To assess the effectiveness of the flashlamp- pumped pulsed dye laser (Photogeneca V, Synosure Corp, Boston, United States) in the treatment of port- wine stains. Methods. One hundred and ninety- four consecutive patients with port- wine stains were treated with a flashlamp- pumped pulsed dye laser in Peking Union Medical College Hospital from January 1998 to August 1999. Results. Of 194 patients who completed treatment, 56.2% had more than 60% fading of the lesion and only 6.7% had less than 20% fading. An average of 3.6 treatments were needed to achieve more than 60% fading. The response was better in children than in adults, although the difference was not significant. Pigmentary change (usually transient) occurred in 3.1% of patients. Conclusions. This study confirms the efficacy of the flashlamp- pumped pulsed dye laser in the treatment of port- wine stains in children and adults. 展开更多

Early Pulsed Dye Laser Treatment for Infantile Hemangiomas: An Open Study for 50 Japanese Infancies

Infantile hemangiomas, occurring at birth or during the first weeks of life, are benign vascular tumors with an initial proliferative phase and a later involutional phase. The massive proliferation on the key regions and unpredicted regression may threaten?life. Thus, early management of infantile hemangiomas is necessary for affected infancies. Recent studies showed that propranolol for infantile hemangiomas had high efficacy for disappearance or reduction of infantile hemangiomas. However, a most recent study demonstrated existence of propranolol-resistant infantile hemangiomas. Therefore, we retrospectively evaluated efficacy of established early pulsed dye laser treatment for infantile hemangiomas in the 50 Japanese infancies which were applied in the period from 2000 to 2005, because our series included from the plaque subtype to the more proliferative tumor subtype. High efficacy in both subtypes suggests that early pulsed dye laser treatment can still be included as the first option for the mixed type of infantile hemangiomas and that pulsed dye laser treatment may be useful as the second line for the propranolol-resistant mixed type of infantile hemangiomas. 展开更多

The purpose of the study is to evatuate the feasibility of pulsed dye laser(PDL)throm bolysis In vitro and the effect of PDL on vascular function and structure invivo.In vitro,fifty hum an throm biwere divided into laser treated group(n=35),irritation group(n=11),and controgroup(n=4).In the laser treated group thethrom biwere irradiated by PDL with 300 pulses at 100m J/pulse.No irradiation wasgiven to the throm bi in the irritation group,while the catheter was put to and frointo the throm bi for there times,Neither irradintion nor irritation was given to thethrom bi in the controlgroup.Sixteen healthy dogs were studied in vivo.Under theguidance of the guide-w ire PDL catheter was put into the left coronary arteries(n=16)from which PDL was em itted for 1380±60 pulses at 100m J/pulse,whereasthe rightcoronary srteries(n=16)which did not receieve PDL em ission were servedas irritation group.Then the PDL were irradiated directly to the right coronaryarterial wall in the irritation group without the guidance of the guide-wire.AfterPDL irradiation.the weight of the throm bi was reduced from 1.79±0.23g to0.61±0.12g while in the irritation group theweight of the throm biwas only reducedfrom 1.65±0.19 to 1.42±0.15g.In these two aroups the reduction of throm biweightwere 68%±6% and 15%±7% resoectively(P【0.001).No self-dissolution of thethrom bi occurred in the control group.The finding under the contrast phasem icroscope indicated 91% of the debris size were smaller than the size of redcells.In vivo study,coronary angiography showed that no changes in luminalduam eter of both groups.Nomechanicalor thermal in jury to the coronary arterieswas found excepta light irregularity of the intima.PDL Throm bolysis was effectivein vitro.There was no mechanicaldam age or therm al in jury to the vessels during theprocedure of intracoronary PDL irradiation in vivo. 展开更多

Cutaneous infections with the human papilloma virus, despite being almost always asymptomatic, can induce slow-growing lesions knowed as warts that can remain subclinical for long periods of time. There are several therapeutical options namely keratolytics, retinoids, imiquimod, cryotherapy, electrosurgery, lasers and classic surgery, but none has antiviral effects. We describe the case of a patient with large warts located on the dorsal surface of the right hand, treated with cryotherapy, CO2 laser and topical imiquimod having either no clinical response or quick relapse. Treatment with PDL was started and after 13 sessions, the patient was without lesions, with a good cosmetic result and so far without relapse. This case report is relevant to us and deserves to be published because of the size of the lesions and the great response to PDL despite all the failures with the other treatments. 展开更多

