Objective Human adenovirus(HAdV)infection is common and can develop to serious conditions with high mortality,yet the mechanism of HAdV infection remains unclear.In the present study,the serum metabolite profiles of HAdV-7-infected patients with pneumonia or upper respiratory tract infection(URTI)were explored.Methods In total,35 patients were enrolled in the study following an outbreak of HAdV-7 in the army,of whom 14 had pneumonia and 21 had URTI.Blood samples were collected at the acute stage and at the recovery stage and were analyzed by untargeted metabolomics.Results Over 90% of the differential metabolites identified between the pneumonia patients and URTI patients were lipids and lipid-like molecules,including glycerophospholipids,fatty acyls,and sphingolipids.The metabolic pathways that were significantly enriched were primarily the lipid metabolism pathways,including sphingolipid metabolism,glycerophospholipid metabolism,and linoleic acid metabolism.The sphingolipid metabolism was identified as a significantly differential pathway between the pneumonia patients and URTI patients and between the acute and recovery stages for the pneumonia patients,but not between the acute and recovery stages for the URTI patients.Ceramide and lactosylceramide,involved in sphingolipid metabolism,were significantly higher in the pneumonia patients than in the URTI patients with good discrimination abilities[area under curve(AUC)0.742 and 0.716,respectively;combination AUC 0.801].Conclusion Our results suggested that HAdV modulated lipid metabolism for both the patients with URTI and pneumonia,especially the sphingolipid metabolism involving ceramide and lactosylceramide,which might thus be a potential intervention target in the treatment of HAdV infection. 展开更多

Objective The aim of this study was to explore the prevalent characteristics of HBoV1 and its co-infection.Methods PCR was used to detect HBoV1-DNA(HBoV1) and other viruses.A multivariate logistic regression model was used to explore possibility of co-detected for related viruses.Results The positivity rates in Nanjing and Lanzhou were 9.38%(74/789) and 11.62%(161/1386),respectively(P〉0.05).The HBoV1 positive group was younger than negative group(P〈0.05).Seasonal differences were noted,with a higher frequency of infection in December and July.HBoV1-positive children [72.34%(169/235)] were co-infected with other respiratory viruses.Multifactorial analysis showed no correlations between HBoV1 and the clinical classification,region,gender,age,or treatment as an outpatient or in a hospital.Correlations were identified between HBoV1 infections with ADV(OR=1.53,95% CI 1.03-2.28),RSV(OR=0.71,95% CI 0.52-0.98),and IFVA(OR=1.77,95% CI 1.00-3.13).Conclusions Presence of HBoV1 in nasopharyngeal aspirates did not correlate with region or gender,although the prevalence of HBoV1 was higher in younger children.There were no correlations between HBoV1 and other variables,except for the season and ADV,RSV,or IFVA infections. 展开更多

Objective:To examine the effects of human bocavirus type 1(HBoV1)on the course of lower respiratory tract infections in cases of monoinfection and coinfection,and the effects of HBoV1 viral load on the disease in children under six years old hospitalized with a diagnosis of HBoV1-associated lower respiratory tract infections.Methods:Children under six years of age,who were hospitalized with the diagnosis of lower respiratory tract infection due to HBoV1 between 1 January 2021 and 1 January 2022 were included in the study.Laboratory confirmation of the respiratory pathogens was performed using polymerase chain reaction(PCR).Results:Fifty-four(16.4%)children with HBoV1 among 329 children whose PCR was positive with bacterial/viral agent in nasopharyngeal swab samples were included in the study.There were 28(51.9%)males and 26(48.1%)females with a median age 23.4 months[interquartile range(IQR):13.2,30.0 months](min-max:1 month-68 months).HBoV1 was detected as a monoinfecton in 26(48.1%)children,and as a coinfection with other respiratory agents in 28 children(51.9%).In multiple regression analysis,coinfection(P=0.032)was associated with the length of hospitalization(P<0.001;R^(2)=0.166).There was a negative correlation(r=−0.281,P=0.040)between cough and cycle threshold.Fever was found to be positively correlated with C-reactive protein(r=0.568,P<0.001)and procalcitonin(r=0.472;P=0.001).Conclusions:Although we found a higher HBoV1 viral load in children with more cough symptoms in our study,it had no effect on the severity of the disease,such as length of hospital stay and need for intensive care.Coinfection was found to affect the length of hospitalization. 展开更多

