHODS: Our retrospective comparative study included 51 patients(60 eyes) received implantation of an accommodating IOL(Tetraflex;16 patients,20 eyes),a refractive multifocal IOL(Re Zoom;18 patients,20 eyes),or a diffractive multifocal IOL(ZMA00;17 patients,20 eyes).Subjective refraction,visual acuity,contrast sensitivity(CS),intraocular aberration,and subjective photic phenomena were detected at 3mo after surgery.RESULTS: The spherical equivalent in the three groups was-0.38±0.54 D,0.14±0.56 D,and 0.35±0.41 D,respectively.No statistically significant differences were found in uncorrected and corrected distance visual acuity and uncorrected intermediate visual acuity among the groups(P=0.39).The Re Zoom group had significantly better distance-corrected intermediate visual acuity than the ZMA00 group(P=0.003).The ZMA00 group had significantly better near visual acuity than the other groups(P〈0.05).Better contrast sensitivity values were observed in the Tetraflex group under most of the spatial frequencies conditions(P=0.025).The total aberration was lowest in the ZMA00 group(P=0.000),and the spherical aberration was highest in the Tetraflex group(P=0.000).The three groups had similar frequency of ghosting and glare,and the Tetraflex group had a low rate of halos(P=0.01).CONCLUSION: Both accommodating and multifocal IOLs can successfully restore distance and uncorrected intermediate visual acuities.Tetraflex accommodating IOLs perform better in CS and with less halos of photic phenomena.Re Zoom refractive multifocal IOLs havebetter performance in distance-corrected intermediate visual acuity than ZMA00 diffractive multifocal IOLs,and the latter achieved better near visual acuity and efficiently decreased the optical aberration. 展开更多

Encouraging Visual Outcome of a New Accommodative Intraocular Lens: A Retrospective Long-Term Study with a Mean Follow-Up of 5.3 Years

Purpose: To assess the visual outcome after implantation of the new Hydrophylic IOL type 41 B/G accommodating intraocular lens (AIOL). Methods: The presented lens was implanted during cataract surgery. All patients were offered follow-ups, allowing two postoperative measurements to be performed at 3 months and between 2020 and 2021. The mean time between lens implantation and last follow-up was 5.3 years (min. 1 year, max. 10 years). Excluded were patients with age-related macular degeneration or amblyopia. Patients with a foreign lens implanted into the second eye were included in a separate control group. Corrected distance (CDVA) and uncorrected distance visual acuity (UDVA) at 5 m, and corrected near (CNVA) and uncorrected near visual acuity (UNVA) at 40 cm were assessed. Furthermore, the postoperative spherical equivalent (SE), the dependence on spectacles and the occurrence of optical phenomena were evaluated. Results: The final study cohort consists of 65 patients with 119 implanted AIOLs. Significantly better visual results were obtained in both postoperative follow-ups compared with the preoperative results. The mean values of the last follow-up for the UNVA, CNVA, UDVA, and CDVA were 0.107 ± 0.10;0.039 ± 0.08;0.097 ± 0.11;and 0.040 ± 0.09 logMAR, respectively. Visual outcomes remained on a high level for up to 10 years and showed significantly better results compared to the control group. Postoperative SE was significantly improved. Nearly 70% of patients were no longer dependent on glasses. Furthermore, the occurrence of disturbing optical phenomena was denied by all patients. Conclusion: The results of this AIOL are particularly promising, especially since gratifying visual results could still be measured 10 years after implantation. 展开更多

Background The TetraFlex accommodating intraocular lens (IOL) was designed to supply the patients both satisfied far and near vision after cataract surgery.So we need to evaluate the safety, distance and near visual acuity, subjective accommodation and IOL mobility with the TetraFlex accommodating IOL implantation.Methods Fifty eyes of 42 study-eligible cataract patients, who gave informed consent at a single eye clinic in China over a 10-month period, underwent phacoemulsification with TetraFlex IOL implantation.At three months postoperation,uncorrected visual acuities (UCVA), best corrected visual acuities (BCVA), distance-corrected near visual acuities (DCNVA), subjective accommodation using the defocus method, and pilocarpine-induced IOL mobility were measured.Results No postoperative complications were noted in the study.Three months postoperation UCVA and BCVA were 20/40 or bettter in 82% (41/50) and 92% (46/50) of eyes, respectively; 66% (33/50) of the eyes had DCNVA of Jager (J) 4or better at 3 months.In addition, the mean subjective accommodation was (0.94±0.61) diopters (D) (range from 0.50 to 1.50 D) and pilocarpine-induced IOL mobility was (337±124) μm (range from 121 to 501 μm) with the TetraFlex.High relationship (r2=0.901, P 〈0.01) was found between these two measurements.Conclusion Implantation of the TetraFlex is safe and leads to excellent uncorrected distance vision and good uncorrected near vision. 展开更多

