The technology of tunnel boring machine(TBM)has been widely applied for underground construction worldwide;however,how to ensure the TBM tunneling process safe and efficient remains a major concern.Advance rate is a k...The technology of tunnel boring machine(TBM)has been widely applied for underground construction worldwide;however,how to ensure the TBM tunneling process safe and efficient remains a major concern.Advance rate is a key parameter of TBM operation and reflects the TBM-ground interaction,for which a reliable prediction helps optimize the TBM performance.Here,we develop a hybrid neural network model,called Attention-ResNet-LSTM,for accurate prediction of the TBM advance rate.A database including geological properties and TBM operational parameters from the Yangtze River Natural Gas Pipeline Project is used to train and test this deep learning model.The evolutionary polynomial regression method is adopted to aid the selection of input parameters.The results of numerical exper-iments show that our Attention-ResNet-LSTM model outperforms other commonly-used intelligent models with a lower root mean square error and a lower mean absolute percentage error.Further,parametric analyses are conducted to explore the effects of the sequence length of historical data and the model architecture on the prediction accuracy.A correlation analysis between the input and output parameters is also implemented to provide guidance for adjusting relevant TBM operational parameters.The performance of our hybrid intelligent model is demonstrated in a case study of TBM tunneling through a complex ground with variable strata.Finally,data collected from the Baimang River Tunnel Project in Shenzhen of China are used to further test the generalization of our model.The results indicate that,compared to the conventional ResNet-LSTM model,our model has a better predictive capability for scenarios with unknown datasets due to its self-adaptive characteristic.展开更多
BACKGROUND Locally advanced gastric cancer(LAGC)is a common malignant tumor.In recent years,neoadjuvant chemotherapy has gradually become popular for the treatment of LAGC.AIM To investigate the efficacy of oxaliplati...BACKGROUND Locally advanced gastric cancer(LAGC)is a common malignant tumor.In recent years,neoadjuvant chemotherapy has gradually become popular for the treatment of LAGC.AIM To investigate the efficacy of oxaliplatin combined with a tigio neoadjuvant chemotherapy regimen vs a conventional chemotherapy regimen for LAGC.METHODS Ninety patients with LAGC were selected and randomly divided into control and study groups with 45 patients in each group,according to the numerical table method.The control group was treated with conventional chemotherapy,and the study group was treated with oxaliplatin combined with tigio-neoadjuvant che-motherapy.The primary outcome measures were the clinical objective response rate(ORR)and surgical resection rate(SRR),whereas the secondary outcome measures were safety and Karnofsky Performance Status score.RESULTS The ORR in the study group was 80.00%,which was significantly higher than that of the control group(57.78%).In the study group,SRR was 75.56%,which was significantly higher than that of the control group(57.78%).There were 15.56%adverse reactions in the study group and 35.56%in the control group.These differences were statistically significant between the two groups.CONCLUSION The combination of oxaliplatin and tigio before surgery as neoadjuvant chemotherapy for patients with LAGC can effectively improve the ORR and SRR and is safe.展开更多
This study aims to develop several optimization techniques for predicting advance rate of tunnel boring machine(TBM)in different weathered zones of granite.For this purpose,extensive field and laboratory studies have ...This study aims to develop several optimization techniques for predicting advance rate of tunnel boring machine(TBM)in different weathered zones of granite.For this purpose,extensive field and laboratory studies have been conducted along the 12,649 m of the Pahang-Selangor raw water transfer tunnel in Malaysia.Rock properties consisting of uniaxial compressive strength(UCS),Brazilian tensile strength(BTS),rock mass rating(RMR),rock quality designation(RQD),quartz content(q)and weathered zone as well as machine specifications including thrust force and revolution per minute(RPM)were measured to establish comprehensive datasets for optimization.Accordingly,to estimate the advance rate of TBM,two new hybrid optimization techniques,i.e.an artificial neural network(ANN)combined with both imperialist competitive algorithm(ICA)and particle swarm optimization(PSO),were developed for mechanical tunneling in granitic rocks.Further,the new hybrid optimization techniques were compared and the best one was chosen among them to be used for practice.To evaluate the accuracy of the proposed models for both testing and training datasets,various statistical indices including coefficient of determination(R^2),root mean square error(RMSE)and variance account for(VAF)were utilized herein.The values of R^2,RMSE,and VAF ranged in 0.939-0.961,0.022-0.036,and 93.899-96.145,respectively,with the PSO-ANN hybrid technique demonstrating the best performance.It is concluded that both the optimization techniques,i.e.PSO-ANN and ICA-ANN,could be utilized for predicting the advance rate of TBMs;however,the PSO-ANN technique is superior.展开更多
