Practice and Exploration of Economic Benefit Evaluation of Medical Equipment in Our Hospital

Objective The evaluation index of medical equipment's economic benefit is based on the usage of medical equipment,the traditional data collection method is time-consuming,laborious and not entirely accurate.The usage of medical equipment is obtained by designing data query statements from the HIS system.Methods First the charging items are in correspondence with the device's name included,second fees and other relevant data are extracted from charging module in HIS.Through a rough estimate of the recovery period and an increase or decrease ratio,the economic benefit of the medical equipment can be analyzed.Results Through the method of the benefit analysis of the medical equipment,we can clearly find out the different economic benefit of the equipment,and finally analyze the reasons.Conclusion Practice has proved that,this methad,it can greatly reduce human,material resources required in data collection and improve the accuracy of the data.It can help hospital managers timely to grasp the operating costs of medical equipment and other information,and also provide scientific data for hospital managers when they purchase reasonable medical equipment.

Experiences of Social Isolation in Older Hospitalized Patients with COVID-19 and Their Close Relatives: A Qualitative Study

Background: Proximity between older patients and their close relatives is essential during hospitalisation. During the first wave of the pandemic, the Danish Patient Safety Authority restricted no hospital visitors. Aim: To explore how older patients with COVID-19 and their close relatives experienced physical separation during hospitalization. Method: A qualitative study using semi-structured interviews and thematic analysis was employed. Findings: Six interviews were conducted: three patients with a mean age of 81 years and three close relatives—two daughters and one spouse. The patients felt boredom, loneliness, and a sense of imprisonment, yet they felt safe and satisfied. Isolation was known beforehand from the media. Close relatives emphasised that information, involvement, and collaboration with hospital staff were crucial. Conclusion: Although older patients with COVID-19 and their close relatives widely accept their situation during hospitalization, they experience negative consequences from social isolation.

Emergency management of nursing human resources and supplies to respond to coronavirus disease 2019 epidemic

Objective:To introduce the emergency management of nursing human resources and supplies of a large general hospital when facing the outbreak of coronavirus disease 2019(COVID-19).Method:The Nursing Department of the hospital fully executed its functional authority to establish a three-level echelon of sustainable support,allocate human recourses dynamically,organize pre-service training,supervise the key working steps,formulate positive incentive methods,and deploy medical supplies scientifically.Result:By taking these strategies,the hospital effectively improved the coping capacity of the nursing team and played a positive role in the prevention and treatment of COVID-19.Conclusion:The emergency management of nursing human resources and material resources for COVID-19 of the hospital is successful.But several deficiencies were identified as well,which indicated that the hospital needs to establish an efficient emergency management system,and pay attention to the practice of nursing emergency plans to enhance coping capacities in public health emergencies.

Shaking table test study on performance and functionality of a typical telecommunication room after earthquakes

Large numbers of basic transceiver stations,where the telecommunication room is one of the main components,comprise an important part of the telecommunication system.After earthquakes,considerable economic loss from telecommunication systems is often associated with seismic damage and functional loss of the telecommunication room.However,research related to this has been limited.In this study,shaking table tests were conducted for a full-scale typical telecommunication room,including a light-steel house and the necessary communication and power supply equipment.The tests not only focused on the seismic damage to all the structures but also considered the functions of the communication and power supply of the equipment.The interactions between these facilities and their effects on communication function were also investigated.Compared with the damage to structures,the interruption of the power supply due to earthquakes is a weak link.Finally,the damage indexes,together with their threshold values of different damage states for the communication and power supply equipment,were derived from the test results.The results of this research can contribute to the literature gaps regarding seismic performance studies of telecommunication rooms,and can serve as a valuable reference for future research on its seismic fragility and economic losses evaluation.

目的开发医院设备管理系统,便于医学工程科人员及时了解设备运行情况。方法基于Android平台,将设备信息数据库、代理服务器和客户端通过网络进行互连。结果该系统的代理服务器可通过自定义的应用协议与客户端进行网络通信以及数据传输,使工作人员通过客户端及时查询到设备运行信息。结论该系统安装在智能手机上,维修人员可实时查询设备运行信息,从而提高设备维护保养的效率。

Intelligent Examination Monitoring and Maintenance for the Safety of Operation of Flexible Manufacturing Systems

Based on the system of electric power supply for flexible manufacturing systems (FMS), a study has been carried out on the intelligent safety examination, monitoring and maintenance of its running environment. On the basis of the specific feature of the power supply network of an FMS, real time monitoring system of the power supply network and the fault diagnostic expert system for the power equipment have been designed. This system can diagnose not only definite fault phenomena, but also fuzzy, uncertain fault phenomena as well. Fault diagnostic knowledge base for the power equipment has been founded hierarchy architecture model and the method of fault tree analysis. Feasibility of this system has been proved by computer simulation.