Repeated respiratory tract infection is a frequently-occurring disease during childhood. At present, western medicine doctors generally adopt anti-infectives and immunomodulators to treat the disease, while traditional Chinese medicine doctors mainly administer decoction of Chinese herbs. The authors treated 46 cases of repeated respiratory tract infection from March 1990 to April 1996 by applying mild-moxibustion over points on the back with satisfactory therapeutic results. A report follows.Clinical Data All the 86 cases were outpatients in our hospital with duration of common cold for over 10 days and characterized by relapse of respiratory tract infection. There were over 7-time relapse of respiratory tract infection on each case within a year. Eighty-six cases were randomly divided into treatment group (46 cases) and control group (40 cases). Of the 46 cases in the treatment group, 22 were boys and 24 girls. 17 cases (36.9%) were 6 months to 4 years old, 18 (39.1%) 4 to 6 years, and 11 (23.9%) 6 to 12 years. 目的分析2023年我院呼吸道感染的流行病学特征及血清学指标变化,为呼吸道感染疾病的诊断和治疗提供依据。方法选取2023年1月至2023年12月就诊于北京佑安医院的483例呼吸道病原体IgM阳性者为研究对象,采用间接免疫荧光法检测血清中的肺... 目的分析2023年我院呼吸道感染的流行病学特征及血清学指标变化,为呼吸道感染疾病的诊断和治疗提供依据。方法选取2023年1月至2023年12月就诊于北京佑安医院的483例呼吸道病原体IgM阳性者为研究对象,采用间接免疫荧光法检测血清中的肺炎支原体(MP)、肺炎衣原体(CP)、甲型流感病毒(INFA)、副流感病毒(PIV)、呼吸道合胞病毒(RSV)、腺病毒(ADV)、柯萨奇病毒(COX)、嗜肺军团菌(LP)、乙型流感病毒(INFB)9种病原体免疫球蛋白(Ig)M抗体。采用卡方检验对不同性别、年龄、季节患者的血清呼吸道病原体IgM阳性率进行统计分析,采用Kru’skal-Wallis检验分析不同病原体感染者间的血清学指标。结果(1)483例呼吸道感染患者中,单一病原体IgM阳性者占比为39.3%(190/483),>2种病原体IgM阳性者占比为60.7%(293/483)。单一病原体IgM阳性患者中,位居前3位的依次为INFB(39.5%,75/190)、INFA(24.2%,46/190)及MP(20%,38/190);混合病原体IgM阳性患者中,以2种病原体混合感染为主,占比为61.7%(181/293),其中以病原体INFA+INFB为主,占比43.3%(127/293),其次为MP+INFA+INFB,占比17.4%(51/293)。(2)女性患者血清MP、ADV IgM阳性率高于男性,INFB IgM阳性率低于男性,差异均具有统计学意义(P<0.05)。(3)0~18、19~45、46~59、>60岁4个年龄段患者血清MP、COX病原体IgM阳性率比较,差异均有统计学意义(P均<0.05),其中0~18岁MP、COX阳性率分别高于19~45、46~59、>60岁(P均<0.05),19~45岁MP阳性率高于46~59、>60岁(P均<0.05)。(4)春、夏、秋、冬4个季节血清MP、INFA IgM阳性率比较,差异有统计学意义(P<0.05);其中春季MP阳性率高于秋季(P<0.05),冬季阳性率分明高于夏季、秋季(P均<0.05);INFA阳性率冬季高于夏季(P<0.05)。(5)将483例呼吸道感染者分为MP IgM阳性组、病毒IgM阳性组、混合感染组,3组间eGFR、EOS、PLT比较,差异有统计学意义(P<0.05),其中混合感染组中EOS高于MP IgM阳性组(P<0.05),eGFR、PLT高于病毒IgM阳性组(P均<0.05),肝功能指标在3组间比较差异无统计学意义(P>0.05)。结论2023年全年以MP、INFA、INFB病原体感染为主,且混合感染多见;女性及年龄小的人群更易感染MP;春季和冬季更易发生MP感染,甲型流感病毒更易发生于冬季;混合感染可能更易引起机体的免疫反应。 展开更多