两种激光治疗儿童中心点突起型蜘蛛痣的临床研究

目的:探研2种激光方法在儿童中心点突起型蜘蛛痣治疗中的疗效、不良反应及并发症。方法:选取2020年10月至2022年10月期间由河北省儿童医院整形外科门诊确诊的中心点突起型蜘蛛痣患儿60例为研究对象,采用数字表法随机分组,分为595nm脉冲... 目的:探研2种激光方法在儿童中心点突起型蜘蛛痣治疗中的疗效、不良反应及并发症。方法:选取2020年10月至2022年10月期间由河北省儿童医院整形外科门诊确诊的中心点突起型蜘蛛痣患儿60例为研究对象,采用数字表法随机分组,分为595nm脉冲染料激光组,超脉冲CO_(2)点阵激光组,每组30例。对患儿的治疗次数、治疗效果、不良反应以及并发症情况进行评估。结果:经过随访得知,595nm组较CO_(2)组治疗次数多,差异有统计学意义(P<0.05);两组一次治疗后治愈率分别为80%、95%,差异有统计学意义(P<0.05);两组经多次治疗后治愈率差异无统计学意义(P>0.05);595nm组较CO_(2)组不良反应大,差异有统计学意义(P<0.05);随访12周,并发症差异无统计学意义(P>0.05)。结论:595nm脉冲染料激光及超脉冲CO_(2)点阵激光在儿童中心点突起型蜘蛛痣的治疗中均效果显著,各有优缺点,可根据病灶特点及患儿要求选择适当的治疗方法。 展开更多

目的:回顾性研究Vbeam 595 nm脉冲染料激光器(PDL)治疗婴幼儿血管瘤(IHs)中期临床效果,分析探索其可能的影响因素。方法:收集2020年1月至2022年12月唐山市妇幼保健院行PDL治疗的219例IHs患儿,并至少随访6个月,分析治疗效果与月龄、血管... 目的:回顾性研究Vbeam 595 nm脉冲染料激光器(PDL)治疗婴幼儿血管瘤(IHs)中期临床效果,分析探索其可能的影响因素。方法:收集2020年1月至2022年12月唐山市妇幼保健院行PDL治疗的219例IHs患儿,并至少随访6个月,分析治疗效果与月龄、血管瘤部位、厚度及治疗次数等之间的关系。结果:219例患儿总体中位月龄7个月,平均月龄(4~10)个月,总计219处病损。月龄、病损特征等一般资料在性别间差异无统计学意义(P>0.05)。治疗总体有效率92.7%,中位治疗次数3次,平均(2~4)次。男女性别间比较、治疗次数及治疗结局差异无统计学意义(P>0.05)。疗效差的6人,月龄13(8,19)个月;疗效一般的10人,月龄13(8,14)个月;疗效好的31人,月龄8(4,10)个月;疗效优的172人,月龄6(4,9)个月;不同疗效患儿间月龄差异具有统计学意义(Z=24.863,P<0.001),疗效优的患儿月龄较小。血管瘤患儿的治疗结局与其病变厚度、面积大小及治疗次数有相关性(r=-0.32、-0.46、-0.46,P<0.05),治疗有效主要集中在2~4次的治疗中。治疗引起的不良反应为皮肤红斑,总体半年后残留并发症率为5.48%。结论:595 nm PDL联合动态冷却装置(DCD)治疗为IHs提供了一种安全、有效的治疗方式,在早期对IHs进行干预,可以提高治疗效果,减少治疗疗程,从而进一步减少相应并发症可能。 展开更多

