The number of solid organ transplantations performed annually is increasing and are increasing in the following order:Kidney, liver, heart, lung, pancreas, small bowel, and uterine transplants. However, the outcomes of transplants are impro-ving(organ survival>90%after the 1st year). Therefore, there is a high probability that a general surgeon will be faced with the management of a transplant patient with acute abdomen.Surgical problems in immunocompromised patients may not only include graft-related problems but also nongraft-related problems. The perioperative regulation of immunosuppression, the treatment of accompanying problems of immunosuppression, the administration of cortisol and, above all,the realization of a rapidly deteriorating situation and the accurate evaluation and interpretation of clinical manifestations are particularly important in these patients. The perioperative assessment and preparation includes evaluation of the patient’s cardiovascular system and determining if the patient has hypertension or suppression of the hypothalamic-pituitary-adrenal axis,or if the patient has had any coagulation mechanism abnormalities or thromboembolic episodes.Immunosuppression in transplant patients is associated with the use of calci-neurin inhibitors, corticosteroids, and antiproliferation agents. Many times, the clinical picture is atypical, resulting in delays in diagnosis and treatment and leading to increased morbidity and mortality.Multidetector computed tomo-graphy is of utmost importance for early diagnosis and management.Transplant recipients are prone to infections, especially specific infections caused by cytomegalovirus and Clostridium difficile, and they are predisposed to intraop-erative or postoperative complications that require great care and vigilance. It is necessary to follow evidence-based therapeutic protocols. Thus,it is required that the clinician choose the correct therapeutic plan for the patient(conservative, emergency open surgery or minimally invasive surgery, including laparoscopic or even robotic surgery). 展开更多

The large global outbreak of coronavirus disease 2019(COVID-19)has seriously endangered the health care system in China and globally. The sudden surge of patients with severe acute respiratory syndrome coronavirus 2 infection has revealed the shortage of critical care medicine resources and intensivists.Currently, the management of non-critically ill patients with COVID-19 is performed mostly by non-intensive care unit(ICU)physicians, who lack the required professional knowledge, training, and practice in critical care medicine, especially in terms of continuous monitoring of the respiratory function, intervention, and feedback on treatment effects. This clinical problem needs an urgent solution. Therefore, here,we propose a series of clinical strategies for non-ICU physicians aimed at the standardization of the management of non-critically ill patients with COVID-19 from the perspective of critical care medicine.Isolation management is performed to facilitate the implementation of hierarchical monitoring and intervention to ensure the reasonable distribution of scarce critical care medical resources and intensivists, highlight the key patients, timely detection of disease progression, and early and appropriate intervention and organ function support, and thus improve the prognosis.Different management objectives are also set based on the high-risk factors and the severity of patients with COVID-19.The approaches suggested herein will facilitate the timely detection of disease progression, and thus ensure the provision of early and appropriate intervention and organ function support, which will eventually improve the prognosis. 展开更多

Objective: To investigate patients' attitudes towards cancer pain management and analyze the factors influencing these attitudes. Methods: The self-developed Demographic and Disease-Related Information Questionnaires, Pain Management Barriers Questionnaire-Taiwan form(BQT), and Pain Knowledge Questionnaire were administered to 363 pairs of hospitalized cancer patients and their caregivers from the oncology departments of 7 hospitals in Beijing, China. Results: The average patient score for attitudes towards pain management was 2.96±0.49. The dimension scores indicated good attitudes in three areas(scores 〈2.5), "Desire to be good"(2.22±1.04), "Fatalism"(2.08±0.81) and"Religious fatalism"(1.86±1.00), and poor attitudes in six areas(scores ≥2.5), "Tolerance"(3.83±0.96), "Use of analgesics as needed(p.r.n.)"(3.73±1.01), "Addiction"(3.44±1.05), "Disease progression"(3.28±1.26), "Distraction of physicians"(3.16±1.07) and "Side effects"(2.99±0.68). Two factors were entered into the regression equation:the caregivers' attitudes towards cancer pain management and the patients' pain knowledge. These two factors explained 23.2% of the total variance in the patients' average scores for their attitudes towards cancer pain management. Conclusions: The patients' attitudes towards cancer pain management were poor and could be influenced by the caregivers' attitudes and the patients' pain knowledge, and thus need to be improved. 展开更多

Hemodynamic monitoring and optimization improve postoperative outcome during high-risk surgery. However, hemodynamic management practices among Chinese anesthesiologists are largely unknown. This study sought to evaluate the current intraoperative hemodynamic management practices for high-risk surgery patients in China. From September 2010 to November 2011,we surveyed anesthesiologists working in the operating rooms of 265 hospitals representing 28 Chinese provinces. All questionnaires were distributed to department chairs of anesthesiology or practicing anesthesiologists.Once completed, the 29-item questionnaires were collected and analyzed. Two hundred and 10 questionnaires from 265 hospitals in China were collected. We found that 91.4%of anesthesiologists monitored invasive arterial pressure,82.9%monitored central venous pressure(CVP),13.3%monitored cardiac output(CO),10.5%monitored mixed venous saturation, and less than 2%monitored pulse pressure variation(PPV) or systolic pressure variation(SPV) during high-risk surgery. The majority(88%) of anesthesiologists relied on clinical experience as an indicator for volume expansion and more than 80%relied on blood pressure,CVP and urine output.Anesthesiologists in China do not own enough attention on hemodynamic parameters such as PPV,SPV and CO during fluid management in high-risk surgical patients. The lack of CO monitoring may be attributed largely to the limited access to technologies, the cost of the devices and the lack of education on how to use them. There is a need for improving access to these technologies as well as an opportunity to create guidelines and education for hemodynamic optimization in China. 展开更多

Telemedicine is the use of information and communication technology to deliver healthcare at a distance. It has been resorted to during the COVID-19 pandemic to lessen the need for in-person patient care decreasing the risk of transmission, and it can be of benefit afterward in the management of cardiac disease. The elderly population has unique challenges concerning the use of telehealth technologies. We thus review the advances in telemedicine technologies in treating elderly cardiac patients including in our discussion only studies with a mean age of participants above 60.Remote monitoring of blood pressure, weight, and symptoms, along with home ECG recording has been found to be superior to usual in-clinic follow up.Combining remote monitoring with video conferencing with physicians, patient education websites, and applications is also of benefit.Remote monitoring of Implantable Cardioverter Defibrillators(ICD)and Cardiac Resynchronization Therapy Defibrillators(CRT-D)is also beneficial but can be at the cost of an increase in both appropriate and inappropriate interventions.Implantable sensing devices compatible with remote monitoring have been developed and have been shown to improve care and cost-effectiveness.New smartphone software can detect arrhythmias using home ECG recordings and can detect atrial fibrillation using smartphone cameras.Remote monitoring of implanted pacemakers has shown non-inferiority to in clinic follow up.On the other hand,small-scale questionnaire-based studies demonstrated the willingness of the elderly cardiac patients to use such technologies, and their satisfaction with their use and ease of use.Large-scale studies should further investigate useability in samples more representative of the general elderly population with more diverse socioeconomic and educational backgrounds.Accordingly,it seems that studying integrating multiple technologies into telehealth programs is of great value.Further efforts should also be put in validating the technologies for specific diseases along with the legal and reimbursement aspects of the use of telehealth. 展开更多

In 1956, my former Head and Professor of the Department of Obstetrics and Gynecology KT LIM, asked me to take the initiative to establish a reproductive endocrinology unit. As China is a developing country, we were then short of diagnostic facilities and therapeutic agents.
