BACKGROUND Acute recurrent pancreatitis(ARP)is characterized by episodes of acute pancreatitis in an otherwise normal gland.When no cause of ARP is identifiable,the diagnosis of"idiopathic"ARP is given.Mutat...BACKGROUND Acute recurrent pancreatitis(ARP)is characterized by episodes of acute pancreatitis in an otherwise normal gland.When no cause of ARP is identifiable,the diagnosis of"idiopathic"ARP is given.Mutations in the cystic fibrosis transmembrane conductance regulator(CFTR)gene increase the risk of ARP by 3-to 4-times compared to the general population,while cystic fibrosis(CF)patients present with a 40-to 80-times higher risk of developing pancreatitis.CASE SUMMARY In non-classical CF or CFTR-related disorders,CFTR functional tests can help to ensure a proper diagnosis.We applied an individualized combination of standardized and new CFTR functional bioassays for a patient referred to the Verona CF Center for evaluation after several episodes of acute pancreatitis.The CFTR genotype was G542X+/-with IVS8Tn:T7/9 polymorphism.The sweat(Cl-)values were borderline.Intestinal current measurements were performed according to the European Cystic Fibrosis Society Standardized Operating Procedure.Recent nasal surgery for deviated septum did not allow for nasal potential difference measurements.Lung function and sputum cultures were normal;azoospermia was excluded.Pancreas divisum was excluded by imaging but hypoplasia of the left hepatic lobe was detected.Innovative tests applied in this case include sweat rate measurement by image analysis,CFTR function in monocytes evaluated using a membrane potential-sensitive fluorescent probe,and the intestinal organoids forskolin-induced swelling assay.CONCLUSION Combination of innovative CFTR functional assays might support a controversial diagnosis when CFTR-related disorders and/or non-classical CF are suspected.展开更多

Rome Ⅰ, the first edition was published in 1994, with symptom-based diagnostic criteria for...Functional gastrointestinal disorders (FGIDs) represent a common and important class of disorders within gastroenterology. Rome Ⅰ, the first edition was published in 1994, with symptom-based diagnostic criteria for FGIDs. These criteria began to change the diagnostic approach to F-GIDs, and no longer considered "diagnoses of exclusion" but rather "diagnoses of inclusion". Rome Ⅱ, the second edition published in 2000, resulted from the continual process of analyzing new scientific and clinical evidence in the study of F-GIDs. Rome Ⅱ, diagnostic criteria for irritable bowel syndrome (IBS), was extended with a focus on the frequency of symptoms occurring twelve weeks (not necessarily consecutive weeks) within twelve months. ROlE Ⅲ, the third edition, conservative one, was published in September 2006, with changes made only where there is good evidence to do so. Some of the differences between Rome Ⅱ and Rome Ⅲ criteria are highlighted in this issue.展开更多
AIM:To investigate the pathophysiology of functional heartburn(FH) in Japanese patients.METHODS:A total of 111 patients with proton pump inhibitor(PPI)-refractory non-erosive gastroesophageal reflux disease underwent ...AIM:To investigate the pathophysiology of functional heartburn(FH) in Japanese patients.METHODS:A total of 111 patients with proton pump inhibitor(PPI)-refractory non-erosive gastroesophageal reflux disease underwent intraesophageal pressure testing and 24-h multichannel intraluminal impedancep H(24MII-p H) testing.The patients also completed several questionnaires while they were receiving the PPI treatment, including the questionnaire for the diagnosis of reflux disease(QUEST), the frequency scale for the symptoms of gastroesophageal reflux disease(FSSG), the gastrointestinal symptoms rating scale(GSRS), SF-36, and the Cornell Medical Index(CMI).The subjects were classified into FH and endoscopy-negative reflux disease(ENRD) groups based on the Rome Ⅲ criteria.RESULTS:Thirty-three patients with esophageal motility disorder were excluded from this study, while 22 patients with abnormal esophageal acid exposure time(p H-POS) and 34 with hypersensitive esophagus(HE) were included in the ENRD group.The FH group included 22 patients with no reflux involvement.Sex, age, and body mass index did not differ significantly between the groups.The mean SF-36 values were < 50(normal) for all scales in these groups, with no significant differences.The GSRS scores in these groups were not different and showed overlap with other gastrointestinal symptoms.The QUEST and the FSSG scores did not differ significantly between the groups.Neuroticism was diagnosed using the CMI questionnaire in 17 of the 78 included subjects within the p H-POS(n=4),HE(n=8),and FH(n=5)groups,with no significant differences.CONCLUSION:Clinical characteristics of the FH and PPI-refractory ENRD groups were similar.Therefore,esophageal function should be examined via manometry and 24MII-p H testing to differentiate between them.展开更多