目的观察湿润烧伤膏联合脉冲染料激光对提升儿童大面积鲜红斑痣整体治疗效果的影响,以期指导临床相关疾病治疗。方法本文为随机对照试验,研究对象选自2022年1月至2022年9月湖南省人民医院收治60例大面积鲜红斑痣患儿,采用随机数字表法... 目的观察湿润烧伤膏联合脉冲染料激光对提升儿童大面积鲜红斑痣整体治疗效果的影响,以期指导临床相关疾病治疗。方法本文为随机对照试验,研究对象选自2022年1月至2022年9月湖南省人民医院收治60例大面积鲜红斑痣患儿,采用随机数字表法将其分为两组,各30例。对照组男童12例,女童18例,年龄(6.25±1.02)岁;试验组男童10例,女童20例,年龄(6.35±1.05)岁。对照组采取595 nm脉冲染料激光治疗联合常规抗炎软膏护理创面,试验组采取595 nm脉冲染料激光联合湿润烧伤膏护理创面。对比两组治疗效果、红痣瘢痕消退程度[采用温哥华瘢痕量表(VSS)评估]、疼痛程度(采用国际数字疼痛分级标准评估)、不良反应。采用t检验、χ^(2)检验、秩和检验。结果试验组治疗总有效率高于对照组[96.67%(29/30)比70.00%(21/30)],差异有统计学意义(χ^(2)=7.680,P<0.05)。治疗结束时,试验组红痣瘢痕消退程度(VSS评分)低于对照组[(1.25±0.85)分比(4.02±1.12)分],差异有统计学意义(t=10.791,P<0.05);治疗结束时,试验组疼痛程度低于对照组[(1.02±0.25)比(3.02±0.52)],差异有统计学意义(t=18.986,P<0.05)。试验组不良反应总发生率低于对照组[10.00%(3/30)比43.33%(13/30)],差异有统计学意义(χ^(2)=8.523,P<0.05)。结论595 nm脉冲染料激光联合湿润烧伤膏可显著提高儿童大面积鲜红斑痣的治疗效果,利于红痣瘢痕消退,改善疼痛程度,提升患儿治疗过程中的舒适度,且安全可靠。 展开更多

目的探讨双波长脉冲染料激光对血管瘤内皮细胞(HemECs)的增殖和凋亡的影响。方法体外分离培养HemECs并随机分为4组:脉冲染料激光组(PDL,595 nm波长)、Nd∶YAG激光(1064 nm波长)组、双波长组(595 nm PDL+1064 nm Nd∶YAG)、对照组(不照... 目的探讨双波长脉冲染料激光对血管瘤内皮细胞(HemECs)的增殖和凋亡的影响。方法体外分离培养HemECs并随机分为4组:脉冲染料激光组(PDL,595 nm波长)、Nd∶YAG激光(1064 nm波长)组、双波长组(595 nm PDL+1064 nm Nd∶YAG)、对照组(不照射激光)。于照射结束后第1、3、7天,采用CCK-8法检测各组HemECs的增殖能力,流式细胞术检测凋亡率,Western blot检测新生血管生成相关蛋白VEGF、bFGF的表达水平。结果激光照射后HemECs细胞较对照组细胞逐渐变长、变疏。照射后第1天,与对照组相比,双波长组HemECs的增殖抑制率显著升高(P=0.015),而PDL组、Nd∶YAG组的增殖抑制率均无明显改变(均P>0.05);照射后第3、7天,与对照组相比,PDL组、Nd∶YAG组及双波长组的HemECs的增殖活性、VEGF和bFGF蛋白表达水平均显著降低(均P<0.001),增殖抑制率与凋亡率均显著升高(均P<0.001),且双波长组较PDL组、Nd∶YAG组更明显(均P<0.05)。结论双波长脉冲染料激光可显著抑制HemECs增殖、促进其凋亡,降低血管生成因子水平,其作用较595 nm PDL或1064 nm Nd∶YAG单波长激光照射更明显。 展开更多

目的:探究长脉冲染料激光联合局部封闭治疗面颈部增生性瘢痕的效果及对皮肤屏障功能、Sawada分级和温哥华瘢痕量表(Vancouver scar scale,VSS)评分的影响。方法:选取2021年1月-2022年8月于笔者医院门诊就诊的71例面颈部增生性瘢痕患者,... 目的:探究长脉冲染料激光联合局部封闭治疗面颈部增生性瘢痕的效果及对皮肤屏障功能、Sawada分级和温哥华瘢痕量表(Vancouver scar scale,VSS)评分的影响。方法:选取2021年1月-2022年8月于笔者医院门诊就诊的71例面颈部增生性瘢痕患者,按照随机数字表法分为对照组(n=35例,局部封闭治疗)与观察组(n=36,长脉冲染料激光联合局部封闭治疗),所有患者治疗结束后随访12个月,评估疗效并比较两组患者治疗前后瘢痕皮肤经皮水分丢失量、角质层含水量、红斑指数、临床症状Sawada分级、VSS评分及不良反应发生情况。结果:观察组治疗总体有效率为94.44%,高于对照组的77.14%(P<0.05);观察组治疗后皮肤经皮水分丢失量及红斑指数低于对照组,角质层含水量高于对照组(P<0.05);观察组治疗后瘢痕颜色、高度、硬度、瘙痒程度、触痛感觉评分低于对照组(P<0.05);观察组治疗后血管分布、柔软程度、色泽、厚度评分低于对照组(P<0.05);观察组疼痛、轻度色素减退、萎缩性瘢痕、红肿及总体不良反应发生率低于对照组(P<0.05)。结论:长脉冲染料激光联合局部封闭治疗面颈部增生性瘢痕患者,可明显改善其皮肤屏障功能及瘢痕情况,提升疗效,值得临床推广使用。 展开更多