Interest in the possibility of storing and transporting natural gas in the form of clathrate hydrates has been increasing in recent years, particularly in some gas-importing and exporting countries.The technologies ne...Interest in the possibility of storing and transporting natural gas in the form of clathrate hydrates has been increasing in recent years, particularly in some gas-importing and exporting countries.The technologies necessary for realizing this possibility may be classified into those relevant to the four serial processes (a) the formation of a hydrate, (b) the processing (dewatering, pelletizing, etc. ) of the formed hydrate, (c) the storage and transportation of the processed hydrate, and (d) the regasification (dissociation) of the hydrate. The technological development of any of these processes is still at an early stage. For hydrate formation, for example, various rival operations have been proposed. However,many of them have never been subjected to actual tests for practical use. More efforts are required for examining the different hydrate-formation technologies and for rating them by comparison. The general design of the processing of the formed hydrate inevitably depends on both the hydrate-formation process and the storage/transportation process, hence it has a wide variability. The major uncertainty in the storage-process design lies in the as-yet unclarified utility of the "self-preservation" effect of the naturalgas hydrates. The process design as well as the relevant cost evaluation should strongly depend on whether the hydrates are well preserved at atmospheric pressure in large-scale storage facilities. The regasification process has been studied less extensively than the former processes. The state of the art of the technological development in each of the serial processes is reviewed, placing emphasis on the hydrate formation process.展开更多
A new approach for prediction of face advance rete (FAR) prior to mining operation and determination of the operation efficiency after mining operation in retreat longwall mining panel is presented based upon the conc...A new approach for prediction of face advance rete (FAR) prior to mining operation and determination of the operation efficiency after mining operation in retreat longwall mining panel is presented based upon the concepts of rock engineering system (RES). For this purpose, six longwall panels considered in Parvadeh-I coal mine. Seven major effective parameters on FAR was selected including coal mine roof rating, gas propagation, safety factor of longwall face, ratio of joint spacing to cutting depth at longwall face, longwall face inclination, panel width, floor rock mass rating. To performance evaluation of the presented model, the relationship between the average vulnerability indexes of advance operation with FAR was determined in considered panels with coefficient of determination (R2) equal to 0.884 that indicate relatively acceptable correlation and compatibility. Investigations of the research indicated that it is possible to determine the actual operation efficiency under fair conditions by a RES-based model. The inevitable reduction of FAR for each longwall panel was determined by presented model that the difference amount between the maximum possible practical face advance rate (FARmpp) and recorded actual face advance rate (FARa) indicate the operation efficiency. Applied approach in this paper can be used to prediction of FAR in retreat longwall mining panel for same conditions that can have many benefits, including better and more accurate planning for the sales market and mine operation. Also, presented method in this paper can be applied as a useful tool to determination of actual operation efficiency for other sections and extraction methods in coal mines.展开更多
AIM:To determine the feasibility and safety of high dose rate intraluminal brachytherapy(HDR-ILBT) boost during preoperative chemoradiation for rectal cancer.METHODS:Between 2008 and 2009,thirty-six patients with loca...AIM:To determine the feasibility and safety of high dose rate intraluminal brachytherapy(HDR-ILBT) boost during preoperative chemoradiation for rectal cancer.METHODS:Between 2008 and 2009,thirty-six patients with locally advanced rectal cancer(≥ T3 or N+),were treated initially with concurrent capecitabine(825 mg/m2 oral twice daily) and pelvic external beam radiotherapy(EBRT)(45 Gy in 25 fractions),then were randomized to group A;HDR-ILBT group(n = 17) to receive 5.5-7 Gy×2 to gross tumor volume(GTV) and group B;EBRT group(n = 19) to receive 5.4 Gy×3 fractions to GTV with EBRT.All patients underwent total mesorectal excision.RESULTS:Grade 3 acute toxicities were registered in 12 patients(70.6%) in group A and in 8(42.1%) in group B.Complete pathologic response of T stage(ypT0) in group A was registered in 10 patients(58.8%) and in group B,3 patients(15.8%) had ypT0(P < 0.0001).Sphincter preservation was reported in 6/9 patients(66.7%) in group A and in 5/10 patients(50%) in group B(P < 0.01).Overall radiological response was 68.15% and 66.04% in Group A and B,respectively.During a median follow up of 18 mo,late grade 1 and 2 sequelae were registered in 3 patients(17.6%) and 4 patients(21.1%) in the groups A and B,respectively.CONCLUSION:HDR-ILBT was found to be effective dose escalation technique in preoperative chemoradiation for rectal cancers,with higher response rates,downstaging and with manageable acute toxicities.展开更多