Presbyopia still remains the last frontier of refractive surgery.Its surgical management is under constant evolution due to the limitations that exist today with respect to its management,which is probably in relation with the multifactorial basis in which presbyopia is clinically developed in the human.Until currently,virtually all surgical techniques that have been proposed for its correction are based on the induction of pseudoaccommodation in the presbyopic eye,including multifocality.However,the real restoration of accommodation is more complex,and it has been tried by the use of different,so called,“accommodative”pseudophakic intraocular lenses(AIOL).Overall,the reported results with these lenses by independent authors have been modest in relation with the restoration of the accommodative power of the eye and these modest benefits are usually lost with time due to the long term changes in the capsular bag.This fact made these lenses to be almost abandoned in the last few years,but there are currently other AIOL models being used with innovative mechanisms of action and different anatomical support outside the capsular bag that offer encouraging preliminary results that could bring a new potential of application to these types of lenses.In this article,we will update the modern refractive surgeon about the fundamentals and provide updated information about the outcomes of AIOLs by reviewing the concept of accommodation,the different attempts that have been accomplished in the past,their demonstrated published results in human clinical trials,and the future alternatives that may arrive in the near future. 展开更多

Purpose:.To compare early visual function between patients undergoing phacoemulsification combined with multifocal and accommodative intraocular lens implantation.Methods: A total of 112 patients with age-related cataract undergoing phacoemulsification in our hospital were recruited for this study and randomly assigned into multifocal(56 eyes;ZAM00 group) and accommodative(56 eyes; FLEX group)intraocular lens groups. Visual acuity and contrast sensitivity were statistically compared between the two groups.Results:.No significant difference was found in uncorrected distant visual acuity between the ZMA00 and FLEX groups at1 week, or 1, 3, and 6 months after operation(all P>0.05).At postoperative 6 months, no statistical significance was noted in distant and intermediate best-corrected visual acuity or in contrast sensitivity between the two groups(all P>0.05). Patients in the ZMA00 group were superior to their counterparts in the FLEX group regarding near best corrected visual acuity, reading speed, and spectacle independence(all P<0.05).Conclusion: ZMA00 and FLEX IOL implantation can provide excellent distant and intermediate visual acuity for patients with age-related cataract. ZMA00 IOL is superior to FLEX in terms of near visual acuity. 展开更多

Background:Presbyopia has become a global disease affecting the world's aging population.Among various treatments,cataract extraction and intraocular lens(IOL)implantation have become the most popular and common methods of presbyopia correction.During the twentieth century,IOLs have underwent significant innovation and advancements to meet the patients'high demands for functional vision at all distances.Main Text:To meet the increasing needs for excellent near and intermediate vision for daily activities,some premium IOLs with more than one focus have been developed,for example,the refractive MfIOLs,diffractive MfIOLs,extended depth of field(EDOF)IOLs,and accommodating IOLs(AIOLs)were introduced to meet this need.In addition,the add-on MfIOLs have been explored as promising supplementary IOLs for pseudophakic presbyopia.When selecting the MfIOLs,the IOLs'features,patients’characteristics,preoperative eye conditions,and treatment expectations should be considered.Conclusions:In this review,we focus on the multifocal IOLs(MfIOLs)commonly used for presbyopia correction and systematically summarized their optical designs and clinical outcomes.More evidence-based studies are required to provide guidelines for MfIOL selection,provide maximum visual benefits,and develop personalized visual solutions in the future. 展开更多