目的分析重组人碱性成纤维细胞生长因子辅助脉冲染料激光治疗玫瑰痤疮的临床效果。方法选取2022年11月-2023年10月与航空总医院皮肤科收治的玫瑰痤疮患者。在采用单纯的染料激光治疗的患者群体中随机抽取40例患者纳入对照组。染料激光... 目的分析重组人碱性成纤维细胞生长因子辅助脉冲染料激光治疗玫瑰痤疮的临床效果。方法选取2022年11月-2023年10月与航空总医院皮肤科收治的玫瑰痤疮患者。在采用单纯的染料激光治疗的患者群体中随机抽取40例患者纳入对照组。染料激光联合生长因子治疗的患者群体中随机抽取40例纳入观察组。比较两组患者治疗前后玫瑰痤疮症状评分、皮肤屏障功能指标、Acne-QOL评分。比较患者治疗后的临床效果。结果治疗前,两组患者玫瑰痤疮症状评分、皮肤屏障功能指标、Acne-QOL评分等比较差异无统计学意义。治疗后,观察组玫瑰痤疮症状评分低于对照组(P<0.05),TEWL低于对照组,角质层含水量、EI高于对照组(P<0.05)。观察组Acne-QOL(96.45±18.47)分,与对照组(82.49±16.53)分比较差异无统计学意义(t=3.56,P=0.001)。观察组临床效果优于对照组,差异有统计学意义(近似χ^(2)=4.15,P=0.042)。结论重组人碱性成纤维细胞生长因子显著提高脉冲染料激光治疗玫瑰痤疮的临床效果,修复玫瑰痤疮患者的皮肤屏障功能,值得临床进一步研究与推广。 展开更多

The objective of this study was to evaluate the effect of the pulsed dye laser (585 nm) on the production of collagen and the mRNA expression of collagen related gene in fibroblasts in vitro. Methods Cultured fibroblasts were treated with a 585-nm flashlamp-pumped pulsed dye laser ( fluence 3 J/cm^2, 4 J/cm^2, spot size 7 mm, pulse duration 450 12s). The production of collagen and the mRNA expression of transforming growth factor (TGF)-β1, SMAD2, SMAD3, SMAD4, SMAD7 and type Ⅰ procollagen α1, α2 in fibroblasts were investigated by colorimetry or real time polymerase chain reaction. Results The production of collagen was significantly up-regulated after treatment with a 585-nm flashlamp-pumped pulsed dye laser with a fluence of 3 J/cm^2 (P 〈0.001). The mRNA expression of TGF- β1, SMAD2, SMAD3, SMAD4, SMAD7 and procollagen I was significantly up-regulated after treatment with a 585-nm flashlamp-pumped pulsed dye laser with a fluence of 3 J/cm^2 (P 〈0.001). No significant difference of mRNA expression of SMAD2, SMAD3, SMAD4, SMAD7 and type Ⅰ procollagen was found between controls and fibroblasts treated with pulsed dye laser with a fluence of 4 J/cm^2 (P 〉0.05). Conclusions Lower fluence (3 J/cm^2) pulsed dye laser increased the collagen production in fibroblasts by up-regulating TGF-β1, SMAD2, SMAD3, SMAD4, SMAD7 and type Ⅰ procollagen mRNA expression. These may be the reason it can be effectively used in the treatment of wrinkles. 展开更多

Laser induced periodic surface structures(LIPSS)represent a kind of top down approach to produce highly reproducible nano/microstructures without going for any sophisticated process of lithography.This method is much simpler and cost effective.In this work,LIPSS on Si surfaces were generated using femtosecond laser pulses of 800 nm wavelength.Photocatalytic substrates were prepared by depositing TiO2 thin films on top of the structured and unstructured Si wafer.The coatings were produced by sputtering from a Ti target in two different types of oxygen atmospheres.In first case,the oxygen pressure within the sputtering chamber was chosen to be high(3×10^–2 mbar)whereas it was one order of magnitude lower in second case(2.1×10^–3 mbar).In photocatalytic dye decomposition study of Methylene blue dye it was found that in the presence of LIPSS the activity can be enhanced by 2.1 and 3.3 times with high pressure and low pressure grown TiO2 thin films,respectively.The increase in photocatalytic activity is attributed to the enlargement of effective surface area.In comparative study,the dye decomposition rates of TiO2 thin films grown on LIPSS are found to be much higher than the value for standard reference thin film material Pilkington Activ^TM. 展开更多