Background Human adenovirus(HAdV)infection can cause a variety of diseases.It is a major pathogen of pediatric acute respiratory tract infections(ARIs)and can be life-threatening in younger children.We described the epidemiology and subtypes shifting of HAdV among children with ARI in Guangzhou,China.Methods We conducted a retrospective study of 161,079 children diagnosed with acute respiratory illness at the Guangzhou Women and Children’s Medical Center between 2010 and 2021.HAdV specimens were detected by real-time PCR and the hexon gene was used for phylogenetic analysis.Results Before the COVID-19 outbreak in Guangzhou,the annual frequency of adenovirus infection detected during this period ranged from 3.92%to 13.58%,with an epidemic peak every four to fve years.HAdV demonstrated a clear seasonal distribution,with the lowest positivity in March and peaking during summer(July or August)every year.A signifcant increase in HAdV cases was recorded for 2018 and 2019,which coincided with a shift in the dominant HAdV subtype from HAdV-3 to HAdV-7.The latter was associated with a more severe disease compared to HAdV-3.The average mortality proportion for children infected with HAdV from 2016 to 2019 was 0.38%but increased to 20%in severe cases.After COVID-19 emerged,HAdV cases dropped to 2.68%,suggesting that non-pharmaceutical interventions probably reduced the transmission of HAdV in the community.Conclusion Our study provides the foundation for the understanding of the epidemiology of HAdV and its associated risks in children in Southern China. 展开更多

Human metapneumovirus(HMPV) infection is one of the leading causes of hospitalization in young children with acute respiratory illness. In this study, we prospectively collected respiratory tract samples from children who were hospitalized with acute lower respiratory tract infection in six hospitals in China from 2017 to 2019. HMPV was detected in 145 out of 2733 samples(5.3%) from the hospitalized children. The majority of HMPV-positive children were under the age of two(67.6%), with a median age of one year. HMPV can independently cause acute lower respiratory tract infection in young children, while all patients showed mild clinical symptoms. Of all the co-infected patients, HMPV was most commonly detected with enterovirus(EV) or rhinovirus(RhV)(38.0%),followed by respiratory syncytial virus(RSV)(32.0%). The highest detection rate occurred from March to May in both northern and southern China. Out of 145 HMPV positive samples, 48 were successfully typed, of which 36strains were subgrouped into subtypes A2c(75%), eight strains were included in subtype B1(16.7%), and four strains were included in subtype B2(8.3%). Moreover, 16 A2c strains contained 111-nucleotide duplications in the G gene. Twenty-seven complete HMPV genomes were successfully obtained, and 25(92.6%) strains belonged to subtype A2c, whereas one strain was included in subgroup B1 and another was included in subgroup B2. A total of 277 mutations were observed in the complete genomes of 25 A2c strains. All results presented here improve our understanding of clinical characteristics and molecular epidemiology of HMPV infection in children. 展开更多

Human respiratory syncytial virus(RSV) is a major pathogen of acute lower respiratory tract infection among young children. To investigate the prevalence and genetic characteristics of RSV in China, we performed a molecular epidemiological study during 2015–2019. A total of 964 RSV-positive specimens were identified from 5529 enrolled patients during a multi-center study. RSV subgroup A(RSV-A) was the predominant subgroup during this research period except in2016. Totally, 535 sequences of the second hypervariable region(HVR-2) of the G gene were obtained. Combined with182 Chinese sequences from GenBank, phylogenetic trees showed that 521 RSV-A sequences fell in genotypes ON1(512),NA1(6) and GA5(3), respectively;while 196 RSV-B sequences fell in BA9(193) and SAB4(3). ON1 and BA9 were the only genotypes after December 2015. Genotypes ON1 and BA9 can be separated into 10 and 7 lineages, respectively. The HVR-2 of genotype ON1 had six amino acid changes with a frequency more than 10%, while two substitutions H258 Q and H266 L were co-occurrences. The HVR-2 of genotype BA9 had nine amino acid substitutions with a frequency more than10%, while the sequences with T290 I and T312 I were all from 2018 to 2019. One N-glycosylation site at 237 was identified among ON1 sequences, while two N-glycosylation sites(296 and 310) were identified in the 60-nucleotide duplication region of BA9. To conclusion, ON1 and BA9 were the predominant genotypes in China during 2015–2019. For the genotypes ON1 and BA9, the G gene exhibited relatively high diversity and evolved continuously. 展开更多