目的比较植入多焦点(multifocal intraocular lenses,MIOL)、可调节(accommodative intraocular lens,AIOL)和单焦点(monofocal/single focus intraocular lens,SIOL)人工晶状体植入术后的视觉质量。方法采用同期随机对照研究,选取2010... 目的比较植入多焦点(multifocal intraocular lenses,MIOL)、可调节(accommodative intraocular lens,AIOL)和单焦点(monofocal/single focus intraocular lens,SIOL)人工晶状体植入术后的视觉质量。方法采用同期随机对照研究,选取2010年9月至2011年4月期间,在本科行白内障超声乳化联合人工晶状体植入术的患者,分别植入3种人工晶状体,其中多焦点ZMA00组30例(43只眼),可调节FLEX组33例(49只眼),单焦点AKROS组32例(47只眼)。术后1 d,1、3个月随访,测定患者裸眼及矫正远视力、矫正远视力下近视力及中视力;离焦法测伪调节力;MFVA-100多功能视力检测仪测量对比敏感度视力,并问卷调查其脱镜率。结果裸眼远视力ZMA00组和FLEX组略高于AKROS组,但差异无统计学意义(P>0.05),矫正远视力3组无明显差异(P>0.05);矫正远视力下中视力,ZMA00和FLEX组均明显优于AKROS组(P<0.05),但ZMA00和FLEX 2组间无差异(P>0.05);矫正远视力下近视力及人工晶体眼伪调节力均表现为ZMA00组最佳,FLEX组次之,AKROS组最低,差异有统计学意义(P<0.05)。脱镜率ZMA00组86.7%(26/30),FLEX组63.6%(21/33),均优于AKROS组18.8%(6/32),差异有统计学意义(P<0.05);在各百分比对比度下,3组患者的对比敏感度视力无显著差异(P>0.05)。结论 3种人工晶状体均可获得较好的远视力,但多焦点和可调节人工晶状体均拥有更佳的远、中、近视力及较高的调节力和脱镜率。非球面多焦点人工晶状体未带来对比敏感度的缺失,且在近视力及伪调节力方面,比可调节人工晶状体具有更大的优势。 展开更多

目的:探讨1CU可调节人工晶状体植入眼内后术眼拟调节能力。方法:通过对2003-06至今我院行白内障超声乳化联合可调节人工晶状体1CU术后对术眼裸眼远、近视力,矫正视力,矫正远视力下的近视力进行测量,并运用主觉近点法、离交法、动态视网... 目的:探讨1CU可调节人工晶状体植入眼内后术眼拟调节能力。方法:通过对2003-06至今我院行白内障超声乳化联合可调节人工晶状体1CU术后对术眼裸眼远、近视力,矫正视力,矫正远视力下的近视力进行测量,并运用主觉近点法、离交法、动态视网膜检影法以及A超测量10g/L匹罗卡品点眼前后前房深度,检测术眼拟调节能力,同期以单焦人工晶状体Acrysof作为临床对比,评价可调节人工晶状体1CU临床应用效果。结果:1CU组的裸眼近视力和矫正远视力下的近视力好于Acrysof组;带状光检影测量1CU组的调节力为0.99±0.45D,Acrysof组的调节力为0.25±0.22D,两组差异有显著性意义(t=5.623,P<0.05);主觉近点法测量1CU组的调节力为1.60±0.55D,Acrysof组的调节力为0.42±0.22D,两组差异有显著性意义(t=2.147,P<0.05);离焦法测量1CU组的调节力为1.46±0.52D,Acrysof组的调节力为0.52±0.35D,两组差异有显著性意义(t=2.647,P<0.05);滴用匹罗卡品眼药水后分别测量1CU组和Acrysof组前房深度(ACD)变化,两组比较有显著性差异(t=4.374,P<0.05)。结论:1CU可调节人工晶状体可以明显提高近视力,使白内障患者术后获得一定的调节力。 展开更多

目的对比植入非对称区域折射型多焦点人工晶状体(multifocal intraocular lens,MIOL;SBL-3)与可调节型IOL(accommodating IOL,AIOL;Tetraflex HD)后的视觉质量。方法前瞻性队列研究。选取在我院诊断为单纯性白内障,并行白内障超声乳化联... 目的对比植入非对称区域折射型多焦点人工晶状体(multifocal intraocular lens,MIOL;SBL-3)与可调节型IOL(accommodating IOL,AIOL;Tetraflex HD)后的视觉质量。方法前瞻性队列研究。选取在我院诊断为单纯性白内障,并行白内障超声乳化联合IOL植入术的患者26例(38眼)为研究对象,其中植入SBL-3 MIOL者18眼为MIOL组,植入Tetraflex HD AIOL者20眼为AIOL组。术后1个月、3个月分别检查患者裸眼远、近视力,最佳矫正远、近视力及远视力矫正下近视力,3个月时使用视觉质量分析系统评估患者斯特列尔比率、调制传递函数截止频率(MTFcutoff)值及客观调节幅度。采用SPSS 18.0分析数据。结果术后3个月时两组患者间裸眼远视力及最佳矫正远、近视力差异均无统计学意义(均为P>0.05);MIOL组裸眼近视力及远视力矫正下近视力分别为(0.245±0.079)log MAR、(0.279±0.074)log MAR,均优于AIOL组的(0.303±0.094)log MAR和(0.339±0.077)log MAR,差异均有统计学意义(均为P<0.05);术后3个月时,视觉质量分析系统测得MIOL组和AIOL组斯特列尔比率值分别为0.19±0.29和0.15±0.05,客观调节幅度分别为(2.72±0.49)D和(2.19±0.49)D,两组相比差异均有统计学意义(均为P<0.05)。结论非对称区域折射型MIOL与AIOL在视觉质量方面具有差异性。 展开更多