The peak energy density w0P of second harmonic pulses of a mode-locked titanium sapphire laser at wavelength λP = 400 nm is determined by nonlinear energy transmission measurement TE through the dye ADS084BE (1,4-bis(9-ethyl-3-car-bazovinylene)-2-methoxy-5-(2’-ethyl-hexyloxy)-benzene) in tetrahydrofuran. TE(w0P) calibration curves are calculated for laser pulse peak energy density reading w0P from measured pulse energy transmissions TE. The ground-state absorption cross-section σP and the excited-state absorption cross-section σex at λP, and the number density N0 of the retinal Schiff base isoform RetA in pH 7.4 buffer of the blue-light adapted recombinant rhodopsin fragment of the histidine kinase rhodopsin HKR1 from Chlamydomonas reinhardtii were determined by picosecond titanium sapphire second harmonic laser pulse energy transmission measurement TE through RetA as a function of laser input peak energy density w0P. 目的探讨小儿手指挛缩性瘢痕激光治疗经验。方法采用自身前后对照研究方法,选取2021年12月至2022年12月,昆明市儿童医院皮肤科收治的30例手指挛缩性瘢痕患儿。其中17例瘢痕外观色红、充血明显,使用脉冲染料激光(PDL,585 nm)联合超脉冲CO... 目的探讨小儿手指挛缩性瘢痕激光治疗经验。方法采用自身前后对照研究方法,选取2021年12月至2022年12月,昆明市儿童医院皮肤科收治的30例手指挛缩性瘢痕患儿。其中17例瘢痕外观色红、充血明显,使用脉冲染料激光(PDL,585 nm)联合超脉冲CO_(2)点阵激光进行治疗;13例肤色瘢痕单独使用超脉冲CO_(2)点阵激光进行治疗。每例患儿治疗3次,每次治疗间隔2个月。治疗前、治疗后分别采用温哥华瘢痕评分量表(VSS)对瘢痕情况进行评分,并记录治疗前后受累关节活动度的变化以及不良反应发生情况,治疗结束8周后进行疗效评价。结果所有病例治疗后与治疗前相比,VSS评分均明显下降(P<0.05),受累关节活动度明显优于首次治疗前(P<0.05)。疗效评价结果显示,显效19例、有效8例、无效3例,显效率63.3%,有效率90.0%。治疗区域治疗后均未观察到水疱、感染、增生性瘢痕形成等不良反应。结论激光治疗小儿手指挛缩性瘢痕效果明显,相关并发症少,值得临床推广。 展开更多

目的:探讨不同间隔时间脉冲染料激光(Pulsed dye laser,PDL)治疗痤疮瘢痕的临床疗效。方法:回顾性分析2018年8月-2020年10月笔者科室采用PDL治疗的150例痤疮瘢痕患者病例资料,按照治疗周期不同分为A组(50例)、B组(51例)和C组(49例),A组... 目的:探讨不同间隔时间脉冲染料激光(Pulsed dye laser,PDL)治疗痤疮瘢痕的临床疗效。方法:回顾性分析2018年8月-2020年10月笔者科室采用PDL治疗的150例痤疮瘢痕患者病例资料,按照治疗周期不同分为A组(50例)、B组(51例)和C组(49例),A组每2周治疗一次,B组每3周治疗一次,C组每4周治疗一次,共治疗3个月,比较治疗前后三组患者的痤疮瘢痕疗效、VISIA面部检测、皮肤生理指标及瘢痕恢复情况。结果:B组治疗后总有效率为78.43%,高于A组和C组(χ^(2)=7.594,P<0.05);B组治疗后色斑、皱纹、纹理、毛孔及紫质均低于A组和C组,差异具有统计学意义(P<0.05);B组患者治疗后红斑指数、黑素指数及TEWL低于A组和C组,角质层含水量高于A组和C组,差异具有统计学意义(P<0.05);B组患者治疗后的红斑持续时间、水肿时间及创面疼痛消退时间低于A组和C组,差异具有统计学意义(P<0.05)。结论:间隔时间为3周的脉冲染料激光治疗痤疮瘢痕可以快速促进成纤维细胞生成真皮胶原蛋白,降低瘢痕增生和VISIA分值,改善皮肤生理状态,同时加快患者恢复时间。 展开更多