Medical errors, infection-control breaches and the use of adulterated and misbranded medical devices

Several well-publicized cases of improper cleaning,disinfection or sterilization of contaminated reusable medical equipment that posed an increased risk of patientto-patient disease transmission were reported within the past few years,resulting in the notification of approximately 20 000 patients.These medical errors,the specific infection-control standards they breached,and assessments of the risk of infection associated with each are discussed.Other topics discussed include the Food and Drug Administration’s(FDA)regulation of medical devices and infection-control products;the use of adulterated,misbranded,and investigational devices;consent decrees and associated Certificates of Medical Necessity;and informed patient consent.Focus is placed on liquid chemical sterilization,its history,and the FDA’s recent censure and discontinuation of a medical device labeled with this claim,namely,the STERIS System 1 processor.Recommendations are provided for healthcare facilities,regulatory agencies,manufacturers of reusable medical devices,and professional healthcare organizations and administrations to improve public health and prevent healthcareassociated infections.

基于Web和Android系统的医疗设备管理系统的设计与实现

随着医疗设备技术的不断发展,医院的规模不断扩大,传统人为粗放的管理模式愈发不适用于大型现代化医院医疗设备的管理。为实现医院现有的大量先进医疗设备科学高效管理,通过需求调研和分析后,设计并实现了基于Web和Android系统的大型医院医疗设备管理系统,通过该系统可以使医疗设备管理相关人员的工作完成信息化管理。经过测试和运行,发现该系统可以稳定运行,并从医疗设备购入到报废全生命周期的各个环节,使用对应的模块可方便相关人员更方便快捷的完成工作,实现了医疗设备的精细化管理。

Discussion on Question of Importing Large-scale Equipment from Other Countries by Installment

Successful cases have not been rare, that is to import funds accelerate hospital building by means of raising funds, borrowing, renting, jointing, and so on. The same characteristics is raising money at first and then working, and the investor will earn high proportional benefits. 'Importing funds' has solved some practical problems, but it also causes some negative effects, such as difficulty in management, being difficult to work, inequitable benefit allocation, little benefits earned by the hospital, a big venture, and so on.By means of installment, the hospital has imported much equipment, such as very expensive Magnetic Resonating Instrument (MRI), spiral CT, straight accelerator, ECT and so on. Connecting with practice, the authors has analyzed objectively the profits and the harm of importing equipment by installment. There are four reasons for applying installment: firstly, it solves the problem of shortage of funds of the hospital; secondly, it efficiently avoids the inconvenience resulted from the management by both jointing sides together; thirdly, it raises the investing earnings of the hospital; finally, it is advantages which are as follows. Importing large-scale foreign equipment from abroad by installment has not been popular in the native hospital, and there are not relatively regular and ripe rules to be followed. Moreover, because effective institution is required to act as an economic guarantor, an extra content of work will, be added to practice.The authors prefer that attentions should be paid to the five aspects in importing equipment by means of installment; selecting the suitable importing project, selecting a favorable cooperating partner and deciding suitable cooperating items; selecting an excellent company or institution to guarantee for the hospital, and grasping the practice of the contract.

对103家三级公立医院医疗设备配置现状的调查与分析

目的:调查103家三级公立医院医疗设备配置现状,分析比对我国不同区域医院的医疗设备占比,为科学合理地配置医疗设备提供依据。方法:采用文献研究和专家咨询法设计调查问卷,通过“中国医学装备”公众号和“好医工”平台发布电子问卷进行调查,分析医疗设备配置数量、国产占比与医院所在区域及医院床位数的相关性。结果:共回收有效问卷103份,涉及华东、华北、华中、华南、西南、西北和东北的103家三级公立医院,其中华北占18%,华东占37%,华南占19%。华北、华南和华东3个样本区域三级公立医院医疗设备配置的平均台件数为9785台/家、9245台/家和8153台/家,甲乙类设备配置数量较少,与床位数无相关性;华北与华东在床位数、每百张床设备台件数以及每百张床设备金额上差异有统计学意义(t=-2.582、2.939、4.653,P<0.05);生命支持类设备配置数量均与床位数有显著正相关,国产占有率尤其是生命支持类设备的国产占有率有所提高。结论:分析我国公立医院医疗设备配置情况,可为医院更好地适应新版甲乙类大型医用设备目录的调整提供数据支撑,为三级医院在大型医用设备配置和国产化的示范推广提供参考依据,为相关卫生管理部门提供借鉴。

文章基于突发公共卫生事件状态下公立医院医疗物资供应冲突发生的环境、事发经过、情境结果及冲突条件等要素,运用冲突管理有关理论探索冲突的构成要素及其结果、冲突中人员管理的一般性方法及冲突解决途径。在研究中不难发现,强制型管理风格在应对医疗物资全方位供应复杂性工作面前适用性略显不足,知识型员工在保健与激励因素共同负强化下使得人员管理更加如履薄冰,合作型冲突管理能够较为有效地解决冲突窘境。组织中的各类冲突在所难免,冲突的发生与领导管理风格、组织目标一致性、团队成员的激励程度、人际关系的维系与促进等因素息息相关,应关注冲突的管理方法,通过替代性方案建设性地加以解决至关重要。