目的:研究Tetraflex可调节人工晶状体(IOL)植入术后视力、调节幅度、对比敏感度及主观视觉功能的临床应用效果。方法:病例对照研究。选择2010-03/2012-12在我院接受白内障超声乳化摘除联合囊袋内IOL植入术的白内障患者48例72眼,其中23... 目的:研究Tetraflex可调节人工晶状体(IOL)植入术后视力、调节幅度、对比敏感度及主观视觉功能的临床应用效果。方法:病例对照研究。选择2010-03/2012-12在我院接受白内障超声乳化摘除联合囊袋内IOL植入术的白内障患者48例72眼,其中23例35眼植入Tetraflex可调节IOL(Tetraflex组),25例37眼植入AcrySof SA60AT单焦点IOL(SA60AT组),对比研究两组患者术后1,3,6mo裸眼远视力、最佳矫正远视力、裸眼近视力、最佳矫正近视力、最佳矫正远视力下近视力、拟调节力、脱镜率以及患者满意度情况,采用主观移近法测量调节力。采用SPSS13.0统计软件包进行统计学处理。结果:术后随访期间,两组患者的裸眼远视力、最佳矫正近视力和最佳矫正远视力差异无统计学意义(P>0.05);Tetraflex组患者的裸眼近视力和最佳矫正远视力下近视力与单焦组比较差异有统计学意义(P<0.05)。术后Tetraflex组拟调节力均高于SA60AT组(P<0.05),于术后6mo Tetraflex组拟调节力有所下降,与1,3mo比较差异均有统计学意义(P<0.05),两组间对比敏感度差异无统计学意义。脱镜率及和主观视功能满意度,Tetraflex可调节IOL组均明显优于单焦组,差异有统计学意义(P<0.05)。结论:可调节IOL可提供较好的远近视力,增加拟调节力,有效降低患者术后对老视镜的依赖,改善主观视功能。但其拟调节力仍需要进一步的研究。 展开更多

目的:评价福来视(Tetraflex)可调节人工晶状体植入术的临床效果。方法:白内障患者21例30眼行超声乳化手术并植入Tetraflex可调节人工晶状体。20例29眼同年龄白内障患者植入传统单焦点人工晶状体做对照。所有患者术后1d;1wk;1,3mo常规行... 目的:评价福来视(Tetraflex)可调节人工晶状体植入术的临床效果。方法:白内障患者21例30眼行超声乳化手术并植入Tetraflex可调节人工晶状体。20例29眼同年龄白内障患者植入传统单焦点人工晶状体做对照。所有患者术后1d;1wk;1,3mo常规行裂隙灯检查,屈光、裸眼及矫正远视力和近视力检查。术后3mo检查调节幅度及20g/L毛果芸香碱滴眼前后的前房深度。结果:术后两组均可获得良好的裸眼远视力及矫正远视力,Tetraflex组裸眼近视力明显好于传统单焦点组。动态检影法Tetraflex组的调节幅度为2.25±0.35D,单焦点组为0.65±0.25D(P<0.01)。滴20g/L毛果芸香碱后前房深度变浅分别为0.40±0.20mm,0.15±0.11mm,两组差异有显著意义(P<0.01)。结论:Tetraflex可调节人工晶状体是安全有效的,可为患者提供良好的远近视力。 展开更多

