Neuropathological characteristics of abnormal white matter functional signaling in adolescents with major depression

BACKGROUND Major depression disorder(MDD)constitutes a significant mental health concern.Epidemiological surveys indicate that the lifetime prevalence of depression in adolescents is much higher than that in adults,with a corresponding increased risk of suicide.In studying brain dysfunction associated with MDD in adolescents,research on brain white matter(WM)is sparse.Some researchers even mistakenly regard the signals generated by the WM as noise points.In fact,studies have shown that WM exhibits similar blood oxygen level-dependent signal fluctuations.The alterations in WM signals and their relationship with disease severity in adolescents with MDD remain unclear.AIM To explore potential abnormalities in WM functional signals in adolescents with MDD.METHODS This study involved 48 adolescent patients with MDD and 31 healthy controls(HC).All participants were assessed using the Patient Health Questionnaire-9 Scale and the mini international neuropsychiatric interview(MINI)suicide inventory.In addition,a Siemens Skyra 3.0T magnetic resonance scanner was used to obtain the subjects'image data.The DPABI software was utilized to calculate the WM signal of the fractional amplitude of low frequency fluctuations(fALFF)and regional homogeneity,followed by a two-sample t-test between the MDD and HC groups.Independent component analysis(ICA)was also used to evaluate the WM functional signal.Pearson’s correlation was performed to assess the relationship between statistical test results and clinical scales.RESULTS Compared to HC,individuals with MDD demonstrated a decrease in the fALFF of WM in the corpus callosum body,left posterior limb of the internal capsule,right superior corona radiata,and bilateral posterior corona radiata[P<0.001,family-wise error(FWE)voxel correction].The regional homogeneity of WM increased in the right posterior limb of internal capsule and left superior corona radiata,and decreased in the left superior longitudinal fasciculus(P<0.001,FWE voxel correction).The ICA results of WM overlapped with those of regional homogeneity.The fALFF of WM signal in the left posterior limb of the internal capsule was negatively correlated with the MINI suicide scale(P=0.026,r=-0.32),and the right posterior corona radiata was also negatively correlated with the MINI suicide scale(P=0.047,r=-0.288).CONCLUSION Adolescents with MDD involves changes in WM functional signals,and these differences in brain regions may increase the risk of suicide. 展开更多

目的运用静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)并结合低频振幅分数(fractional amplitude of low-frequency fluctation,fALFF)和度中心性(degree centrality,DC)来探讨躯体症状障碍(som... 目的运用静息态功能磁共振成像(resting-state functional magnetic resonance imaging,rs-fMRI)并结合低频振幅分数(fractional amplitude of low-frequency fluctation,fALFF)和度中心性(degree centrality,DC)来探讨躯体症状障碍(somatic symptom disorder,SSD)患者是否存在某些脑功能上的异常,从而为躯体症状障碍的临床诊疗提供参考信息。材料与方法45例右利手SSD患者和43名右利手健康对照组参与静息态磁共振扫描,fALFF和DC分析探测SSD患者静息态下自发性脑活动上的特征。SPM12软件用于磁共振数据的分析,组分析采用双样本t检验fALFF和DC统计阈值为体素水平P<0.001,团块水平经过family wise error(FWE)校正,P<0.05,且大于10个体素的团块将被报道。结果静息态脑功能结果显示SSD患者在边缘叶、丘脑处(t=4.91)fALFF值显著高于健康对照组。SSD患者在额叶、扣带回(t=5.20)的DC值显著低于健康对照组。结论这些异常的区域可能与SSD的发病机理有关,这些区域的功能异常可能会导致个体情绪处理、自我感觉和身体知觉功能的异常。 展开更多