Oncological screening of CRC has a major role in the management of the disease and it is mostly performed by colonoscopy. Anyway, effectiveness of endoscopic screening for CRC strictly depends on adequate detection and removal of potentially precancerous lesions, and accuracy of colonoscopy in detection of adenomas is still suboptimal. For this reason, several technological advances have been implemented in order to improve the diagnostic sensitivity of colonoscopy in adenoma detection. Among these:(1) Visual technologies such as chromoendoscopy and narrow band imaging;(2) optical innovation as high definition endoscopy, full-spectrum endoscopy or Third Eye Retroscope; and(3) mechanical advances as Cap assisted colonoscopy, Endocuff, Endoring and G-Eye endoscope. Methods: We conducted a retrospective chart review in intensiv...Background: The optimal rate of feeding advancement after initiation of early enteral nutrition (EEN) for underweight, critically ill patients is unknown. Methods: We conducted a retrospective chart review in intensive care unit (ICU) patients with a body mass index (BMI) < 20.0 kg/m<sup>2</sup>. Patients were categorized into Group R, which reached the energy target within 3 days of EEN initiation, and Group S, which reached the energy target 4 or more days after EEN initiation. Results: A total of 65 patients with a median age of 73 years were included in the study. No significant differences were observed between the two groups for all-cause mortality, ICU-free days, or length of hospital stay. Ventilator-free days (VFDs) were significantly fewer in Group R than in Group S (18.0 [0.0 - 22.0] vs. 21.0 [16.3 - 24.8] days;P = 0.046). A significantly higher number of patients requiring mechanical ventilation (MV) at hospital discharge were observed in Group R than in Group S (29% vs. 8%;P = 0.030). Multivariable analyses with adjustment for confounders found that days required to reach target energy intake after EEN initiation were significantly and independently associated with the requirement for MV at hospital discharge, but not with VFDs. Conclusion: A slow rate of feeding advancement after initiation of EEN in critically ill patients having a BMI of <20.0 kg/m<sup>2</sup> might be associated with a reduced requirement for MV at hospital discharge. These results require confirmation in a large multicenter trial of underweight, critically ill patients.展开更多

Patients with locally advanced perihilar CCA requiring NACCRT were defined as follows: (1) a mass involving unilateral branches of the portal vein or hepatic artery with insufficient volume of the anticipated remnant lobe; or (2) an infiltrating mass in the main portal vein that was too long for reconstruction, identified at preoperative staging.RESULTSThe median disease-free survival (DFS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 26.0 and 15.1 mo, respectively (P = 0.91). The median overall survival (OS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 32.9 and 27.1 mo, respectively (P = 0.26). The NACCRT group showed a downstaging tendency compared to the non-NACCRT group as compared with the tumor stage confirmed by histological examination after surgery and the tumor stage confirmed by imaging test at the time of diagnosis (P = 0.01).CONCLUSIONNACCRT does not prolong DFS and OS in localized or locally advanced perihilar CCA. However, NACCRT may allow tumor downstaging and improve tumor resectability.展开更多
Objective The aim of the study was to evaluate the role of postoperative sequential chemotherapy and radiotherapy in patients with locally advanced gastric cancer.Methods From January 2003 to December 2010, 146 gastri...Objective The aim of the study was to evaluate the role of postoperative sequential chemotherapy and radiotherapy in patients with locally advanced gastric cancer.Methods From January 2003 to December 2010, 146 gastric cancer patients at our institution(Department of Oncology, Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, Wuhan, China) received postoperative sequential chemotherapy and radiotherapy after radical surgery. Radiotherapy was administered as a dose of 4500 cGy in 25 fractions. For patients with positive margins, the dose was raised to 5040 cGy in 28 fractions. Three cycles of m FOLFOX or PF(cisplatin, 5-fluorouracil) chemotherapy regimen were applied before and after radiotherapy. Three-and 5-year survival rates were analyzed; any adverse effects with respect to hematology, hepatic and renal function, or the gastrointestinal tract that occurred during the treatment were evaluated.Results This cohort consisted of non-metastatic patients: 104 men and 42 women with a median age of 51.0 years. The full