However, along with the dramatically economic growth, there constantly emerges environmental and social constraints which may be great challenges of China's future sustainable development. In order to optimize the developmental mode, the latest national plan, "The 12th National Economic and Social Development Plan (2011-2015)", is establishing the "Main Function Zoning Strategy", which could be considered as a new prototype of national spatial plan and may cover all Chinese territories. Based on the latest "Main Function Zoning Strategy" stipulated in "The 12th National Economic and Social Development Plan (2011-2015)", the article builds main interlinks between the spatial development strategy and urban regulatory planning. Secondly, regulation factors in Chinese regulatory planning, which may implement the task of main function zoning, have been clarified and categorized. For each regulation factor, its regulation capability is defined by applying a coding method developed by the author. Finally, according to the output of the study, the spatial regulation approaches to implementing the "Main Function Zoning Strategy" in Chinese regulatory planning are analyzed.展开更多

In bacteria, RpoS expression is under strict control and is mainly regulat...RpoS protein is a σ factor of RNA polymerase that can control the expression of a group-specific gene, thus playing a vital role in bacteria. In bacteria, RpoS expression is under strict control and is mainly regulated at three levels: transcription level, translation level and post-translational level. Environmental stress enters bacterial cells through signal transduction and leads to a series of variations in microenvironment, thereby causing changes of regulator and controlling its levels based on the direct and indirect interaction between regulator and RpoS protein. In addition, RpoS protein has played special roles in bacteria, therefore the changes of RpoS protein levels will lead to variations in expression levels of a large number of genes, thereby causing variations of bacterial response to different environmental stress and changes of certain characteristics of bacteria, which provides a new strategy for the control of bacterial diseases in the future. This paper reviewed the recent progress on the regulation of RpoS protein expression and its function in several common bacteria. Due to the functional complexity of RpoS protein, there are still a lot of unknown functions to be further identified.展开更多

In this study, we functionally characterized Ta ARR1, a cytokinin(CK) response regulator gene in Triticum aestivum, in mediating the N starvation adaptation in plants. Ta ARR1 harbors two conserved domains specified by plant ARR family members;subcellular localization analysis indicated its target onto nucleus after endoplasmic reticulum assortment. Ta ARR1 displayed modified expression upon the N starvation stressor, showing upregulated expression in roots and leaves over a 27-h N starvation treatment and whose induced transcripts were gradually recovered along with progression of the N recovery treatment. The tobacco lines overexpressing Ta ARR1 displayed improved low-N stress tolerance, displaying enlarged phenotype, increased biomass and N accumulation, and enhanced glutamine synthetase(GS) activities compared with wild type(WT) following the N starvation treatment. Expression analysis on genes encoding the nitrate transporter(NRT) and GS proteins in Nicotiana tabacum revealed that Nt NRT2.2 and Nt GS3 are upregulated in expression in the N-deprived transgenic lines, whose expression patterns were contrasted to other above family genes that were unaltered on transcripts between the transgenic lines and WT. Transgene analysis validated the function of Nt NRT2.2 and Nt GS3 in regulating N accumulation, GS activity, growth traits, and N use efficiency in plants. These results suggested the internal connection between the Ta ARR1-mediated N starvation tolerance and the modified transcription of distinct N acquisitionand assimilation-associated genes. Our investigation together indicates that Ta ARR1 is essential in plant N starvation adaptation due to the gene function in transcriptionally regulating distinct NRT and GS genes that affect plant N uptake and assimilation under the N starvation condition.展开更多
AIM:To investigate the relationship between plasma acylated ghrelin levels and the pathophysiology of functional dyspepsia.METHODS:Twenty-two female patients with functional dyspepsia and twelve healthy volunteers wer...AIM:To investigate the relationship between plasma acylated ghrelin levels and the pathophysiology of functional dyspepsia.METHODS:Twenty-two female patients with functional dyspepsia and twelve healthy volunteers were recruited for the study.The functional dyspepsia patients were each diagnosed based on the Rome Ⅲ criteria.Eligible patients completed a questionnaire concerning the severity of 10 symptoms.Plasma acylated ghrelin levels before and after a meal were determined in the study participants using a commercial human acylated enzyme immunoassay kit;electrogastrograms were performed for 50 min before and after a standardized 10-min meal containing 265 kcal.RESULTS:There were no significant differences in plasma acylated ghrelin levels between healthy volunteers and patients with functional dyspepsia.However,in patients with functional dyspepsia,there was a negative correlation between fasting plasma acylated ghrelin levels and the sum score of epigastric pain(r =-0.427,P = 0.047) and a positive correlation between the postprandial/fasting plasma acylated ghrelin ratio and the sum score of early satiety(r = 0.428,P =0.047).Additionally,there was a negative correlation between fasting acylated ghrelin plasma levels and fasting normogastria(%)(r =-0.522,P = 0.013).Interestingly,two functional dyspepsia patients showed paradoxically elevated plasma acylated ghrelin levels after the meal.CONCLUSION:Abnormal plasma acylated ghrelin levels before or after a meal may be related to several of the dyspeptic symptoms seen in patients with functional dyspepsia.展开更多