目的 观察太极拳对慢性疲劳综合征(CFS)患者的疗效及对功能性磁共振成像中分数低频振幅(fALFF)的特征性影响。方法 招募CFS患者20例设为试验组,并选择性别、年龄与试验组差异无统计学意义的健康受试者20例设为对照组。两组受试者均接受... 目的 观察太极拳对慢性疲劳综合征(CFS)患者的疗效及对功能性磁共振成像中分数低频振幅(fALFF)的特征性影响。方法 招募CFS患者20例设为试验组,并选择性别、年龄与试验组差异无统计学意义的健康受试者20例设为对照组。两组受试者均接受为期4周的二十四式太极拳训练。在练习太极拳前后进行疲劳量表14 (FS-14)、生活质量量表(SF-36)及匹兹堡睡眠质量指数问卷(PSQI)评分;并进行脑部功能性磁共振成像检测,分析CFS患者和健康受试者fALFF值的差异,以及两者练习太极拳前后脑fALFF值的变化和差异,并对试验组受试者练习太极拳前后FS-14量表差值与脑fALFF差值进行相关性分析。结果练习前试验组FS-14评分、PSQI评分明显高于对照组,SF-36评分明显低于对照组(P<0.01)。与练习前比较,试验组练习后FS-14评分、PSQI评分均降低,SF-36评分升高(P<0.01);而对照组练习前后FS-14评分、PSQI评分差异无统计学意义(P>0.05),练习后SF-36评分较练习前显著升高(P<0.05)。与练习前比较,试验组受试者练习太极拳后fALFF值显著减弱的脑区包括左侧眶回/额极皮层、右侧前扣带回、右外侧颞叶,fALFF值显著增强的脑区包括左侧感觉运动皮层、右侧视觉皮层。试验组受试者练习前后全脑fALFF差值与FS-14评分差值呈负相关(r=-0.842,P<0.001)。结论 太极拳运动可改善CFS患者疲劳状态和睡眠,提高生活质量,可能与对患者脑部感觉运动皮层、左侧额叶、右外侧颞叶、前扣带回、视觉皮层的fALFF具有特异性影响有关。 展开更多

We want to explore the analgesic brain effect of the moxibustion at heat-sensitized Yaoyangguan(GV3)in patients with lumbar disc herniation(LDH)and myofascial pain syndrome(MPS).In an assessor-blinded observational study,we will include 15 LDH and 15 MPS.They will accept same treatment of heat-sensitive moxibustion at Yaoyangguan(GV3).The resting-state functionality magnetic resonance imaging image data of brain activities before and after treatment will be analyzed by mean fractional amplitude of low-frequency fluctuation,regional homogeneity analysis and brain functional connection.We select seed of first sensory cortex,second sensory cortex,insula cortex,periaqueductal gray and anterior cingulate cortex as the regions of interest to analyse the relationship between brain functional connectivity of pain-related networks and clinical data.Our study could disclose key brain targets and central response characteristics of the analgesic brain effect and the brain functional connection of heat-sensitive moxibustion. 展开更多

Background:Juvenile myoclonic epilepsy(JME)is the most common syndrome of idiopathic generalized epilepsy.Although resting-state functional magnetic resonance imaging(rs-fMRI)studies have found thalamocortical circuit dysfunction in patients with JME,the pathophysiological mechanism of JME remains unclear.In this study,we used three complementary parameters of rs-fMRI to investigate aberrant brain activity in JME patients in comparison to that of healthy controls.Methods:Rs-fMRI and clinical data were acquired from 49 patients with JME undergoing monotherapy and 44 ageand sex-matched healthy controls.After fMRI data preprocessing,the fractional amplitude of low-frequency fluctuation(fALFF),regional homogeneity(ReHo),and degree centrality(DC)were calculated and compared between the two groups.Correlation analysis was conducted to explore the relationship between local brain abnormalities and clinical features in JME patients.Results:Compared with the controls,the JME patients exhibited significantly decreased fALFF,ReHo and DC in the cerebellum,inferior parietal lobe,and visual cortex(including the fusiform and the lingual and middle occipital gyri),and increased DC in the right orbitofrontal cortex.In the JME patients,there were no regions with reduced ReHo compared to the controls.No significant correlation was observed between regional abnormalities of fALFF,ReHo or DC,and clinical features.Conclusions:We demonstrated a wide range of abnormal functional activity in the brains of patients with JME,including the prefrontal cortex,visual cortex,default mode network,and cerebellum.The results suggest dysfunctions of the cerebello-cerebral circuits,which provide a clue on the potential pathogenesis of JME. 展开更多
