Frontal assessment battery: A tool for screening minimal hepatic encephalopathy?

AIM To apply the Frontal Assessment Battery to cirrhotic patients with or without overt hepatic encephalopathy(OHE) and controls. METHODS The frontal assessment battery(FAB) was applied to 87 patients with liver cirrhosis(16 with and 71 without OHE) and 40 control subjects without cirrhosis treated at the alcohol and liver outpatient clinics and the gastroenterology ward of the Cassiano Ant?nio de Moraes University Hospital(Hospital Universitário Cassiano Ant?nio de Moraes- HUCAM), Espírito Santo, Brazil.RESULTS The average FAB score was lower for the cirrhotic than for the non-cirrhotic patients(10.6 ± 3.67 vs 12.25 ± 2.72, P = 0.015). The FAB score was lower for the cirrhotic patients with OHE than for the patients without OHE(8.25 ± 4.55 vs 11.14 ± 3.25, P = 0.027). The total FAB score was lower for the cirrhotic patients without OHE than for the non-cirrhotic patients, although this difference was not significant(11.14 ± 3.25 vs 12.25 ± 2.72, P = 0.067). Nevertheless, the difference in the scores on the subtest that assessed the ability to inhibit a response previously conditioned to a stimulus was significant(1.72 ± 0.93 vs 2.2 ± 0.85, P = 0.011).CONCLUSION The present study indicates that the FAB is a promising tool for outpatient minimal HE screening and the assessment of HE severity. 展开更多

A Motor Programming Task Activates the Prefrontal Cortex More than a Sensitivity-to-Interference Task or an Inhibitory Control Task in Older Adults 被引量:2

The objectives of this study were to detect age-related differences in activation of the prefrontal cortex (PFC) during the tasks of hand motions and to determine an activity-related task type activating the PFC. PFC activation during three tasks, three subtests of the Frontal Assessment Battery (FAB), was investigated in 77 healthy adults by using near-infrared spectroscopy (NIRS). The tasks were a motor programming task (FAB 3), a sensitivity-to-interference task (FAB 4) and an inhibitory control task (FAB 5). We divided participants into three age groups of Younger (20 - 39 years), Middle-aged (40 - 59 years), and Older (60 - 81 years), and compared relative changes in oxygenated hemoglobin concentration in the PFC during the tasks. The activation in the frontal pole (FP) and the dorsolateral prefrontal cortex (DLPFC) during a motor programming task and a sensitivity-to-interference task showed no main effects by age. The results indicated that they were not likely to be affected by age-related cognitive decline compared to an inhibitory control task. In addition, in the Older group, a motor programming task induced significantly greater activation than a sensitivi-ty-to-interference task at eleven channels out of twelve on which we focused (p < 0.05). It was suggested that some characteristic factors included in the motor programming task such as repetition of a series of hand motions and attention to action have the potential to contribute to PFC activation in older adults. These findings provide a clue to understanding daily activities available to suppress cognitive decline of older adults by activating the PFC. 展开更多

目的筛查可反映肌萎缩侧索硬化(ALS)患者执行功能损害的量表,并确定其参考值。方法通过对比ALS患者和对照组蒙特利尔认知评估(MOCA)执行功能、快速词汇测验(rapid verbal retrieval,RVR)、韦氏成人智力测验(Wechsler Adult Intelligence... 目的筛查可反映肌萎缩侧索硬化(ALS)患者执行功能损害的量表,并确定其参考值。方法通过对比ALS患者和对照组蒙特利尔认知评估(MOCA)执行功能、快速词汇测验(rapid verbal retrieval,RVR)、韦氏成人智力测验(Wechsler Adult Intelligence Scale,WAIS)数字广度测验和额叶功能评定表(frontal assessment battery,FAB)评分,确定患者有无执行功能损害,并通过ROC曲线从WAIS数字广度、RVR(动物、水果、蔬菜)、FAB、"大"组词和MOCA执行亚项7项量表中筛选出敏感的执行功能损害的量表。结果病例组MOCA、RVR、WAIS数字广度测验、FAB评分明显低于对照组(P<0.05),其中MOCA执行功能、RVR(动物、蔬菜、水果)评分、WAIS数字广度测验中顺向和逆向评分、FAB中概念化能力、"大"组词、程序性控制、对干扰敏感性、抑制性控制评分明显低于对照组(P<0.05),而"发"组词和FAB量表中自主性抓握在两组间差异无明显统计学意义(P>0.05)。对ALS执行功能障碍测查,诊断效能从高到低依次为WAIS数字广度、RVR(动物)、FAB、"大"组词、RVR(水果)、RVR(蔬菜)和MOCA执行功能。结论 26.74%ALS患者存在执行功能障碍,筛查ALS患者执行功能障碍时,WAIS数字广度、RVR(动物)、FAB有较高诊断效能。 展开更多