course of sequential chemotherapy and radiotherapy(4500–5040 cGy) was completed by 129 patients(88.4%). Seventeen regional relapses(9.8%) and 46 distant relapses(23.8%) were recorded. Fifty patients(34.2%) died during follow-up. The 3-and 5-year overall survival rates(OS) were 60% and 54%, and disease-free survival rates(DFS) were 53% and 47%, respectively. There were no significant differences in survival rate with respect to age, sex, histopathology, N stage, site of the tumor, or margin status. Multivariate analysis showed that only the depth of tumor invasion(T stage) was an independent prognostic factor for OS(P = 0.009) and DFS(P = 0.006). The rates of grades 3 and 4 neutropenia and vomiting were 9.6% and 3.4%, respectively, during the treatment.Conclusion Postoperative sequential chemotherapy with an m FOLFOX or PF regimen and radiotherapy were found to be an effective means of treating advanced gastric cancer patients with T3–T4 disease. The adverse effects of this treatment were tolerable.展开更多

The study was to evaluate the efficacy and toxicities of cisplatin plus 5-fluorouracil combined with concurrent radiotherapy for locally advanced nasopharyngeal carcinoma. Methods: Sixty-six patients with locally advanced nasopharyngeal carcinoma received chemotherapy of cisplatin plus 5-fluorouracil jointing concurrent radiotherapy; concurrent radiotherapy started on day 1 in the first cycle of chemotherapy of PF, 5-fluorouraci1500 mg/m^2 intravenous infusion on days 1-5, cisplatin 80 mg/m2 intravenous infusion on days 1-3, 21 days for a cycle, a total of three cycles; nasopharyngeal lesions and positive lymph node were given a total amount of 70 Gy, prophylactic neck radiation were given the amount of 50 Gy, radiotherapy five times a week, Gy/f. Results: All patients who can be evaluated, the response rate (RR) was 100%; 1-, 3- and 5-year overall survival (OS) were 100%, 86.4%, 21.2%, respectively; 3-year, 5-year disease-free survival (DFS) were 72.7% and 18.2%, respective- ly; the average survival time and median survival time were 49.0 months and 48.5 months, respectively; the average survival time and median survival time of DFS were 46.1 months and 46.5 months, respectively. Conclusion: For patients with locally advanced nasopharyngeal carcinoma, who accepted RT combining concurrent chemotherapy of cisplatin plus 5-fluorouracil, clinical efficacy is satisfaction and toxicities could be tolerated.展开更多

背景随着我国经济的高速发展和现代人生育观念的转变,高龄产妇的数量正在逐年增加。相较于适龄产妇,高龄产妇更可能因自身、家庭、工作等原因产生抑郁症状。目的系统评价我国高龄产妇(分娩时年龄≥35岁)产后抑郁检出率,为早期预防和干...背景随着我国经济的高速发展和现代人生育观念的转变,高龄产妇的数量正在逐年增加。相较于适龄产妇,高龄产妇更可能因自身、家庭、工作等原因产生抑郁症状。目的系统评价我国高龄产妇(分娩时年龄≥35岁)产后抑郁检出率,为早期预防和干预提供相关依据。方法在中国知网、万方数据知识服务平台、维普网、中国生物医学文献数据库和PubMed、Cochrane Library、Embase、Web of Science数据库中以主题词和自由词相结合的方式检索与中国高龄产妇产后抑郁检出率相关的文献,检索时限为数据库建库至2023年7月。文献筛选、资料提取和质量评价过程由2名研究员独立进行。使用Stata 16.0软件进行数据分析。结果共纳入21篇文献,样本量合计5163例。Meta分析结果显示,我国高龄产妇产后抑郁总检出率为20.0%(95%CI=17.4%~22.6%)。亚组分析结果显示,年龄<40岁高龄产妇产后抑郁检出率为19.5%(95%CI=13.8%~25.3%),≥40岁为40.3%(95%CI=11.4%~69.3%);文化程度高中以上为19.7%(95%CI=11.7%~27.7%),高中及以下为30.7%(95%CI=19.1%~42.3%);初产妇为21.1%(95%CI=14.4%~27.9%),经产妇为16.2%(95%CI=12.9%~19.6%);自然分娩为16.4%(95%CI=12.2%~20.6%),剖宫产为27.8%(95%CI=20.9%~34.8%);新生儿性别男性为20.7%(95%CI=15.6%~25.8%),女性为27.3%(95%CI=20.5%~34.0%);有妊娠并发症为38.7%(95%CI=22.6%~54.8%),无妊娠并发症为11.7%(95%CI=7.6%~15.8%);有不良孕产史为29.5%(95%CI=17.9%~41.1%),无不良孕产史为27.7%(95%CI=16.6%~38.8%);发表年份为2020年以前为18.0%(95%CI=16.5%~19.4%),发表年份为2020年及以后为19.5%(95%CI=18.0%~21.0%);调查地区北方为20.4%(95%CI=18.2%~22.6%),南方为18.2%(95%CI=17.0%~19.4%);爱丁堡产后抑郁量表(EPDS)评分≥13分为20.0%(95%CI=18.5%~21.5%),≥10分为16.9%(95%CI=15.2%~18.5%)。Egger’s检验(t=1.76,P=0.095)和Begg’s检验(Z=1.48,P=0.147)提示不存在明显发表偏倚。结论我国高龄产妇产后抑郁检出率较高(20.0%),其中年龄≥40岁、文化程度高中及以下、初产妇、剖宫产、新生儿性别为女性、有妊娠并发症、有不良孕产史、发表年份为2020年及以后、调查地区为北方、EPDS评分≥13分的高龄产妇产后抑郁检出率较高,应重视以上高龄产妇的心理状况,并制定相应的预防和干预措施。展开更多

文摘BACKGROUND Locally advanced gastric cancer(LAGC)is a common malignant tumor.In recent years,neoadjuvant chemotherapy has gradually become popular for the treatment of LAGC.AIM To investigate the efficacy of oxaliplatin combined with a tigio neoadjuvant chemotherapy regimen vs a conventional chemotherapy regimen for LAGC.METHODS Ninety patients with LAGC were selected and randomly divided into control and study groups with 45 patients in each group,according to the numerical table method.The control group was treated with conventional chemotherapy,and the study group was treated with oxaliplatin combined with tigio-neoadjuvant che-motherapy.The primary outcome measures were the clinical objective response rate(ORR)and surgical resection rate(SRR),whereas the secondary outcome measures were safety and Karnofsky Performance Status score.RESULTS The ORR in the study group was 80.00%,which was significantly higher than that of the control group(57.78%).In the study group,SRR was 75.56%,which was significantly higher than that of the control group(57.78%).There were 15.56%adverse reactions in the study group and 35.56%in the control group.These differences were statistically significant between the two groups.CONCLUSION The combination of oxaliplatin and tigio before surgery as neoadjuvant chemotherapy for patients with LAGC can effectively improve the ORR and SRR and is safe.