背景后发性白内障的发生使1CU可调节人工晶状体(1CUAIOL)的拟调节力及远近视功能明显下降,影响其远期临床效果。目前国内外关于后发性白内障对IOL眼内移动度的影响及Nd:YAG激光后囊膜切开术后是否可恢复IOL眼内移动度的研究报道较... 背景后发性白内障的发生使1CU可调节人工晶状体(1CUAIOL)的拟调节力及远近视功能明显下降,影响其远期临床效果。目前国内外关于后发性白内障对IOL眼内移动度的影响及Nd:YAG激光后囊膜切开术后是否可恢复IOL眼内移动度的研究报道较少。目的探讨后发性白内障及Nd:YAG激光后囊膜切开术对1CUAIOL眼内移动度的影响。方法采用系列病例观察试验设计。收集因单纯性白内障行白内障超声乳化摘出术联合1CUAIOL植入术后发生后发性白内障者20例24眼,分别于IOL植入术后3个月、Nd:YAG激光后囊膜切开术前1d、Nd:YAG激光后囊膜切开术后3个月进行随访观察,检测术眼最佳矫正远视力下的近视力(DCNVA),并用IOLMaster检测10g/L匹罗卡品滴眼液点眼前及点眼后的前房深度,计算其差值作为IOL的眼内移动度。比较上述3个时间点IOL的眼内移动度变化,评价后发性白内障及激光治疗对1CUAIOL眼内移动的影响。结果1CUAIOL植入术后3个月IOL的眼内移动度为(0.44±O.21)mm,发生后发性白内障后,术眼行Nd:YAG激光后囊膜切开术前1dIOL的眼内移动度为(0.27±0.11)mm,Nd:YAG激光后囊膜切开术后3个月IOL的眼内移动度为(0.34±0.10)mm,3个时间点间IOL的眼内移动度差异有统计学意义(F=7.180,P=0.001),与IOL植入术后3个月比较,Nd:YAG激光后囊膜切开术前1dIOL的眼内移动度明显下降,差异有统计学意义(P=0.006);与后囊膜切开术前1d相比,Nd:YAG激光后囊膜切开术后3个月IOL的眼内移动度略有增加,但差异无统计学意义(P=0.059)。1CUAIOL植入术后3个月术眼DCNVA为(3.1±O.9)J,Nd:YAG激光后囊膜切开术前1d为(6.2±0.8)J,Nd:YAG激光后囊膜切开术后3个月为(3.4±O.7)J,3个时间点间的差异有统计学意义(F=110。270,P=0.000),其中Nd:YAG激光后囊膜切开术前1d术眼DCNVA明显低于IOL植入术后3个月,差异有统计学意义(P〈0.05),与后囊膜切开术前1d相比,Nd:YAG激光后囊膜切开术后3个月术眼DCNVA明显增加,差异有统计学意义(P〈0.05)。IOL植入术后3个月、Nd:YAG激光后囊膜切开术前1d、Nd:YAG激光后囊膜切开术后3个月DCNVA与1CUAIOL的眼内移动度均无显著的相关性(r,=-0.150,P1=0.486;r2=-0.320,P:=0.122:r3=-0.100,P3=0.633)。结论后发性白内障使1CUAIOL的眼内移动度明显下降,Nd:YAG激光后囊膜切开术对IOL的移动度并无明显影响,但术后术眼仍可恢复较好的视近功能,可能与多种因素产生的拟调节有关。 展开更多

目的比较超声乳化白内障吸出术后植入Array多焦点人工晶状体和1cu可调节人工晶状体后的调节能力。方法26例(40眼)年龄相关性白内障患者行超声乳化白内障吸出及人工晶状体植入术,根据植入的人工晶状体的种类不同分为2组:Array组:14例20眼... 目的比较超声乳化白内障吸出术后植入Array多焦点人工晶状体和1cu可调节人工晶状体后的调节能力。方法26例(40眼)年龄相关性白内障患者行超声乳化白内障吸出及人工晶状体植入术,根据植入的人工晶状体的种类不同分为2组:Array组:14例20眼,植入Array多焦点人工晶状体;1cu组:12例20眼,植入1cu可调节人工晶状体。分别于术后3个月、18个月比较2组患者的最佳矫正远视力、35cm处最佳矫正远视力基础上获得的近视力及调节幅度。结果术后3个月时,2组患者的最佳矫正远视力、35cm处最佳矫正远视力基础上获得的近视力及调节幅度差异均无统计学意义(均为P>0.05)。术后18个月时,Array组患者的35cm处最佳矫正远视力基础上获得的近视力为0.36±0.12,优于1cu组的0.28±0.10,差异有统计学意义(t=2.29,P<0.05);Array组的调节幅度为(1.39±0.40)D,大于1cu组的(0.87±0.30)D,差异有显著统计学意义(t=4.65,P<0.001)。术后18个月时,Array组患者的35cm处最佳矫正远视力基础上获得的近视力及调节幅度与术后3个月(0.35±0.10,1.31D±0.50D)比较差异均无统计学意义(均为P>0.05);1cu组患者的35cm处最佳矫正远视力基础上获得的近视力及调节幅度与术后3个月(0.38±0.11,1.44D±0.40D)相比均出现一定程度的下降(均为P<0.05)。结论植入1cu可调节人工晶状体或Array多焦点人工晶状体均能为患者提供较好的近视力及调节能力,但植入1cu可调节人工晶状体患者的远期调节能力出现一定程度的下降。 展开更多