智能物流机器人在医院后勤物资配送管理中的应用

目的开展智能物流机器人在医院后勤物流配送管理中的应用研究,以提高医院后勤及运行保障精细化管理的水平。方法基于智能订单调度系统和交通调度系统,打通智能物流机器人与医院信息系统的通道,整合即时定位与地图构建技术和5G技术,利用物联网技术将医院电梯、门禁与机器人紧密结合,实现智能物流机器人自动配送物资。结果提升了医院物资整体配送效率,降低了配送环节差错率,提高了医院整体运行效率,降低了医院的管理难度及运营成本。结论智能物流机器人具有高效、便捷、安全的特点,在医院物资配送管理中的应用效果良好,具有较强的可行性与推广性,可有效提高医院后勤精细化管理水平。

分立式开关电源系统抗震性能的振动台试验研究

开关电源是通信机楼和通信基站中最重要的设备之一,相当于机楼和基站内供电系统的“心脏”,若在地震作用下发生功能失效将导致通信机楼和基站完全停止工作。开关电源的抗震性能研究是通信系统抗震韧性评估研究的基础。本文以通信机楼内常用的分立式开关电源系统为研究对象,通过振动台试验研究其在地震下的特征损伤模式、损伤发展过程以及其供电功能在地震作用下的变化等。试验过程中为了模拟开关电源在通信机楼中的实际使用状态,为开关电源连接了380 V电压输入和一台模拟负载,组成了一个完整的用电系统。试验中台面激励采用了1条人工地震动、1条天然地震动和1条正弦拍波,逐步增大台面PGA直至振动台位移超限停止试验。从试验结果看:当PGA达到1.0 g时电源柜开始损伤,表现为柜门与主框架磨损掉漆,PGA在1.2~1.5 g时,电源柜顶部连接松动,面板连接螺丝松动,螺栓孔发生冲切破坏,PGA达到2.0 g时,试验结束,电源柜的主体未发生严重破坏,交流柜与直流柜前门轴变形;另外即使在较大PGA下,正弦拍波也无法对电源柜激起较大位移而造成破坏。试验过程中电气信号始终稳定,该开关电源柜抗震性能良好。

JCI理念为框架的模式在消毒供应室医用器械管理中的应用效果

目的 探讨国际医疗卫生机构认证联合委员会(JCI)理念为框架管理模式在消毒供应室医用器械管理中的应用效果。方法 采用随机数字表法将2019年1月至2020年1月在消毒供应室工作的30例医护人员,随机分为对照组和试验组,每组15例。对照组采用常规管理模式,试验组采用以JCI理念为框架的管理模式。观察2组护理服务质量、综合素质、医护人员满意度和器械管理效果进行评价。结果 2组护理质量比较,试验组护理流程、质量监管、安全意识、感染风险评分显著高于对照组(P<0.05)。2组管理前综合素质评分比较,差异无统计学意义(P>0.05)。2组管理后安全知识掌握、主动服务意识、风险防控能力和专业素养均高于管理前,差异均有统计学意义(P<0.05)。试验组管理后安全知识掌握、主动服务意识、风险防控能力和专业素养均高于对照组,差异均有统计学意义(P<0.05)。试验组医务人员满意率,灭菌、清洗、检查、包装、发放和回收合格率明显高于对照组,差异均有统计学意义(P<0.05)。结论 JCI理念为框架管理模式在消毒供应室医用器械管理中的应用效果良好,能有效提高护理质量与护理安全性,增加医护人员满意度,可用于医疗管理之中。

SPD项目的全流程信息化精细化方案在医用耗材管理中的有效性研究

目的 探讨供应-加工-配送(Supply-Processing-Distribution,SPD)项目的全流程信息化精细化方案在南京市第一医院医用耗材管理中的有效性。方法 通过分析医院医用耗材管理现状,建立标准分类编码以及以医疗器械唯一标识为核心的基础数据库,采用SPD模式、引入第三方供应链信息管理平台、射频识别技术及智能货柜系统、医院信息系统医嘱扫码计费等配套技术对医用耗材进行全流程信息化精细化管理。比较管理前(2020年)与管理后(2022年)百元耗材占比、高值耗材占比、医用耗材合理使用率,以及工作人员每周申请、领取、盘库、计费医用耗材时所耗时间、每周库存占用资金数据,以及条形码漏贴、粘贴错误例数和计费错误例数。结果 管理后百元耗材占比、高值耗材占比显著低于管理前,而医用耗材使用合格率高于管理前(均P<0.05)。管理后护理人员每周申请、领取、盘库、计费医用耗材的时间均低于管理前(P<0.05)。管理后可收费耗材每周库存金额显著低于管理前(P<0.05)。管理后医用耗材条形码漏贴率、误贴率显著低于管理前,医用耗材收费名称错误率、遗漏计费率显著低于管理前(均P<0.05)。结论 SPD项目的全流程信息化精细化方案可有效控制医院医用耗材占比,提高医用耗材的合理使用率,以及提高医护人员工作效率,具有良好的临床运用效果。