文摘This study aims to develop several optimization techniques for predicting advance rate of tunnel boring machine(TBM)in different weathered zones of granite.For this purpose,extensive field and laboratory studies have been conducted along the 12,649 m of the Pahang-Selangor raw water transfer tunnel in Malaysia.Rock properties consisting of uniaxial compressive strength(UCS),Brazilian tensile strength(BTS),rock mass rating(RMR),rock quality designation(RQD),quartz content(q)and weathered zone as well as machine specifications including thrust force and revolution per minute(RPM)were measured to establish comprehensive datasets for optimization.Accordingly,to estimate the advance rate of TBM,two new hybrid optimization techniques,i.e.an artificial neural network(ANN)combined with both imperialist competitive algorithm(ICA)and particle swarm optimization(PSO),were developed for mechanical tunneling in granitic rocks.Further,the new hybrid optimization techniques were compared and the best one was chosen among them to be used for practice.To evaluate the accuracy of the proposed models for both testing and training datasets,various statistical indices including coefficient of determination(R^2),root mean square error(RMSE)and variance account for(VAF)were utilized herein.The values of R^2,RMSE,and VAF ranged in 0.939-0.961,0.022-0.036,and 93.899-96.145,respectively,with the PSO-ANN hybrid technique demonstrating the best performance.It is concluded that both the optimization techniques,i.e.PSO-ANN and ICA-ANN,could be utilized for predicting the advance rate of TBMs;however,the PSO-ANN technique is superior.
文摘Interest in the possibility of storing and transporting natural gas in the form of clathrate hydrates has been increasing in recent years, particularly in some gas-importing and exporting countries.The technologies necessary for realizing this possibility may be classified into those relevant to the four serial processes (a) the formation of a hydrate, (b) the processing (dewatering, pelletizing, etc. ) of the formed hydrate, (c) the storage and transportation of the processed hydrate, and (d) the regasification (dissociation) of the hydrate. The technological development of any of these processes is still at an early stage. For hydrate formation, for example, various rival operations have been proposed. However,many of them have never been subjected to actual tests for practical use. More efforts are required for examining the different hydrate-formation technologies and for rating them by comparison. The general design of the processing of the formed hydrate inevitably depends on both the hydrate-formation process and the storage/transportation process, hence it has a wide variability. The major uncertainty in the storage-process design lies in the as-yet unclarified utility of the "self-preservation" effect of the naturalgas hydrates. The process design as well as the relevant cost evaluation should strongly depend on whether the hydrates are well preserved at atmospheric pressure in large-scale storage facilities. The regasification process has been studied less extensively than the former processes. The state of the art of the technological development in each of the serial processes is reviewed, placing emphasis on the hydrate formation process.
文摘A new approach for prediction of face advance rete (FAR) prior to mining operation and determination of the operation efficiency after mining operation in retreat longwall mining panel is presented based upon the concepts of rock engineering system (RES). For this purpose, six longwall panels considered in Parvadeh-I coal mine. Seven major effective parameters on FAR was selected including coal mine roof rating, gas propagation, safety factor of longwall face, ratio of joint spacing to cutting depth at longwall face, longwall face inclination, panel width, floor rock mass rating. To performance evaluation of the presented model, the relationship between the average vulnerability indexes of advance operation with FAR was determined in considered panels with coefficient of determination (R2) equal to 0.884 that indicate relatively acceptable correlation and compatibility. Investigations of the research indicated that it is possible to determine the actual operation efficiency under fair conditions by a RES-based model. The inevitable reduction of FAR for each longwall panel was determined by presented model that the difference amount between the maximum possible practical face advance rate (FARmpp) and recorded actual face advance rate (FARa) indicate the operation efficiency. Applied approach in this paper can be used to prediction of FAR in retreat longwall mining panel for same conditions that can have many benefits, including better and more accurate planning for the sales market and mine operation. Also, presented method in this paper can be applied as a useful tool to determination of actual operation efficiency for other sections and extraction methods in coal mines.