目的探讨江苏淮安地区住院治疗的呼吸道感染患儿肺炎支原体(mycoplasma pneumoniae,MP)感染的流行病学特征,为该地区MP感染的防控和诊治提供线索。方法收集2017年1月1日至2021年12月31日因呼吸道感染在淮安市淮安医院儿科住院患儿的血标... 目的探讨江苏淮安地区住院治疗的呼吸道感染患儿肺炎支原体(mycoplasma pneumoniae,MP)感染的流行病学特征,为该地区MP感染的防控和诊治提供线索。方法收集2017年1月1日至2021年12月31日因呼吸道感染在淮安市淮安医院儿科住院患儿的血标本,化学发光法检测MP-IgM抗体,分析并比较不同年份、不同性别、不同年龄和不同季节MP的检出情况。结果9836例呼吸道感染患儿中,MP阳性检出率为21.38%(2103/9836)。2017~2021年MP检出率分别为16.64%(310/1863)、25.30%(533/2107)、26.22%(521/1987)、19.60%(354/1806)、18.57%(385/2073),2018年和2019年MP阳性检出率较其他年份明显升高,差异有统计学意义(χ^(2)=84.949,P<0.001)。女性患儿MP阳性检出率25.71%(1006/3913)高于男性患儿的18.52%(1097/5923),差异有统计学意义(χ^(2)=72.430,P<0.001)。0~1岁、>1~3岁、>3~6岁和>6~16岁MP检出率分别为8.50%(325/3824)、23.48%(635/2704)、29.55%(651/2203)、44.52%(492/1105)。MP检出率随着年龄增大而逐步升高(χ^(2)=824.217,P<0.001)。春夏秋冬MP检出率依次为14.79%(330/2231)、31.14%(839/2694)、21.62%(624/2886)、15.31%(310/2025)。检出率最高的季节为夏季,其次为秋季,冬春两季检出率最低(χ^(2)=254.889,P<0.001)。五年间2018年春季和夏季分别为MP检出率最高和最低的季节。结论淮安地区呼吸道感染患儿MP检出率较高,不同年份的检出率存在波动。女性患儿的检出率高于男性患儿,MP感染可见于各个年龄段,年龄越大MP检出率越高。夏季为淮安地区MP感染的高发季节。 展开更多

目的 探讨呼吸道病原体感染住院儿童的病原谱特征,为相关感染性疾病的诊治和预防控制提供依据。方法 收集2022年6月至2023年3月深圳市龙岗区妇幼保健院采集住院儿童呼吸道病原体样本2108份,采用多重聚合酶链反应技术对样本进行甲型流行... 目的 探讨呼吸道病原体感染住院儿童的病原谱特征,为相关感染性疾病的诊治和预防控制提供依据。方法 收集2022年6月至2023年3月深圳市龙岗区妇幼保健院采集住院儿童呼吸道病原体样本2108份,采用多重聚合酶链反应技术对样本进行甲型流行性感冒病毒(influenza A virus,FluA)、乙型流行性感冒病毒(influenza B virus,FluB)、呼吸道合胞病毒(respiratory syncytial virus,RSV)、腺病毒(respiratory adenovirus,ADV)、鼻病毒(respiratory rhinovirus,HRV)、肺炎支原体(mycoplasma pneumoniae,MP)6种病原体检测,分析总体检出情况,并对不同年龄、性别、疾病、月份、时间段的病原体分布特点进行比较。结果 病原体的阳性总检出率为50.05%,单一感染中HRV(333例,15.80%)、RSV(237例,11.24%)、FluA(194例,9.20%)占比较高,混合感染中以HRV与RSV组合频次最高;住院患儿不同年龄组病原体单一感染、混合感染(3种病原体混合感染除外)的阳性检出率差异均有统计学意义(均P<0.05)。不同性别住院患儿病原体单一感染、混合感染的阳性检出率差异均无统计学意义(均P>0.05)。FluA在急性支气管炎患儿中检出率最高(27.24%),支气管肺炎患儿中以RSV阳性检出率最高(19.14%),急性咽炎患儿中以ADV阳性检出率最高(47.06%),喘息性支气管炎患儿中以HRV和RSV阳性检出率最高(50.00%和48.08%),重症肺炎患儿中以MP阳性检出率最高(36.73%)。FluA检出率在2022年7月和2023年3月有2个小高峰,RSV在2022年12月检出率最高,HRV检出率在2022年11月和2023年2月有2个小高峰,ADV和MP每月均有检出。结论 呼吸道感染住院患儿感染的主要病原体是HRV和RSV,同时应注意防控重叠感染。 展开更多