Impaired gastric accommodation is one of the most important etiologic factors in the pathophysiology of functional dyspepsia.Ultrasound is a potential alternative method to study changes in gastric volume as a reflect...Impaired gastric accommodation is one of the most important etiologic factors in the pathophysiology of functional dyspepsia.Ultrasound is a potential alternative method to study changes in gastric volume as a reflection of gastric accommodation.Ultrasound is suitable for patients because it is a non-invasive,easily repeated and non-radioactive procedure,and a previous study has demonstrated the feasibility of 3-dimensional ultrasound in examining functional dyspepsia.The brief article by Fan et al demonstrated that both the proximal gastric area and volume,measured by 2-and 3-dimensional ultrasound respectively,were significantly smaller in patients with functional dyspepsia than in healthy controls.These results are very interesting,but we raise the relevant point that it should have been mandatory to study both changes in gastric volume and their relationship with upper gastrointestinal symptoms in functional dyspepsia.In fact,the relationship between cardinal symptoms and several pathophysiologic mechanisms in functional dyspepsia remains a matter of debate.Moreover,further evaluation of distal gastric volume that has been previously implicated in the origin of functional dyspeptic symptoms is advisable.Therefore,impaired gastric accommodation does not serve as a clear marker of the cardinal symptoms experienced by patients with functional dyspepsia in daily life.展开更多

In addition, RpoS protein has played special roles in bacteria, therefore the changes of RpoS protein levels will lead to variations in expression levels of a large number of genes, thereby causing variations of bacterial response to different environmental stress and changes of certain characteristics of bacteria, which provides a new strategy for the control of bacterial diseases in the future. This paper reviewed the recent progress on the regulation of RpoS protein expression and its function in several common bacteria. Due to the functional complexity of RpoS protein, there are still a lot of unknown functions to be further identified.

文摘Newest planning methods implemented by Chinese government are promoting a coordinated regional development and shaping an orderly spatial structure by applying the regulation of territorial function. This article analyzes the problems of spatial planning and regional strategy caused by the wrongly-set primary goal of economic development; it states that the three-fold objective of competitiveness, sustainability, and welfare fairness shall be the principal for China to implement the spatial regulation in the new era; it discusses about theoretical thoughts and technology framework of conducting the ′Major Function Oriented Zone′ based on their different major functions that each region plays in urbanization and industrialization, ecological constructions, grain productions, and protection of natural and cultural heritages; it introduces the new concept of ′Major Function Oriented Zone′ that include the major functions category, the stereo regional equilibrium mode, the two-level zoning specification, and the territorial development intensity; it offers a zoning scheme that defines development-optimized and development-prioritized zones as regions with massive urbanization and industrialization, development-restricted zones as ecological constructing or grain producing regions, development-prohibited zones as natural and cultural heritage protecting regions; and finally it addresses the main obstacle for implementing ′Major Function Oriented Zone′, which is the institutional arrangement of the supreme goal of high GDP growth rate that is currently being implemented.