文摘AIM:To determine the feasibility and safety of high dose rate intraluminal brachytherapy(HDR-ILBT) boost during preoperative chemoradiation for rectal cancer.METHODS:Between 2008 and 2009,thirty-six patients with locally advanced rectal cancer(≥ T3 or N+),were treated initially with concurrent capecitabine(825 mg/m2 oral twice daily) and pelvic external beam radiotherapy(EBRT)(45 Gy in 25 fractions),then were randomized to group A;HDR-ILBT group(n = 17) to receive 5.5-7 Gy×2 to gross tumor volume(GTV) and group B;EBRT group(n = 19) to receive 5.4 Gy×3 fractions to GTV with EBRT.All patients underwent total mesorectal excision.RESULTS:Grade 3 acute toxicities were registered in 12 patients(70.6%) in group A and in 8(42.1%) in group B.Complete pathologic response of T stage(ypT0) in group A was registered in 10 patients(58.8%) and in group B,3 patients(15.8%) had ypT0(P < 0.0001).Sphincter preservation was reported in 6/9 patients(66.7%) in group A and in 5/10 patients(50%) in group B(P < 0.01).Overall radiological response was 68.15% and 66.04% in Group A and B,respectively.During a median follow up of 18 mo,late grade 1 and 2 sequelae were registered in 3 patients(17.6%) and 4 patients(21.1%) in the groups A and B,respectively.CONCLUSION:HDR-ILBT was found to be effective dose escalation technique in preoperative chemoradiation for rectal cancers,with higher response rates,downstaging and with manageable acute toxicities.
文摘Colorectal cancer(CRC) is the third most common cancer in males and second in females, and globally the fourth cause for cancer death worldwide. Oncological screening of CRC has a major role in the management of the disease and it is mostly performed by colonoscopy. Anyway, effectiveness of endoscopic screening for CRC strictly depends on adequate detection and removal of potentially precancerous lesions, and accuracy of colonoscopy in detection of adenomas is still suboptimal. For this reason, several technological advances have been implemented in order to improve the diagnostic sensitivity of colonoscopy in adenoma detection. Among these:(1) Visual technologies such as chromoendoscopy and narrow band imaging;(2) optical innovation as high definition endoscopy, full-spectrum endoscopy or Third Eye Retroscope; and(3) mechanical advances as Cap assisted colonoscopy, Endocuff, Endoring and G-Eye endoscope. All these technologies advances have been tested over time by clinical studies with mixed results. Which of them is more likely to be successful in the next future?

文摘In this short review,we discuss a few recent advances in calculating the nonradiative decay rates for point defects in semiconductors.We briefly review the debates and connections of using different formalisms to calculate the multi-phonon processes.We connect Dr.Huang's formula with Marcus theory formula in the high temperature limit,and point out that Huang's formula provide an analytical expression for the phonon induced electron coupling constant in the Marcus theory formula.We also discussed the validity of 1D formula in dealing with the electron transition processes,and practical ways to correct the anharmonic effects.
文摘Background: The optimal rate of feeding advancement after initiation of early enteral nutrition (EEN) for underweight, critically ill patients is unknown. Methods: We conducted a retrospective chart review in intensive care unit (ICU) patients with a body mass index (BMI) < 20.0 kg/m<sup>2</sup>. Patients were categorized into Group R, which reached the energy target within 3 days of EEN initiation, and Group S, which reached the energy target 4 or more days after EEN initiation. Results: A total of 65 patients with a median age of 73 years were included in the study. No significant differences were observed between the two groups for all-cause mortality, ICU-free days, or length of hospital stay. Ventilator-free days (VFDs) were significantly fewer in Group R than in Group S (18.0 [0.0 - 22.0] vs. 21.0 [16.3 - 24.8] days;P = 0.046). A significantly higher number of patients requiring mechanical ventilation (MV) at hospital discharge were observed in Group R than in Group S (29% vs. 8%;P = 0.030). Multivariable analyses with adjustment for confounders found that days required to reach target energy intake after EEN initiation were significantly and independently associated with the requirement for MV at hospital discharge, but not with VFDs. Conclusion: A slow rate of feeding advancement after initiation of EEN in critically ill patients having a BMI of <20.0 kg/m<sup>2</sup> might be associated with a reduced requirement for MV at hospital discharge. These results require confirmation in a large multicenter trial of underweight, critically ill patients.