目的调查本地区急性呼吸道感染患者肺炎支原体(MP)和肺炎衣原体(CP)的流行病学特征。方法对2021年2月至2023年8月厦门大学附属成功医院的4896例急性呼吸道感染患者进行回顾性研究,其中男2886例、女2010例,年龄1 d~102岁。分析患者血清M... 目的调查本地区急性呼吸道感染患者肺炎支原体(MP)和肺炎衣原体(CP)的流行病学特征。方法对2021年2月至2023年8月厦门大学附属成功医院的4896例急性呼吸道感染患者进行回顾性研究,其中男2886例、女2010例,年龄1 d~102岁。分析患者血清MP免疫球蛋白M(MP-IgM)抗体和CP-IgM抗体检出情况,并对比MP和CP感染类型、人群及季节分布特征。统计学方法采用χ^(2)检验。结果2021年至2023年急性呼吸道感染患者中MP-IgM抗体和CP-IgM抗体阳性检出率为14.48%(709/4896),其中MP阳性率为12.58%(616/4896),CP阳性率为1.63%(80/4896),MP+CP阳性检出率为0.27%(13/4896),以单一MP感染为主。女性MP阳性率高于男性[17.31%(348/2010)比9.74%(281/2886)],差异有统计学意义(χ^(2)=60.748,P<0.001);不同性别CP阳性率比较,差异无统计学意义(χ^(2)=0.360,P>0.05)。MP、CP在不同年龄段感染率差异均有统计学意义(χ^(2)=237.938、55.070,均P<0.05),MP以0~14岁儿童为主要感染对象,CP以6~40岁为高发年龄。夏季总MP阳性率[15.30%(268/1752)]、总CP阳性率[2.40%(42/1752)]均高于其他季节;不同季节总MP阳性率比较,差异有统计学意义(χ^(2)=16.747,P<0.001),MP感染高发于夏秋季;而不同季节总CP阳性率比较,差异无统计学意义(χ^(2)=4.114,P=0.249)。结论MP、CP是急性呼吸道疾病的重要病原体,其中以MP感染为主,呈现季节、性别和年龄差异。 展开更多