文摘Plant N starvation response is closely associated with the N signaling components that involve transduction of the low-N cues. In this study, we functionally characterized Ta ARR1, a cytokinin(CK) response regulator gene in Triticum aestivum, in mediating the N starvation adaptation in plants. Ta ARR1 harbors two conserved domains specified by plant ARR family members;subcellular localization analysis indicated its target onto nucleus after endoplasmic reticulum assortment. Ta ARR1 displayed modified expression upon the N starvation stressor, showing upregulated expression in roots and leaves over a 27-h N starvation treatment and whose induced transcripts were gradually recovered along with progression of the N recovery treatment. The tobacco lines overexpressing Ta ARR1 displayed improved low-N stress tolerance, displaying enlarged phenotype, increased biomass and N accumulation, and enhanced glutamine synthetase(GS) activities compared with wild type(WT) following the N starvation treatment. Expression analysis on genes encoding the nitrate transporter(NRT) and GS proteins in Nicotiana tabacum revealed that Nt NRT2.2 and Nt GS3 are upregulated in expression in the N-deprived transgenic lines, whose expression patterns were contrasted to other above family genes that were unaltered on transcripts between the transgenic lines and WT. Transgene analysis validated the function of Nt NRT2.2 and Nt GS3 in regulating N accumulation, GS activity, growth traits, and N use efficiency in plants. These results suggested the internal connection between the Ta ARR1-mediated N starvation tolerance and the modified transcription of distinct N acquisitionand assimilation-associated genes. Our investigation together indicates that Ta ARR1 is essential in plant N starvation adaptation due to the gene function in transcriptionally regulating distinct NRT and GS genes that affect plant N uptake and assimilation under the N starvation condition.
文摘AIM:To investigate the relationship between plasma acylated ghrelin levels and the pathophysiology of functional dyspepsia.METHODS:Twenty-two female patients with functional dyspepsia and twelve healthy volunteers were recruited for the study.The functional dyspepsia patients were each diagnosed based on the Rome Ⅲ criteria.Eligible patients completed a questionnaire concerning the severity of 10 symptoms.Plasma acylated ghrelin levels before and after a meal were determined in the study participants using a commercial human acylated enzyme immunoassay kit;electrogastrograms were performed for 50 min before and after a standardized 10-min meal containing 265 kcal.RESULTS:There were no significant differences in plasma acylated ghrelin levels between healthy volunteers and patients with functional dyspepsia.However,in patients with functional dyspepsia,there was a negative correlation between fasting plasma acylated ghrelin levels and the sum score of epigastric pain(r =-0.427,P = 0.047) and a positive correlation between the postprandial/fasting plasma acylated ghrelin ratio and the sum score of early satiety(r = 0.428,P =0.047).Additionally,there was a negative correlation between fasting acylated ghrelin plasma levels and fasting normogastria(%)(r =-0.522,P = 0.013).Interestingly,two functional dyspepsia patients showed paradoxically elevated plasma acylated ghrelin levels after the meal.CONCLUSION:Abnormal plasma acylated ghrelin levels before or after a meal may be related to several of the dyspeptic symptoms seen in patients with functional dyspepsia.

文摘Impaired gastric accommodation is one of the most important etiologic factors in the pathophysiology of functional dyspepsia.Ultrasound is a potential alternative method to study changes in gastric volume as a reflection of gastric accommodation.Ultrasound is suitable for patients because it is a non-invasive,easily repeated and non-radioactive procedure,and a previous study has demonstrated the feasibility of 3-dimensional ultrasound in examining functional dyspepsia.The brief article by Fan et al demonstrated that both the proximal gastric area and volume,measured by 2-and 3-dimensional ultrasound respectively,were significantly smaller in patients with functional dyspepsia than in healthy controls.These results are very interesting,but we raise the relevant point that it should have been mandatory to study both changes in gastric volume and their relationship with upper gastrointestinal symptoms in functional dyspepsia.In fact,the relationship between cardinal symptoms and several pathophysiologic mechanisms in functional dyspepsia remains a matter of debate.Moreover,further evaluation of distal gastric volume that has been previously implicated in the origin of functional dyspeptic symptoms is advisable.Therefore,impaired gastric accommodation does not serve as a clear marker of the cardinal symptoms experienced by patients with functional dyspepsia in daily life.