文摘To clarify the role of neoadjuvant concurrent chemoradiotherapy (NACCRT) followed by surgical resection for localized or locally advanced perihilar cholangiocarcinoma (CCA).METHODSWe retrospectively reviewed 57 patients who underwent surgical resection with or without NACCRT for perihilar CCA; 12 patients received NACCRT and 45 patients did not received NACCRT. Patients with locally advanced perihilar CCA requiring NACCRT were defined as follows: (1) a mass involving unilateral branches of the portal vein or hepatic artery with insufficient volume of the anticipated remnant lobe; or (2) an infiltrating mass in the main portal vein that was too long for reconstruction, identified at preoperative staging.RESULTSThe median disease-free survival (DFS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 26.0 and 15.1 mo, respectively (P = 0.91). The median overall survival (OS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 32.9 and 27.1 mo, respectively (P = 0.26). The NACCRT group showed a downstaging tendency compared to the non-NACCRT group as compared with the tumor stage confirmed by histological examination after surgery and the tumor stage confirmed by imaging test at the time of diagnosis (P = 0.01).CONCLUSIONNACCRT does not prolong DFS and OS in localized or locally advanced perihilar CCA. However, NACCRT may allow tumor downstaging and improve tumor resectability.
文摘Objective The aim of the study was to evaluate the role of postoperative sequential chemotherapy and radiotherapy in patients with locally advanced gastric cancer.Methods From January 2003 to December 2010, 146 gastric cancer patients at our institution(Department of Oncology, Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, Wuhan, China) received postoperative sequential chemotherapy and radiotherapy after radical surgery. Radiotherapy was administered as a dose of 4500 cGy in 25 fractions. For patients with positive margins, the dose was raised to 5040 cGy in 28 fractions. Three cycles of m FOLFOX or PF(cisplatin, 5-fluorouracil) chemotherapy regimen were applied before and after radiotherapy. Three-and 5-year survival rates were analyzed; any adverse effects with respect to hematology, hepatic and renal function, or the gastrointestinal tract that occurred during the treatment were evaluated.Results This cohort consisted of non-metastatic patients: 104 men and 42 women with a median age of 51.0 years. The full course of sequential chemotherapy and radiotherapy(4500–5040 cGy) was completed by 129 patients(88.4%). Seventeen regional relapses(9.8%) and 46 distant relapses(23.8%) were recorded. Fifty patients(34.2%) died during follow-up. The 3-and 5-year overall survival rates(OS) were 60% and 54%, and disease-free survival rates(DFS) were 53% and 47%, respectively. There were no significant differences in survival rate with respect to age, sex, histopathology, N stage, site of the tumor, or margin status. Multivariate analysis showed that only the depth of tumor invasion(T stage) was an independent prognostic factor for OS(P = 0.009) and DFS(P = 0.006). The rates of grades 3 and 4 neutropenia and vomiting were 9.6% and 3.4%, respectively, during the treatment.Conclusion Postoperative sequential chemotherapy with an m FOLFOX or PF regimen and radiotherapy were found to be an effective means of treating advanced gastric cancer patients with T3–T4 disease. The adverse effects of this treatment were tolerable.

文摘Fault fracture zones and water-bearing bodies in front of the driving head are the main disasters in mine laneways,thus it is important to perform their advanced detection and prediction in advance in order to provide reliable technical support for the excavation.Based on the electromagnetic induction theory,we analyzed the characteristics of primary and secondary fields with a positive and negative wave form of current,proposed the fine processing of the advanced detection with variation rate of apparent resistivity and introduced in detail the computational formulae and procedures.The result of physical simulation experiments illustrate that the tectonic interface of modules can be judged by first-order rate of apparent resistivity with a boundary error of 5%,and the position of water body determined by the fine analysis method agrees well with the result of borehole drilling.This shows that in terms of distinguishing structure and aqueous anomalies,the first-order rate of apparent resistivity is more sensitive than the secondorder rate of apparent resistivity.However,some remaining problems are suggested for future solutions.