目的探讨2023年厦门地区急性呼吸道感染儿童的呼吸道病原学流行特征。方法收集2023年厦门市患急性呼吸道感染(ARTI)的儿童1939个病例咽拭子,进行甲型流感(FluA)、甲型HpN;流感(FluA-H_(1)N_(1))、甲型HN2流感(FluA-H_(3)N_(2)).鼻病毒(R... 目的探讨2023年厦门地区急性呼吸道感染儿童的呼吸道病原学流行特征。方法收集2023年厦门市患急性呼吸道感染(ARTI)的儿童1939个病例咽拭子,进行甲型流感(FluA)、甲型HpN;流感(FluA-H_(1)N_(1))、甲型HN2流感(FluA-H_(3)N_(2)).鼻病毒(RVs)、呼吸道合胞病毒(RSV)、偏肺病毒(MPV)、腺病毒(ADV)、副流感病毒(PIV)、博卡病毒(BoV)、冠状病毒(CoV),乙型流感病毒(FIuB)、肺炎支原体(MP)和衣原体的病原体检测,并对以上呼吸道13项病原体检测数据进行分析。结果ARTI儿童中,75.66%的患儿至少有1种呼吸道病原体呈阳性;不同年龄阶段中,学龄期儿童组呼吸道13项病原体阳性检出率79.41%,占比最高,其余依次是学龄前期儿童组、幼儿期儿童组及婴儿期儿童组(P<0.05);PIV、RSV和衣原体感染多发生在婴儿期,阳性检出率分别为12.16%、23.87%、2.25%,RVs、BoV感染多发生在幼儿期,阳性检出率分别为25.13%、3.54%,ADV感染多发生在学龄前期,阳性检出率为11.85%,FluA、FluA-H_(3)N_(2)、MP和FluB感染多发生在学龄期,阳性检出率分别为13.32%.8.13%、49.31%、和7.79%(P<0.05);FluA、FluA-H_(3)N_(2)、FluB、MP病原体检出率随患者年龄阶段增长占比增加,PIV、RSV病原体检出率随患者年龄阶段增长占比减小(P<0.05)。FluA-H_(3)N_(2)、RVs感染多发生在春季,阳性检出率分别为12.80%、21.48%,PIV、RSV感染多发生在夏季,阳性检出率分别为10.40%、24.32%,MPV、MP和BoV感染多发生在秋季,阳性检出率分别为8.54%、48.93%、2.96%,FluA、FluA-H_(3)N_(2)、ADV、FuB和衣原体感染多发生在冬季,阳性检出率分别为20.88%、17.78%、9.79%、14.95%和1.80%(P<0.05)。结论2023年厦门市ARTI儿童呼吸道13项病原体流行与性别、季节性和年龄阶段相关,不同季节性和年龄阶段病原体流行类型呈交替出现,因此需要加强ARTI儿童呼吸道病原学监测,进行精准临床治疗。 展开更多

In the past few years,a variety of extradigestive disorders, including cardiovascular,skin,rheumatic and liver diseases, have been associated with Helicobacter pylori(H.pylori) infection.The activation of inflammatory mediators by H.pylori seems to be the pathogenetic mechanism underlying the observed associations.The present review summarizes the current literature,including our own studies,concerning the association between H.pyloriinfection and respiratory diseases. A small number of epidemiological and serologic,case- control studies suggest that H.pylori infection may be associated with the development of chronic bronchitis.A frequent coexistence of pulmonary tuberculosis and H.pylori infection has also been found.Moreover,recent studies have shown an increased H.pyloriseroprevalence in patients with bronchiectasis and in those with lung cancer.On the other hand,bronchial asthma seems not to be related with H.pylori infection. All associations between H.pylori infection and respiratory diseases are primarily based on case-control studies, concerning relatively small numbers of patients.Moreover, there is a lack of studies focused on the pathogenetic link between respiratory diseases and H.pylori infection. Therefore,we believe that larger studies should be undertaken to confirm the observed results and to clarify the underlying pathogenetic mechanisms. 展开更多

Respiratory syncytial virus(RSV) is the most frequent and important cause of lower respiratory tract infection in infants and children. It is a seasonal virus, with peak rates of infection occurring annually in the cold season in temperate climates, and in the rainy season, as temperatures fall, in tropical climates. High risk groups for severe RSV disease include infants below six mo of age, premature infants with or without chronic lung disease, infants with hemodynamically significant congenital heart disease, infants with immunodeficiency or cystic fibrosis, and infants with neuromuscular diseases. Mortality rates associated with RSV infection are generally low in previous healthy infants(below 1%), but increase significantly in children with underlying chronic conditions and comorbidities. Following early RSV lower respiratory tract infection, some patients experience recurrent episodes of wheezing mimicking early childhood asthma with persistence of lung function abnormalities until adolescence. There is currently no RSV vaccine available, but promising candidate vaccines are in development. Palivizumab, a monoclonal RSV antibody that is the only tool for immunoprophylaxis in high-riskinfants, lowers the burden of RSV infection in certain carefully selected patient groups. 展开更多