文摘Objective: The prognosis of patients with nasopharyngeal carcinoma (NPC) depends on the stage of the disease at diagnosis. Unfortunately, at diagnosis, most of patients have locally advanced, non-metastatic stage III or IVa disease. The study was to evaluate the efficacy and toxicities of cisplatin plus 5-fluorouracil combined with concurrent radiotherapy for locally advanced nasopharyngeal carcinoma. Methods: Sixty-six patients with locally advanced nasopharyngeal carcinoma received chemotherapy of cisplatin plus 5-fluorouracil jointing concurrent radiotherapy; concurrent radiotherapy started on day 1 in the first cycle of chemotherapy of PF, 5-fluorouraci1500 mg/m^2 intravenous infusion on days 1-5, cisplatin 80 mg/m2 intravenous infusion on days 1-3, 21 days for a cycle, a total of three cycles; nasopharyngeal lesions and positive lymph node were given a total amount of 70 Gy, prophylactic neck radiation were given the amount of 50 Gy, radiotherapy five times a week, Gy/f. Results: All patients who can be evaluated, the response rate (RR) was 100%; 1-, 3- and 5-year overall survival (OS) were 100%, 86.4%, 21.2%, respectively; 3-year, 5-year disease-free survival (DFS) were 72.7% and 18.2%, respective- ly; the average survival time and median survival time were 49.0 months and 48.5 months, respectively; the average survival time and median survival time of DFS were 46.1 months and 46.5 months, respectively. Conclusion: For patients with locally advanced nasopharyngeal carcinoma, who accepted RT combining concurrent chemotherapy of cisplatin plus 5-fluorouracil, clinical efficacy is satisfaction and toxicities could be tolerated.

文摘Cutting breeding is an important method of asexual reproduction of Xanthoceras sorbifolium Bunge.At present,it has been found that the factors influencing the cuttings of X.sorbifolium mainly include cutting material type,substrate type,cutting season,cutting material length and thickness,mother tree age,cutting material pretreatment method,hormone species concentration and soaking time.Besides,different regions,control conditions,and germplasm types have different cutting rooting rates.This paper introduced some of the problems in the cuttings of X.sorbifolium,and came up with some recommendations to provide a reference for the future research and technical promotion of X.sorbifolium cuttings.
文摘Presently most of research results on strata behaviors were concluded under the normal (moderate) advance rate conditions. Comparing with normal advance rate, fast advance rate and slow advance rate may cause some new changes of strata behaviors. The characteristics of ground pressure behaviors in working face with great mining height at different forward speeds were studied by similarity model experiment in laboratory. Its conclusion provides a theoretic basis for ground pressure controlling.
文摘背景随着我国经济的高速发展和现代人生育观念的转变,高龄产妇的数量正在逐年增加。相较于适龄产妇,高龄产妇更可能因自身、家庭、工作等原因产生抑郁症状。目的系统评价我国高龄产妇(分娩时年龄≥35岁)产后抑郁检出率,为早期预防和干预提供相关依据。方法在中国知网、万方数据知识服务平台、维普网、中国生物医学文献数据库和PubMed、Cochrane Library、Embase、Web of Science数据库中以主题词和自由词相结合的方式检索与中国高龄产妇产后抑郁检出率相关的文献,检索时限为数据库建库至2023年7月。文献筛选、资料提取和质量评价过程由2名研究员独立进行。使用Stata 16.0软件进行数据分析。结果共纳入21篇文献,样本量合计5163例。Meta分析结果显示,我国高龄产妇产后抑郁总检出率为20.0%(95%CI=17.4%~22.6%)。亚组分析结果显示,年龄<40岁高龄产妇产后抑郁检出率为19.5%(95%CI=13.8%~25.3%),≥40岁为40.3%(95%CI=11.4%~69.3%);文化程度高中以上为19.7%(95%CI=11.7%~27.7%),高中及以下为30.7%(95%CI=19.1%~42.3%);初产妇为21.1%(95%CI=14.4%~27.9%),经产妇为16.2%(95%CI=12.9%~19.6%);自然分娩为16.4%(95%CI=12.2%~20.6%),剖宫产为27.8%(95%CI=20.9%~34.8%);新生儿性别男性为20.7%(95%CI=15.6%~25.8%),女性为27.3%(95%CI=20.5%~34.0%);有妊娠并发症为38.7%(95%CI=22.6%~54.8%),无妊娠并发症为11.7%(95%CI=7.6%~15.8%);有不良孕产史为29.5%(95%CI=17.9%~41.1%),无不良孕产史为27.7%(95%CI=16.6%~38.8%);发表年份为2020年以前为18.0%(95%CI=16.5%~19.4%),发表年份为2020年及以后为19.5%(95%CI=18.0%~21.0%);调查地区北方为20.4%(95%CI=18.2%~22.6%),南方为18.2%(95%CI=17.0%~19.4%);爱丁堡产后抑郁量表(EPDS)评分≥13分为20.0%(95%CI=18.5%~21.5%),≥10分为16.9%(95%CI=15.2%~18.5%)。Egger’s检验(t=1.76,P=0.095)和Begg’s检验(Z=1.48,P=0.147)提示不存在明显发表偏倚。结论我国高龄产妇产后抑郁检出率较高(20.0%),其中年龄≥40岁、文化程度高中及以下、初产妇、剖宫产、新生儿性别为女性、有妊娠并发症、有不良孕产史、发表年份为2020年及以后、调查地区为北方、EPDS评分≥13分的高龄产妇产后抑郁检出率较高,应重视以上高龄产妇的心理状况,并制定相应的预防和干预措施。
